갑상선 종양에서 RASSF1A 메틸화와 BRAF 유전자 변이에 관한 연구 (Relation between RASSF1A Methylation and BRAF Mutation in Thyroid Tumor)

  • Background and Objectives: Hypermethylation of the tumor suppressor gene RASSF1A and activating mutation of BRAF gene have been recently reported in thyroid cancers. To investigate the role of these two epigenetic and genetic alterations in thyroid tumor progression, methylation of RASSF1A and BRAF mutation were examined in thyroid tumors. Materials and Methods: During 2007 to 2017, 69 papillary carcinomas, 18 nodular hyperplasia, 3 follicular carcinomas, and 13 follicular adenomas were selected. The methylation-specific polymerase chain reaction (MSP) technique was used in detecting RASSF1A methylation and polymerase chain reaction (PCR)-single-stranded conformation polymorphism and sequencing were used for BRAF gene mutation study. Results: The hypermethylation of the RASSF1A gene was found in 84.6%, 100% and 57.9% of follicular adenomas, follicular carcinomas, and papillary carcinomas, respectively. Nodular hyperplasia showed a hypermethylation in 33.3%. The BRAF mutation at V600E was found in 60.7% of papillary carcinoma and 27.0% of nodular hyperplasia, but none of follicular neoplasms. The BRAF mutation was correlated with the lymph node metastasis and MACIS clinical stage. There is an inverse correlation between RASSF1A methylation and BRAF mutation in thyroid lesions. Conclusion: Epigenetic inactivation of RASSF1A through aberrant methylation is considered to be an early step in thyroid tumorigenesis, and the BRAF mutation plays an important role in the carcinogenesis of papillary carcinoma, providing a genetic marker.

  • In order to know morphological changes on the female genital organs by Ivermectin(IVM) administration, the histopathological observation was carried out in the organs of rat and mouse treated with the overdose of IVM. In the microscopical findings of the uterus, there were many mitotic figures, epithelial hyperplasia and papillary foldings in the endometrial surface. The increased prevalance of uterine glands, uterine epithelia and glands hyperplasia were markedly presented on diverse patterns adenoma-like structure and single nodular or multiple polyp-like adenoma. In ovary, primary and mature follicles were decreased in number, and hypoplasia of ovarian follicles, atretic follicles, follicular cysts and ovarian atropy were observed. It was considered that IVM administration resulted in follicular hypoplasia and atropy of ovary, and hyperplasia of uterine gland and endometrial surface epithelium might be transformed to neoplasia of glandular structures.

Development of multifocal nodular lesions of a liver mimicking hepatic metastasis, following resection of an insulinoma in a child

  • Insulinoma, which arises from insulin-producing pancreatic beta cells, is a rare tumor in children. Only 5%-10% of insulinomas are malignant and undergo metastasis. We report a case of an 11-year-old girl who experienced hypoglycemia-related seizures induced by an insulinoma; after resection of the primary tumor, she developed hepatic focal nodular hyperplasia (FNH). Laboratory test results indicated marked hypoglycemia with hyperinsulinemia. Abdominal ultrasonography (US) and computed tomography results were normal; however, magnetic resonance imaging (MRI) showed a solid mass in the pancreatic tail. Therefore, laparoscopic distal pancreatectomy was performed. Two months after the surgery, an abdominal MRI revealed multiple nodular lesions in the liver. An US-guided liver biopsy was then performed, and histological examination revealed FNH without necrosis or mitotic activity. The patient has been free of hypoglycemia for 2 years, and recent MRI studies showed a decrease in the size of FNH lesions, without any evidence of metastasis. Even though no metastatic lesions are noted on imaging, close observation and follow-up imaging studies are required in a child with insulinoma that has malignant potential on histopathologic findings.

  • 호산구 증가증을 동반한 ANGIOLYMPHOID HYPERPLASIA는 주로 두경부의 표층에 발생되고 진단에 어려움이 있는 특이한 질환이다. 일본에서 처음 보고된 이래 주로 동양에서 종종 보고되어온 KIMUR'S DISEASE와 많은 논란이 되고 있다. 저자등은 좌측 안면부에 거대한 유동성의 종괴를 주소로 환자로서 술전 진단에 상당한 어려움이 있었고 술후 최종적으로 ALHE로 진단된 32세 여자환자를 문헌고찰과 더불어 보고하는 바이다.

Multifocal nodular lymphoid hyperplasia of the lung

  • Nodular lymphoid hyperplasia (NLH) is a benign lymphoproliferative disease that can affect the lung. Because of its rarity, little is known about the etiology and natural history of NLH. Most cases are usually asymptomatic and found incidentally on imaging studies. Imaging finding of NLH has shown most commonly as a solitary lesion, although multifocal pulmonary nodules may be seen. Surgical resection has proved curative in the cases previously described. We report a rare case of NLH in a 55 year-old man who presented with bilateral multiple pulmonary nodules on chest radiography. Open biopsy was performed from the upper and lower lobe of the left lung. The lesions were pathologically diagnosed as pulmonary NLH. Multifocal residual nodules in both lungs remain stable without spontaneous regression during the 3 years of follow-up.

