2009년 국민건강영양조사에 근거한 Significant Caries(SiC) Index (Significant Caries(SiC) Index Based on 2009 Korea National Health and Nutrition Examination Survey)

  • 본 연구는 2009 국민건강영양조사자료를 근거로 6세 이상 인구의 DMFT index(9,271명)를 구하고 그 중 상위 1/3인 고위험군을 분류하여 SiC index(2,517명)를 산출하였다. 이에 치아우식증 고위험군의 존재를 확인하고 고위험군에 대한 예방전략의 필요성을 강조하고자 하였다. 통계 분석은 STATA 11.0 프로그램을 이용하여 t-test 분석을 하였다. 1. DMFT index의 연구대상자는 남자가 45.0%(4,174명), 여자가 55.0%(5,097명)이었으며, SiC index 대상자를 산출한 결과 전체 2,517명이었으며, 남자는 36.3%(914명), 여자는 63.7%(1,603명)로 SiC index에서 여자가 더 많았다. 2. DMFT index와 SiC index의 평균을 보면 연령이 증가할수록 두 집단 간 평균의 차이가 컸으며, 특히 13, 14세는 평균 4 이상, 15세 이상부터는 평균 2배 이상의 차이를 보였다. 3. 성별에 따라 DMFT index를 비교한 결과 14세부터 남녀 간에 통계적으로 유의하였으나 결과는 모든 연령에서 여자가 남자보다 높게 나타났다. SiC index는 성별에 따른 통계적 유의성이 없었으며, 9세와 14세에서 약 1개 여자가 더 많게 나타났다. 4. 지역별 DMFT index를 비교한 결과 60대와 70세 이상에서 도시보다 전원지역이 더 높았다. SiC index는 60대에서 도시가 15.49, 전원지역이 17.04로 약 1.5개 정도 전원지역이 높았다. 이상의 결과를 볼 때 DMFT index는 연령이 증가할수록 성별의 차이를 보였으나 SiC index는 성별, 지역별 큰 차이를 보이지 않았다. 고위험군은 성별과 지역별 차이 없이 포괄적이고 집중적인 예방관리가 필요하며, 고위험군의 구강건강증진을 위한 구강보건정책이 수립되어야 한다.

The framingham risk score, diet, and inflammatory markers in Korean men with metabolic syndrome

  • The Framingham risk score (FRS) has been used to assess the risk of a cardiovascular event and to identify patients for risk factor modifications. Therefore, the purpose of this study was to evaluate the relationship of the FRS with dietary intake and inflammatory biomarkers. We conducted a cross-sectional study of 180 men ($49.2{\pm}10.2$ years) with MS. Serum levels of high sensitive C-reactive protein (hs-CRP), interleukin-6 (IL-6), and adiponectin were examined. Participants were asked to complete the food frequency questionnaire (FFQ) using the previous 1 year as a reference point. The absolute cardiovascular disease (CVD) risk percentage over 10 years was calculated to estimate the FRS, which was classified as low risk (< 10%), intermediate risk (10-20%), and high risk (> 20%). Mean intake of polyunsaturated fatty acids was lower in subjects who had > 20% FRS than in subjects who had < 10% FRS ($3.7{\pm}1.9$ g/day vs. $4.7{\pm}1.9$ g/day; P < 0.05). Significant differences in the Index of Nutritional Quality of protein, phosphorus, iron, vitamin A, vitamin $B_1$, niacin, vitamin $B_6$, and vitamin C were observed between the > 20% FRS group and the < 10% FRS group (P < 0.05). IL-6 concentrations were significantly lower in subjects with a < 10% FRS than in subjects who were 10-20% FRS or > 20% FRS ($0.91{\pm}0.26$ vs. $1.48{\pm}033$ vs. $2.72{\pm}0.57$ pg/mL, respectively; P < 0.05). IL-6 and dietary intake of polyunsaturated fatty acids together explained 6.6% of the variation in FRS levels in a stepwise multiple regression model. Our results provide some evidence that dietary intake in the higher CVD risk group was inferior to that in the lower risk group and that dietary fat intake and IL-6 were associated with FRS and MS in Korean men.

