  • Regularly participating in physical activity is known to improve quality of life and body composition in elderly with stroke. However, comparatively less physical activity is performed by the stroke survivors. The factors related to inactivity in elderly female stroke survivors have not been elucidated. Therefore, this study aims to compare the quality of life factors and limb compositions between the active and inactive elderly female stroke survivors. Forty nine subjects between the ages of 65 to 75 years were selected from the KNHANES data between the years 2009 to 2011. In addition, 186 agematched healthy peers were also selected for limb composition comparisons. The subjects were groups based on walking days per week: walkers; 3 days or more, non-walkers; less than 3 days per week. BMI and waist circumference were within the obesity ranges for both the non-walkers and walkers. As results, the trend for greater fat (${\pm}10%$) and lean mass (${\pm}30%$) differences were observed for non-walker and walkers, respectively. Significantly greater reasons for function limitation by stroke and hypertension were reported with significantly greater self-care difficulty was shown by the walkers. In conclusion, elderly female stroke survivals may require customized motivation and continuous support to participate in physical activity regularly.

  • Purpose: This study estimated the incidence of delirium and associated risk factors and outcomes in ICU patients with acute poisoning. Methods: Data were collected from ICU patients over 18 years of age that were admitted via the emergency center after presenting with poisoning from 2010 to 2015. Delirium was assessed retrospectively using the Intensive Care Delirium Screening Checklist (ICDSC). Risk factors were evaluated by univariate and multivariate analysis. Results: A total of 199 patients participated in this study and 68 (34.2%) were diagnosed with delirium based on the ICDSC score. The delirium group showed a significantly higher association with prolonged length of stay in the hospital and ICU in comparison with the non-delirium group. The delirium group was associated with greater use of physical restraint. A statistically greater number of patients with pharmaceutical substance poisoning developed delirium over a short period of time than those with non-pharmaceutical substance poisoning. There was no significant difference between the two groups with respect to age, sex, past history, GCS score, vital signs, application of ventilator care and renal replacement therapy. Conclusion: The finding that the delirium group had a greater length of stay in both the hospital and the ICU is consistent with the results of previous worldwide studies of the effects of delirium on the prognosis of patients who were admitted to the ICU, suggesting the possibility for domestic application. Additionally, use of physical restraint was positively related to the incidence of delirium. Thus, interventions for minimizing the use of physical restraints and considering alternatives are needed.

  • The present study was conducted to investigate physical characteristics, food habits of pregnant women with gestational diabetes mellites(GDM). The subjects were consecutive pregnant women, 21 GDM and 43 non-GDM controls who received antenatal care at the Obstetrics and Gynecology clinic. There were no significant difference in the mean age between the two groups(GDM:30.8years, non-GDM:30.5years). Significant differences were observed in several anthropometric characteristics such as height(GDM:159.7cm, non-GDM:162.3cm), prepregnancy weight(GDM:61.6kg, non-GDM:54.1kg), body mass index(GDM:23.9kg/$m^2$, non-GDM:20.5kg/$m^2$), and triceps skin fold thicknesses(GDM:30.9mm, non-GDM:24.7mm) between the two groups. The GDM group had an high incidence of(52.4%) family history of diabetes mellitus than those of control group(23.3%). In the GDM group, more irregularities of meals and more bed-time snacking were observed than those of control group. The intake amount of carbohydrates as well as the total energy were significantly higher in the GDM group when thery were compared with the control. These results suggest that the GDM women in Korea do exhibit clearly distinctive anthropometric and dietary characteristics. Nutritional management such as weight control before conception and throughout pregnancy, monitoring total energy and carbohydrate intakes, and implementing more rigorous meal scheduling might be beneficial for the prevention or reduction of the prevalence of the GDM or its complications. • Purpose: The purpose of this study was to develop the Self-Care Non-adherence Risk Assessment Scale (SCNRAS) for patients with chronic illness in South Korea. Methods: This study was conducted from April to July, 2020 and utilized a convenience sampling method to recruit 336 patients with chronic illness from three hospitals located in South Korea. The content, factorial structure, item-convergent/discriminant validity, convergent validity, internal consistency reliability, and test-retest reliability of the scale were evaluated. The data were analyzed using exploratory and confirmatory factor analyses, Pearson's correlation coefficient, Cronbach's α, and intra-class correlation coefficient. Results: The exploratory and confirmatory factor analyses yielded six-factors. Convergent validity was demonstrated using measures of defining issues. Internal consistency reliability and test-retest reliability were found to be acceptable, as indicated by a Cronbach's α of .65~.81 and an intra-class correlation coefficient of .93~.98. The Self-Care Non-adherence Risk Assessment Scale for patients with chronic illness is a new instrument that comprehensively measures the knowledge, skill, physical function status, access to health care, social support, motivation, and confidence. It comprises 18 items scored on a 5-point Likert scale. The validity and reliability of the scale were verified. Conclusion: The scale developed through this study is expected to screen those who need nursing intervention early by predicting the self-care non-adherence risk group.

