세포교정영양요법(OCNT)을 이용한 대상포진 후 신경통 개선 사례 (Improvement of Symptoms in Patients with Postherpetic Neuralgia Using Ortho-Cellular Nutrition Therapy)

  • Objective: To improve patients' postherpetic neuralgia (PHN) symptoms using Ortho-Cellular Nutrition Therapy (OCNT). Methods: A Korean woman in her forties suffering from neuralgia due to herpes zoster was treated with OCNT for approximately six months. Results: After initiating OCNT, the patient gradually experienced reduced neuralgia symptoms in the chest, flank, and back areas. Approximately six months later, she no longer felt discomfort from these symptoms. Conclusion: OCNT can be beneficial in alleviating symptoms of neuralgia in patients suffering from PHN.

양측성 삼차신경통 -증례 보고- (Bilateral Trigeminal Neuralgia -Four cases-)

  • Bilateral trigeminal neuralgia is uncommon and usually occurs in 1% to 11% of patients with trigeminal neuralgia. There has been no report of a patient with bilateral trigeminal neuralgia in Korea. Based on our review, the incidence of bilateral trigeminal neuralgia among the trigeminal neuralgia patients who visited our pain clinic numbered 0.7% (4 out of 531). This is much lower than those of other previous studies. This lower incidence might be explained by the fact that we excluded those patients with mild bilateral trigeminal neuralgia which did not require treatment and also patients whose symptom on controlateral side was relieved by injection of local anesthetics for that side. Among the four patients with bilateral trigeminal neuralgia, three patients received alcohol block of the branches of trigeminal nerve and one refused the treatment. Overall, three patients maintained good or excellent pain relief without any serious complications throughout the follow-up period.

  • Trigeminal neuralgia is nerve systematic disease appearing in the distribution scope of trigeminal nerve. It's characterized by extreme pain accompanying with a repeated and simultaneous fit from several seconds to 1-2 minutes. The oriental medical name of trigeminal neuralgia is generally Dootong, Doopoong, Myuntong, Pyundootong, Pyundoopoong, and Myuntong is the nearest in Occidental medicine. So I study for general prescription of Trigeminal neuralgia and point out Galgunhaegui-tang(葛根解肌湯). Trigeminal neuralgia patients were given Galgunhaegui-tang. After taking Galgunhaegui-tang, the patients's Trigeminal neuralgia was improved.

Intractable Occipital Neuralgia Caused by an Entrapment in the Semispinalis Capitis

  • Occipital neuralgia is a rare pain syndrome characterized by periodic lancinating pain involving the occipital nerve complex. We present a unique case of entrapment of the greater occipital nerve (GON) within the semispinalis capitis, which was thought to be the cause of occipital neuralgia. A 66-year-old woman with refractory left occipital neuralgia revealed an abnormally low-loop of the left posterior inferior cerebellar artery on the magnetic resonance imaging, suggesting possible vascular compression of the upper cervical roots. During exploration, however, the GON was found to be entrapped at the perforation site of the semispinalis capitis. There was no other compression of the GON or of C1 and C2 dorsal roots in their intracranial course. Postoperatively, the patient experienced almost complete relief of typical neuralgic pain. Although occipital neuralgia has been reported to occur by stretching of the GON by inferior oblique muscle or C1-C2 arthrosis, peripheral compression in the transmuscular course of the GON in the semispinalis capitis as a cause of refractory occipital neuralgia has not been reported and this should be considered when assessing surgical options for refractory occipital neuralgia.

한국인 성인남녀에서 신경통과 혈액정보 및 체형정보와의 연관성 연구 (Study of association of neuralgia with blood parameters and anthropometric indices in Korean adult men and women)

  • 신경통은 중증의 고통을 수반하는 질병으로써 인간의 삶의 질에 매우 강한 영향을 주는 질병이며, 국내에서도 노화에 따라 유병율이 증가하는 추세이다. 그동안 신경통에 대한 국내외연구들은 주로 신경통과 연관성이 있는 정신질환, 인구학정 정보, 영양성분 등 과의 연관성 분석이 대부분이고, 혈액정보와의 연관성 연구는 매우 드물다. 따라서, 본 연구에서는 신경통과 혈액 파라미터들과의 연관성을 도출하고 신경통과 연관성이 있는 임상지표를 도출하고자 한다. 본 연구를 위해 질병관리본부의 국민건강영양조사 데이터를 기반으로 binary logistic regression을 이용하여 분석하였다. 실험결과 남녀 모두에서 나이가 신경통과 가장 연관성이 높은 경향이 있는 것으로 나타났으며, 여성에서는 복부둘레와 헤마토크리트 수치가 신경통과 연관성이 있는 것으로 나타났고, 남성에서는 공복시혈당과 헤모글로빈이 신경통과 연관성이 있는 것으로 나타났다. 또한 복부둘레 및 혈액정보들과 신경통사이에서의 연관성이 남녀에서 차이가 있는 것을 밝혀내었다.