일부 흡연 남성의 식사섭취와 대사증후군 위험도 (Nutrient Intake and Risk of Metabolic Syndrome in Male Smokers)

  • This study was intended to investigate the risk of metabolic syndrome (MS) in healthy 92 male smokers (mean age: $44.4\;{\pm}\;7.8\;yrs$). We investigated the anthropometric assessment and dietary intake survey for 2 days by 24-recall method, also blood pressure and serum lipids were measured. The average numbers of cigarettes smoking a day were 21.3/d, smoking duration were 21.5 years. The proportion of fat energy was 24.9% and intakes of vitamin B2, folate, calcium, potassum and fiber were lower than KDRI. The body mass index (BMI) and body fat % were 24.8, 23.9% respectively. The systolic ($134.1\;{\pm}\;1.4\;mmHg$) and the diastolic blood ($87.9\;{\pm}\;1.1\;mmHg$) pressure were in borderline hypertension. Among biochemical parameters, TG ($173.6\;{\pm}\;9.4\;mg/dL$) and fasting blood glucose ($109.0\;{\pm}\;2.4\;mg/dL$) levels were out of normal values. The most occurred problem among the risk factors related to MS was the borderline hypertension (63%) in subjects. Regarding the correlations of anthropometric data with biochemical factors, TG was significantly correlated with the BMI, body fat % and waist circumference. Smoking years showed positive correlation with AI. These results suggest that the smoking habit has significant relations with the risk factors of MS. Therefore, quitting is necessary to prevent MS, and nutrition education and dietary management program are required to prevent the degenerative disease.

지역 규모에 따른 농촌주민의 건강위험행동 관계 연구 (Study on the Relationship between Health Risk Behaviors of Rural Residents by Regional Scale)

  • Objective: This study aimed to analyze the correlation between factors affecting health risk behaviors of rural residents according to regional scale. Methods: Restricted-access data from the 2016~2021 Korea National Health and Nutrition Examination Survey and the multivariate probit model were used. As for health risk behaviors, smoking, drinking, lack of aerobic exercise, low level of healthy eating index, unvaccination, and non-participation in health examination were considered. Results: Controlling for individuals' socio-demographic characteristics, in general, correlation coefficients between unobservable factors affecting health risk behaviors were significant. However, the magnitude and statistical significance of the correlation coefficients varied by regional scale (dong/eup/myeon). This suggests that rural residents engage in health risk behaviors due to their different characteristics compared with urban dwellers, which also varies by whether residents are located in eup or myeon area. Conclusion: It is necessary to differentiate health care services between urban and rural areas in terms of type of service and programs based on the relationship between unobservable factors affecting each type of health risk behaviors.

Relation of Serum Oxygen Radical Absorbance Capacity with Metabolic Risk Factors in Human Volunteers

  • Oxygen radical absorbance capacity(ORAC) is known to be a sensitive and simple method to determine total antioxidant capacity(TAC) in biological samples. While TAC has received great attention with its relation to pathogenesis in the progression of several diseases, little is known about association of ORAC with metabolic risk factors. The aim of this study is to evaluate the relationship between ORAC and serum lipid profiles, fasting glucose and anthropometric measures. One hundred seventeen volunteers participated in the study. Perchloric acid treated serum was used to determine $ORAC_{pca}$. Mean$ORAC_{pca}$ of subjects whose serum total cholesterol(TC) concentrations were $\geq$ 200 mg/dl was significantly(P < 0.05) higher than that of subjects whose TC concentrations were < 200mg/dl. There were significantly positive correlations between $ORAC_{pca}$ and serum concentrations of TC(P < 0.05) and low density lipoprotein (LDL) cholesterol(P < 0.01). The positive relation between cholesterol concentrations and $ORAC_{pca}$ in serum may suggest an elevated TAC against oxidative stress associated with the cardiovascular risk factors. The Effect of Bifidobacteria and Various Oligosaccharides Consumption on the Risk of Colon Cancer in Rats

