대학병원 종사자의 감염성 폐기물에 대한 지식, 태도 및 실천 (A University Hospital Employee's Knowledge, Attitude Toward, and Practice of Hospital Infectious Wastes)

  • Purpose: This study was to provide baseline data to arrange systematic management for the infectious waste. Methods: This data was collected by self-reported questionnaires from a total of 419 subjects, ie nurses, nurse's aides and laboratory technicians working at a university hospital located in G city. The collected data were analyzed by t-test or ANOVA, Tukey test, Pearson's correlation, and multiple regression analysis using SPSS/WIN 12.0. Results: The knowledge of the infectious waste was statistically significant in type of staff and level of education. Attitude was statistically significant in type of staff, age group, level of education, working period, and marital status. And also the practice of the subject was statistically significant in type of staff, age group, education level, the working periods, and marital status. There was positive association between attitude and practice(r=.63, ${\rho}$< 001). By means of multiple stepwise regression analysis, total variance explained by the attitude towards infectious wastes, single employee, and the working periods less than ten years was 44% of the practice of infectious wastes. Conclusion: An educational program focusing on strategy to change employee's attitude can be effective for building a well-organized management system.

일 종합병원 병동 간호단위의 간호효율성 평가-자료포락분석의 적용 (Evaluation of the Efficiency of General Nursing Units using Data Envelopment Analysis (DEA))

  • Purpose: The aim of this study was to evaluate the efficiency of nursing units in a hospital using DEA. Methods: Data were collected using the medical information system of a general hospital in a city. Input variables were number of nurses, number of nurse-aides, number of beds, and overhead costs. Output variables included number of admitted patients, rate of bed utilization, satisfaction of discharged patients, and prevention rate of safety accidents and sores. EMS Window version 3.1 was used to measure the efficiency score and descriptive statistics were applied to analyze the general characteristics of variables. Results: The average efficiency score of 18 general nursing units was approximately .99. Nine nursing units had a 1.00 efficiency score. Conclusion: Our findings reveal that nursing units operated very efficiently. To increase efficiency of inefficient nursing units, we recommend results of the DEA slack analysis as a benchmark of the most efficient nursing unit.

  • The purpose of this study was to survey the knowledge level, attitude and practice of nurses toward their work. The subjects of the study were composed of 98 nurses from 3 general hospitals, 1 oriental medical hospital, 2 health centers and several community health posts and schools. Data were collected from May to October, 1998. In data analysis, an SPSS PC program was utilized for descriptions. 1) 16 nurses (16.3%) experienced medical accidents on the 7 nurses(7.1%) 1 time, 6 nurses (6.1%) 2 times, and 3 nurses(3.1%) 3 times. 2) Concerning knowledge of their legal obligations ; the prohibition of telling secrets was .89, the prohibition of reading medical records was .58, the keeping of medical records was 1.0 and the teaching of recuperation was. 79. The total mean score was. 86. Concerning attitude and practice; the prohibition of telling secrets was 81.6%, 63.3%. The prohibition of reading medical records was 61.2%, 60.2%. The keeping of medical records was 98%, 98%. The explanation for treatment, care and test was 91.8%, 66.3%. The teaching for recuperation was 63.3%, 63.3%. 3) Knowledge of their legal responsibilities; 29. 6% of the subjects thought that they should report a medical accident to their headnurse, but 75.5% of the subjects actually reported to the headnurse. 39.8% of the subjects thought that nurses were liable for the faults of nursing aides. The total mean score was .45. 46% of the subjects asked a senior staff's advide on difficult affairs. Nurses obeyed legal obligations when concern ing the protection of a client, but were passive when concerning self protection. Also, headnurses were required as adviser, guide and advocate.

Difficulties Faced by Working Mothers and Potential Solutions to these Problems: A Survey of Nursing Personnel in a Korean Teaching Hospital

