Iron status in small for gestational age and appropriate for gestational age infants at birth

  • Purpose: This study compared the iron statuses of small for gestational age (SGA) and appropriate for gestational age (AGA) infants at birth. Methods: The clinical data of 904 newborn infants admitted to the neonatal intensive care unit were reviewed. Blood samples were drawn from the infants within 24 hours after birth. Serum ferritin level was used as a marker of total iron status. Results: In this study, 115 SGA (GA, $36.5{\pm}2.9weeks$; birth weight [BW], $1,975{\pm}594.5g$) and 717 AGA (GA, $35.1{\pm}3.5weeks$; BW, $2,420.3{\pm}768.7g$) infants were included. The SGA infants had higher hematocrit levels ($50.6%{\pm}5.8%$ vs. $47.7%{\pm}5.7%$, P<0.05) than the AGA infants. No difference in serum ferritin level (ng/mL) was found between the groups (mean [95% confidence interval]: SGA vs. AGA infants, 139.0 [70.0-237.0] vs. 141.0 [82.5-228.5]). After adjusting for gestational age, the SGA infants had lower ferritin levels (147.1 ng/mL [116.3-178.0 ng/mL] vs. 189.4 ng/mL [178.0-200.8 ng/mL], P<0.05). Total body iron stores were also lower in the SGA infants than in the AGA infants (185.6 [153.4-211.7] vs 202.2 [168.7-241.9], P<0.05). Conclusion: The SGA infants had lower ferritin and total body iron stores than the AGA infants. The SGA infants affected by maternal hypertension who were born at late preterm had an additional risk of inadequate iron store. Iron deficiency should be monitored in these infants during follow-up.

Causes of Transfer of Neonates (Born after ≥34 Weeks of Gestation) to the Neonatal Intensive Care Unit Owing to Respiratory Distress and their Clinical Features

  • Purpose: Respiratory morbidity is the most common problem among neonates admitted to neonatal intensive care units. Therefore, the aim of this study was to make a differential diagnosis between transient tachypnea of the newborn (TTN), respiratory distress syndrome (RDS), and pneumonia through comparison of clinical features and test results. Methods: This retrospective study was conducted in 86 infants with TTN, RDS, or pneumonia. These were infants who had respiratory distress, were born after ${\geq}34$ weeks of gestation, and transferred to the neonatal intensive care unit of Kosin University Gospel Hospital between June 1, 2011 and June 30, 2016. Results: The numbers (percentage) of infants with TTN, RDS, and pneumonia were 51 (59.3%), 20 (23.3%), and 15 (17.4%), respectively. Late-preterm and early-term newborns accounted for 65.1% of the infants. Tachypnea was observed in 74.4% of the neonates. The median age at admission was 4 hours (0 to 116) after birth. The infants with RDS had significantly lower birth weights, pH levels, base excess and oxygen saturation levels at admission, longer duration of total ventilator therapy, and hospital stay than those in the other two groups. The infants with pneumonia showed significantly high initial high-sensitivity C-reactive protein levels and significant chest radiographic findings. Conclusion: Early differential diagnosis for TTN, RDS, and pneumonia is challenging because they show similar respiratory symptoms at an early stage. Clinical features and test results can be used to determine the etiology of respiratory distress and early antibiotic treatment.

Effects of Sensory Stimulation Therapy on the Improvement of Growth and Behavioral Development for Instituted Infants (감각자극 요법이 시설 영아의 신체 및 행동발달에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effectiveness of sensory stimulation therapy on the physical growth and behaviors of infants accommodated at two Korean orphanages. Method: Thirty-eight full term infants were assigned to a control (n=20) or an experimental (n=18) group within 14 dys of birth. In addition to routine orphanage care, infants in the experimental group received 15 minutes of massage twice a day, 5 days a week for 4 weeks. Result: Compared to the control group, the experimental group had gained significantly more weight and had larger increases in length and head circumference after the 4-week intervention period. In addition, the experimental group showed significantly better behavioral performance in the scoring of habituation (e.g., light), motor (e.g., motor maturity), and state range (e.g., peak of excitement, rapidity of build-up) behavioral clusters of Brazelton's NBAS. Conclusion: These data demonstrate that sensory stimulation therapy may facilitate the physical and behavioral development of the newborn infants placed in the orphanages.

