서울 지역 일부 중학생의 식이 패턴에 따른 식생활 평가 (Dietary Assessment Using Dietary Pattern Analysis of Middle School Students in Seoul)

  • The objectives of this study were to identify dietary patterns of Korean middle school students and to investigate the characteristics of dietary intake of subjects with different dietary pattern. Three-day diet records were obtained from 163 male and 155 female 7th graders in Seoul, Korea. Food items from the diet records were aggregated into 22 food groups before subjected to factor analysis. Four dietary patterns emerged from factor analysis with different factor score. Cluster analysis using factor score classified subjects into three groups named 'Traditional' (n = 42), 'Westernized' (n = 135), and 'Intermediate' (n = 145). Major nutrient intake and dietary quality assessed by NAR (Nutrients Adequacy Ratio), MAR (Mean Adequacy Ratio), DDS (Dietary Diversity Score), and DVS (Dietary Variety Score) of the three groups were compared. Mean energy intakes of three groups were 1783, 1916, 1578 kcal in Traditional, Westernized, and Intermediate diet group respectively. Differences in nutrient intake of the groups were significant in all nutrients except vitamin B$_1$. Percent energy from fat was significantly higher in Westernized and Intermediate diet group, and cholesterol intake of Westernized diet group was higher than 300mg. NARs of most nutrients were higher in Traditional and Westernized than Intermediate diet group except vitamin E. Traditional and Westernized diet groups had the highest MAR of fourteen nutrients. DDS was the highest in Westernized and DVS was the highest in Traditional and Westernized diet group. Traditional diet groups had 22% of energy consumption from breakfast, significantly higher than other diet groups. In conclusion, these results suggest that Korean teenagers with Traditional diet pattern have lower diet in % energy from fat, diversity of food and regularity of meals. Future studies need to focus on the relationship between dietary patterns and health status of Korean teenagers.

농촌지역 노인의 고혈압 유병율과 영양위험요인 (The Prevalence of Hypertension and Related Nutritional Risk Factors of Elderly Living in a Rural Area)

  • Objectives: The prevalence of hypertension in Korean rural elderly was significantly higher than that of the general population. Determining the potential risk factors of hypertension would be useful for managing and improving the treatment and prevention of hypertension in rural areas. Methods: We studied 336 elderly individuals 110 males, 226 females) aged between 65 years and 95 years residing in the rural area, S-gun Jeonbuk. Health-related habits, frequency of intake of food groups, nutrient intakes, anthropometric and biochemical measurements were assessed. Subjects were defined as hypertensive if SBP was ${\geq}140mmHg$ or if DBP was ${\geq}90mmHg$ or take an antihypertensive drug. Results: The rate of prevalence of hypertension in the study group was 51.8% (male 40.0%, female 57.5%). The risk of occurrence of hypertension was higher among females (OR, 1.98), 75 years old or older (OR, 1.62), BMI ${\geq}25kg/m^2$ (OR, 2.84), acceptable range (upper end) of body fat (OR, 2.29) and unhealthy (too high) range of body fat (OR, 3.28), hypertriglyceridemia (OR, 2.17) and hypercholesterolemia (OR, 5.42), low protein intakes (OR, 1.78). However, health related habits, frequencies of intake of food groups and most nutrient intakes except for protein did not show any significant relationship with the occurrence of hypertension. Conclusions: To reduce the risk of occurrence of hypertension among elderly individuals in rural areas, it is needed to avoid increase of body fat, 25 or higher BMI ($kg/m^2$) and hyperlipidemia and low intake of proteins.

Implementation and Evaluaion of Nutrition Education Program for Elementary School Children

  • This study was designed to develop, implement and evaluate a nutrition education program for elementary school children. Subjects consisted of 47 children in the 6th grade in Seoul, Korea. They received five sessions of nutrition education from June to July, 1999. Topics for nutrition education included function of nutrients, food sources of nutrients, balanced diet, meal planning, and adequate energy intake. Each session lasted 40 minutes. To evaluate program effectiveness, one group pretest-posttest design was used. Subjects were given a pretest and a posttest on nutrition knowledge, eating attitudes, eating behavior, and dietary intake. All data were statistically analyzed mainly using the paired t-test and $\chi$$^2$-test. There was a significant difference between pretest and posttest score on nutrition knowledge(p<0.001), subscales of nutrition knowledge including food sources of nutrients, function of nutrients, and balanced diet(p<0.001). These changes in nutrition knowledge were also shown both in boys(p<0.001) and in girls(p<0.001). Eating attitudes, subscales of eating attitudes, and eating behavior were not significantly changed after nutrition education, although some changes were observed in these variables. Dietary intakes were not significantly were not significantly changed after nutrition education for all subjects ; however, different patterns in dietary intakes were observed when examined by sex. There was an increase in nutrient intakes among boys, while there was a significant decrease in nutrient intakes among girls after program implementation. Especially, it was noted that girls had diets deficient in iron, calcium, and vitamin A, even after nutrition education. It was concluded that a five-week nutrition education program can improve nutrition knowledge or some dietary intake, but this might not be enough to induce changes in food attitudes, eating behavior or dietary intakes. It might be necessary to incorporate diverse approaches, targeting sub-groups of children, and follow-up in nutrition education for elementary school children. Sarcopenic obesity can be negatively associated with active physical activity and adequate intake of some nutrients in Korean elderly: Findings from the Korea National Health and Nutrition Examination Survey (2008-2011)

