The purposes of this study were to investigate blood profiles and nutrient intakes of groups that are different in obese levels, and to find the credible predictor of insulin resistance. The subjects were classified as normal weight (%IBW${\leq}$ 110), obese without MS and obese with MS according to IDF definition of the risk group in metabolic syndrome (MS). Subjects of this study were included 137 (59 boys, 78 girls) free living children and adolescents (mean age $12.6{\pm}3.4$ years) in Gangneung area, South Korea. %IBW of normal weight (94.9%), obese without MS (123.8%) and obese with MS (131.5%) were significantly different among groups. HOMA-IR had positive correlations with TG (r = 0.634), waist circumference (r = 0.553), atherogenic index (r = 0.513), %IBW (r = 0.453) and ALT (r = 0.360), but showed negative correlations with HDL cholesterol (r = -0.417, p < 0.001). HOMA-IR showed positive correlation with polyunsaturated fatty acid intake (p < 0.05). The energy intake of obese with MS was 1762 kcal/day which was not significantly different from those of normal weight and obese without MS. Total fatty acid intakes of two obese groups were significantly higher than that of normal weight. The results of this study suggest that waist circumference and ALT as well as TG, atherogenic index and weight can be credible indices to predict the insulin resistance in children and in adolescents. In addition, nutrition education and adequate diet should be provided to prevent MS in children and in adolescents.
Weight-controlling can be supported by a proper prescription of energy intake. The individual energy requirement is usually determined through resting energy expenditure (REE) and physical activity. Because REE contributes to 60-70% of daily energy expenditure, the assessment of REE is very important. REE is often predicted using various equations, which are usually based on the body weight, height, age, gender, and so on. The aim of this study is to validate the published predictive equations for resting energy expenditure in 76 normal weight and 52 obese Korean children and adolescents in the 7-18 years old age group. The open-circuit indirect calorimetry using a ventilated hood system was used to measure REE. Sixteen REE predictive equations were included, which were based on weight and/or height of children and adolescents, or which were commonly used in clinical settings despite its use based on adults. The accuracy of the equations was evaluated on bias, RMSPE, and percentage of accurate prediction. The means of age and height were not significantly different among the groups. Weight and BMI were significantly higher in obese group (64.0 kg, $25.9kg/m^2$) than in the non-obese group (44.8 kg, $19.0kg/m^2$). For the obese group, the Molnar, Mifflin, Liu, and Harris-Benedict equations provided the accurate predictions of > 70% (87%, 79% 77%, and 73%, respectively). On the other hand, for non-obese group, only the Molnar equation had a high level of accuracy (bias of 0.6%, RMSPE of 90.4 kcal/d, and accurate prediction of 72%). The accurate prediction of the Schofield (W/WH), WHO (W/WH), and Henry (W/WH) equations was less than 60% for all groups. Our results showed that the Molnar equation appears to be the most accurate and precise for both the non-obese and the obese groups. This equation might be useful for clinical professionals when calculating energy needs in Korean children and adolescents.
This study set out to investigate the effects of 16-week nutrition education and exercise intervention run by a public health center on the physical activities and dietary patterns of obese children at the before, after, and after one month point. After the program, the obese index of the obese children dropped by 5.9% from 38.4% to 32.5%. One month after the end of the program, their obese index increased by 1.4% to 33.9% (p < 0.001). The obese children's hours of walking of physical activities increased to 2.6 hours from 1.2 hours after the program and remained at the 2.6 hours range one month after the end of the program (p < 0.01). The total scores of their food habits increased to 10.4 points from 9.1 points after the program and 10.3 points one month after the end of the program with significant differences (p < 0.01). The total scores of their eating behavior made a significant increase to 6.5 points from 5.3 points after the program and then dropped to 5.9 points one month after the end of the program (p < 0.05). As for changes to their dietary pattern scores according to the obese index, only the food habits scores showed significant main effects of term (p < 0.05). According to these results, there is still a need to develop proper programs to help them increase regular exercise, improve their physical activities by cutting down time with TV, computer, Internet, and video games, and enhance their nutritional knowledge and to provide them with ongoing management and guidance until the improved food habits and eating behavior become part of their habits.
