목빗근에 대한 허혈성 압박법이 위등세모근의 만성 통증에 미치는 영향 (Effect of Ischemic Compression on Sternomastoidcleido Muscle on Chronic pain in Upper Trapezius Muscle)

  • 최근 스마트폰 사용 등과 같은 이유로 잘못된 자세가 발생하고 있다. 이로 인하여 근육들의 불균형 단축이 발생하고 있는데 특히 목빗근이 단축되면 더부신경이 압박되어 위등세모근의 허혈성 통증을 일으킬 수 있다. 그리하여 본 연구는 목빗근 1/3지점의 더부신경 포착점에 허혈성 압박법을 중재하여 위등세모근의 통증에 미치는 영향을 알아보았다. 본 연구는 여성 8명을 대상으로 4주간 주 2회 중재하였고, 중재 전, 후 그리고 중재 종료 3주 후 평가하였다. 대상자들의 통증을 알아보기 위해 시각적 사상 척도와 맥길 통증 설문지 그리고 압력 통각계를 사용하였다. 본 연구 결과 치료 후 통증이 유의하게 감소한 것을 확인하였으며, 중재 종료 3주 후 역시 통증 감소에 효과가 있는 것을 확인하여 목빗근의 더부신경 압박점의 중재가 위등세모근의 통증 감소와 지속성이 있음을 확인하였다. 차후 연구에서 대상자를 늘려 위등세모근의 통증 감소뿐만 아니라 목의 기능개선도 확인할 필요성이 있을 것이다.

  • Oral prostaglandin E1 (PGE1) is a medicine that is clinically applied during a treatment of patients suffering with vascular disease with chronic arterial obstruction because it has vasodilation and anti-platelet effects. The mechanisms of lumbosacral symptoms associated with spinal stenosis probably include vascular insufficiency with hypoxic injury to the cauda equina and the nerve roots. Thus, increasing the blood supply would be beneficial to improve the pathophysiologic condition. Several studies on the improvement of clinical symptoms of spinal stenosis by PGE1 treatment have been reported on. In this case, 47-year old female underwent posterior compression and posterolateral fusion with a cage at L2-4 due to L3 compression fracture, and she did not show improvement of the radiating pain of her right leg after the operation. Therefore, she received repetitive epidural catheterization and adhesiolysis, epidural block and physical therapy, but her symptoms deteriorated after temporary improvement. Finally, she was given PGE1 and the radiculopathy was completely improved, although some muscle weakness still remained.

추궁 절제술 후의 요하지통에 대한 경막외 Steroid 주입요법 (Epidural Steroid Therapy as a Treatment of Post-laminectomy Low Back Pain)

  • Epidural steroid therapy has been well-established for the treatment of sciatica and low back pain. Disappointing results following surgical decompression or discectomy pain owing for to nerve root compression have led to trials of corticosteroids injected either systemically or into the intrathecal or epidural space to treat intervertebral disc. Epidural steroid is less effective in the patients with low back pain who have a history of surgical operation, so that the use of epidural morphine and methylprednisolone has been advocated for the amelioration of chronic low back pain in the post-laminectomy pain("failed back") syndrome over the past several years. We treated 47 patients with low back pain who had a history of one or two surgical procedures. We concluded that epidural steroid therapy is less effective in the patients with "failed back" syndrome than in the virgin back furthermore, there is a greater risk of complication such as inadvertent dural puncture and corresponding motor paralysis and headache.

Does Aging Matter? The Efficacy of Carpal Tunnel Release in the Elderly

  • Open release remains the gold standard in the treatment of carpal tunnel syndrome in cases where conservative treatment fails. However, the efficacy of carpal tunnel release in the elderly has been debated in the literature throughout the years. This review aims to review the current evidence pertaining to the efficacy of carpal tunnel release in the elderly. Based on the current evidence, the outcome of carpal tunnel release is unpredictable in the elderly. Elderly patients are also less satisfied with the operation compared to younger patients. The authors recommend that these messages be conveyed to elderly patients before surgery. Moreover, open carpal tunnel release should be offered in the early stages of treatment whenever operative management is indicated.