  • BACKGROUND/OBJECTIVES: The aim of this study was to analyze the factors related to sarcopenic obesity among the elderly in South Korea. SUBJECTS/METHODS: A total of 3,367 elderly (${\geq}65years$) from the Korea National Health and Nutrition Examination Survey (2008-2011) were included in this analysis. The subjects were assessed to determine their sarcopenia and obesity status. Sarcopenia was assessed by determining their appendicular skeletal muscle mass (ASM). Obesity was defined by the waist circumference. An association of sarcopenic obesity and the related factors was analyzed using multiple logistic regression models. RESULTS: The risk of sarcopenic obesity of the subjects was decreased by active physical activity. After adjusting for age, smoking, and alcohol consumption, the risk of sarcopenic obesity in men of the highest level group (Q4) decreased by 45% (OR = 0.550, 95% CI = 0.334-0.905, P trend 0.018) compared to that in the reference group (Q1). Among the women, the risk of sarcopenic obesity in the Q3 and Q4 groups decreased by 29.0% (OR = 0.710, 95% CI = 0.512-0,984) and 56.7% (OR = 0.433, 95% CI = 0.281-0.668), respectively, compared to that in the Q1 group (P trend < 0.001). The mean daily energy intake was higher in the non-sarcopenia group than in the sarcopenia group. The risk of sarcopenic obesity in subjects not meeting the recommended intakes of energy, riboflavin, and vitamin C increased significantly by 25.4%, and 36.6%, and 32.6%, respectively, compared to that in the subjects meeting the recommended nutrient intake. CONCLUSION: Active physical activity as well as an adequate intake of energy and some vitamins might be negatively associated with the development of sarcopenia and sarcopenic obesity in the elderly.

Micronutrients as Supportive Care for Gastrointestinal Cancer Patients; Benefits and Concerns

  • Cancer is the first leading cause of death in Korea and the second leading cause of death in the USA. There is extensive research into prevention of cancer and the support of oncology patients with diet or dietary supplements. In vitro and in vivo animal studies have indicated that antioxidants, including beta-carotene, alpha-tocopherol, and ascorbic acid, can yield anti-cancer effects in addition to providing protection against oxidative damage. Although many observational studies have shown that consuming fruits and vegetables can reduce the risk of some cancers, the results of several large-scale human intervention trials testing the benefits of a single or combined higher-dose of individual micronutrients have been inconsistent. Cancer can cause profound metabolic and physiological changes which may affect patients' nutrient requirements. Although the optimal route of nutrient delivery is through diet, cancer patients often suffer symptoms that disrupt their food intake, including anorexia, premature satiety, altered taste and smell, and changes in bowel mobility. In particular, micronutrient deficits can slow postoperative healing, contribute to depression symptoms, and decrease immune competence. Cancer patients are generally motivated to take dietary supplements to improve responses to treatment and quality of life. The Physician's Health Study II (PHS II) randomized controlled trial reported recently that daily multivitamin supplementation significantly, albeit modestly, reduced the risk of total cancer. Although evidence of multivitamin use benefits is limited in cancer patients, taking dietary supplements with constituents in the range of the recommended daily allowance according to the Dietary Reference Intake (DRI) recommendation is generally considered to be safe.

