Surgical Outcomes in Small Cell Lung Cancer

  • Background: The experience of a single-institution regarding surgery for small cell lung cancer (SCLC) was reviewed to evaluate the surgical outcomes and prognoses. Materials and Methods: From July 1990 to December 2009, thirty-four patients (28 male) underwent major pulmonary resection and lymph node dissection for SCLC. Lobectomy was performed in 24 patients, pneumonectomy in eight, bilobectomy in one, and segmentectomy in one. Surgical complications, mortality, the disease-free survival (DFS) rate, and the overall survival rate were analyzed retrospectively. Results: The median follow-up period was 26 months (range, 4 to 241 months), and there was one surgical mortality (2.9%). Six patients (17.6%) experienced recurrence, all of which were systemic. Eight patients died during follow-up; four died of disease progression and the other four died of pneumonia or of another non-cancerous cause. The three-year DFS rate was $79.2{\pm}2.6%$ and the overall survival rate was $66.4{\pm}10.5%$. Recurrence or death was significantly prevalent in the patients with lymph node metastasis (p=0.001) as well as in those who did not undergo adjuvant chemotherapy (p=0.008). The three-year survival rate was significantly greater in the patients with pathologic stage I/II cancer than in those with stage III cancer (84% vs. 13%, p=0.001). Conclusion: Major pulmonary resection for small cell lung cancer is feasible in selected patients. Patients with pathologic stage I or II disease showed an excellent survival rate after surgery and adjuvant treatment. Prospective randomized studies will be needed to define the role of surgery in early-stage small cell lung cancer.

일부 종합병원 간호사의 건강증진생활양식에 대한 실천정도와 영향요인에 관한 연구 (A Study on Health-Promoting Lifestyle and Its Affecting Factors of Hospital Nurses)

  • The chances of disease pattern increased the importance of Health-promoting Lifestyle and a large part of the Health-promoting Lifestyle is associated with individual's habit. Health-promoting Lifestyle among nurses is very important because nurses could be a role model of patients and possibly influence many patients. The purpose of this study was oat on1y to assess how hospital nurses practice their Health-promoting Lifestyle but also to identify those affecting determinants. The subjects were 392 nurses working at 3 different hospital in Seoul. These data were collected by self administered questionnaire from April 27 to May 20, 2000. This study examined Health-promoting Lifestyle using In Sook Park's model, individual characteristics. Behavior-specific Cognitions and Affect factors using Pender's model and tried to fine out their relationships. The instruments used in this study were Health-promoting Lifestyle Profile developed by In Sock Park(1995). Likert's four-point scale was used also in this research. The percentage, mean standard deviation, AVNOVA, Pearson's correlation coefficient and multiple regression in the SAS package were used to analyze the data. The results of this study were as follows; 1. 52.3% of sample were aged between 25 and 29, 67.1% were single, 55.6% were university graduates, 51.8% earned 1.5 to 2.0 million won, 57.9% slept for d to 8 hours, 74.5% stated they were healthy. 2. 32.7% of sample worked in surgical gird department, 82.4% worked in 3 shift, 26.3% have been working as nurses for 5 to 7 years. Average score of Perceived self-efficacy was 3.63, Perceived benefits of action was 3.25, Social support was 2.75, and Perceived barriers to action was 1.87 which was the lowest score. 3. The average score if the performance in Health-promoting Lifestyle variable was 2.45 which was lower than previous study. The sanitary life Was 3.18 which was the highest score, harmonious relationship 3.13, emotional support 2.90. The variable with the highest degree of performance was the sanitary life, whereas the one with the west degree was the health diet. 4. There were no significant difference in perceived benefits of action with individual's a character, but in Perceived bait his of action there were significant differences with age(p<.01), marital status(p<.05), housing(p<.05), and Perceived health status(p<.05). In Perceived self-efficacy, there were significant differences with educational level(p<05), sleeping hours(p<.05), and BMI(p<.05). In Social support here were significant difference with housing and sleeping hours.

파종기가 귀리의 생육 및 수량에 미치는 영향 (Effects of Seeding Date on Growth and Yield in Oats)

