단기간 기관내 삽관전, 후 음성지표의 측정

  • 배경 및 목적: 전신마취를 위하여 시행한 기관내 삽관은 삽관튜브와 성대내면의 접촉에 의한 압력과 마찰로 후두 미세한 손상을 주게된다. 저자들은 단기간 기관내 삽관 전,후의 음성분석을 통하여 손상의 유무와 회복을 측정할 수 있는 객관적인 음성지표를 찾아보고자 하였다. 대상 및 방법: 만성 중이염 수술시 전신마취를 목적으로 경구기관 튜브를 거치한 성인 남자 10명과 여자 15명 환자를 대상으로 수술 1일전과 술후 24시간 후 각각 "a"음을 연장 발성시켜 CSL 4300B (KAY elemetrics Corp)의 MDVP(multidimensional voice program)을 이용하여 harmonic to noise ratio(NHR), Jitter, Shimmer, Fundamental frequency를 측정 비교하였다. 결 과: 남녀 모두에서 Jitter, Shimmer는 각각 평균 0.70%에서 1.06%, 1.92%에서2.28%로 증가되는 경향을 보였으나 통계학적 유의성은 없었다. Fundamental frequency는 여자에서 평균 220Hz에서 221Hz로 남자는 125Hz에서 128Hz로 변화를 보이지 않았고 harmonic to noise ratio(NHR)또한 평균 0.11로 수술 전, 후 변화를 관찰할 수 없었다. 결 론: 이상의 결과로 2내지 6시간의 단기간 삽관으로 인한 성대의 손상은 경미하여 24시간 이내에 회복되는 것으로 판단된다. 향후 6시간 이상의 기관내삽관이나 수일이상의 장기간 삽관후의 음성지표의 측정등의 연구가 필요할 것으로 사료된다.

A study on the clinical utility of voiced sentences in acoustic analysis for pathological voice evaluation (장애음성의 음향학적 분석에서 유성음 문장의 임상적 유용성에 관한 연구)

  • This study aimed to investigate the clinical utility of voiced sentence tasks for voice evaluation. To this end, we analyzed the correlation between perturbation-based acoustic measurements [jitter percent (jitter), shimmer percent (shimmer), Noise to Harmonic Ratio (NHR)] using sustained vowel phonation, and cepstrum-based acoustic measurements [Cepstral Peak Prominence (CPP), Low/High spectral ratio (L/H ratio)] using voiced sentences. As a result of analyzing data collected from 65 patients with voice disorders, there was a significant correlation between the CPP and jitter (r = -.624, p = .000), shimmer (r = -.530, p = .000), NHR (r = -.469, p = .000).This suggests that the cepstrum measurement of voiced sentences can be used as an alternative to the analysis limitations of the pathological voice such as not possible perturbation-based acoustic measurement, and result difference according to the analysis section.

Acoustic and Physiologic Characteristics of Newborn Infants' Communication Intent via Crying (신생아 울음의 의사소통 의도와 관련된 음향학적 특성)

  • The purpose of this study was to investigate the acoustic characteristics of crying infants according to the communication intents such as hunger and pain in terms of acoustic differences in the fundamental frequency ($F_0$), jitter, shimmer, noise-to-harmonic ratio(NHR), habitual pitch, and intensity. The subjects were 20 healthy, normal infants, less than seven days old, from the city of Seoul and were born after 38 to 42 weeks(full term) of pregnancy. The sound of crying was recorded for three minutes. The crying due to pain was induced by means of the inborn metabolism error test, whereas the crying due to hunger was verified by means of the rooting reflex by waiting for the designated eating time. The results were as follows: (1) the fundamental frequency, noise-to-harmonic ratio(NHR), and intensity of the infants' crying due to pain was higher than that by hunger, showing a significant difference between the mean values. (2) the infants' crying due to hunger and that by pain did not have a significant difference in the mean jitter and shimmer values but both of them were largely outside of the normal threshold values(jitter by 1.04% and shimmer by 3.81%). This study was significant in the sense that it showed the acoustic characteristics of infants' crying from hunger and pain were very different from each other according to the communication intents in terms of the six acoustic parameters.