2008 한국 성인을 위한 식생활목표와 식생활지침 (2008 Dietary Goals and Dietary Guidelines for Korean Adults)

  • Recently the Ministry of Health and Welfare, Republic of Korea, announced the “Dietary Guidelines for Korean Adults (DGKA)”, which includes ten Dietary Goals, six Dietary Guidelines, and twenty-three Action Guidelines. DGKA are developed as the revision of the 2003 Dietary Guidelines for Koreans, targeting adult population. Dietary Guidelines are developed for general purpose as well as for different age groups. They are revised periodically to accommodate changes in diet and health problems of the population. The process of developing new DGKA can be summarized as 1) selection of focus areas, 2) analysis and review of available data for each area selected, and 3) derivation of guidelines based on the analyzed data, and 4) finalizing the guidelines after open discussions among the experts and general public. Five focus areas were selected by examining the Nutrition Goals of the Health Plan 2010 of Korea, soliciting proposals from the experts in the related fields, and reviewing existing and international guidelines. Five areas selected were 1) adequate intake of nutrients and foods, 2) balance of energy intake and physical activities, 3) alcohol intake, 4) food security and nutrition service, and 5) food safety. Adequacy of nutrient and food intakes of the Korean adult population was assessed using 2005 Korea National Health and Nutrition Examination Survey (KNHANES) data. Newly developed Dietary Reference Intakes for Koreans were used as reference values to assess the prevalence of inadequacies and excesses in nutrient intakes. Energy balance was examined with energy intake of 2005 KNHANES survey and results of physical activity questionnaire in the survey. Alcohol intake was also examined using 2005 KNHANES results of dietary intakes as well as the results of questionnaire survey on alcohol intakes. Food security, nutrition services, and food safety were analyzed using various government data and published results on the issues. Ten Dietary Goals and six Dietary Guidelines were developed after data analysis and were subjected to reviews of experts and general public. The final DGKA are: 1) Eat a variety of foods from each food group, 2) Increase physical activity and maintain healthy weight, 3) Eat proper amount of clean foods, 4) Avoid salty foods and try to eat foods with bland taste, 5) Avoid foods with high fat contents and deep-fried foods, and 6) When you drink alcohol, limit the amount. Twenty-three action guidelines are developed in order to achieve these guidelines in actual diet and life among the population. The government is disseminating the guidelines with “337” slogan and emblem. “337” indicates everyone should practice “3” guidelines of promoting good eating practice, “3” guidelines to limit or decrease in your diet, and you should practice them for “7” days a week. The guidelines will be useful in promoting healthy food habits and good nutritional status which will result in decrease nutrition related health problems in Korea.

남녀 청소년에서 연령별 영양소 섭취 및 식사의 다양성 평가: 2010~2012 국민건강영양조사 자료를 이용하여 (Evaluation of nutrient intake and food variety by age in Korean adolescents: Based on 2010~2012 Korean National Health and Nutrition Examination Survey)

  • 본 연구에서 우리나라 국가단위 대규모 데이터를 사용하여 청소년을 대상으로 성별 연령군에 따른 식사 섭취의 다양성에 대하여 분석한 결과는 다음과 같다. 1. 식습관 조사 결과 12~14세군의 경우 아침결식자의 비율이 13.54%로 15~18세군의 23.16%에 비해 유의적으로 낮은 비율을 보였으며 (p < 0.0001), 이는 남학생과 여학생에서 모두 같은 결과를 보였다. 또한 영양교육 수혜 여부를 조사한 결과, 12~14세군이 18.07%로 15~18세군의 11.49%에 비해 유의적으로 높게 나타났으며 (p = 0.0020), 남학생과 여학생에서 모두 같은 양상을 보였다. 2. 영양 밀도 분석 결과 15~18세군은 12~14세군에 비해 지방의 섭취 밀도가 높은 반면 (p = 0.0069), 탄수화물의 섭취 밀도는 유의적으로 낮게 나타났다 (p = 0.0063). 또한 영양소별 밀도는 성별에 따라 상이한 결과를 보여 여학생의 경우 15~18세군이 12~14세군에 비해 지방의 섭취 밀도가 유의적으로 높게 나타났지만 (p = 0.0494), 남학생의 경우 15~18세군의 비타민 B1 (p = 0.0324), 나이아신 (p = 0.0227) 및 나트륨 (p = 0.0344)의 섭취 밀도가 12~14세군에 비해 유의적으로 높게 나타났다. 3. 식품군별 섭취량 분석 결과 15~18세군이 12~14세군에 비해 채소류 (p = 0.0386), 육류 (p = 0.0099), 유지류 (p = 0.0093), 음료류 (p < 0.0001) 및 양념류 (p = 0.0056)에서 유의적으로 높은 섭취량을 보인 반면, 우유 및 유제품류 (p = 0.0084)의 섭취에서는 15~18세군이 12~14세군에 비해 유의적으로 낮게 나타났다. 또한 음료류의 경우 남학생 (p < 0.0001)과 여학생 (p < 0.0001)에서 모두 15~18세군이 12~14세군에 비해 유의적으로 높은 섭취량을 보였다. 4. 섭취한 식품 가짓수를 분석한 결과 12~14세군과 15~18세군의 1일 섭취한 식품 가짓수는 각각 33.00개와 32.76개로 두군간 유의한 차이는 보이지 않았다. 그러나 음료류 (p < 0.0001)의 경우 15~18세군이 12~14세군에 비해 유의적으로 높은 섭취 가짓수를 보인 반면, 우유 및 유제품류 (p = 0.0253)에서는 15~18세군이 12~14세군에 비해 유의적으로 낮은 섭취 가짓수를 가지는 것으로 나타났다. 또한 음료류의 경우 남학생 (p < 0.0001)과 여학생 (p < 0.0001)에서 모두 15~18세군이 12~14세군에 비해 유의적으로 높은 섭취 가짓수를 보였다. 5. 식품군점수를 분석한 결과, 12~14세군과 15~18세군의 1일 섭취한 식품군점수는 각각 4.44점과 4.33점으로 두 군간 유의한 차이는 보이지 않았다. 그러나 12~14세 남학생에서 식품군점수 = 6인 대상자의 비율이 16.97%로 15~18세 남학생의 11.45%에 비해 유의적으로 높게 나타났다 (p = 0.0404). 이와 같은 결과를 통하여 15~18세 청소년의 경우 12~14세 청소년에 비하여 식품군 섭취의 측면에서 식사의 다양성 저하의 우려를 가지고 있음을 제안할 수 있으며, 본 연구는 향후 청소년들을 대상으로 다양한 식품 섭취의 중요성 및 섭취 방법 등에 대한 세분화된 영양교육시 기초자료가 될 수 있을 것으로 사료된다.

