Quantitative and Qualitative Assessment of Dietary Intake in Metabolic Syndrome Patients (대사증후군 환자의 양적 및 질적 식사섭취상태 평가)

  • The purpose of this study was to compare the quantitative and qualitative assessment of dietary intake between patients with metabolic syndrome (MetS) and healthy subjects and to investigate dietary factors related to MetS. Anthropometric measurements, blood analysis, and dietary intake as assessed by 24-hour recall were conducted in MetS patients (n=15) and healthy subjects (n=25). In order to assess the quantity and quality of dietary intake, daily nutrient intake, nutrient density, nutrient intake to dietary reference intake (DRI), nutrient adequacy ratio (NAR), food intake, dietary diversity score (DDS), and dietary variety score (DVS) were analyzed. The statistical differences between MetS patients and controls were analyzed using the SAS software program. Daily energy intake and food intake were not significantly different between the two groups (2,154.3 kcal vs. 1,872.9 kcal; 1,280.0 g vs. 1,261.6 g). There were also no significant differences in daily nutrient intake, nutrient intake ratio to DRI, NAR, or DVS between the MetS group and the control group. However, daily intake of eggs and milk in MetS patients was significantly lower than in the control group (9.0 g/day vs. 30.3 g/day, p<0.05; 0 g/day vs. 49.7 g/day, p<0.05). These results indicate that low intake of eggs and dairy products may be related to the development of MetS.

Energy Value of Breakfast and Its Relation to Total Daily Nutrient Intake and Serum Lipid in Korean Urban Adults (도시성인 아침식사의 열량기 및 이의 영양소 섭취와 혈청 지질 수준간의 관련성 연구)

  • The purpose of this study was to assess the energy value of breakfast and its relation to total daily nutrient intake and serum lipid. Dietary intakes were evaluated through the 3-day dietary recalls(interview for 1day and self-report for 2 days) from 333 Korean urban adults aged 20-49 year. Serum lipids in fasting blood samples were measured form 98 of those 333 adults. Low energy breakfast(<15% of daily energy intake) was consumed by 22.6% of males and 18.5% of females. 59.4 of males and 43.3% of females consumed a breakfast of average energy intake(15-25% daily energy intake) ; and 18.1 of males and 38.2% of females consumed a significant contribution to a total daily nutrient intake. The daily nutrient intake except crude fiber and vitamin A, B, B, and C in males and except vitamin C in females significantly increased as the energy value of breakfast increased. The differences in energy and nutrient intakes at breakfast were not made up for by other meals. To make matters worse, the satisfactory-energy breakfast group took more energy at dinner compared with the low-energy and average-energy breakfast group took more energy at dinner compared with the low-energy and average-energy breakfast groups in females. Among serum lipid parameters, TG was negatively correlated with energy provided at breakfast, and total cholesterol and LDL-cholesterol had a negative correlation with energy and carbohydrates provided at breakfast in males. For females TG was positively correlated with the ratio of carbohydrates to energy at breakfast but negatively correlated with the ratio of fat to energy at breakfast. These results suggest that satisfactory energy intakes at breakfast have positive effects on the adequacy of daily nutrient intake and may positively affect the serum lipid status.

A Comparison between Postmenopausal Osteoporotic Women and Normal Women of Their Nutrient Intakes and the Evaluation of Diet Quality (폐경 후 골다공증 여성과 정상 여성의 영양소 섭취상태와 식사의 질 평가에 관한 비교 연구)