목 적 : 지속적으로 증가하는 소아 비만은 성인 비만으로 이행되기 쉽고 합병증으로 고혈압, 지방간, 동맥경화증이 동반될 수 있다. Carnitine은 장쇄 지방산이 미토콘드리아로 이동할 때 필요한 조효소로 지방산 대사에서 중요한 역할을 한다. 지방간을 가진 비만 소아에서 혈중 지방산과 carnitine 농도를 측정함으로써 L-carnitine을 임상적으로 비만 치료에 적용할 수 있는지를 알아보고자 본 연구를 실시하였다. 방 법 : 7-18세의 지방간으로 진단받은 비만아 9명과 정상 대조군 소아 10명을 대상으로 하였다. 혈장을 sodium borate를 섞어 원심분리 후 하층을 methylene chloride를 이용하여 계층 분리하였고, MSTFA와 acetonitrile을 넣고 유도체화 반응을 시켰다. GC-MS 자동 분석기로 혈청 지방산 fraction을 정량 분석하였고, carnitine(free, acyl, total)은 cycling technique을 이용하여 415 nm에서 정량 분석하였다. 결 과 : 혈중 총 지방산은 지방간이 동반된 비만아 군에서 대조군에 비해 유의하게 증가하였고 특히 장쇄 지방산(myristic acid, palmitoleic acid, palmitic acid, linoleic acid, oleic acid, stearic acid)이 의미 있게 증가되어 있었다. 총 carnitine과 유리 carnitine 농도가 비만아에서 정상아에 비해 유의하게 증가하였으나 acyl carnitine은 유의한 차이가 없었다. 결 론 : 지방간이 동반된 비만아에서 장쇄 지방산이 뚜렷이 증가되었으며, 비만아군에서 정상아에 비해 혈청 carnitine이 증가되어 있음을 알 수 있었다. 이것을 기초로 하여 비만아에서 L-carnitine 투여 후 지방산 대사의 변화에 대해 추후에 연구할 예정이다.
This study determines the body size of preteen children and conducts questionnaire investigations into the awareness of real purchasers or parents of and satisfaction with children's clothes and fit, in order to provide the basic data for planning and setting up the size range for the preteens market in which the sales of children clothes are gradually increasing. The findings of this study are as follow: Analyzing the obesity of the children against the Rohrer index showed that almost 30% of the subjects were obese children and that it is necessary to research the sizes of the clothes for obese children. It was also found that older children selected a more appropriate clothe size. Investigations into the awareness of children on their body image found that there was a significant difference in the perception of body shape, body weight, shoulder breath, arm thickness, chest girt, waist girth, hip girth, and thigh thickness. Slim or standard type children had the greatest consideration for the length of clothes to purchase while obese children had the greatest consideration for bosom size and waist girth. However, in terms of the association between fitness and size selection, obese children did not select a bigger size but an appropriate one for their body. The findings show that it is imperative to develop appropriate size clothes for obese children.
This study investigates thermoregulatory behaviors of obese children for their thermal comfort. Nine normal-weight children (NWC) and nine obese children (OC) participated in the study. All subjects were fourth-graders and sixth-graders in elementary school. An informed consent form was obtained from each child and his or her mother. The questionnaire method was used, and the experiment was conducted to determine the preferred amount of clothing at $17^{\circ}C$ and the preferred temperature at $33^{\circ}C$. According to the results, OC were more susceptible to heat than NWC, whereas there was no significant difference in their susceptibility to cold. There were no significant differences of the oral temperature and the mean skin temperature between the two groups, but changes in forearm blood flow were less in OC than in NWC. There was no significant difference in the preferred amount of clothing between OC and NWC, and the preferred temperature was higher in NWC than in OC. The results based on the questionnaire are consistent with those based on the experiment. A compositive study of physical exercise, eating behavior, and wearing behavior should be useful for developing programs for preventing and treating obese children.