  • 귀리의 생육기를 예측하는데 유효적산온도(GDD)의 이용성을 검토하고 제주지방에서 종실용 귀리의 파종적기를 구명하고자 1991년과 1992년에 귀리 1006를 10월 9일부터 12월 9일까지 15일 간격으로 5회(10월9일, 10월24일, 11월9일, 11월24일, 12월9일) 파종하여 귀리의 생육일수 및 GDD, 생육 및 수량관련 형질을 조사한 결과를 요약하면, 다음과 같다. 1. 두 해 모두 만파할수록 출아일수는 길어졌으나 출수일수와 성숙일수는 짧아졌다. 파종부터 출아까지 GDD는 대체로 만파할수록 적어지는 경향이나 파종기 이동에 따른 경향이 출아일수보다 현저하지 않았다. 두 해 모두 출아부터 출수까지의 GDD는 만파할수록 적어졌고 출수부터 성숙까지의 GDD도 11월 24일까지는 만파할수록 적어졌으나 12월 9일 파종구에서는 더이상 적어지지 않았다. 2. 파종기가 10월 9일에서 12월 9일로 늦어짐에 따라 1992년 파종의 출수기 엽면적지수는 7.7에서 5.1로, 성숙기 건물중은 1920kg/10a에서 823kg로 감소되었고, 2개년 평균 간장은 120cm에서 89cm로 짧아졌다. 3. 두 해 모두 수장, 1수입수, l 중은 파종기간 유의한 차이가 없었고 $m^2당 수수, 1000입중, 종실수량은 11월 9일 파종구에서 가장 많았고 이보다 조파하였거나 만파하였을 때 감소되었다. 따라서 제주지방에서 종실용 귀리의 파종적기는 11월 상순으로 판단된다.

사과원에서 피복식물이 토양생물상에 미치는 영향 (Effects of Cover Plants on Soil Biota: A Study in an Apple Orchard)

  • 사과원 토양의 지속적인 생산성을 위해 토양생물의 변화를 파악하는 것이 중요하므로, 피복식물 재배에 따른 미생물 PLFA 및 미소동물의 개체밀도 변화를 조사하였다. 피복식물은 두과식물인 호밀, 귀리, 들묵새와 두과식물인 자운영, 레드클로버, 크림슨클로버, 헤어리배치를 대상으로 하였다. 청경구에 비하여 피복식물을 재배하였을 때 미생물 PLFA 수치 및 미소동물이 증가하였기 때문에 청경구보다는 피복식물로 토양을 관리하는 것이 토양생물을 증가시키는데 유리할 것으로 사료된다. 미생물 PLFA 수치에서는 피복식물간 차이가 적었지만, 화본과식물 중에서는 들묵새와 두과식물 중에서는 자운영이 토양의 미소절족동물의 개체밀도를 증가시키는 효과가 비교적 큰 것으로 나타났다. 본 결과는 적절한 피복식물 선정을 통한 효과적인 유기물분해와 안정적인 양분공급을 위한 기초자료로 활용될 수 있을 것이다.

우분 분해에 대한 지렁이와 소똥구리의 상호작용 (Interaction between Earthworm and Dung Beetles on Cattle Dung Decomposition)

  • 우분의 분해에 있어 소똥구리와 지렁이를 함께 처리한 구에서 단독으로 소똥구리와 지렁이를 처리한 것 보다 분해율이 높았고, 지렁이와 소똥구리가 우분 분해 활동을 한 다음에 식물체를 파종했을 때 식물체 생장은 비료를 처리한 것과 동일한 효과가 인정되었다. 우분 함수율 78%까지는 소똥구리에 의해 분해 활동이 일어나며, 30%까지는 지렁이에 의해, 30% 이하에서는 두 동물군 모두 분해 활동을 하지 않았다. 소똥구리와 지렁이에 의해 운반된 우분은 초기 우분의 약 72%이며, 이중 10.2%는 소똥구리의 brood ball 형성에 이용되었다. 지렁이의 활동이 소똥구리의 brood ball 형성에 저해요인이 되지 않았고, 소똥구리의 활동 또한 지렁이의 생육에 영향을 미치지 않아 지렁이와 소똥구리는 우분에서 공간적 분포를 같이 하면서 먹이 경쟁을 하기 보다는 상호보완적인 작용을 하는 것으로 밝혀졌다.

Methane Production Potential of Feed Ingredients as Measured by In Vitro Gas Test

  • This study was conducted to investigate in vitro methane production of feed ingredients and relationship between the content of crude nutrients and methane production. Feed ingredients (total 26) were grouped as grains (5 ingredients), brans and hulls (8), oil seed meals (9) roughages (3), and animal by-product (1) from their nutrient composition and their methane production protential were measured by in vitro gas test. Among the groups, the in vitro methane productions for both 6 and 24 h incubation were highest in grains, followed by brans and hulls, oil meals and roughages, animal byproducts. Within the group of grains, methane production from wheat flour was the highest, followed by wheat, corn, tapioca, and then oat. Within the brans and hulls, soybean hull showed the highest methane production and cotton seed hull, the lowest. Methane production from oil meals was lower compared with grains and brans and hulls, and in decreasing order production from canola meal was followed by soybean meal, coconut meal, and corn germ meal (p<0.01). Three ingredients were selected and the interactions among feed ingredients were evaluated for methane production. Correlation coefficient between measured and estimated values of the combinations were 0.91. Methane production from each feed ingredient was decreased with increasing amount of crude fiber (CF), protein (CP) and ether extract (EE), whereas positive relationship was noted with the concentrations of N-free extract (NFE). The multiple regression equation (n=134) for methane production and nutrient concentrations was as follows. Methane production (ml/0.2 g DM)=(0.032${\times}$CP)-(0.057${\times}$EE)-(0.012${\times}$CF)+(0.124${\times}$NFE) (p<0.01; $R^2$=0.929). Positive relationship was noted for CP and NFE and negative relationship for CF and EE. It seems possible to predict methane production potential from nutritional composition of the ingredients for their effective application on formulating less methane emitting rations.

