The effect of the traditional herbal medicine Yukmijihwangtang-Jahage(YJ) on the improvement of insulin resistance and lipid profile was studied using a model for non-insulin dependent diabetes mellitus, lean (Fa/-) and obese (fa/fa) Zucker rats. Yukmijihwangtang-Jahage feeding for 4 weeks resulted in a significant decrease in the concentration of plasma triglyceride in both lean and obese Zucker rats. Furthermore, Yukmijihwangtang-Jahage markedly decreased both plasma cholesterol and fasting plasma insulin, and significantly decreased the postprandial glucose level at 30 min during oral glucose tolerance test in obese Zucker rats. Although there was no statistical significance, the crude glucose transporter 4 protein level of Yukmijihwangtang-Jahage dieted obese rats tended to increase when compared to that of obese control rats. Therefore, the present results suggested that Yukmijihwangtang-Jahage may be useful in prevention and improvement of metabolic disorders characterized by hyperinsulinemia states such as non-insulin dependent diabetes mellitus, syndrome X and coronary artery disease.
Non-insulin-dependent diabetes mellitus (NIDDM) is characterized by insulin resistance and impaired insulim secretion. The transgenic technology, in which a specific gene can be introduced or deleted to study its function, has been established. A number of transgenic mice, altered the expression of genes potentially involved in insulin action or pancreatic ${\beta}$-cell function, have recently been developed to address questions concerning NIDDM. Thransgenic mice model may help understanding the molecular basis of complex patho-physiologies of NIDDM. This review outlines the new insights obtained from the studies of transgenic mice that overxpress or show decreased expression of putative key genes involved in the regulation of insulin resistance and pancreatic ${\beta}$-cell function, therefore in the control of glucose homeostasis.
This study was to investigate the risk factors for the development of diabetes mellitus on healthy offsprings of Korea non-insulin dependent diabetes mellitus(NIDDM) patients. Forty four offsprings who have at least 1 parent with NIDDM and age, sex and body mass index(BMI) matched forty four healthy control subjects were collected for this study. BMI, skinfold thickness, waist/hip circumference ratio(WHR), fasting blood glucose, serum insulin, serum lipid profile, nutrient intakes and food behaviors were measured. There were no significant difference in BMI, WHR, fasting blood glucose, insulin, total cholesterol, triglyceride and daily nutrients intake between offsprings and controls. But skinfold thickness, LDL-cholesterol level and sweety food eating frequency were significantly higher in male offsprings than in male controls. And HDL-cholesterol was significantly lower in male offsprings than in male controls. Although there were no significant difference, offsprings had a overeating habit and ate more confectionery and greasy food than controls.
The purpose of this study is to determine the effect of diabetes education program on Glucose Metabolism(blood sugar, HbA1c) and Lipid Metabolism(total cholesterol, triglyceride, low density lipoprotein, high density lipoprotein), Self-efficacy in non-insulin independent diabetes mellitus. The study design was a non equivalent control group pre-test post-test design. Data for the study were collected from March 12 to June 19, 2001. Sixty-two research subjects were assigned to experimental(36) and control(26) groups. The collected data was analyzed using the Chi-Square test, t-test by spsswin program The results are as follows : 1. Experimental group had higher level of glucose metabolism than control group(FBS ; t=-3.317, p=.002, HbA1c; t=-4.956, p=.000). 2. Level of lipid metabolism were partly a significant different between experimental group and control group(Triglyceride ; t=-2.108, p=.039). 3. Experimental group had higher efficacy score than control group(t=4.651, p=.000). In conclusion, the study supported the effects of diabetes education program to increase metabolism an d self-efficacy. Further study with a longitudinal design is suggested to verify the effect of diabetes education program in NIDDM and standardized diabetes education program.