  • Background: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy caused by compression of the median nerve beneath the transverse carpal ligament. CTS can be correctly diagnosed by the patients' description of symptoms and electrophysiological tests that measure nerve conduction through the wrist. Many previous studies reported various risk factors of CTS, such as obesity, diabetes mellitus, thyroid disease and trauma. Obesity is associated with both hyperlipidemia and CTS. This study focused on the relationship between severity of CTS and serum lipid level. Methods: One hundred fourteen patients with CTS and 74 controls were divided into four groups according to the severity; normal, mild, moderate and severe. And then serum total cholesterol (TC), triglyceride (TG), low-density lipoprotein (LDL-C) and high-density lipoprotein (HDL-C) were measured in each group. Results: There was a positive correlation between TG and CTS severity (p<0.001). But TC, LDL-C and HDL-C were not correlated with CTS severity. Conclusions: These results suggest that high serum TG may act as an aggravating factor of CTS.

포착신경병증의 약침치료에 대한 국내 임상 연구 동향: 주제범위 문헌고찰 (Domestic Clinical Research Trends of Pharmacopuncture Treatment for Nerve Entrapment Syndroeme: A Scoping Review)

  • Objectives The purpose of this study is to check the research trends of pharmacopuncture treatment in nerve entrapment syndrome, identify specific techniques, identify which pharmacopuncture are used, and provide directions for future research. Methods This study was conducted based on the five steps suggested by Arksey and O'Malley. We searched five domestic databases (Research Information Sharing Service, Oriental Medicine Advanced Searching Integrated System, Korean studies Information Service System, Science ON, and KMBASE) and identified studies with key search terms like "nerve entrapment" And "pharmacopuncture" until June 23, 2023. Results Twenty-nine studies were finally selected. among them, 25 papers were non-comparative studies (86.2%). The most common disease was carpal tube syndrome (n=10). All the investigated studies were treated by injecting pharmacopuncture into the pathway of the entraped nerve. The depth of pharmacopuncture injection was mentioned only in 13 studies. As for the pharmacopuncture used, sweet bee venom was 8 studies and bee venom was 6 studies, and about half of the pharmacopuncture manufactured with Bee venom as the main component accounted for. Conclusions This study is a scoping review of the pharmacopuncture treatment for nerve entrapment, which was first conducted in Korea. The treatment is mainly performed on the path way of the entraped nerve. After that, it is necessary to study the standardization of the specific technique method of pharmacopuncture and the uniformity of evaluation criteria.

양성(良性) 종격동(縱隔洞) 종진(腫疹)의 외과적(外科的) 치료(治療) (Surgical Treatment of Benign Mediastinal Tumor)

  • This is a report on the cases of benign mediastinal tumors in the Department of Thoracic and Cardiovascular Surgery, Chonnam University Hospital during the period from 1961 to 1975. Age distribution was from 18 to 62 years old with the highest incidence in the 3rd decade, and sex ratio of male to female was 7 : 8. The tumor were classified as follows; 6 cases of teratoma 5 cases of neurogenic tumor one case of pericardial cyst one case of cystic hygroma one case of dermoid cyst one case of bronchogenic cyst. The symptomatic patients were 10 cases (66.7%) and asymptomatic patients were 5 cases (33.3%), who were found incidentally by routine chest n-ray. The symptoms occurred by compression to the adjacent nerve system in 7 cases, by perforation into the lung with infection in one case of teratoma and by infection of bronchogenic cyst in one case and of teratoma in one case. Complications were Pancoast's syndrome including Horner's syndrome 2 cases, middle lobe syndrome 2 cases, intercostal neuralgia 1 case and bronchitis 1 case. All tumors were surgically resectable with good recovery. In all 10 cases of symptomatic patients, their symptoms disappeared dramatically after operation.

