인삼의 품질과 약리활성 물질과의 상관성 (Relationship of Saponin and Non-saponin for the Quality of Ginseng)

  • It has generally been accepted that quality of ginseng should be determined not by the content of a single component but by composition and balance of total active principles. However, there still can be an exception with a product in which a given ginsenoside is used for the treatment of a specific disease. Although ginsenosides have been regarded to be major active components of ginseng and employed as index components for the quality control, it does not consistent with the traditional concept on ginseng quality creterion; main root has been more highly appreciated than the lateral or fine root. Content of ginsenosides in the lateral or fine root is much higher than that in main root. However, the ratio of protopanaxadiol (PD) and protopanaxatriol (PT) saponins existing in various part of ginseng root is greatly different. The ratio of PD/PT saponins in main root is well balanced but the thinner the root is the higher the ratio. Thus far, a total of 34 different kinds of ginsenosides have been isolated from Korean (red) ginseng, and their pharmacological activities were elucidated partly. Interestingly, different ginsenoside shows similar or contrary effects to each other in biological systems, thus indicating the significance of absolute content of single ginsenoside as well as compositional patterns of each ginsenoside. Therefore, pharmacological activities of ginseng should be determined as a wholly concept. In these regards, standardization of ginseng material (fresh ginseng root) should be preceded to the standardization of ginseng products because ginsenoside content and non-saponin active principles such as polysaccharides and nitrogen (N)-containing compound including proteins are significantly different from part to part of the root. In other words, the main root contains less ginsenosides than other lateral or fine roots. Contents of polysaccharides and N-containing compound in main root is higher. However, the quality control of ginseng products focused on non-saponin compounds has limitation in applying to the analytical method, because of the difficult chemical analysis of these compounds. Content of ginsenosides, and ratios of PD/PT and ginsenoside Rb,/Rg, are inversely proportional to the diameter of ginseng root. Therefore, these can be served as the chemical parameters for the indirect method of evaluating from what part of the root does the material originate. Furthermore, contents of polysaccharides and N-containing compounds show inverse relationship to saponin content. Therefore, it seems that index for analytical chemistry of saponin can be applied to the indirect method of evaluating not only saponin but also non-saponin compounds of ginseng. From these viewpoints, it is strongly recommended that quality of ginseng or ginseng products be judged not only by the absolute content of given ginsenoside but also by varieties and compositional balance of ginsenosides, including contents of non-saponin active principles.

항암화학요법을 받는 암 환자의 오심, 구토관리를 위한 환자용 지침 개발 (Development of Cancer Patient Guide for Nausea & Vomiting Management in Chemotherapy)

  • Purpose: This study was conducted to develop a Cancer Patient Guide with patients involvement using evidenced based practice research. The purpose of this patient guide was to help patients undergoing chemotherapy to manage their nausea and vomiting based on evidence. Methods: The design of the research was a methodological study. The participants consisted of seven cancer patients who were asked about their ' need for nausea and vomiting management, and secondly, 16 expert & 15 cancer patients to evaluate the Cancer Patient Guide using the DESCERN tool. Results: 1) Sixty-four relevant research evidences based articles were reviewed. 2) Patients were interviewed as to their needs in controlling nausea and vomiting. 3) The preliminary Cancer Patient Guide utilizing the research evidenced and the cancer patients interviews was then evaluated and revised by the experts and cancer patients. Lastly, the Cancer Patient which included an overview of chemotherapy, pathophysiology of nausea & vomiting, pharmacological and non-pharmacological interventions was finalized with each intervention supported by research evidence and patients' narratives of their experience. Conclusion: The Cancer Patient Guide was developed using evidenced based research and cancer patients in-put and be used to improve patients' self-management skill of nausea and vomiting in chemotherapy. The guide t also provides evidence based patient friendly information and contributes as a baseline data for developing and evaluating evidence-based guide for patients.