  • Purpose: The purpose of this study was to identify difficulties that working mothers face and solutions to the identified problems. Methods: The study design was a cross-sectional, descriptive survey. A survey with 8 items rated on a 5-point scale (1~5) and 5 open-ended questions was administered to 48 married nurses and nurse aides working in 5 nursing units of a teaching hospital in South Korea in April 2013. Results: The score of satisfaction with childrearing patterns (mean=$3.31{\pm}0.79$) was higher than that of satisfaction with spousal support (mean=$3.08{\pm}0.85$). The score of working mother's turnover intention (mean=$2.40{\pm}1.03$) was lowest among the 8 items, but partners did not want their wives to quit work (mean=$3.60{\pm}1.22$). Satisfaction with company employment benefits for childrearing (mean=$2.90{\pm}0.72$) had the lowest score among the 4 satisfaction types analyzed. The turnover intention and satisfactions with childrearing (r=-.51, p<.001), spousal support (r=-.43, p=.002), supervisor's support (r=-.36, p=.013), and company benefits (r=-.37, p=.009) showed significant negative correlations. Conclusion: According to these results, familial support for childrearing is highly correlated with employed mothers' turnover intention. So improvement of familial support for childrearing will reduce married nurses' turnover intention. In addition, well-organized nursery facilities are recommended for enabling working mothers to continue their careers. Furthermore, more family-friendly welfare policies such as a flextime systems or compulsory paternity leave should be reinforced in the workplace.

한의원 환경 및 경영 현황 변화에 대한 연구;한의원 시설 및 인력, 환자, 매출액을 중심으로 (Status of changes in the business environment of Oriental medicine clinics;With the focus on their facilities, staff, patients and service fee revenue)

  • Objectives: The purpose of this survey is to monitor the changes that have occurred in the business environment surrounding Oriental medicine clinics, with the focus on their facilities, staff, patients, and service fee revenue. Method adopted: A questionnaire was sent in December 2006 to 1,000 Oriental medicine clinics nationwide, of which 122 (or 12 percent of them) replied. Previous questionnaires similar to this one in nature were checked for comparison. Results: As a result of analyzing the aforesaid replies, the average Oriental medicine clinic appears to have a floor size of 156 square meters (= 47.2 pyeong) and is equipped with 6.6 beds. The number of helpers and nurse's aids at each Oriental medicine clinics comes to 3.2 and 1.58, respectively. The number of patients coming to see a practitioner of Oriental medicine stands at 36.3 persons per day, of whom 32.06 come for acupuncture treatment. 50.2 percent of the Oriental medicine clinics' service fee revenue is paid from the health insurance. Each clinic spends on average 2.42 million won per month on the purchase of medicinal substances and so forth. The foregoing indicates a 27 percent increase from 1999 in terms of floor size, a 30 percent increase in the number of beds, a 47 percent increase in the number of helpers, a 45 percent increase in the number of nurse's aides, and an 11 percent increase in the number of patients who visit a practitioner of oriental medicine. As for the latter figure, there was an increase of 3.64 over a seven-year period. The number of patients coming for acupuncture treatment increased by 7.06 in the same period, whereas the number of those coming for medication treatment decreased by 4.28 percent. Health insurance payments as a proportion of Oriental medicine clinics' service fee revenues increased by 23.9 percentage points from 26.29 percent in 1997 to 50.2 percent in 2006. The amount that a clinic spends on the purchase of medicinal substances, etc, decreased by 250,000 won or by 9.3 percent from 1999. The estimated value of the domestic Oriental medical service market for 2006 stood at 2,422.2 billion won in total. Conclusion: Oriental medicine clinics in Korea appear to be getting larger, with an increase in the number of beds and helpers. Health insurance payments now account for a greater proportion of Oriental medicine clinics' service fee revenues, and management conditions at the clinics are deteriorating.

소양면 지역사회 환경기초조사 (A Basie Community Health Survey in Rural Korea (Soyang-Myun))

  • 1. Introduction Community medicine with the concept of comprehensive medical care and an ideal medical care delivery system not only for an individual or family but for the whole community has emerged. In April 1970, the Presbyterian Medical Center started a hospital based community health service project in order to improve the health of the people in rural areas. Prior to commencing a comprehensive medical care system, a family survey was needed. The major objective of this survey was to obtain information concerning the people and their environment so as to be able to plan and implement a comprehensive medical care program in Soyang-Myun. 2. Survey Method An interview using a family record form was carried out for each household. This family record form was designed to get information about demography, family planning, environmental sanitation and vital statistics. Prior to beginning, the members of the survey team were trained in interviewing techniques for three days. The team consisted of a public health nurse, four nurse-aides, a sanitarian and four health extension workers who are working in our project, The survey was carried out during the period November 1971 to March 1972. 3. Project area 1) Population of Soyang-Myun was 11,668; male, 5,962 and female, 5,706. 병원 간호행정 개선을 위한 연구 (A Study for Improvement of Nursing Service Administration)