일부 초등학생의 아토피피부염 유무에 따른 식습관, 식품섭취양상 및 영양소섭취상태 비교 연구 (Comparative Study on Eating Habits, Dietary Intake Patterns, and Nutrient Intakes Between Elementary School Children With and Without Atopic Dermatitis)

  • 본 연구는 아토피피부염을 가지고 있는 아동과 정상 아동의 식습관, 식품섭취양상 및 영양소섭취상태 비교를 위해 서울 소재 1개교와 울산의 2개 초등학교에 재학 중인 5, 6학년 388명을 대상으로 설문지를 이용하여 실시되었다. 조사는 2008년 7월 7일부터 7월 18일까지 총 2주간 실시하였으며, 조사대상자 중 아토피피부염군은 16.7%(65명), 정상 아동군은 83.3%(323명)로 나타났다. 아토피피부염 아동군은 남학생 29명, 여학생 36명으로 남아보다는 여아에게서 더 호발 되었으며 대부분 7-13세에 아토피피부염이 유발된 것으로 나타났다. 아토피피부염 아동군 중 병원치료를 받고 있는 경우는 20.0%였고 아토피피부염 유발에 영향을 주는 식품인 튀김, 패스트푸드, 우유 등의 섭취를 제한하고 있었다. 식습관의 경우, 아토피피부염 아동군이 정상 아동군에 비해 식습관이 불규칙적인 것으로 나타났으며(p<0.05), 아토피피부염의 유발과 연관 있는 식품 중 우유, 돼지고기, 게, 메밀, 닭고기, 고등어, 복숭아, 토마토의 섭취량이 정상 아동군에 비해 아토피피부염 아동군에서 높게 나타났다. 또한, 아토피피부염 아동군이 열량(p<0.001), 총 단백질(p<0.05), 식물성 단백질(p<0.001), 당질(p<0.001), 식이섬유소(p< 0.01), 칼륨(p<0.05), 아연(p<0.001), 비타민 B1(p<0.001), 나이아신(p<0.01), 비타민 C(p<0.05) 및 엽산(p<0.01) 등의 섭취량이 정상 아동군에 비해 유의적으로 적었다. 본 연구결과 아토피피부염 아동은 정상아동에 비해 식습관이 불규칙적이고, 아토피피부염 유발식품의 섭취량은 많았으며, 대부분의 영양소 섭취량이 정상아동에 비해 적었다. 이러한 결과는 식품과 연관성이 높은 소아 아토피피부염에 부정적인 영향을 줄 수 있을 것으로 보이며, 아토피피부염 아동에 대해 적절한 식품의 선택과 균형 잡힌 식생활 및 식습관 교정의 필요성이 매우 절실한 것으로 판단된다. 따라서 아토피피부염을 악화시키는 원인식품의 섭취를 제한하는 것과 더불어 올바른 영양교육 및 지도가 이루어져야할 것으로 생각된다.
