Laryngeal Findings and Phonetic Characteristics in Prelingually Deaf Patients (언어습득기 이전 청각장애인의 후두소견 및 음성학적 특성)

  • Background and Objectives : There are few studies reported that specifically examine the laryngeal function in patients with profound hearing loss or deafness, This study was designed to examine videostroboscopic findings and phonetic characteristics in adult patients with prelingually deaf. Materials and Method: Sixteen patients (seven males, nine females) diagnosed as prelingually deaf aged from 19 to 54 years, and were compared with a 20 normal control group with no laryngeal pathology and normal hearing group, Videostroboscopic evaluations were rated by experienced judges on various parameters describing the structure and function of the laryngeal mechanism during comfortable pitch and loudness phonations. Acoustic analysis test were done, and a nasalance test performed to measure rabbit, baby, and mother passage. CSL were measured to determine the first and two formant frequencies of vowels /a/, /i/, /u/, Statistical analysis was done using Mann-Whitney U or Wilcoxon signed ranks test. Results: Videostroboscopic findings showed phase symmetry but significantly more occurrences decrement in the amplitude of vibration, mucosal wave, irregularity of the vibration and increased glottal gap size during the closed phase of phonation, In addition, group of prelingually deaf patients were observed to have significantly more occurrences of abnormal supraglottic activities during phonation. The percentage of shimmer in the group of prelingually deaf patients were higher than in the control group. Characteristics of vowels were lower of the second formant of the vowel /i/. Nasalance in prelingually deaf patients showed normal nasality for all passages, Conclusion: Prelingually deaf patients show stroboscopic abnormal findings without any mucosal lesion, suggesting that they have considerable functional voice disorder. We suggest that prelingually deaf adults should perform vocal training for normalized laryngeal function after cochlear implantation.

  • Our present study was performed to investigate acoustically the Korean normal adults' voices, with enough large number of subjects to be reliable. 120 Korean normal adults (60 males and 60 females) of the age of 20 to 39 years produced sustained three vowels, /a/, /i/, and /u/ and read a part of 'Taking a Walk' paragraph, and by analyzing them acoustically with MDVP of CSL, we could get the fundamental frequency ($F_{0}$), jitter, shimmer and NHR of sustained vowels: speaking fundamental frequency ($SF_{0}$), highest speaking frequency (SFhi), lowest speaking frequency (SFlo) of continuous speech. As results, on the average, male voices showed 118.1$\sim$122.6 Hz in $F_{0}$, 0.467$\sim$0.659% in jitter, 1.538$\sim$2.674% in shimmer, 0.117$\sim$0.114 in NHR, 120.8 Hz in $SF_{0}$, 183.2 Hz in SFhi, 82.6 Hz in SFlo. And, female voices showed 211.6∼220.3 Hz in F0, 0.678∼0.935% in jitter, 1.478∼2.582% in shimmer, 0.098∼0.114 in NHR, 217.1 Hz in $SF_{0}$, 340.9 Hz in SFhi, 136.0 Hz in SFlo. Among the 7 parameters, every parameters except shimmer showed the significant difference between male and female voices. And, when we compared the three vowels, they showed significant differences one another in shimmer and NHR of both genders, but not in $F_{0}$ of males and jitter of females.

Speech Outcomes in 5-Year-Old Korean Children with Bilateral Cleft Lip and Palate

  • Background Among the cleft types, bilateral cleft lip and palate (BCLP) generally requires multiple surgical procedures and extended speech therapy to achieve normal speech development. This study aimed to describe speech outcomes in 5-year-old Korean children with BCLP and examine whether normal speech could be achieved before starting school. Methods The retrospective study analyzed 52 children with complete BCLP who underwent primary palatal surgery at a tertiary medical center. Three speech-language pathologists made perceptual judgments on recordings from a speech follow-up assessment of 5-year-old children. They assessed the children's speech in terms of articulation, speech intelligibility, resonance, and voice using the Cleft Audit Protocol for Speech-Augmented-Korean Modification. Results The results indicated that at the age of five, 65 to 70% of children with BCLP presented articulation and resonance within normal or acceptable ranges. Further, seven children with BCLP (13.5%) needed both additional speech therapy and palatal surgery for persistent velopharyngeal insufficiency and speech problems even at the age of five. Conclusion This study confirmed that routine follow-up speech assessments are essential as a substantial number of children with BCLP require secondary surgical procedures and extended speech therapy to achieve normal speech development.

A Study of Correlation Between Severity of Vocal Polyp and Acoustic Parameters (성대용종의 중증도와 음향지수의 상관관계)

  • Background and Objectives: Vocal polyp is the most common disease that causes hoarseness and its incidence is increased currently. The purposes of this study are to investigate the correlation between severity of vocal polyp and acoustic parameters and compare these data with those of the normal Korean. Materials and Methods: We analyzed the acoustic parameters of sustained vowel for 70 vocal polyp patients and 20 normal controls. A CSL(computerized speech lab) was used to carry out the analysis of the voice sample and statistical analysis was used Spearman correlation coefficient & t-test. Results: According to the conclusion of correlation analysis, 21 parameters of all the 34 parameters are significant. Conclusion: These data will be served as basic data for the evaluation of postoperative assessment of the patients with vocal polyp.