Acoustic and Physiologic Characteristics of Newborn Infants' Communication Intent via Crying (신생아 울음의 의사소통 의도와 관련된 음향학적 특성)

  • The purpose of this study was to investigate the acoustic characteristics of crying infants according to the communication intents such as hunger and pain in terms of acoustic differences in the fundamental frequency ($F_0$), jitter, shimmer, noise-to-harmonic ratio(NHR), habitual pitch, and intensity. The subjects were 20 healthy, normal infants, less than seven days old, from the city of Seoul and were born after 38 to 42 weeks(full term) of pregnancy. The sound of crying was recorded for three minutes. The crying due to pain was induced by means of the inborn metabolism error test, whereas the crying due to hunger was verified by means of the rooting reflex by waiting for the designated eating time. The results were as follows: (1) the fundamental frequency, noise-to-harmonic ratio(NHR), and intensity of the infants' crying due to pain was higher than that by hunger, showing a significant difference between the mean values. (2) the infants' crying due to hunger and that by pain did not have a significant difference in the mean jitter and shimmer values but both of them were largely outside of the normal threshold values(jitter by 1.04% and shimmer by 3.81%). This study was significant in the sense that it showed the acoustic characteristics of infants' crying from hunger and pain were very different from each other according to the communication intents in terms of the six acoustic parameters.

Epidural hematoma treated by aspiration of accompanying cephalhematoma in a newborn infant (신생아에서 동반된 두혈종 흡인으로 치료된 경막외출혈 1례)

  • Epidural hematoma is relatively rare in newborn infants and frequently associated with instrumental deliveries or other complications during labor and delivery. Although surgical evacuation has been the most common therapy, many other procedures have been described. Although many epidural hematomas require surgical evacuation rather than nonsurgical management, conservative or aspiration of hematoma have been attempted. In the case of EDH associated with cephalhematoma, aspiration of cephalhematoma could be attempted because frequent features of these combination were communication between these hematoma. We report a case of successful nonsurgical management of epidural hematoma through the aspiration of accompanying cephalhematoma in a 5-day-old newborn infant.

Maternal Nutritional Status at the End of Pregnancy, and Correlation among Pregnancy Weight Gain, Birth Weight and Serum Leptin Levels (산모의 임신말기 영양상태와 임신 중 체중증가, 출생체중과 혈청 렙틴 농도와의 상관관계에 관한 연구)

  • The necessity of adequate pregnancy weight gain for optimal pregnancy outcome has been recognized. However, the specific components of pregnancy weight gain that might be critical for fetal growth and development have not been elucidated clearly. The purpose of this study was to investigate the correlation of pregnancy weight gain and birth weight with serum leptin levels in women delivered newborns. The subjects were recruited from K university hospital. The subject's characteristic data (age $32.1\;{\pm}\;4.3\;y$, gestational age $39.5\;{\pm}\;1.1wk$, pre-pregnancy weight $58.0\;{\pm}\;8.6\;kg$, pregnancy weight gain $12.7\;{\pm}\;5.5\;kg$, newborn's birth weight $3.5\;{\pm}\;0.5\;kg$) were gathered. Maternal dietary assessment was carried out at the end of pregnancy. After delivery, blood samples were collected from 20 mother-newborn pairs. Serum levels of various lipids and leptin were analyzed. Maternal daily consumption of iron, zinc, folate were lower than the RDA of each nutrient and index of nutritional quality was less than 1 showing that the quality of maternal diet was low. The levels of serum leptin of mothers and infants were $10.2\;{\pm}\;6.7\;ng/ml$ and $1.7\;{\pm}\;0.6\;ng/ml$, respectively. The serum leptin concentrations of male infants $(1.9\;{\pm}\;0.7\;ng\;ml)$ were not different from that of females $(1.7{\pm}0.5\;ng/ml)$. A negative correlation was found between the maternal pre-pregnant BMI and weight gain during pregnancy (r = -0.54, p < 0.05). There was a positive correlation between the pregnancy weight gain and the newborn's birth weight (r=0.59, p < 0.01 There were also positive correlation between newborn's birth weight and newborn's serum leptin levels (r = 0.57, p < 0.01). No correlations were found between maternal serum leptin levels and that of newborn's. Efforts should be made to attain adequate diet and weight gain during the pregnancy to reduce the likelihood of low or over birth weight of newborns.

A Diagnostic Algorithm after Newborn Screening for 21-hydroxylase Deficiency (선천성 부신 과형성증(21-hydroxylase 결핍)의 신생아 선별 검사 후 진단 알고리즘)

  • 21-hydroxylase deficiency (21-OHD), most common form of congenial adrenal hyperplasia, is categorized into classical forms, including the salt-wasting (SW) and the simple virilizing (SV) types, and nonclassical (NC) forms based on the severity of the disease. Newborn screening for 21-OHD has been performed in Korea since 2006. $17{\alpha}$-hydroxyprogesterone (17-OHP) is a marker for 21-OHD and is measured using a radioimmunoassay or a fluoroimmunoassay. Premature and low birth weight infants are likely to give false positive 17-OHP findings, therefore, cutoff values for these infants should be determined based on gestational weeks or birth weight. ACTH simulation test is helpful when the 17-OHP shows equivocal increase, and it is gold standard for diagnosis of NC type. Recently, liquid chromatography linked with tandem mass spectrometry was developed for rapid, highly specific, and sensitive analysis of multiple analytes. Molecular analysis of CYP21A2 is useful for confirming diagnosis of mild SV or NC type, predicting prognoses, and genetic counseling. In order to make newborn screening for 21-OHD more efficient, early detection of boy with SW type, early determination of girl with ambiguous genitalia, detection of NC type, and overcoming of false positive in premature and low birth weight infants should be considered. Above all, early treatment should be started when the patient is suspected as having 21- OHD clinically before confirming the diagnosis to prevent adrenal crisis. Here, author reviewed recent articles of guideline and proposed guideline for 21-OHD.