Energy Metabolism and Methane Production in Faunated and Defaunated Sheep Fed Two Diets with Same Concentrate to Roughage Ratio (70:30) but Varying in Composition

  • Two calorimetric experiments were performed to investigate the effects of two diets with same concentrate: roughage ratio (70:30) but varying in composition on energy metabolism and methane production in faunated (F) and defaunated (DF) Muzaffarnagari sheep. For experiment I, ten animals were divided equally into two groups of which one was kept normally F as such while other was DF using 10% sodium lauryl sulphate. All the animals were offered diet I which comprised of oat hay and concentrate mixture I (CM I) containing maize grain (93%) as a major ingredient in 70:30 ratio. Similarly, the experiment II was conducted for which four F and four DF sheep (same as used for experiment I) were switched to diet II that consisted of maize hay and CM II (maize grain 59% + molasses 36%). Through diet II, DM intake in DF sheep was significantly (p<0.05) lower. Intake of GE through both the diets was similar in F and DF sheep. Digestibility of DM, OM, CP and GE and also metabolisability (ME/GE) was similar in F and DF sheep on both the diets. Total urinary energy loss did not differ in F and DF on both the diets, but methane energy loss as a percent of GE in DF was significantly (p<0.05) lower on diet I (3.75 vs 2.48), while it did not differ on diet II (3.20 vs 3.60). Heat production was significantly (p<0.01) reduced in DF on both the diets. Although, efficiency of utilisation of ME for maintenance calculated as per ARC (1984) did not differ in F and DF on both the diets, efficiency for maintenance and growth was higher (0.60 vs 0.672) on diet I in DF. It was inferred that methane production in DF sheep reduces on good quality hay-based diet supplemented with slowly fermentable carbohydrate (maize grain) but supplementation of molasses (rapidly fermentable CHO) nullify this effect when sheep were fed diets with concentrate: roughage ratio of 70:30.

Effect of condensed tannins from Ficus infectoria and Psidium guajava leaf meal mixture on nutrient metabolism, methane emission and performance of lambs

  • Objective: The study examined the effect of condensed tannins (CT) containing Ficus infectoria and Psidium guajava leaf meal mixture (LMM) supplementation on nutrient metabolism, methane emission and performance of lambs. Methods: Twenty four lambs of ~6 months age (average body weight $10.1{\pm}0.60kg$) were randomly divided into 4 dietary treatments (CT-0, CT-1, CT-1.5, and CT-2 containing 0, 1.0, 1.5, and 2.0 percent CT through LMM, respectively) consisting of 6 lambs each in a completely randomized design. All the lambs were offered a basal diet of wheat straw ad libitum, oat hay (100 g/d) along with required amount of concentrate mixture to meet their nutrient requirements for a period of 6 months. After 3 months of experimental feeding, a metabolism trial of 6 days duration was conducted on all 24 lambs to determine nutrient digestibility and nitrogen balance. Urinary excretion of purine derivatives and microbial protein synthesis were determined using high performance liquid chromatography. Respiration chamber study was started at the mid of 5th month of experimental feeding trial. Whole energy balance trials were conducted on individual lamb one after the other, in an open circuit respiration calorimeter. Results: Intake of dry matter and organic matter (g/d) was significantly (p<0.05) higher in CT-1.5 than control. Digestibility of various nutrients did not differ irrespective of treatments. Nitrogen retention and microbial nitrogen synthesis (g/d) was significantly (p<0.01) higher in CT-1.5 and CT-2 groups relative to CT-0.Total body weight gain (kg) and average daily gain (g) were significantly (linear, p<0.01) higher in CT-1.5 followed by CT-1 and CT-0, respectively. Feed conversion ratio (FCR) by lambs was significantly (linear, p<0.01) better in CT-1.5 followed by CT-2 and CT-0, respectively. Total wool yield (g; g/d) was linearly (p<0.05) higher for CT-1.5 than CT-0. Methane emission was linearly decreased (p<0.05) in CT groups and reduction was highest (p<0.01) in CT-2 followed by CT-1.5 and CT-1. Methane energy (kcal/d) was linearly decreased (p<0.05) in CT groups. Conclusion: The CT supplementation at 1% to 2% of the diet through Ficus infectoria and Psidium guajava LMM significantly improved nitrogen metabolism, growth performance, wool yield, FCR and reduced methane emission by lambs.

