특징적인 Warthin-Finkeldey형의 다유핵 거대세포 소견을 보인 기무라 림프절염의 세침흡인 생검소견 - 1예 보고 - (Fine Needle Aspiration Cytology of Kimura's Lymphadenitis with Characteristic Warthin-Finkeldey Type Polykaryocytes - A Case Report -)

  • Kimura's disease is a chronic Inflammatory disorder of unknown etiology, presenting usually as a painless subcutaneous swelling in the head and neck region or in the lymph nodes. We experienced a case of Kimura's lymphadenitis with characteristic Warthin-Finkeldey type polykaryocytes by fine needle aspiration cytology. The patient was a 10-year old male with two enlarged lymph nodes in the postauricular area. Fine needle aspiration cytology from the lymph nodes disclosed hypercellular smears with some scattered eosinophils and polykaryocytes in a polymorphous lymphoid background. There were also fragmented vessel wails and activated endothelial cell clusters in the slightly necrotic background. The Warthin-Finkeldey type polykaryocytes had three to thirty nuclei and prominent nucleoli with cytoplasmic borders. Their nuclei were arranged in grapevine or ring shaped clusters. As these polykaryocytes could also be found in lymph nodes and extranodal tissues of both reactive and neoplastic lymphoid disorders, polykaryocytes themselves are clinically nonspecific. However, the morphologic features of the Warthin-Finkeldey type giant cells are quite different from the foreign body type or Langhans' type giant cells. When the characteristic cytologic features of Kimura's disease such as significant number of eosinophils in a background of lymphoid cells asd proliferation of vessels and endothelial cells are also observed in the smear, it is possible to suggest this diagnosis in the appropriate clinical setting.

Intracranial Plasma Cell Granuloma

  • Plasma cell granuloma is a tumor-like disease characterized by non-neoplastic polyclonal proliferation of plasma cells and other mononuclear cells. This disease occurs most frequently in the lung and upper respiratory tract, while the involvement of the central nervous system is very rare. A 44-year-old female patient presented with nausea and progressive visual disturbance. Brain magnetic resonance imaging (MRI) revealed the mass along the right tentorium with low signal intensity in the T2 weighted image (T2WI) and fluid-attenuated inversion recovery (FLAIR) sequence, and an isosignal intensity in T1 weighted image (T1WI), the latter of which was enhanced after administration of gadolinium-diethylenetriamine penta-acetic acid (Gd-DTPA). The thickest portion of the tentorium was partially excised via the combined suboccipital and infratentorial approach. The histopathological examination indicated a diagnosis of plasma cell granuloma. Postoperative steroid therapy was administered for remnant tumor control. Although a follow up MRI scan taken 20 months after the operation showed a slight decrease in tumor size, the lesion had extended to the falx and left frontal convexity along with parenchymal edema at 32 months after the operation and the clinical status was aggravated. The mass was removed from the left frontal convexity. Radiation therapy was given, together with steroid administration.

Large Sized Common Iliac Artery Aneurysm with Thrombus Developing a Diagnostic Confusion in a Patient with Sciatica

  • The causes of sciatica are variable and include musculoskeletal, dermatologic, infectious, neoplastic, and vascular disorders. In many cases, the symptom is usually caused by degenerative disease in the spine with the compression or irritation of spinal nerve. On the other hands, there are also several announced extra-spinal causes including aneurysm, diabetes, and radiation for sciatica in a low rate. Among the extra-spinal cases, aneurysms arising from iliac vessels are sometimes developing a diagnostic confusion with the spinal causes, and delayed diagnosis can lead to poor prognosis. It is very important to pay attention weather the aneurysmal cause is involved in the symptom of sciatica.

천골의 세침흡인검사로 진단된 전이성 간세포암종 (Cytologic Diagnosis of Metastatic Hepatocellular Carcinoma by Aspiration Cytology of Sacrum)

  • Bone metastasis of hepatocellular carcinoma appears to be peculiar when clinical manifestation of liver disease is not apparent, and initial diagnosis of metastatic hepatocellular carcinoma by fine needle aspiration cytology is rarely obtained. We experienced a case of 45-year-old man with metastatic hepatocellular carcinoma in the sacrum, which was diagnosed by fine needle aspiration cytology. The intrahepatic mass, measuring 1.2 cm in diameter and kept unchanged in size for two years, was never proved to be hepatocellular carcinoma histopathologically. The aspirated neoplastic cells were mostly in sheets, showing abundant acidophilic cytoplasm and large, round, centrally located nuclei with single, prominent acidophilic mucleoli. In the cell block section, diagnosis of metastatic well-differentiated hepatocellular carcinoma was made without difficulty, and definite trabecular fashion with sinusoidal endothelial cell lining was found.