Acoustic characteristics of speech-language pathologists related to their subjective vocal fatigue (언어재활사의 주관적 음성피로도와 관련된 음향적 특성)

  • In addition to administering a questionnaire (J-survey), which questions individuals on subjective vocal fatigue, voice samples were collected before and after speech-language pathology sessions from 50 female speech-language pathologists in their 20s and 30s in the Daejeon and Chungnam areas. We identified significant differences in Korean Vocal Fatigue Index scores between the fatigue and non-fatigue groups, with the most prominent differences in sections one and two. Regarding acoustic phonetic characteristics, both groups showed a pattern in which low-frequency band energy was relatively low, and high-frequency band energy was increased after the treatment sessions. This trend was well reflected in the low-to-high ratio of vowels, slope LTAS, energy in the third formant, and energy in the 4,000-8,000 Hz range. A difference between the groups was observed only in the vowel energy of the low-frequency band (0-4,000 Hz) before treatment, with the non-fatigue group having a higher value than the fatigue group. This characteristic could be interpreted as a result of voice abuse and higher muscle tonus caused by long-term voice work. The perturbation parameter and shimmer local was lowered in the non-fatigue group after treatment, and the noise-to-harmonics ratio (NHR) was lowered in both groups following treatment. The decrease in NHR and the fall of shimmer local could be attributed to vocal cord hypertension, but it could be concluded that the effective voice use of speech-language pathologists also contributed to this effect, especially in the non-fatigue group. In the case of the non-fatigue group, the rhamonics-to-noise ratio increased significantly after treatment, indicating that the harmonic structure was more stable after treatment.

Acoustic Analysis of Voice Change According to Extent of Thyroidectomy (갑상선 수술범위에 따른 음성의 음향적 분석)

  • Voice complication without the laryngeal nerve injury can occur after thyroidectomy. The purpose of this study is to investigate voice changes according to extent of thyroidectomy with acoustic analysis. Thirty-five female patients with papillary thyroid carcinoma took voice evaluation at before and 1 month, and 3 months after thyroidectomy. Acoustic analysis parameters were speaking fundamental frequency(SFF), min $F_0$, max $F_0$, dynamic range $F_0$, jitter, shimmer, noise-to-harmonic ratio(NHR), and Cepstral prominence peak(CPP). Repeated-measured analysis of variance was applied. Time-related voice changes showed significant differences in all parameters except NHR. At 1 month after surgery, voice quality was worse and pitch was decreasing, but voice quality and pitch were improving at 3-month follow-up. Voice changes according to the extent of surgery were in SFF, max $F_0$, and dynamic range $F_0$. Time by surgery-related voice change existed only in min $F_0$. The result showed that the severity of voice complication depended on the extend of thyroidectomy which had a negative impact on $F_0$-related parameters. The deterioration of voice quality at 1 month after thyroidectomy may be affected by the loss of thyroid hormone in the blood. The descent of $F_0$-related parameters may be impacted by laryngeal fixation of surgical site adhesion.

Acoustic Qualities of Phonation in Hearing-impaired Male Adults (청각장애 성인 남성의 음성 특성)

  • The purposes of this experiment were to compare and analyze some voice parameters of the hearing impaired male adults and to suggest a basic data on the speech intervention for the hearing impaired. Voice analysis of four sustained vowels(/a/, /i/, /${\partial}$/, /u/, fundamental Sequency(F0), jitter percent, shimmer percent, and Noise to Harmonic Ratio(NHR) was conducted for the deaf young male adults using a sign laguage(N=5, aged 16-20) and the normal hearing young male adults(N=10, aged 18-20) by using MDVP(Multi-Dimensional Voice Program) in CSL. F0, jitter, and shimmer in the deaf group were significantly higher than those in the normal hearing group. The average of F0 was 151 Hz, which was lower than the results of the previous studies, and there were no significant differences among the sustained vowels. In both groups, the values of the voice parameters were stable on the /a/ or /${\partial}$/, those closed to the standard scores.

Acoustic Characteristics on the Adolescent Period Aged from 16 to 18 Years (16~18세 청소년기 음성의 음향음성학적 특성)