  • This study examined the effect of viable bifidobacteria and non-digestible carbohydrates on the cecal pH, colonic neoplastic lesion (aberrant crypt) and proliferating cell nuclear antigen (PCNA) labeling index in carcinogen-treated mts. Animals received s.c. injection of dimethylhydrazine (DMH) (15 mg/kg body weight) twice 3 days apart. Three days after the second carcinogen administration, the treatments were begun. 1he treatments were basal diet (AIN-76) with skim milk (Basal/skim), or the following diets with daily gavage of $10^8$ bifidobacteria: basal (Basal/bifido), $2\%$ fructo-oligosaccharide (FOS/bifido), $2\%$ soybean oligosaccharide (SBO/bifido), $2\%$ wheat bran oligosaccharide (WBO/bifido) and $8.4\%$ wheat bran (WB/bifido). After 4 weeks of treatment, cecal pH was measured using a pH probe. The number of aberrant crypt (AC), aberrant crypt foci (ACF) and crypt multiplicity were enumerated and colonic PCNA labeling index was determined using immunohistochemistry. Cecal pH was significantly reduced in SBO/bifido and FOS/bifido groups compared to control group. However, there were no significant differences in either number of AC or rates of cell proliferation as shown by PCNA labeling index among the groups, although mts fed FOS/bifido reduced the numbers of ACF compared to Basal/skim group. The SBO/bifido group did not reduce the number of ACF or PCNA labeling index. Also, other oligosaccharides did not reduce the risk of colon cancer compared to control group. The concomitant reduction of cecal pH and number of ACF suggest that the combination of bifidobacteria and FOS may reduce the risk of colon cancer.

The association of blood pressure with body mass index and waist circumference in normal weight and overweight adolescents

  • Purpose: Overweight can be defined by the body mass index (BMI) and is likely associated with an increased cardiovascular disease risk. However, waist circumference (WC), a central adiposity index, may be a better indicator of cardiovascular disease risk. Studies comparing the effects of BMI and WC on cardiovascular risk factors, such as high blood pressure (BP), are rare in adolescents. Methods: We analyzed the correlations of BMI and WC with BP in 3,363 Korean adolescents (aged 10-19 years), using data from the Korean National Health and Nutrition Examination Surveys (2009-2011). Results: Systolic BP (SBP) in both sexes and diastolic BP (DBP) in boys were higher in the high BMI (>85th percentile) and high WC (>90th percentile) groups. High BMI and high WC were positively correlated with high SBP (>90th percentile) in both sexes, and high DBP (>90th percentile), in boys. BMI maintained its positive associations with SBP, DBP, high SBP, and high DBP in the normal weight ($BMI{\leq}85th$ percentile) and overweight (BMI>85th percentile) groups in both boys and girls, as well as in all subjects of both sexes, even after adjustment for WC. However, WC did not correlate with SBP, DBP, high SBP, or high DBP after adjustment for BMI in any group in either sex. Conclusion: In Korean adolescents, BMI correlated better with BP and high BP levels than WC. Further, BMI was positively associated with BP and high BP in the normal weight group as well as in the overweight group.

Apolipoprotein E 다형성과 고지혈증 위험 유무에 따른 혈중 지질농도, 영양소 섭취, 생활습관 및 위험요인과의 관계 (Apolipoprotein E Phenotypes and the Relationship Among Lipid Levels, Nutrient Intakes, Lifestyles and Risk Factors Between Subjects with and without Hyperlipidemic Risk)

  • This study was performed to investigate Apolipoprotein E phenotypes and the relationship among lipid levels, nutrient intakes, lifestyles and risk factors between subjects with and without hyperlipidemic risk. The data were collected from 675 industrial male workers who had completed annual medical examination. Compared to the normal group, the hyperlipidemic risk group in Apo E3 and E4 had significantly higher BMI (p < 0.05) and showed significantly higher body fat (%), waist circumference and WHR in all types of Apo E (p < 0.05). In addition, the hyperlipidemic risk group had significantly higher total cholesterol, LDL-cholesterol, triglyceride and AI than the normal group in all types of Apo E (p < 0.05). Intakes of protein, calcium, phosphorus, iron, vitamin A, vitamin B1, vitamin B2, vitamin C and niacin in Apo E3 were significantly lower in the hyperlipidemic risk group than in the normal group (p < 0.05). In the logistic regression analysis, after adjustment for other factors, Apo E2 + E4, waist and WHR were the significant risk factors associated with hyperlipidemia, but protein intakes were associated with significantly lower risks of hyperlipidemia (p < 0.05). In conclusion, genetic factor (Apo E2 or Apo E4), anthropometric index and nutrient intake seem to influence hyperlidemic risk. Further studies and efforts will be needed to evaluate the independent relationships among hyperlipidemic risk factors.