This study was designed to assess the body fat distribution, and also to investigate the effects of body fat on glucose tolerance and on insulin secretion pattern by body mass index in offspring of parents with NIDDM. The subjects consisted of twenty parents with NIDDM who had been admitted to the Department of Internal Medicine or had been seen in the outpatient clinic at Kangnam St. Mary's Hospital, Catholic University between February to March, 1995. Twenty offspring were randomly selected from forty six offspring of twenty parents with NIDDM. As a control group, twenty healthy people without a family history of diabetes mellitus were matched by sex, age and body mass index(BMI). The results are as follows : 1. Mean fasting serum glucose and insulin levels and insulin / glucose ratio were significantly greater in offspring than in the control subjects with BMI 25kg /㎡ in the offspring and in the BMI<25kg /㎡ control subjects (P<0.05). 2. The total glucose area and insulin area were significantly greater in both the offsping and the control subjects with BMI≥25kg /㎡ than in both the offspring and the control subjects with BMI<25 kg /㎡(P<0.05). 3. Upper body skinfold thickness, Waist hip ratio(WHR), serum levels of total cholesterol and triglyceride(TG), total dietary calorie intake and protein intake in both the offspring and the control subjects with BMI≥25kg /㎡ were greater than those with BMI<25kg /㎡(P<0.05). On the other hand, HDL-cholesterol in both the offspring and the control subjects with BMI≥25kg /㎡ was lower than those with BMI< 25kg /㎡(P<0.05). 4. The major variables influencing the total glucose area were subscapular skinfold thickness and WHR and the major variables influencing the total insulin area were suprailiac skinfold thickness, WHR, TG and free fatty acid. In the light of the results, glucose intolerance and insulin resistance were affected by body mass index, Upper body fat, WHR and lipids(TG, Free fatty acid), it is implied that these are influencing factors on total glucose area and total insulin area. The identification of these factors might provide a useful tool to identify individuals at high risk of diabetes mellitus. Therefore, various nursing intervention programs to reduce obesity could be given to both the offspring of parents with NIDDM and to the obese healthy controls before diabetes mellitus develops.
Cytokines produced by immune cells infiltrating pancreatic islets have been incriminated as important mediators of $\beta$-cell destruction in insulin-dependent diabetes mellitus. In non insulin-dependent diabetes, cytokines are also associated with impaired $\beta$-cell function in high glucose condition. By the screening of various natural products blocking $\beta$-cell destruction, we have recently found that epigallocatechin gallate (EGCG) can prevent the in vitro destruction of RINm5F cell, an insulinoma cell line, that is induced by cytokines. In that study we suggested that EGCG could prevent cytokine-induced $\beta$-cell destruction by down-regulation of nitric oxide synthase (NOS) through inhibition of NF-kB activation. Here, to verify the in vivo antidiabetogenic effect of EGCG, we examined the possibility that EGCG could also prevent the experimental autoimmune diabetes induced by the treatment of multiple low doses of streptozotocin (MLD-STZ), which is recognized as an inducer of type I autoimmune diabetes. Administration of EGCG (100 mg/day/kg for 10 days) during the MLD-STZ induction of diabetes reduced the increase of blood glucose levels caused by MLD-STZ. Ex vivo analysis of $\beta$-islets showed that EGCG downregulates the MLD-STZ-induced expression of inducible NOS (iNOS). In addition, morphological examination showed that EGCG treatment ameliorated the decrease of islet mass induced by MLD-STZ. In combination these results suggest that EGCG could prevent the onset of MLD-STZ-induced diabetes by protecting pancreatic islets. Our results therefore revealed the possible therapeutic value of EGCG for the prevention of diabetes mellitus progression.
This study compared the nutrient intake of obese versus non-obese non-insulin dependent diabetes mellitus (NIDDM) patients for Diabetes Medical Nutrition Therapy. The study was conducted at medical hospitals in Gyeonggi and Seoul from April 2009 to November 2009. Fifty-six adult male NIDDM patients were enrolled and divided into two groups: 36 into an obese group (BMI ${\geq}25$) and 20 into a non-obese group (BMI<25). To conduct this study, anthropometric measurements, and daily nutrient intake of obese and non-obese NIDDM patients were measured. Daily nutrient intake was estimated by 24hr-recall and analyzed by the CAN program. In the results, anthropometric measurements of the two groups showed significant differences in weight and BMI (p<0.001). Daily nutrient intake of the two groups showed no significant differences, except for vitamin E intake (p<0.05). The total energy intake of the non-obese and obese groups were $2,669.9{\pm}964$ kcal and $2,555.4{\pm}803$ kcal, respectively, which were both above 113% of the recommended Dietary Reference Intakes for Korean (KDRIs). Cholesterol and sodium intake were $378.1{\pm}215.6$ mg and $6,478.9{\pm}2755.1$ mg, respectively for the non-obese group. Cholesterol and sodium intake were $308.1{\pm}155.6$ mg and $6,306.8{\pm}2788.9$ mg, respectively, for the obese group. Both groups were above 150% of the recommended levels set by the Korean Diabetes Association (KDA). However, their antioxidant nutrient intake was appropriate. Meanwhile, their fiber intake was $10.7{\pm}5.1$ g and $9.8{\pm}5.2$ g, respectively, which was lower than 40% of the recommended intake set by the KDA. The results show that the nutritional education for obese and non-obese NIDDM male patients must aim to reduce total energy, cholesterol, and sodium intake, while increasing fiber intake. In addition, the factors related to a patient's glycosylated hemoglobin, serum lipids, blood pressure, and weight change must be calibrated for the appropriate energy, fat, cholesterol, sodium, and dietary fiber intake.