Epidemiological studies have indicated that a relationship between overweight and cardiovascular disease exists. To assess the relationship of unfavorable changes in serum lipid concentrations, apolipoprotein B, lipoprotein(a), blood sugar and blood pressure to anthropometric variables, nutrient intakes and dietary behavior,64 obese children were chosen as subjects and 24-hour dietary intake, dietary behavior as well as blood pressure, apolipoprotein B, lipoprotein(a), three kinds of cholesterol concentrations were measured in November 1976. More than half(57.8%) of the children had serum cholesterol concentrations over 175mg/d1, and 26.6% of them had over 200mg/d1 and this group could be classified as a high risk group. Abdominal obesity as measured with waist circumference and waist hip ratio was negatively co..elated to high density lipop개tein(HDL) cholesterol. Dietary behavior rather than nutrient intakes appeared to have associations with unfavorable lipid profiles. Total dietary behavior scores and fish and bean product consumption had strong correlations with potentially atherogenic lipoprotein concentrations. This study suggests that fir the early prevention of cardiovascular disease, waist hip ratio together with obese rate should be included in selective programs to identify risk group of children in Korea. furthermore, interventions in cardiovascular disease risk groups in obese children should emphasize maintenance of ideal body weight through reducing body fat and adopting desirable dietary behaviors such as increasing intake of fish or bean products. This study was carried out to provide information on the related factors for child obesity through investigation of children aged 6-12. Data was obtained on family history, dietary habits and physical examinations. Children were classified into normal(98 persons) and obese (108 persons) groups using combinations of the height f3r age, weight fir age, and weight fir height standards used by the World Health Organization. Children's height, weight and BMI showed significant differences between groups except children aged 8 for height. Birth weight, breast feeding and unbalanced diet were not associated with obesity. Overeating was a risk factor in the obese group(Odd ratio : 3.417, 95% CI 1.879-6.212). Taking nutrition pills was correlated to obesity. Odd ratio for taking nutrition pills was 0.109(95% CI : 0.024-0.492) between normal and obese group. Number of brothers didn't have any correlation to obesity. Obesity was not correlated to mother's education level or employmental status. As the result of this study, children's obesity was related to overeating and taking nutrition pills. Therefore, these results suggest that systematic dietary education and continuous parental supervision are necessary to avoid child obesity. (Korean J Nutrition 31(7) : 1158-1164, 1998)
Purpose: To compare normal weight children with obese children in terms of family factors, eating habits, and sociocognitive factors. Method: This study compared 217 obese children with 231 normal standard weight children of 4th-6th graders from 7 elementary schools in Kangwon province. The study also surveyed 817 their parents using questionnaires in which familial factors, eating habits, and sociocognitive factors such as dietary self-efficacy, locus of control and self-esteem were included. Result: Obese children had more obese family members, more family members with chronic metabolic disease, and lower breast-feeding rate than normal weight group. Dietary self-efficacy was positively correlated with eating habits and locus of control while no correlation was found with obesity index.. Self-esteem was negatively correlate with obesity index. Both the parents and the childrens eating habits showed significant positive correlation to the obesity index. Conclusion: These results implied familial factors, eating habits, and sociocognitive factors influenced childhood obesity. Thus, further research targeting to positive attitude toward familial dietary practices combined with significant sociocognitive factors, may lead to prevention and effective management of childhood obesity.
This study was conducted to investigate the effects of wearing behavior of clothing and dietary behavior, and psychological characteristics on the obese children. For this purpose, the questionnaire method was used and the subjects were 1,501 elementary school children in the 4th to the 6th grade and their mothers (1,459), in Seoul, Busan, Daegu, Gwangju, and Andong cities. The children were divided into two groups, such as a normal weight group (NWG) and an obese group (OG). There were significant differences in adaptability to the cold and the heat, and in wearing behavior of clothing for body shape between NWG and OG. Scores for balanced dietary behavior was higher in OG than in NWG while dietary control perceived by their mothers was better in NWG than in OG. Stress was lower and self-esteem was higher in NWG than in OG. Experimental tests are needed to analyze the behavior of obese children on the basis of the obtained results of this study.