Effects of calcium intake, milk and dairy product intake, and blood vitamin D level on osteoporosis risk in Korean adults: analysis of the 2008 and 2009 Korea National Health and Nutrition Examination Survey

  • This study was performed to determine the effects of dietary calcium (Ca) intake, milk and dairy product intake, and serum vitamin D level on bone mineral density. The survey data from the 2008-2009 Korea National Health and Nutrition Examination Survey (KNHANES) for adults (3,819 males, 5,625 females) aged > 20 years were examined; osteoporosis was defined according to the standards for Asian populations (T-score < -2.5). The risk for osteoporosis significantly decreased as Ca intake increased; this effect persisted (quartile 4 vs. quartile 1 of Ca intake: odds ratio [OR] 0.66; 95% confidence interval [CI]: 0.50-0.87) even after adjustment for gender, age, and other factors (body mass index, serum vitamin D, menstruation, female hormone intake, menopausal status, and the number of days per week of muscular strength exercise). Additionally, the risk for osteoporosis significantly decreased as the Ca/P ratio increased (quartile 4 vs. quartile 1: OR 0.76; 95% CI: 0.58-0.98). The degree of risk was 0.96 (0.66-1.38) in those who consumed < 1 portion of milk or dairy products daily, and 0.71 (0.53-0.96) in those who consumed > 1 portion per day, compared with those who had zero intake. The risk for osteoporosis significantly decreased as the serum 25(OH) vitamin D level increased. From these results, we advocate an increase in Ca, milk, and dairy product intake, and that serum 25(OH) vitamin D levels be maintained within the normal range, for the maintenance of bone health and the prevention of osteoporosis in adults.

성인의 만성질환관련 탄수화물 식사지침 연구 (Carbohydrate Intake Associated with Risk Factors of Coronary Heart Disease in the Adults: NHANES III)

  • Recent research reported health risks associate with high carbohydrates diets. Objectives of this study were to evaluate in a cross-sectional study if high carbohydrate diet is associated with coronary heart disease(CHD) risk factors: examined blood concnetration of triglyceride(TG), total cholesterol, high density lipoprotein cholesterol(HDL-C), plasma glucose, systolic blood pressure(BP), body mas index(BMI), wasit-hip ratio(WHR) and waist-stature ratio(WSR). Using the most recent US National Health and Nutrition Examination Survey(IIINHANES III) data, the nationally representative US population (3772 men, 4095 women of 25-64 years of age) was divided into low vs. high carbohydrate diet groups(below 40% vs. above 60% energy intake from cab carbohydrates) and compared by the CHD risk factors. Triglyceride was higher(p<0.001) in the high carbohydrate group, whereas high density-lipoprotein cholesterol(HDL-C)was lower(p<0.01) in the high carbohydrate diet group. In plasma glucose, there was no significant differences between high carbohydrate diet and low carbohydrate diet. In adiposity(BMI, WHR and WSR), it also showed no significant differences, After adjustment for age, ethnicity, alcohol and smoking in upper 60%-carbohydrate diet, Odds Ratio of TG and HDL-C were 1.42 and 1.23 in men and 1.22 and 1.17 in women. 50-60% carbohydrate diet was associated with decreased risk of CHD. Dietary guidelines for Koreans recommend 60-70% of total energy from carbohydrate, as Koreans traditionally consumed high carbohydrate diets. In a cross-sectional population of adults, diets containing 55-60% energy from carbohydrate were suggested as a dietary guideline of carbohydrate intake for Koreans.