  • The purpose of this study was to compare the nutrient intake and diet quality of postmenopausal osteoporotic women to those of control subjects, and to investigate the relationship among diet quality and bone mineral density in postmenopausal women. In this study, we classified the subjects into the postmenopausal osteoporotic women (n =38) and control (n = 43) according to their lumbar spine bone mineral density and age. Dietary intakes, anthropometric measurements and dietary quality indices were measured and evaluated. The average age of osteoporotic and control group were 60.4 yrs and 58.3 yrs, respectively and there was no significant difference. Body weight, body mass index of osteoporotic group were significantly lower than those of control group. The average energy intake of osteoporotic and control group were 1243.3 kcal and 1475.8 kcal, respectively and there was a significant difference. The osteoporotic group consumed significantly lower quantities of protein, plant protein, vitamin A, vitamin $B_1,\;vitamin\;B_2$, folate, vitamin C, calcium, animal calcium, plant calcium, iron and zinc compared to the control group. The osteoporotic group consumed significantly lower of food, vegetables, mushrooms, fruits intakes compared to the control group. In the diet quality, protein, vitamin A, vitamin $B_1,\;vitamin\;B_2$, folate, vitamin C, calcium, iron, Zinc nutrient adequacy ratio (NAR) of osteoporotic group were significantly lower than that of control group. Mean adequacy ratio (MAR) of osteoporotic and control group were 0.63 and 0.78, respectively and there was significant difference. To evaluate nutrient density, Index of nutritional quality (INQ) was calculated by dividing nutrient content per 1,000 kcal of diet with RDA per 1,000 kcal. The average dietary variety score (DVS) of osteoporotic and control group were 22.4 and 33.2, respectively and there was significant difference. DVSs of pulses (p<0.01), seeds (p<0.01) and vegetables (p<0.05) in osteoporotic group were significantly lower than those of the control. In conclusion, postmenopausal osteoporotic women had lower protein, vitamin A, folate, vitamin C, calcium, iron zinc intake quality and vegetables, mushrooms, fruits DVSs than those of the control. Therefore, to promote skeletal health enough energy and food should be consumed, and the maintenance of vitamin and mineral balance by increasing of vitamin A, folate, vitamin C, calcium, iron of intakes are very important.

Evaluation of nutrient and food intake status, and dietary quality according to abdominal obesity based on waist circumference in Korean adults: Based on 2010-2012 Korean National Health and Nutrition Examination Survey (한국인 성인 남녀에서 허리 둘레 기준 복부비만에 따른 영양섭취상태 평가: 2010-2012 국민건강영양조사 자료를 이용하여)

  • Purpose: This study was conducted in order to investigate the nutrient and food intake status, and dietary quality in Korean adults according to abdominal obesity based on waist circumference. Methods: We analyzed data from the combined 2010~2012 KNHANES (Korean National Health and Nutrition Examination Survey). The analysis included 6,974 adults aged 40 to 64 years. In this study, according to abdominal obesity based on waist circumference (male ${\geq}$ 90 cm, female ${\geq}$ 85 cm), we classified the subjects into the obesity group (male, n = 775, female, n = 1,113) and control group (male, n = 2,038, female, n = 3,048). The nutrient and food group intake, ND (nutrient density), NAR (nutrient adequacy ratio), MAR (mean adequacy ratio), INQ (index of nutritional quality), DDS (dietary diversity score), and DVS (dietary variety score) were analyzed using data from the 24-recall method. Results: For male, no significant difference in quality index of the diet was observed between the obesity group and the normal group. In female, in diet quality (ND, NAR, and INQ), vitamin $B_2$ (ND, NAR, and INQ) calcium (NAR), phosphorous (ND, INQ) and potassium (ND) of the obesity group was significantly lower than those of the control group. DDS and DVS in the obesity group (3.57, 30.95) were significantly lower than those of the control group (3.68, 32.84) (p = 0.0043, 0.0002). DVS (DVS ${\geq}$ 39.9) showed association with lower risk of waist obesity in a logistic regression model after adjustments for multiple confounding factors including age, education, income, alcohol intake frequency, smoking, physical activity, energy intake, and body mass index (OR: 0.616, 95% CI: 0.420-0.903). Conclusion: In conclusion, females with abdominal obesity had lower micronutrient intake quality, DVS than those of the control group. In Korean females, food intake variety can adversely affect waist circumference.

Dietary Quality and Foodservice Menu Pattern of American International Schools in Seoul and Gyeonggi Area (서울.경기지역 외국인학교 급식 메뉴패턴과 식사의 질 평가)

  • This study was conducted to identify the combination of foods and foodservice menu patterns, food supply rankings and dietary quality through the a collection of school menus from the fifth week of October 2009, up to the fourth week of November 2009, in three American international schools in Korea. The results of analysis were as follows. For the basic pattern of American international school foodservice menu, dietary patterns comprised of rice, soup, five side dishes, Kimchi and snacks were 23.19%, while menu pattern comprised of rice, soup, four side dishes, Kimchi and snacks were 18.84%. The two dietary patterns accounted for 42.03%. For the foodservice frequency and food rankings from No. 1 to No. 5, vegetables and seasoned vegetables were the highest, followed by cooked rice, Kimchi, soup, and stir-fried foods. For nutrients, the intake ratio of carbohydrates, protein and fat for the calories provided at the three schools was 57:18:24, 53:18:29, and 55:18:27. Compared with 55~70:7~20:15~25 according to by Korean nutrition standards, carbohydrates and protein were adequate, but fat was high. For the recommended nutrient intake (RNI), the provided nutrient ratio was higher than the RI of nutrition standards in schools. For the average nutrient adequacy ratio (NAR), all three schools were above 1. School C had an above-average ratio for vitamin B-complex and vitamin A. Phosphorus showed a significant difference in the nutrient density. For DDS, school C provided all five main food groups every day.