$^{99m}Tc-MDP$를 이용한 Bone scan 시간(時間)에 관(關)한 고찰(考察) (A Study of Bone Scan Time by Use of $^{99m}Tc-MDP$)

  • The bone scan reflects the metabolic reaction of bone to a disease process, whether neoplastic, traumatic or inflammatory. In normal bone tissue, the labeled diphosphate complexes accumulate in the perivascular fluid next to the marrow cavity at the interface between uncalcified and calcified bone matrix. HMDP has the most rapid plasma clearance among the commonly used diphosphonate followed by MDP and then HEDP. I have studied about bone scan time by use of $^{99m}Tc-MDP$, and got 336 images from 112 patients. The result obtained as follow; 1. Accumulation rate of $^{99m}Tc-MDP$ is higher than other age groups in below 30-year old group. 2. Accumulation rate, in 10year old group, is 75.85% on 120min. but other groups are most high on 180 min. 3. The density differants between bone and soft tissue increased with time. 4. Image contrast is good in younger group than old.

  • Natural glycoproteins can induce apoptosis of tumor cells and exert anti-tumor activity by immunomodulatory functions, cytotoxic and anti-inflammation effects, and inhibition of endothelial growth factor. Given their prospects as novel agents, sources of natural antitumor glycoproteins have attracted attention and new research directions in glycoprotein biology are gradually shifting to the direction of cancer treatment and prevention of neoplastic disease. In this review, we summarize the latest findings with regard to the tumor suppressor signature of glycoproteins and underlying regulatory mechanisms.

기관지내 발생한 폐과오종 - 1례 보고 - (Endobronchial Hamartoma - 1 case -)

  • Pulmonary hamartoma has been considered as rare disease, which consists of lung tumor less than 1 %. Originally described by Albrecht in 1904, hamartoma is tumor like malformation-abnormal mixing of the normal components of organ-and is applied also to tumor found in many organs other than the lung. Lately, the major conclusions are that pulmonary hamartoma is neoplastic rather than developmental error in origin. Because pulmonary hamartoma frequently mimics lung cancer, especially in cancer-risk age groups, its clinical significance is great. Recently, we experienced 1 cases of endobronchial hamartoma which located at the right main stem bronchus. The patient was a 54 year old male who was admitted due to symptoms of fever 4 chilliness and dyspnea. Radiologic studies such as chest x-ray, chest tomogram and chest C-T scan revealed that the nearly total haziness of the right lung was caused by endobronchial tumor. The mass was considered as a benign by bronchoscopic exam, so we removed it surgically by tracheobronchotomy without pulmonary resection. Postoperative chest x-ray revealed satisfactory reexpansion of previous collapsed right lung. The patient discharged uneventfully.

Neurofibromatosis type I: points to be considered by general pediatricians

  • Neurofibromatosis type 1 (NF1), a prevalent genetic disease that is transmitted in an autosomal dominant manner, is characterized by multiple cutaneous cafe-au-lait spots and neurofibromas as well as various degrees of neurological, skeletal, and neoplastic manifestations. The clinical features of NF1 increase in frequency with age, while the clinical diagnosis can remain undetermined in some pediatric patients. Importantly, affected patients are at risk for developing tumors of the central and peripheral nervous system. Therefore, adequate counseling for genetic testing, age-appropriate surveillance, and management are important. This review suggests several issues that should be considered to help general pediatricians provide adequate clinical care and genetic counseling to patients with NF1 and their families.

  • Florid cemento-osseous dysplasia (FCOD) is benign, non-neoplastic lesion characterized by multiple sclerosing masses. Cases of bisphosphonate-related osteonecrosis in FCOD have been rarely reported. we present the patient had multiple osteonecrosis with FCOD in the jaw that occurred after receiving bisphosphonates for eight years due to osteoporosis This report discussed the importance of evaluation of the bone disease in the jaw before bisphosphonate treatment and periodic follow-up.

Development of colon cancer in a patient with longstanding colonic diffuse ganglioneuromatosis: a case report

  • Colonic diffuse ganglioneuromatosis is an extremely rare disease in which multiple tumors derived from the ganglion cells, nerve fibers, and supporting cells are distributed in the colon. It is generally considered to be a benign neoplastic condition and is occasionally associated with rare hereditary conditions such as neurofibromatosis type I or multiple endocrine neoplasia type 2B. Here, we report a case of a patient in whom colon cancer developed 12 years after the initial diagnosis of colonic diffuse ganglioneuromatosis, which suggests a possible association between colonic diffuse ganglioneuromatosis and colorectal cancer.