  • Much has teed changed in the field of hospital administration in the It wake of the rapid development of sciences, techniques ana systematic hospital management. However, we still have a long way to go in organization, in the quality of hospital employees and hospital equipment and facilities, and in financial support in order to achieve proper hospital management. The above factors greatly effect the ability of hospitals to fulfill their obligation in patient care and nursing services. The purpose of this study is to determine the optimal methods of standardization and quality nursing so as to improve present nursing services through investigations and analyses of various problems concerning nursing administration. This study has been undertaken during the six month period from October 1971 to March 1972. The 41 comprehensive hospitals have been selected iron amongst the 139 in the whole country. These have been categorized according-to the specific purposes of their establishment, such as 7 university hospitals, 18 national or public hospitals, 12 religious hospitals and 4 enterprise ones. The following conclusions have been acquired thus far from information obtained through interviews with nursing directors who are in charge of the nursing administration in each hospital, and further investigations concerning the purposes of establishment, the organization, personnel arrangements, working conditions, practices of service, and budgets of the nursing service department. 1. The nursing administration along with its activities in this country has been uncritical1y adopted from that of the developed countries. It is necessary for us to re-establish a new medical and nursing system which is adequate for our social environments through continuous study and research. 2. The survey shows that the 7 university hospitals were chiefly concerned with education, medical care and research; the 18 national or public hospitals with medical care, public health and charity work; the 2 religious hospitals with medical care, charity and missionary works; and the 4 enterprise hospitals with public health, medical care and charity works. In general, the main purposes of the hospitals were those of charity organizations in the pursuit of medical care, education and public benefits. 3. The survey shows that in general hospital facilities rate 64 per cent and medical care 60 per-cent against a 100 per cent optimum basis in accordance with the medical treatment law and approved criteria for training hospitals. In these respects, university hospitals have achieved the highest standards, followed by religious ones, enterprise ones, and national or public ones in that order. 4. The ages of nursing directors range from 30 to 50. The level of education achieved by most of the directors is that of graduation from a nursing technical high school and a three year nursing junior college; a very few have graduated from college or have taken graduate courses. 5. As for the career tenure of nurses in the hospitals: one-third of the nurses, or 38 per cent, have worked less than one year; those in the category of one year to two represent 24 pet cent. This means that a total of 62 per cent of the career nurses have been practicing their profession for less than two years. Career nurses with over 5 years experience number only 16 per cent: therefore the efficiency of nursing services has been rated very low. 6. As for the standard of education of the nurses: 62 per cent of them have taken a three year course of nursing in junior colleges, and 22 per cent in nursing technical high schools. College graduate nurses come up to only 15 per cent; and those with graduate course only 0.4 per cent. This indicates that most of the nurses are front nursing technical high schools and three year nursing junior colleges. Accordingly, it is advisable that nursing services be divided according to their functions, such as professional, technical nurses and nurse's aides. 7. The survey also shows that the purpose of nursing service administration in the hospitals has been regulated in writing in 74 per cent of the hospitals and not regulated in writing in 26 per cent of the hospitals. The general purposes of nursing are as follows: patient care, assistance in medical care and education. The main purpose of these nursing services is to establish proper operational and personnel management which focus on in-service education. 8. The nursing service departments belong to the medical departments in almost 60 per cent of the hospitals. Even though the nursing service department is formally separated, about 24 per cent of the hospitals regard it as a functional unit in the medical department. Only 5 per cent of the hospitals keep the department as a separate one. To the contrary, approximately 12 per cent of the hospitals have not established a nursing service department at all but surbodinate it to the other department. In this respect, it is required that a new hospital organization be made to acknowledge the independent function of the nursing department. In 76 per cent of the hospitals they have advisory committees under the nursing department, such as a dormitory self·regulating committee, an in-service education committee and a nursing procedure and policy committee. 