삼차신경통과 반측안면경련에서 CISS 영상의 진단적 유용성 (Diagnostic Usefulness of CISS Image in Preoperative Evaluation of Trigeminal Neuralgia and Hemifacial Spasm)

  • Objectives : Trigeminal neuralgia and hemifacial spasm are caused by vascular compression of the REZ(root entry or exit zone) of the 5th and the 7th cranial nerve. Preoperative detection of neurovascular compression is essential for accurate diagnosis, appropriate treatment, and the good operative results. Three dimensional Fourier Transformation-Constructive Interference in Steady State(3DFT-CISS) images are known to give good contrast between CSF, nerve, and vessels. We applied a 3DFT-CISS imaging technique for the preoperative evaluation of patients with these diseases and estimated the diagnostic accuracy and usefulness of this study. Methods : A series of 71 patients with trigeminal neuralgia and hemifacial spasm were treated by microvascular decompression. Among them 34 patients with trigeminal neuralgia and 24 patients with hemifacial spasm had preoperative CISS images. We compared the radiologic finding with the operative finding, and analysed the diagnostic usefulness of 3DFT-CISS imaging. Results : The sensitivity of CISS images of detecting the neurovascular compression was 90.3% in trigeminal neuralgia and 100% in hemifacial spasm. There were one false-positive and three false-negative cases in trigeminal neuralgia, and one false-positive case in hemifacial spasm. The accuracy in diagnosing the causative vessel was 73.5% in trigeminal neuralgia and 83.3% in hemifacial spasm. Conclusion : CISS image is very useful diagnostic tool for preoperative evaluation of neurovascular compression in patients with trigeminal neuralgia and hemifacial spasm. No additional neuroradiologic examination other than CISS image and MRA is needed for preoperative evaluation of patients with trigeminal neuralgia and hemifacial spasm.

대상포진에 의해 발현된 근위축성 신경통 (Neuralgia Amyotrophy Induced by Herpes Zoster)

  • The etiology of neuralgia amyotrophy remains unclear. Herpes zoster induced neuralgia amyotrophy has been reported in extremely rare cases. In this case report, we describe the clinical features and electrophysiologic findings in a 68-year-old patient with neuralgia amyotrophy associated with herpes zoster infection. We suggest that brachial plexus inflammation due to viral infection may be a direct cause of reversible neuralgia amyotrophy.

  • 1) 에테르-아스피린 국소도포요법은 그 방법이 간단하며 안전하게 사용할 수가 있다. 2) 이 요법은 대상포진성 신경통 환자의 통증을 반감시키나 대상포진후 신경통 환자에서는 그 효과가 입증되지 못했다. 3) 에테르-아스피린 도포는 대상포진의 피부 병변의 빠른 치유에도 도움을 주는 것으로 보인다.

Percutaneous Radiofrequency Thermocoagulation in Trigeminal Neuralgia : Analysis of Early and Late Outcomes of 156 Cases and 209 Interventions

  • Objective : Trigeminal neuralgia is one of the most common causes of facial pain. Our aim is to investigate the efficacy and borders of percutaneous radiofrequency thermocoagulation in the treatment of trigeminal neuralgia. Methods : Between May 2007 and April 2017, 156 patients with trigeminal neuralgia were treated with radiofrequency thermocoagulation. These 156 patients underwent 209 procedures. In our study, we investigated the early and late results of percutaneous radiofrequency thermocoagulation under guiding fluoroscopic imaging in the treatment of trigeminal neuralgia. Barrow Neurological Institute (BNI) pain scale was used for grading the early results. In addition, Kaplan-Meier survival analysis was used to assess long-term outcomes. Of the 156 patients who underwent radiofrequency thermocoagulation for trigeminal neuralgia, 45 had additional disease. Patients with this condition were evaluated with their comorbidities. Early and late results were compared with those without comorbidity. Results : In 193 of 209 interventions BNI pain scale I to III results were obtained. Out of the 193 successful operation 136 patients (65.07%) were discharged as BNI I, 14 (6.70%) as BNI II, 43 (20.58%) as BNI III. Sixteen patients (7.65%) remained uncontrolled (BNI IV and V). While the treatment results of trigeminal neuralgia patients with comorbidity seem more successful in the early period, this difference was not observed in follow-up examinations. Conclusion : Finally, we concluded that percutaneous radiofrequency thermocoagulation of the Gasserian ganglion is a safe and effective method in the treatment of trigeminal neuralgia. However, over time, the effectiveness of the treatment decreases. Neverthless, the reapprability of this intervention gives it a distinct advantage.

Stem cells from human exfoliated deciduous teeth attenuate trigeminal neuralgia in rats by inhibiting endoplasmic reticulum stress

  • Background: The treatment of trigeminal neuralgia remains a challenging issue. Stem cells from human exfoliated deciduous teeth (SHED) provide optimized therapy for chronic pain. This study aimed to investigate the mechanisms underlying the attenuation of trigeminal neuralgia by SHED. Methods: Trigeminal neuralgia was induced by chronic constriction injury of the infraorbital nerve. The mechanical threshold was assessed after model establishment and local SHED transplantation. Endoplasmic reticulum (ER) morphology and Caspase12 expression in trigeminal ganglion (TG) was evaluated as well. BiP expression was observed in PC12 cells induced by tunicamycin. Results: The local transplantation of SHED could relieve trigeminal neuralgia in rats. Further, transmission electron microscopy revealed swelling of the ER in rats with trigeminal neuralgia. Moreover, SHED inhibited the tunicamycin-induced up-regulated expression of BiP mRNA and protein in vitro. Additionally, SHED decreased the up-regulated expression of Caspase12 mRNA and protein in the TG of rats caused by trigeminal neuralgia after chronic constriction injury of the infraorbital nerve mode. Conclusions: This findings demonstrated that SHED could alleviate pain by relieving ER stress which provide potential basic evidence for clinical pain treatment.