  • 47세 여자 환자가 이전부터 있던 흉골상 절흔부에 종괴를 제거하기 위해 내원하였다. 흉부 CT에서 종괴는 상전종격동에 위치하고 있었다. 경부 절개 후에 종괴를 제거하였고 병리학적 검사상 종괴는 결절성 과형성(nodular hyperplasia)을 함유한 이소성 갑상선 조직으로 진단되었다. 저자들은 드문 종격동 이소성 값상선 조직을 치험하였기에 보고하는 바이다.

  • We present two cases of surgically proven focal nodular hyperplasia whou underwent tri contrast-enhance MR imaging using gadolinium chelates, mangafodipir trisodium, and ferumoxides After the unehanced MR images were obtained, dynamic gadolinium-enhanced T1-weighted imagi were performed, then mangafodipir enhanced and ferumoxides-enhanced images were obtained. In one case, the mass was isointense on both T1- and T2-weighted images on the unehanced M images, iso and slightly hyperintense on ferumoxides-enhanced FSE and GRE images, strong hyperintense on the mangafodipir enhanced and gadolinium enhanced arterial phase images. In th other case, the mass was isointense on T2-weighted and hypointense on T1-weighted image isointense on ferumoxides-enhanced images, and hyperintense on mangafodipir enhanced an gadolinium enhanced arterial phase images. Triple contrast enhanced MR images were useful correctly diagnose these two cases preoperatively.

  • Majority of hepatocellular carcinoma is evolved from a well differentiated cancerous condition such as hyperplastic lesions eg. adenomatous hyperplasia in cirrhotic liver or de no vo carcinogenesis and prolifenation along with dedifferentiation. Adenomatous hyperplasia is may be seen in severe acute hepatic injury, like submassive hepatic necrosis, or in chronic liver diseases, particularly liver cirrhosis and it has recently attracted much interest from both clinicians and pathologists because it is regarded as a precursor lesion of hepatocellular carcinoma. Hepatic adenomatous hyperplasia resembling focal nodular hyperplasia might have developed from localized vascular changes associated with chronic liver disease, pre-existing arterial malformation and early stage of angiogenesis in hepatocarcinogenesis. We present a patient who developed hepatocellular carcinoma after hepatic artery ligation.

전립선비대증 초음파 영상에서 GLCM을 이용한 컴퓨터보조진단의 영상분석 (Image Analysis of Computer Aided Diagnosis using Gray Level Co-occurrence Matrix in the Ultrasonography for Benign Prostate Hyperplasia)

  • 전립선 초음파영상은 전립선암, 전립선비대증, 전립선염을 진단하고 전립선암의 생검과 전립선비대에서 전립선 크기 확인 등을 위해서 사용된다. 전립선비대증은 노인 남성의 가장 흔한 질병 중의 하나이다. 전립선은 주변구역, 중심구역, 이행구역과 전방 섬유근 간질부분 4개 구획으로 나누어진다. 전립선비대증은 조직학적으로 전립선 이행구역에서 결절성 증식을 동반한 요도주위의 진행성 과증식이 특징으로 이 결절로 인한 요도 폐쇄를 야기함에 따라 하부요로 증상을 유발한다. 그러므로 본 연구에서는 정상 전립선 이행구역 영상과 전립선비대 이행구역 영상에 대한 컴퓨터 알고리즘을 이용하여 정량적인 분석을 하였다. GLCM을 적용하여 정상영상 60증례와 전립선비대증영상 60증례을 분석영역($50{\times}50$ 픽셀)으로 설정하고, 각 영상에서 Autocorrelation, Contrast, Cluster Prominence, Entropy, Max Probability, Sum average 6가지 파라미터를 비교하여 분석하였다. 결과적으로 Autocorrelation, Cluster Prominence, Entropy, Sum Average 4개의 파라미터에서는 병변의 질감 검출 효율이 92-98%로 높게 나왔다. 이에 전립선 이행구역의 결절성 증식 변화를 정량적인 영상분석으로 확인 할 수 있었다. 향후 전립선비대증 진단에 있어 2차적인 수단으로 가능할 것으로 기대되며, 다양한 전립선 초음파 영상에 있어 기초 자료가 될 것으로 사료된다.

Intravascular Papillary Endothelial Hyperplasia (Masson tumor) of the Skull : Case Report and Literature Review

  • A 10-year-old female patient presented with a rapidly growing nodular mass lesion on her right frontal area. On skull radiography and computed tomography (CT) imaging, this mass had a well-demarcated punch-out lesion with a transdiploic, exophytic soft tissue mass nodule on the frontal scalp. Magnetic resonance (MR) imaging revealed the presence of a $1.5{\times}1.2{\times}1$ cm sized calvarial lesion. This lesion was hypointense on T1 and heterogenous hyperintense on T2 weighted MR images, and exhibited heterogeneous enhancement of the soft tissue filling the punch-out lesion after intravenous administration of gadolinium. En block removal of the tumor with resection of the rim of the normal bone was performed. The pathological diagnosis was intravascular papillary endothelial hyperplasia (IPEH). After surgery, no recurrence was found for 8 months. IPEH is a rare and benign reactive lesion usually found in thrombosed subcutaneous blood vessels. Involvement of skull bone is rare. In this article, we present a case of IPEH involving the calvarium, in a 10-year-old woman.