9. Personnel arrangement and working conditions of nurses 1) The ratio of nurses to patients is as follows: In university hospitals, 1 to 2.9 for hospitalized patients and 1 to 4.0 for out-patients; in religious hospitals, 1 to 2.3 for hospitalized patients and 1 to 5.4 for out-patients. Grouped together this indicates that one nurse covers 2.2 hospitalized patients and 4.3 out-patients on a daily basis. The current medical treatment law stipulates that one nurse should care for 2.5 hospitalized patients or 30.0 out-patients. Therefore the statistics indicate that nursing services are being peformed with an insufficient number of nurses to cover out-patients. The current law concerns the minimum number of nurses and disregards the required number of nurses for operation rooms, recovery rooms, delivery rooms, new-born baby rooms, central supply rooms and emergency rooms. Accordingly, tile medical treatment law has been requested to be amended. 2) The ratio of doctors to nurses: In university hospitals, the ratio is 1 to 1.1; in national of public hospitals, 1 to 0.8; in religious hospitals 1 to 0.5; and in private hospitals 1 to 0.7. The average ratio is 1 to 0.8; generally the ideal ratio is 3 to 1. Since the number of doctors working in hospitals has been recently increasing, the nursing services have consequently teen overloaded, sacrificing the services to the patients. 3) The ratio of nurses to clerical staff is 1 to 0.4. However, the ideal ratio is 5 to 1, that is, 1 to 0.2. This means that clerical personnel far outnumber the nursing staff. 4) The ratio of nurses to nurse's-aides; The average 2.5 to 1 indicates that most of the nursing service are delegated to nurse's-aides owing to the shortage of registered nurses. This is the main cause of the deterioration in the quality of nursing services. It is a real problem in the guest for better nursing services that certain hospitals employ a disproportionate number of nurse's-aides in order to meet financial requirements. 5) As for the working conditions, most of hospitals employ a three-shift day with 8 hours of duty each. However, certain hospitals still use two shifts a day. 6) As for the working environment, most of the hospitals lack welfare and hygienic facilities. 7) The salary basis is the highest in the private university hospitals, with enterprise hospitals next and religious hospitals and national or public ones lowest. 8) Method of employment is made through paper screening, and further that the appointment of nurses is conditional upon the favorable opinion of the nursing directors. 9) The unemployment ratio for one year in 1971 averaged 29 per cent. The reasons for unemployment indicate that the highest is because of marriage up to 40 per cent, and next is because of overseas employment. This high unemployment ratio further causes the deterioration of efficiency in nursing services and supplementary activities. The hospital authorities concerned should take this matter into a jeep consideration in order to reduce unemployment. 10) The importance of in-service education is well recognized and established. 1% has been noted that on the-job nurses. training has been most active, with nursing directors taking charge of the orientation programs of newly employed nurses. However, it is most necessary that a comprehensive study be made of instructors, contents and methods of education with a separate section for in-service education. 10. Nursing services'activities 1) Division of services and job descriptions are urgently required. 81 per rent of the hospitals keep written regulations of services in accordance with nursing service manuals. 19 per cent of the hospitals do not keep written regulations. Most of hospitals delegate to the nursing directors or certain supervisors the power of stipulating service regulations. In 21 per cent of the total hospitals they have policy committees, standardization committees and advisory committees to proceed with the stipulation of regulations. 2) Approximately 81 per cent of the hospitals have service channels in which directors, supervisors, head nurses and staff nurses perform their appropriate services according to the service plans and make up the service reports. In approximately 19 per cent of the hospitals the staff perform their nursing services without utilizing the above channels. 3) In the performance of nursing services, a ward manual is considered the most important one to be utilized in about 32 percent of hospitals. 25 per cent of hospitals indicate they use a kardex; 17 per cent use ward-rounding, and others take advantage of work sheets or coordination with other departments through conferences. 4) In about 78 per cent of hospitals they have records which indicate the status of personnel, and in 22 per cent they have not. 5) It has been advised that morale among nurses may be increased, ensuring more efficient services, by their being able to exchange opinions and views with each other. 6) The satisfactory performance of nursing services rely on the following factors to the degree indicated: approximately 32 per cent to the systematic nursing activities and services; 27 per cent to the head nurses ability for nursing diagnosis; 22 per cent to an effective supervisory system; 16 per cent to the hospital facilities and proper supply, and 3 per cent to effective in·service education. This means that nurses, supervisors, head nurses and directors play the most important roles in the performance of nursing services. 11. About 87 per cent of the hospitals do not have separate budgets for their nursing departments, and only 13 per cent of the hospitals have separate budgets. It is recommended that the planning and execution of the nursing administration be delegated to the pertinent administrators in order to bring about improved proved performances and activities in nursing services.