알츠하이머병의 최신지견 (Recent Advances in Diagnosis and Treatment of Alzheimer's Disease)

  • Alzheimer's disease (AD) is a neurodegenerative disorder in which neuronal loss causes cognitive decline and other neuropsychiatric problems. It can be diagnosed based on history, examination, and appropriate objective assessments, using standard criteria such as the Diagnostic and Statistical Manual of Mental Disorders or the National Institute of Neurological and Communicative Disorders and Stroke and the Alzheimer's Disease and Related Disorders Association (NINCDS-ADRDA). Brain imaging and biomarkers are making progress in the differential diagnoses among the different disorders. The cholinesterase inhibitors, donepezil, rivastigmine and galantamine and N-methyl-D-aspartate receptors antagonist memantine are approved by the US Food and Drug Administration for AD. Recently some acetylcholinesterase inhibitors gained approval for the treatment of severe AD and became available in a higher dose formulation or a patch formulation. Optimal care in AD is multifactorial and it should include early diagnosis and multidisciplinary care with pharmacological and nonpharmacological interventions including exercise interventions, cognitive interventions and maintenance of social networks.

  • Persistent and intractable hiccups (with respective durations of more than 48 hours and 1 month) can result in depression, fatigue, impaired sleep, dehydration, weight loss, malnutrition, and aspiration syndromes. The conventional treatments for hiccups are either non-pharmacological, pharmacological or a nerve block treatment. Pulsed radiofrequency lesioning (PRFL) has been proposed for the modulation of the excited nervous system pathway of pain as a safe and nondestructive treatment method. As placement of the electrode in close proximity to the targeted nerve is very important for the success of PRFL, ultrasound appears to be well suited for this technique. A 74-year-old man suffering from intractable hiccups that had developed after a coronary artery bypass graft and had continued for 7 years was referred to our pain clinic. He had not been treated with conventional methods or medications. We performed PRFL of the phrenic nerve guided by ultrasound and the hiccups disappeared.

기분장애 환자의 비만에 대한 약물치료 (Pharmacotherapy for Obesity in Mood Disorders)

  • 기분장애에서 과체중과 비만의 문제가 일반인구보다 더 심각하고, 심혈관 치명률의 증가와 무관하지 않기 때문에 비만에 대한 치료가 필수적이다. 일반적인 상황에서 비만에 대한 치료와 동일하게 생활습관교정과 같은 비약물치료가 선행되어야 한다. 이 같은 시도가 실패하는 경우에는 약물치료가 필요하다. 기분장애의 비만치료로 공식 승인된 약은 없다. 따라서 일반인구의 비만에 승인된 약을 처방하거나 여러 연구를 통해서 효과가 입증된 약을 처방하게 된다. 여러 치료 지침과 연구 등은 orlistat, metformin, topiramate와 bupropion 등이 효과적임을 지지한다.

Nicotine Addiction: Neurobiology and Mechanism

  • Nicotine, primary component of tobaco produces craving and withdrawal effect both in humans and animals. Nicotine shows a close resemblance to other addictive drugs in molecular, neuroanatomical and pharmacological, particularly the drugs which enhances the cognitive functions. Nicotine mainly shows its action through specific nicotinic acetylcholine receptors located in brain. It stimulates presynaptic acetylcholine receptors thereby enhancing Ach release and metabolism. Dopaminergic system is also stimulated by it, thus increasing the concentration of dopamine in nuclear accumbens. This property of nicotine according to various researchers is responsible for reinforcing behavioral change and dependence of nicotine. Various researchers have also depicted that some non dopaminergic systems are also involved for rewarding effect of nicotinic withdrawal. Neurological systems such as GABAergic, serotonergic, noradrenergic, and brain stem cholinergic may also be involved to mediate the actions of nicotine. Further, the neurobiological pathway to nicotine dependence might perhaps be appropriate to the attachment of nicotine to nicotinic acetylcholine receptors, peruse by stimulation of dopaminergic system and activation of general pharmacological changes that might be responsible for nicotine addiction. It is also suggested that MAO A and B both are restrained by nicotine. This enzyme helps in degradation dopamine, which is mainly responsible for nicotinic actions and dependence. Various questions remain uninsurable to nicotine mechanism and require more research. Also, various genetic methods united with modern instrumental analysis might result for more authentic information for nicotine addiction.