양성 종격동종양의 외과적치료에 대한 임상적 고찰 (Clinical Evaluation of Surgical Treatment of Benign Mediastinal Tumors)

  • This is a report on a total of 8 cases of benign mediastinal tumors and cysts in Department of Thoracic Surgery, Chonnam University Hospital during the period from 1961 to 1969. The patients age was distributed between 18 and 38 year old with the highest incidence in the age group of second decade. Sex ratio of male to female was 3: 5. The tumors were classificed as follow; 3 case:, of neurogenic tumors, 2 cases of teratomas, one case of pericardial cyst, one case of cystic hygroma, and one case of brochogenic cyst. The symptomatic patients were 5 cases(62. 6%) and asymptomatic patients were 3 cases(37.5%). The symptomatic patients had the symptoms not referable to their lesion and the mediastinal tumors of asymptomatic patients were incidently found by routine chest X-ray. The Symptoms occurred by compression to adjacent nerve system in 3 cases. by perforation into the lung with infection in one case and by infection of bronchial cyst in one case. The complications were Pancoast's syndrome including Horner's syndrome(2 cases), middle lobe syndrome (one case), bro:1chial infection(one case) and intercostal neuralgia(one case). All tbe tumors were surgicai[y resectable with good recovery postoperatively. In 5 cases of the symptomatic patents, their symptoms were disappeared dramatically after operation.

흉곽 출구 증후군의 치료 (The Treatment of Thoracic Outlet Syndrome)

  • Purpose: As clinical manifestations of thoracic outlet syndrome are vague pain or symptoms in upper extremity, the diagnosis of the disease is delayed or misdiagnosed as cervical HNP, shoulder pathologies, or peripheral neuropathies. In that reason, many patients spend time for unnecessary or ineffective treatments. We report the results of our thoracic outlet syndrome cases, which were treated by conservative care or surgical treatment. Materials & Methods: Twenty five cases, diagnosed as thoracic outlet syndrome since 1999, were reviewed retrospectively. Physical examinations including Adson's and reverse Adson's test, hyperabduction test, costoclavicular maneuver, and Roo's test, plain radiography of shoulder and cervical spine, MRI of neck or brachial plexus, and EMG were checked. If subjective symptoms were not improved after conservative treatments over three months, surgical treatment were performed. Nine patients were performed operative treatment and the others had conservative treatment in outpatient clinic. Postoperative improvement of symptoms and the follow up period, and the results of conservative care were reviewed. Results: Among five physical examinations, mean 1.75 tests were positive, and EMG has little diagnostic value. MRI were performed in twenty cases and compression of brachial plexus were found in 6 cases (30%). Ten patients out of 16 conservative treatment group had excellent improvement of symptoms, and 5 had good results. Eight patients out of 9 operative treatment group had excellent improvement with mean 5.1 months of follow-up period. Conclusion: Diagnosis of thoracic outlet syndrome is difficult due to bizarre and vague symptoms. However if the diagnosis is suspected by careful physical examinations, radiologic studies, or nerve conduction studies, conservative care should be done as initial treatment and at least after three months, reassess the patient's condition. If the results of conservative treatment is not satisfactory and still the thoracic outlet syndrome is suspected, surgical treatment should be considered. Conservative treatment and operative technique are the valuable for the treatment of this disease.

인칭검사를 이용하여 부분 절개를 시행한 수근관 증후군 환자의 추적관찰 (Follow Up Study of Carpal Tunnel Syndrome Patients Underwent Partial Release of Transverse Carpal Ligament Using Inching Test)

  • Purpose: Complete release of the transverse carpal ligament (TCL) is accepted as the standard treatment for carpal tunnel syndrome (CTS). However, loss of grip and pinch power are reported in some patients after complete release of the TCL. This study was designed to evaluate the effectiveness of complete versus partial carpal tunnel release by using the inching technique. Methods: Nineteen patients (a total of 27 hands) who each had a confirmed diagnosis of CTS were selected from September 2002 to February 2003. The cases were divided into three groups(mild, moderate and severe) based on preoperative electrodiagnostic studies. The patients with partial carpal tunnel syndrome were classified into the mild or moderate groups, while patients with complete carpal tunnel syndrome were classified into the moderate or severe groups. Patient oriented data (functional and symptomatic) were collected and electrophysiologic studies were undertaken preoperatively and postoperatively(on the 2nd week, 1st month, 3rd month and 6th month after surgery).Results: In this study, the mild and moderate groups showed both good functional and symptomatic results and improvements in electrophysiologic studies.Conclusion: Carpal tunnel syndrome patients classified into mild or moderate groups based on nerve conduction studies, and whose precise compression sites were pinpointed using the inching technique, can be treated by partial carpal tunnel release.