노인요양시설과 노인요양공동생활가정 요양보호사의 응급상황 경험 및 응급지식, 응급상황대처능력 비교 (Comparison of Emergency Experience and First Aid Knowledge, Emergency Coping Ability of Elderly Care Facilities and Group Home Caregivers)

  • 본 연구는 노인요양시설과 노인요양공동생활가정 요양보호사의 업무특성에 맞는 응급상황대처능력을 강화시키기 위한 응급교육프로그램을 개발하는데 기초자료로 활용하고자 실시한 조사연구이다. 자료수집은 S 및 G지역 노인요양시설과 노인요양공동생활가정을 대상으로 2020년 7.10~8.15 실시하였으며, 최종 236부를 분석에 사용하였다. 자료는 χ2 검정, t-test, ANOVA, Pearson's correlation coefficient로 분석하였다. 연구결과 응급상황 경험은 시설군 68.9%, 공생군 50.7%로 두군간에 차이가 있고(χ2=8.42, p=.004), 응급처치 담당자는 시설군 간호(조무)사 55.3%, 공생군 시설장 42.7%였으며, 두군간에 차이를 보였고(χ2=27.84, p<.001), 응급처치 경험은 시설군 56.5%, 공생군 68.0%, 응급처치 항목은 시설군 해열처치, 복약도움, 하임리히법, 공생군 119 및 보호자 연락, 해열처치, 하임리히법순으로 실시하였다. 응급처치 지식은 시설군 11.60±2.09점, 공생군 9.08±2.28점으로 집단간 차이가 있었고(t=8.39, p<.001), 응급상황대처능력은 시설군 52.94±5.27점, 공생군 47.33±4.39점으로 두집단 간 차이를 보였다(t=8.00, p<.001). 또 시설군에서 응급상황 경험과 응급상황대처능력 간에 양의 상관관계가 있었다. 본 연구결과 시설과 공생 요양보호사가 경험한 응급상황 경험 및 응급지식, 응급상황대처능력에 유의한 차이가 있음이 확인되었으므로 규모별 기관특성에 맞는 응급교육을 개발하여 적용해보는 것이 중요하다고 생각된다.

노인요양병원 간호종사자의 이직의도에 영향을 미치는 요인 (Factors to Affect Turnover Intention of Nursing Workers at Long-Term care Hospitals )

  • 본 연구는 요양병원 간호종사자인 간호사 및 간호조무사를 대상으로 이직의도에 미치는 영향요인을 파악하기 위한 서술적 조사연구이다. 2022년 8월 12일부터 16일까지 G시 소재 4개 요양병원 간호종사자 146명을 대상으로 실시되었다. 연구결과, 이직의도는 일반적 특성 중 연령, 종교, 결혼상태, 학력정도, 월 평균 급여, 직종, 근무형태 및 이직경험에 대해 유의한 차이가 있었다(p<.05). 이직의도는 직무스트레스(r=.51, p<.001) 및 소진(r=.62, p<.001)과는 유의한 양의 상관관계가 있었으며 조직몰입과는 유의한 음의 상관관계가 있었다(r=.-56, p<.001). 조직몰입은 직무스트레스(r=-.25, p=.002) 및 소진(r=.-.67, p<.001)과 유의한 음의 상관관계가 있었으며 소진은 직무스트레스(r=.56, p<.001)와 유의한 양의 상관관계가 있었다. 이직의도에 영향을 미치는 요인으로는 직무스트레스(β=.32, p<.001), 조직몰입(β=-.30, p=.001), 학력정도(β=.17, p=.022)로 나타났으며 설명력은 49.6%였다. 본 연구결과를 토대로 요양병원 간호종사자의 직무스트레스 감소 및 조직몰입 향상을 위한 근무환경 개선 등을 통해 이직의도 감소를 위한 노력이 필요하리라 생각된다.

  • 본 연구는 의료종사자의 노인에 대한 지식, 소진, 직무만족도와 노인학대 인식 간의 관계를 확인하기 위해 시도되었고 1개 특별시, 3개 광역시에 위치한 8개 보건소, 15개 요양병원, 30개 대학병원 기관에서 근무하는 의사, 간호사, 간호조무사 371명을 대상으로 하였다. 본 연구 결과 노인에 대한 지식은 노인학대 인식과 유의한 양의 상관관계가 있는 것으로 나타났고(r=.14, p<.01), 소진은 직무만족도(r=-.55, p<.01)와 노인학대 인식(r=-.10, p<.05)과 관계에서 유의한 음의 상관관계가 있는 것으로 나타났다. 직무만족도는 노인학대 인식(r=.13, p<.01)과의 관계에서 유의한 양의 상관관계가 있는 것으로 나타났다. 노인학대 인식은 노인에 대한 지식(r=.14, p<.01), 직무만족도(r=.13, p<.01)와 유의한 양의 상관관계가 있는 것으로 나타났고, 소진과 유의한 음의 상관관계가 있는 것으로 나타났다(r=-.10, p<.05).본 연구 결과 의료종사자의 소진을 줄이고 직무만족도 향상을 통해 노인에 대한 긍정적인 인식과 태도를 함양 할 수 있을 것으로 본다.