Effect of Occupational Activities of Mother to Fetus, Newborn Infant, and Delivery Methode (산모의 직업활동이 태아, 신생아 및 분만방법에 미치는 영향)

  • This study compared and analyzed fetus, newborn infant, and delivery methode of 141 women who received medical service from the first examination to the delivery in an obstetrics and gynecology specialized hospital located in Gwangmyeong, Gyeonggi-do from Jan. 2010 to Dec. 2010 and had a regular delivery after 37 weeks of pregnancy (73 non-employed and 68 employed women), depending on employment state of the subjects. The following are the results derived from the study. 1. The weight of the mothers after 37 weeks of pregnancy and the average gestational age was no statistical difference depending on the employment state. 2. In 37 weeks, the average fetal weight in the non-employed group was $2.90{\pm}0.27kg$ and the rate of small for gestational age infants was 2.75%. Meanwhile, the average fetal weight in the employed group was $2.76{\pm}0.28kg$ and the rate of small for gestational age infants was shown to be 10.3%. It is known that the fetal weight of the non-employed group was significantly higher for 37 weeks of pregnancy (p<0.05). 3. The newborn infant weight of the non-employed group was $3.42{\pm}0.43kg$ in average while that of the employed group was $3.18{\pm}0.35kg$. It is known that the newborn infant weight of the non-employed group was significantly higher (p<0.05). 4. In terms of delivery method, cesarean constituted 32.9% in the non-employed group while constituting 16.2% for the employed group. It is known that cesarean was shown to be significantly higher among the non-employed group (p<0.05).

A Study of the Maternal Attachment Behavior During Early Postpartum Period (산모의 신생아에 대한 애착행위에 관한 연구)

  • The purpose of this study was to observe and describe the maternal attachment behavior during the mother's first interactions with her newborn and define the factors contributing to differences in maternal attachment behavior. Observations of the mother's first interaction with her newborn can offer valuable information about the mother-infant relationship, and it provides an opportunity for planning individulized care for them. Data was collected from Sep. 1, 1950 to Oct.30, 1980 at Ewha Womans University Hospital. Maternal attachment behaviors of healthy full-term infants were observed and recorded on the .maternal attachment tool and analysed by the use of means, standard deviations and ANEVA test. The following trends of maternal behaviors were observed: 1. Identifying behaviors was the predominant mode and all of the mothers inspects baby's body features. 2. Modalities of interaction, that is, touching was initiated on the babies extremities and heads (57.3%) rather than the trunks (8.7%) and mothers touched their infants with their fingertips (58.2%) more than palm touching (14.6%) 3. Care-taking activities performed by the mother were negligible at the first interaction. 4. Parity of mother, sex of infant, age of mother, planned pregnancy, length of visits by mother to infant appeared to have significant influence on the maternal attachment behaviors.

The Acute Intermittent Peritoneal Dialysis in Acute Renal Failure of Newborn and Young Infants (신생아와 유아의 급성신부전증에 실시한 급성복막투석)

  • We studied the effects of the acute intermittent peritoneal dialysis in severe acute renal failure of 1 newborn infant and 2 young infants during 18 months period from February 1985 to April 1986. The predisposing illnesses were severe acute gastroenteritis with dehydration. Reye's syndrome, and bilateral nephrolithiasis with hyperuricemia. The concomittent illnesses were severe hypernatremia, hyponatremia, hyperkalemia, hypocalcemia, hypoglycemia, DIC(disseminated intravascular coagulopathy), paralytic ileus, metabolic acidosis and gastrointestinal bleeding. As a dialvsate, Imperinol $solution^R$, 1.5% was used in all cases. The cycles of dialysis were 8, 16, and 41 times in each cases. Observed complications during dialysis were leakage, and abdominal wall and scrotol swelling in 2 cases, hyperglycemia in 1 case, and peritonitis in 1 case. Acinetobacter calcoaceticus was cultured in peritoneal fluid of peritonitis. These complications were treated by stopping dialysis in leakage and abdiminal wall swelling, insulin therapy in hyperglycemia, and intraperitoneal and systemic antibiotics therapy in peritonitis. We experienced improvements of severe acute renal failure with variable concomittant illnesses by acute intermittent peritoneal dialysis despite of the treatable complications of dialysis in all cases.