Evaluation of the Validity of a Simple Screening Test Developed for Identifying Korean Elderly at Risk of Undernutrition (우리 나라 노인의 영양부족위험 진단을 위해 개발된 간이조사표의 타당성 평가)

  • This study intended to evaluate the validity of the simple nutrition screening test that had been developed with the elderly living in Cheongju as a subject. Nutrition screening score(NSS) and reference standards for nutritional and health status(nutrient intakes, mean adequacy ratio, perceive health, and serum albumin, hematocrit, and hemoglobin) were estimated by using the date obtained in 1996 from the 174 elderly living in Taejon, Statistical analysis showed significant correlations between mean adequacy ratio(MAR) and NSS(r=0.341) and also between NSS and biological indices such as albumin and hematocrit, Around 65-75% of the elderly with perceive health and low level of serum albumin, hemoglobin and hematocrit had NSS$\leq$ll. Sensitivity, specificity, and positive predictive values(PPV) were calculated from the crosstabulation of the three categories of NSS(high, moderate, and low nutritional risk) and low categories MAR(< 0.75, undernutrition;$\geq$0.75, normal) to validate the cut-off point for high or low nutritional risk by NSS. It was suggested that point l1 was appropriate as a criterion to determine high risk of undernutrition, but point 16 was better than 17 as criterion to determine low nutritional risk in the Taejon elderly. When point ll was used as a criterion of high nutritional risk, sensitivity, specificity, and PPV are 59.5, 60.5 and 82.1 respectively. When point 16 was used as a criterion of low nutritional risk, sensitivity, specificity, and PPV are 25.6, 95.4, and 64.7%, respectively. In conclusion, nutrition screening test that had been developed can be a simple, easy, and proper instrument to classify the high risk group of undernutrition. A further validation study seems to be required among other groups of individuals for the screening test to the finalized as a more valid instrument identifying Korean elderly at nutrition and health risk(Korean J Nutrition 33(8) : 864-872, 2000)

Nutritional evaluation of new alternative types of dog foods including raw and cooked homemade-style diets

  • Background: New alternative types of pet foods such as raw and cooked homemadestyle diets containing human food ingredients have been introduced due to a trend of pet humanization and diversification of consumer needs. Objectives: To evaluate nutritional adequacy of new alternative types of dog foods containing human food ingredients as maintenance diets for dogs. Methods: Eleven homemade-style foods for adult dogs were purchased from online channel in Korea and analyzed to evaluate nutritional adequacy for adult dogs. Nutrients analyzed included crude protein, amino acids, crude fat, fatty acids, and minerals. Results: Crude protein and amino acids in all products satisfied Association of American Feed Control Officials (AAFCO) requirements. Crude fat in one of 11 products did not meet AAFCO requirements. The most deficient minerals were selenium (10 of 11, 90.9%), copper (five of 11, 45.5%), zinc (five of 11, 45.5%), potassium (three of 11, 27.3%), calcium (three of 11, 27.3%), iron (two of 11, 18.2%), and magnesium (one of 11, 9.1%). Six products were not in the range of the recommended Ca:P ratio in AAFCO dog food maintenance nutrient profiles. Conclusions: This study performed nutritional evaluation of raw and cooked homemadestyle foods as maintenance diets for adult dogs. Some nutritional inadequacies were observed including some minerals, Ca:P ratio, and omega-6:omega-3 fatty acid ratio, although three products (26.2%) satisfied the AAFCO standard except selenium. Overall, the data suggest a need for accurate nutritional adequacy statement for consumers based on proper methods to validate the formula.