Bacillus pumilus TX703 유래 Xylanase 유전자(xynK)의 Cloning과 염기서열 분석 (Molecular Cloning and Analysis of Nucleotide Sequence of Xylanase Gene (xynk) from Bacillus pumilus TX703)

  • Xylanase를 생산하는 내열성 Bacillus pumilus TX703의 chromosomal DNA로부터 xylanase 유전자를 cloning하여 그 염기배열 순서를 결정한 다음 이로부터 유전자 발현에 관련된 구조를 분석하였다. Xylanase 유전자의 cloning을 위해 제한효소 HindIII로 절단한 B. pumilus TX703의 chromosomal DNA와 pUC19을 ligation시켜 E. coli DH5 $\alpha$에 형질전환시킨 후 형질전환체 중에서 xylanase 활성을 나타내는 재조합 plasmid pXES106을 분리하였다. 재조합 plasmid pXES106은 pUC19의 HindIII 부위 내에 2.24 kb의 외래 DNA가 삽입되었고, 이 plasmid DNA를 분리하여 E. coli DH5 $\alpha$에 재형질전환시킨 결과 vector 내에 xylanase 유전자가 cloning되었음을 확인하였다. Cloning된 유전자의 염기배열을 분석한 결과 이 유전자의 총 크기는 2,187 bp였고 이는 409개기 아미노산을 coding 하는 open reading frame 1,227 bp를 포함하고 있었다. 이 염기배열은 ATG개시 codon으로부터 각각 193과 216 base 상류에 TTTAAT의 -10 box와 TCGAAA인 -35 box로 추정되는 염기배열이 존재하였고 -10 box로부터 7 bp하류에 전사개시점인 A가 위치하고 있었다. 또한, 개시 codon으로부터 432 bp 상류에 공통염기배열과 14개의 염기 중 11개의 염기가 일치하는 TGATGGCGTCGGCA의 catabolite responsive element (CRE)가 존재하였다. B. pumilus TX703의 xylanase와 아미노산배열의 유사성이 가장 높은 xylanase는 Hordeum vulgare의 isozyme X-I이었고 본 xylanase는 208번째와 322번째에 glutamic acid 잔기를 가지고 있어 Clostridium thermocellum, Dictyoglomus thermophilum, Thermotoga neapolitana 등에서 밝혀진 바와 같이 glutamic acid 부위가 xylanase의 활성부위라 여겨진다.

맥류미성숙 종자와 초기생육과의 관계 (Growth of Seedling from Immature Kernel Harvested at Different Days after Flowering in Rye, Triticale, Wheat and Oat Cultivars)

  • 입당호밀, 두루호밀, 신기호밀(트리티케일), 그루밀, 메귀리 등 5개 품종에 대하여 미성숙종자의 초기 생육(27일간)을 조사하기 위하여 1985년 10월부터 1987년 2월까지 수원지방에서 행한 시험결과를 요약하면 다음과 같다. 1. 미성숙 종자의 천립중은 성숙기까지 계속 증가를 나타냈는데 (r=0.9000 이상), 입당호밀과 두루호밀은 개화후 35 일 (각각 6월 19일, 6월 24 일), 신기호밀, 그루밀, 메귀리 등은 개화후 30일 (각각 6월 29일, 6월 19일, 7월 9 일)에 천립중이 거의 최대에 달하였다. 2. 미성숙 종자의 발아율은 종자의 천립중 증가와는 상관이 높지 않았으며, 메귀리에서 개화후 5일에 49 %의 발아를 보였고 개화후 10 일부터는 94% 이상을 보였다. 기타 품종들도 어느 일정 시기이후에는 90% 이상의 높은 발아율을 보였다. 3. 미성숙 종자에서 생육한 개체의 초장은 종실의 천립중과 고도의 정의 상관을 보였지만 신기호밀과 메귀리에서 개화후 20 일, 25 일, 30 일 (성숙기)에 수확한 종자의 유식물체간에 그루밀에서는 개화후 25일, 30 일 (성숙기) 간에 차이가 적었다. 4. 미성숙 종자에서 온 유식물체의 생체중과 건물중은 종실의 천립중과 고도의 정의 상관을 보였으나 신기호밀, 그루밀, 메귀리 등에서 개화후 25 일이나 30일에 수확한 종자의 유식물체들 간에 차이가 적었다. 5. 수원지방에서 청예용 종자로서 사용할 경우 입당호밀과 두루호밀은 개화후 35 일 (각각 6월 19일, 6월 24 일 ) 신기호밀, 그루밀, 메귀리 등은 개화후 25 일 (각각 6월 24 일, 6월 14 일, 7월 4 일) 에 수확하여도 청예수량이 크게 감소되지 않을 것으로 보인다.