  • Purpose: This study aimed to investigate the self-care education experience, self-care performance, and home care service needs of patients with rare and incurable diseases who use a home ventilator. Methods: From2 ventilator rental companies and 5 hospitals, records of 162 subjects were collected to obtain data regarding self-care education experience (23 items) and home care service needs (25 items). Results: Of the subjects, 65.4% were male and 55.6% had a myotrophic lateral sclerosis. The items with the highest and lowest levels of self-care education experience were "using the ventilator"(94.0%) and "community-available resource information"(27.0%), respectively. Meanwhile, self-care was performed well in terms of "personal hygiene"(59.6%) but not performed well in "community-available resource information"(23.7%). "Health status assessment" had the highest need(88.2%) of home care services. Generally, the need of home care services was higher for the patients using invasive ventilators than that for those using non-invasive ventilators. Conclusion: Home care nurses need to not only provide physical care for the patients but also strengthen their roles of enhancing patient access to and utilization of various community resources to provide systematic and individualized training and care.

  • Objectives: We examined health care disparities in Korean urban homeless people and individual characteristics associated with the utilization of health care. Methods: We selected a sample of 203 homeless individuals at streets, shelters, and drop-in centers in Seoul and Daejeon by a quota sampling method. We surveyed demographic information, information related to using health care, and health status with a questionnaire. Logistic regression analysis was adopted to identify factors associated with using health care and to reveal health care disparities within the Korean urban homeless population. Results: Among 203 respondents, 89 reported that they had visited health care providers at least once in the past 6 months. Twenty persons (22.5%) in the group that used health care (n = 89) reported feeling discriminated against. After adjustment for age, sex, marital status, educational level, monthly income, perceived health status, Beck Depression Inventory score, homeless period, and other covariates, three factors were significantly associated with medical utilization: female sex (adjusted odds ratio [aOR, 15.95; 95% CI, 3.97 to 64.04], having three or more diseases (aOR, 24.58; 95% CI, 4.23 to 142.78), and non-street residency (aOR, 11.39; 95% CI, 3.58 to 36.24). Conclusions: Health care disparities in Seoul and Daejeon homeless exist in terms of the main place to stay, physical illnesses, and gender. Under the current homeless support system in South Korea, street homeless have poorer accessibility to health care versus non-street homeless. To provide equitable medical aid for homeless people, strategies to overcome barriers against health care for the street homeless are needed.

  • Purpose: To investigate the effect of progressive resistance training (PRT) on body composition, physical fitness, quality of life, lipid and nutritional profile of patients on hemodialysis (HD). Methods: A non equivalent comparison group pretest and posttest design study was used with 40 participants who were randomly assigned to the exercise group (20 participants) and the comparison group (20 participants). The exercise group received PRT for 30 minutes per session, 3 sessions a week, for 12 weeks, while the comparison group received usual care. The PRT consisted of upper and lower body exercises using elastic bands and sandbags. Outcome measures evaluated were: body composition, physical fitness, quality of life, and lipid profile. Results: Skeletal muscle mass, grip, leg muscle strength, and quality of life all improved significantly in the exercise group. Body fat rate, total cholesterol and triglyceride rate decreased significantly in the exercise group. Conclusion: These results suggest that PRT improves body composition, physical fitness, quality of life, and lipid profile of patients on HD. PRT using elastic bands and sandbags can be utilized as part of a regular care plan for these patients.

  • Purpose: This study aimed to investigate the effects of aroma foot baths on stress and sleep in terminally ill cancer patients. Methods: We performed a non-randomized intervention-control study with 30 terminal cancer patients who were admitted to a palliative care unit. Participants responded to questionnaires on stress and sleep before and after a 5-day interval. The intervention group received a daily aroma foot bath for 5 days. We performed multivariate regression analysis to examine the changes in outcomes on stress and sleep for the intervention group compared to the control group. Results: The differences in baseline characteristics between groups, excluding subjective economic status and general weakness, did not show statistical significance. In contrast to the control group, the intervention group showed a statistically significant change in physical stress and psychological stress levels, but significant changes were not observed in quality of sleep. Compared to the control group, the intervention group showed a significant reduction in physical stress (P=0.068) and psychological stress (P=0.021). Conclusion: Aroma foot baths are effective for reducing stress in patients hospitalized with terminal cancer.