Acoustic Characteristics of Stop Consonants in Normal Elderly (일반 노년층 파열음의 음향학적 특성)

  • Changes in speech production in normal elderly might be subtle and gradual. Therefore, an acoustic analysis is appropriate to identify the effect of aging on speech. For this purpose, this study examined four speech parameters; voice onset time (VOT), VOT range, $f_0$ of following vowel($f_0FV$), and $f_0FV$ difference in two age groups, old (mean age 74.57 yrs.) and young (m: 27.43 yrs.). The results show that compared to the older group the younger demonstrated significantly shorter VOTs in lenis and longer in aspirated stop. VOT ranges were relatively broad and consequently overlapped between the phonation types (e.g., lenis, fortis, aspirated). The $f_0FV$ values in the older group which are an integral parameter with VOT were lower compared with the young group. The $f_0FV$ differences in the old female group were significantly narrower than the young female group, therefore, clear distinction became difficult. In conclusion, contrast in temporal information was obscured, and the domain of glottal information was diminished on stop consonants in Korean elderly. The findings suggest that central/peripheral changes by aging could lead to a deficit in coordination between phonation and articulation.

The Relationship between Movements of False Vocal Folds on Phonation and Benign Vocal Folds Lesions (발성시 가성대 형태와 양성 성대 질환의 연관성에 대한 연구)

  • Background and Objectives : Vocal abuse and misuse and muscle tension dysphonia that have various movements of false vocal folds may be related to the development of benign vocal folds lesions, such as vocal nodules, polyps, and cysts. This study was designed to determine whether benign vocal folds lesions were related with movements of false vocal folds on phonation. Material and Methods : One Hundred and seventy eight subjects were studied. All subjects received otolaryngological evaluation including videostroboscopy, objective voice measures. Patients were diagnosed as normal shape of vocal folds (group a), approximation of bilateral false vocal folds (group b), approximation of unilateral false vocal folds (group c), lateralized extension of false vocal folds (group d), and medialized approximation of posterior false vocal folds (group e). We analyzed the results of benign vocal folds lesions in each group. Results : Differences were found between the normal shaped group and the abnormal shaped group. No differences were found between each abnormal groups except group d and e. Conclusion : The shape of false vocal folds was related to the benign vocal folds lesions.

  • Background and Objectives: Complications arising from endotracheal intubation are uncommon but, when they do occur, can be significant. Placement of an endotracheal tube frequently results in trauma to the underlying laryngeal and tracheal tissue, although the trauma is usually reversible. Occasionally, these changes can be of a more permanent nature and result in severe impairment of the airway and/or voice. It is proposed that a common factor-gastroesophageal reflux-might be responsible. This study was performed in order to develop the animal model of LPRD using rats and investigated that LPRD could produce significant damage to larynx especially vocal cords. Materials and Methods : The each four rats were used in the experiment and control study. Each was anesthetized and larynx was exposed and injured in the unilateral aritenoid. Injured site was contact with normal saline(control group) and synthetic gastric juice(experimental group). The larynx was examined after 7days in normal environment. Results : All was survived in the control group and two was survived in the experimental group. In the control group, some inflammation cells was found but in the experimental group, granulation was found. Conclusion : We developed animal model of LPRD using rat and thought LPRD may Play an important role in the development of permanent laryngeal injury.

  • The proposed system is composed of two parts, an AI physical fitness measurement part and an AI physical fitness management part. In the AI fitness measurement part, a guide to physical fitness measurement and accurate calculation of the measured value are performed through deep learning-based pose recognition. Based on these measurements, the AI fitness management part designs personalized exercise programs and provides them to dedicated smart applications. To guide the measurement posture, the posture of the subject to be measured is photographed through a webcam and the skeleton line is extracted. Next, the skeletal line of the learned preparation posture is compared with the extracted skeletal line to determine whether or not it is normal, and voice guidance is provided to maintain the normal posture.

  • Wireless LANs based on the IEEE 802.11 standard are widely spread for use nowadays. Traffic which are conveyed over the WLANs change rapidly from normal data such as Email and Web pages, to multimedia data of high resolution video and voice. To meet QoS (Quality of Service) required by these multimedia traffic, the IEEE 802 committee recently has developed a new standard, IEEE 802.11e. Current IEEE 802.11e, however, is not sufficient to support service differentiations and network performance enhancements, under a varying network environment experiencing as varying channel characteristics and high network loads. Recently, there have been much research to complement this deficiency of 802.11e standard. This paper surveys these research efforts.

  • Aerodynamic analysis study was performed on 14 normal subjects(2 male, 12 female) by nonsense syllables composed of Korean bilabial stop(/p, p', $p^{h}$) and their preceding and/or following vowel /i, a, u/. That is [pi, p'i, phi, pa, p'a, pha, pu, p'u, $p^{h}u$]. All measures were analysed using Aerophone II voice function analyzer and included peak air pressure, mean air pressure, maximum flow rate, volume, mean SPL. As results, first, MSPL and MAP of /p, p', $p^{h}$/ themselves were significantly different. In addition, different vowel enviroment also produced significantliy different aerodynamic chracteristics those consonants.