Controlling dietary behaviors and health-related lifestyle habits is important to manage non-insulin dependent diabetes mellitus (NIDDM). If NIDDM is not treated properly, the prevalence of macro-vascular complications (MC) may increase. The goal of NIDDM therapy is to maintain normal concentrations of blood glucose and lipid profiles by having regular meals, controlling alcohol drinking, quitting smoking, and performing physical exercise regularly. This study was performed to investigate the dietary and health-related lifestyle habits and blood parameters of NIDDM patients of both genders and compared these characteristics between the patients with and without MC. Our results show that the subjects with MC compared to without MC and the female patients than the males controlled their diets more regularly, tended to regulate the number of meals better, and smoke less than those without MC. Although plasma lipid profiles were not significantly different between the genders and between the subjects with and without MC, the men had higher blood glucose and plasma tHcy concentrations than the women and plasma tHcy concentration was higher in the female subjects without MC. These results imply that the male patients of NIDDM without MC might have more problems in maintaining their blood glucose. In addition, smoking may be the most important life-style factor influencing some blood parameters like blood glucose, HbAlc, and total cholesterol in the NIDDM patients.
The present study was conducted to document the association between plasma homocysteine levels and the presence of macrovascular angiopathy with food and nutrient intake patterns among patients with Type II diabetes mellitus in Korea. Plasma total homocysteine concentration was analyzed by HPLC-fluorescence detector method in 127 patients with non-insulin dependent diabetes mellitus. Logistic regression analyses were performed respectively to study the association of plasma homocysteine levels with clinical and dietary characteristics and macroangiopathy (MA). The average plasma homocysteine level of patients with MA was $14.2{\mu}mol/l$, which was significantly higher than that of patients without MA ($11.4{\mu}mol/l$). The proportions of patients with MA showed a significant difference, being 32.3% in hyperhomocysteinemic ($>14.0{\mu}mol/l$) patients and 13.5% in others with homocysteine levels lower than $14.0{\mu}mol/l$. Odds ratios for macroangiopathy by tertile increase of plasma homocysteine concentration were 1.633 ($Q_2$) and 4.831 ($Q_3$), when adjusted for age, sex, and cigarette smoking. Patients with MA consumed reduced amounts of vitamin $B_1,\;B_2$, and folate. The results indicate that the plasma homocysteine levels are significantly increased in NIDDM patients who have macroangiopathy. Dietary management such as increased fruits and vegetables and decreased potatoes and starches might be beneficial for the prevention of macroangiopathy in diabetic patients.
인슐린 비의존성 당뇨병 환자를 대상으로 식이섬유 섭취량을 증가시키기 위한 방법으로서 병원에서 처방된 식사에 더하여 일일 5g의 식이섬유를 포함하는 콩가루 비스켓(15명, 대조군), 20g을 포함하는 비지 비스켓(9명 , 불용성 고식이섬유군) 또는 25g을 포함하는 다시마 비스켓(10명, 수용성 고식이섬유군)을 2주 동안 첨가 섭취시킨 후 당질대사 개선효과를 평가하였다. 실험기간 중 당뇨병 환자들의 일일 열량 섭취량은 세군간에 유의한 차이가 없었고, 하루 실제로 섭취한 평균 총 식이섬유량은 대조군이 $19.1\pm4.3g(13.8\pm8.0g/1000kca1),$ 비지 비스켓군이 $32.5\pm4.1g(19.7\pm3.8g/1000kcal),$ 다시마 비스켓군이 $38.1\pm5.5g(24.5\pm4.5g/1000kcal)이었다.$ 비지 비스켓과 다시마 비스켓의 2주간 섭취 후 공복시 고혈당의 하강폭이 대조군에 비해 유의하게 컸으며 (p<0.05), 경구 포도당 부하 후의 내당능도 뚜렷하게 개선되었다(p<0.05). 경구 포도당 부하검사 및 글루카곤 자극검사를 통해 관찰한 바 혈청 인슐린과 C-peptide 수준 및 혈장 글루카곤의 농도는 2주간의 고식이섬유 첨가물의 섭취에 의해 변화를 보이지 않았다. 당화혈색소 수준은 2주간의 식이 섬유 첨가물 섭취 후에 다소 감소 경향을 보였으나 유의한 변화로는 판정되지 않았고 일일 요당의 배설량은 세군 모두에서 비스켓 섭취전에 비해 유의하게 감소되었다(p<0.05). 2주 동안의 식이섬유 첨가 섭취가 인슐린 비의존성 당뇨병 환자의 당질대사에 긍정적인 효과를 미치는 것으로 관찰되었으므로 비지와 다시마 첨가물이 당뇨병의 식사요법에서 유용한 고식이섬유 식품 첨가물로서의 가능성이 있는 것으로 판단된다.