  • During adolescence the mutational period is characterized by the changes in the laryngeal structure, the length of the vocal cords, and a tone of voice. Usually, adolescents at 15 or 16 reach the voice of adults but the mutational period is sometimes delayed. Therefore, studies on the voice of adolescents between 16 ~ 18 right after the mutational period are required. Accordingly, this paper attempted to provide basic data about the normal standard for patients with voice disorders during this period by evaluating the vocal characteristics of males and females between 16 ~ 18 with an objective device bycomparing and analyzing them by sex and age. The study was conducted on a total of 60 subjects composed of each 10 subjects of each age. The vocal analysis was conducted by MPT (Maximum Phonation Time) measurement, sustained vowels and sentence reading. As for /a/ sustained vowels, fundamental frequency, hereinafter referred to as $F_0$, jitter, shimmer, noise-to-harmonic ratio, hereinafter referred to as NHR were measured by using the Multi-dimensional voice program (MDVP) among the Multi-Speech program of Computerized Speech Lab (Kay Elemetrics). The sentence reading, mean $F_0$, maximum $F_0$ and minimum $F_0$ were measured using the Real-Time Pitch (RTP) Model 5121 among the Multi-Speech program of Computerized Speech Lab (Kay Elemetrics). As a result, according to sex, there were statistically significant differences in $F_0$, jitter, shimmer, mean $F_0$, maximum $F_0$, and minimum $F_0$; and according to age, there were statistically significant differences in MPT. In conclusion, the voice of the adolescents between 16 ~ 18 reached the maturity levels of adults but the voice quality which can be considered on the scale of voice disorders showed transition to the voice of an adult during the mutational period.

Acoustic and Stroboscopic Characteristics of Normal Person's Voices with Advancing Age (연령증가에 따른 정상 노인의 음향분석학적 특징)

  • Anatomic and physiological changes of the larynx with advancing age result in morphologic changes of the vocal fold and reduced control of the phonatory mechanism in elderly individuals and are reflected in increased unstability of fundamental frequency (Fo). The purpose of this study is to increase current understanding of acoustic and stroboscopic characteristics of normal elderly persons voices. First, phonated /a/ vowel productions by 40 normal adults (20 to 40 years, 20 men and 20 women) and 40 normal elderly persons (60 to 80 years,20 men and 20 women) were analyzed, using CSL (model 4300B) acoustic analysis software, to obtain acoustic measures related to fundamental frequency stability nd vocal resonance characteristics. Second, stroboscopic images of the vocal fold behavior in all subjects were analyzed by experienced specialists. In the men, fundamental frequency variation (vFe) (p<0.01), jitter. (p<0.05), and shimmer (p<0.05) for the older group were significantly higher than the value for the adult group. In the stroboscopic findings, edema of vocal fold had a significant finding in aged men (15%). In the women, vFo (p<0.05), jitter (p<0.05), and noise to harmonic ratio (NHR) (p<0.05) for the older group were significantly higher than the value for e adult group and first formant frequency (F1) (p<0.01) and second formant frequency (F2) (p<0.01) for. the older group were significantly lower than the value for the adult group. In the stroboscopic findings, vocal fold atrophy had a significant finding in aged women (25%). Frequency stability, as reflected by vFo, jitter, shimmer, and NHR, decreases with advancing age in men and women and spectral analysis of phonated /a/ vowel productions reveals the lowering of the frequency of F1 and second F2 with advancing age, especially in aged women. Change in the mass of vocal folds, due to atrophy or edema, is considered to be the greatest factor in these acoustic changes.

Acoustic parameter delta of an aspirated voice in stroke patients (뇌졸중 환자 대상 흡인 음성의 음향변수 변동)

  • The present study aimed to investigate the changes of acoustic parameters of the aspirated voice in stroke patients. The eighty-eight subjects diagnosed with cerebro-vascular accident were divided into 32 penetration/aspiration (P/A) and 56 Non-P/A groups according to the videofluroscopic swallowing study (VFSS) results, and 26 control subjects participated. All subjects preformed VFSS and vowel /a/ was recorded three times pre- and post VFSS. Since the variation in the acoustic parameters within a single phonation has been observed, we proposed a delta formula for the acoustic parameters which can reflect the temporal changes of the each parameter in an utterance. We measured from the voice data eight acoustic parameters: fundamental frequency (F0), standard deviation of F0 (F0_SD), Jitter, relative average perturbation (RAP), Shimmer, amplitude perturbation quotient (APQ), harmonic to noise ration (HNR), noise to harmonic ratio (NHR). Then we found parameters which show the meaningful biggest temporal change in an utterance using the suggested delta parameter. Among them, the deltas of shimmer and APQ were significantly different pre- and post VFSS. These deltas of the P/A and the control group were increased after VFSS, while those of the Non-P/A group was descended. The variation patterns of the P/A and the control group were similar but the change width of the P/A group was larger. The large variations in an aspirated phonation of the P/A group are thought to be caused by irregular changes in air resistance due to residual food on the vocal cords.