The Assessment of Dietary Quality in Toddler of 12-36 Months (12~36개월 유아의 식사 질 평가)

  • This research was conducted to evaluate the general dietary quality(the balance and diversity of food intake, nutrient intake and adequacy) of 12 to 36-month-old toddler, through examining their actual food intake. It was conducted for 664(male-357, female-307) healthy toddler who were outpatients from Mar. to Aug., 1999 at university hospitals and private hospitals in Seoul and kyunggi province. The anthropometric parameters were measured. The 24-hr recall method were utilized to analyze the nutrient intake by months, based on the classification of The Korea Child Gorwth Standard. As for the subjects of this research, we found that the younger the age, the more frequent the pattern of intake of only 3 kinds of food groups. The results are that 1) the group with 1 point of the DDS(Dietary Diversity Score) mostly took only foods from the milk group, 2) the group with 2 points only took foods from the grain and milk group, 3) the group with 3 points did not take foods from the fruit and vegetable group, and 4) the group with 4 points took just about all kinds of food groups except for the vegetable group. This means that most of them were not taking various kinds of food even after the age of 12 months, and that they took more than twice the recommended amount of foods from the milk group until the age of 20 months. Thus, one could find a tendency of dependance on the milk group for food intake. Food intake for the vegetable group was severely under the recommended allowance, regardless of the age. The intake of iron was less than two thirds of the recommended allowance, except for the group of 12 to 14-month-olds who had been fed with infant formula. The Nutrient Adequacy Ratio(NAR) of major nutrients(protein, calcium, phosphorous, iron, vitamin A, vitamin B$_1$, vitamin B$_2$, niacin, vitamin C) and the Mean Adequacy Ratio(MAR, shows the general quality of dietary intake) was 0.86$\pm$0.12, but the NAR of iron was 0.62$\pm$0.27, which was extremely low compared to those of other nutrients. Therefore, concerning the subjects of this research, the MAR value alone does not provide enough ground to conclude that the subjects dietary quality is satisfactory. An Assessment of diet quality in the Postmenopausal Women (폐경 후 여성을 대상으로 한 식사의 질 평가 연구)

  • This study was performed to assess diet quality in the postmenopausal women. The data of dietary intakes were obtained using food frequency questionnaires which were collected from 151 postmenopausal women in urban area. We evaluated nutrient intake and diet quality. Diet quality was assessed by NAR(nutrient adequacy ratio), FGIP(food group intake pattern), DDS(dietary diversity score), DVS(dietary variety score), FGS(food group score), DQI(diet quality index). Nutrient intakes of the subjects were close to Korean RDA. Nutrient composition of the diet(in percentage of total energy) was 64% carbohydrate, 15% protein, and 19% protein, and 19% total fat, that was close to 65 : 15 : 20. NAR of most nutrients, except vitamin A, E, were higher than 0.75 and MAR was 0.88. NAR and MAR of the subjects in theis study were higher than those of the subjects of rural area in other studies. The subjects who consumed the five food groups were 60.9%. The mean of DDS was 4.5. The subjects who consumed more than 61 dish items during a month were 61.5%. The mean of DVS was 62.4 and it was significantly higher in the elder age group. 67.6% of the subjects were below FGS of 12. The mean of FGS was 10.6 and few subjects consumed serving numbers of each food groups which were recommended for Koreans. 55.0% of the subjects were DQI scores of 0, 1, and 2. The mean of DQI was 2.3 and few subjects followed the five dietary guidelines for Korean.

Repeatability of a Semi-quantitative Food Frequency Questionnaire or the Korean Elderly (한국노인을 위한 반정량적 식품섭취빈도조사지의 신뢰도 검증)

  • In order to evaluate the repeatability of nutrient values estimated from a semiquantitative food frequency questionnaire(SFFQ) for elderly Korean, a sample of 100 participants(45 men, 55 women) were interviewed to complete a SFFQ developed by this study twice at an approximately 3 month interval. The mean age of subjects was 70.4 years. Males(mein=8.75 years) had more education thu females(1.4 years). In males, Nutrient values calculated from the repeat questionnaire were not significantly different from the baseline results by paired t test. Correlation coefficients of most nutrients ranged from 0.4-0.6. For most nutrients,60-73% males classified into the lowest tertile in baseline data belonged to the same tertile in a repeat questionnaire, but these values appeared to be less than 50% for females. These results were comparable to previously published results and confirm that repeatability of nutrient intakes estimated using SFFQs was acceptable. The repeatability of SFFQ was higher for males than females, which implies that years of schooling should be considered in the development of SFFQ for the elderly. Only f3r males, Mean Adequacy Ratio(MAR) of nutrient intake was higher for those who were less nutritionally at risk and less depressed, and physically functioned better. Considering the educational level of the Korean elderly overall, more studies on the development of simpler dietary assessment methods f3r this population are needed.(Korean J Nutrition 31(7) : 1183-1191, 1998)