섬망에 대한 정신신체의학적 중재 (Psychosomatic Intervention of Delirium)

  • 섬망은 일반 의학 상태와는 독립적으로 입원 기간 및 사망율을 증가시켜 환자의 전반적 예후에 부정적인 영향을 미친다. 다양한 내, 외과적 치료 환경에서 보고된 섬망의 발생 빈도는 15~70%로 알려졌으며, 정신신체 의학적 중재가 요구되는 흔한 정신과 의뢰 사유 중 하나이다. 섬망의 치료는 1) 위험 인자와 촉발 요인의 확인, 2) 지남력 회복 및 기능 유지를 위한 적절한 치료 환경의 구성과 환자 가족 및 치료진에 대한 교육을 제공하는 비약물적 중재, 그리고 3) 심한 초조, 환각, 그리고 행동 상의 문제 등을 조절하기 위한 약물학적 중재로 구분될 수 있다. 본고를 통해, 섬망에 대한 정신신체의학적 중재의 전반에 대해 기술하고 실제 임상에서 섬망치료 시에 흔히 접하게 되는 문제에 대해서도 논의하고자 한다.

유구치에서 electrosurgery와 MTA를 이용한 치수절단술 : 성공률에 대한 후향적 연구 (PULPOTOMY IN PRIMARY MOLAR TEETH USING ELECTROSURGERY AND MTA : A RETROSPECTIVE STUDY OF SURVIVAL RATES)

  • This study investigates the success rate of pulpotomy using electrosurgery operated on a primary molar in caries. The pulpotomy using electrosurgery was done on 253 primary molars of 111 young patients from 2 to 9 years old during the period of the first day of January 2011 to the last day of December 2015. After the amputation of pulp and hemostasis of primary molar were done using electrosurgery, MTA as pulp capping material was applied to the primary molar and the tooth was restored with the stainless steel crown. The follow up period after the treatment ranged from 4-46 months. The clinical and radiographic success rate ranged from 92.1 - 94.3%. Which is comparable to formocresol and ferric sulfate pulpotomy. Due to its non-pharmacological characteristic, electrosurgery can minimize harmful effect on the pulp tissue. Its fast bleeding control makes it easy and safe to use in pediatric and disabled patients in comparatively simple manner. Electrosurgery can be an alternative for pulp therapy considering the side effects of pharmacological ways.

  • Medication-related osteonecrosis of the jaw (MRONJ) has recently associated to the increase in antiresorptive and anti-angiogenic drugs prescriptions in the treatment of oncologic and osteoporotic patients. The physiopathogenesis of MRONJ remains unclear and available treatments are unsatisfactory. Newer pharmacological treatments have shown good results, but are not curative and could have major side effects. At the same time as pharmacological treatments, mesenchymal stem cells (MSCs) have emerged as a promising therapeutic modality for tissue regeneration and repair. MSCs are multipotential non-hematopoietic progenitor cells capable to differentiating into multiple lineages of the mesenchyme. Bone marrow MSCs can differentiate into osteogenic cells and display immunological properties and secrete paracrine anti-inflammatory factors in damaged tissues. The immunomodulatory, reparative, and anti-inflammatory properties of bone marrow MSCs have been tested in a variety of animal models of MRONJ and applied in specific clinical settings. The aim of this review is to discuss critically the immunogenicity and immunomodulatory properties of MSCs, both in vitro and in vivo, the possible underlying mechanisms of their effects, and their potential clinical use as modulators of immune responses in MRONJ, and to identify clinical safety and recommendations for future research.

Comparative evaluation of the efficacy of wound healing with and without dehydrated human amniotic/chorionic membrane in alveoloplasty: a pilot study

  • Objectives: Wound healing is an integral part of any surgical procedure. Appropriate wound closure is critical to any successful surgical procedure, especially intraoral procedures. Various factors aid in wound healing, both pharmacological and non-pharmacological. Dehydrated human amniotic/chorionic membrane (dHACM) is an emerging bioinert material that contains anti-inflammatory properties, angiogenetic properties, osteogenic potential, and various growth factors. The purpose of this study was to evaluate the efficacy of wound healing properties of dHACM in bilateral alveoloplasty patients. Materials and Methods: A prospective split-mouth study was conducted on 10 patients. Site A received sutures with dHACM and site B was sutured without dHACM. Wound healing was assessed with the Landry, Turnbull, and Howley Index. Results: Sites A and site B were compared. A P<0.05 and a test value of 22 was obtained, indicating a statistical difference between the two sites. Conclusion: Our study showed better healing with dHACM than without.
