개에서 아밀로이드 생산 치원성종양 2예 (Amyloid-producing Odontogenic Tumor in Two Dogs)

  • Amyloid-producing odontogenic tumors were described in the gingival masses of two Maltese dogs. Gingival masses were surgically removed and submitted for diagnosis. On histopathology, the masses were poorly demarcated, infiltrative, and consisted of cuboidal and columnar epithelial cells with palisading pattern. Nodular deposit of congophilic amyloid-like material and mineralization were another features of the tumors. Immunohistochemically, the neoplasticd cells were positive to pancytokeratin and neuron-specific enolase but were negative to vimentin. The amorphous homogeneous eosinophilic materials were positive to Congo red stain and showed apple-green color under the polarized microscope. Based on these results, both cases were diagnosed as amyloid-producing odontogenic tumors in dogs.

  • Surgery, radioiodine therapy, and thyroxine treatment represent established therapeutic measures of proven efficacy for the treatment of well-differentiated thyroid cancer. However, in some cases, dedifferentiation is noted and it makes tumors refractory to conventional treatment. Recently, retinoic acid redifferentiation therapy was evaluated in several in vitro and in vivo studios. We report a patient with papillary carcinoma in whom metastatic lesions became radioiodine negative on high-dose therapy. Redifferentiation therapy with retinoic acid induced radioiodine uptake in some of metastatic tissues. Side effects such as xerostomia and cheilosis were mild. We recommend retinoic acid redifferentiation therapy as an option for the treatment of thyroid cancer with negative radioiodine uptake after high-dose radioiodine therapy.

Prognostic Value of Peritoneal Washing Cytology in Gynecologic Malignancies: a Controversial Issue

  • Purpose: To evaluate the prognostic impact of peritoneal washing cytology in patients with endometrial and ovarian cancers. Materials and Methods: We retrospectively identified 86 individuals with ovarian carcinomas, ovarian borderline tumors and endometrial adenocarcinomas. The patients had been treated at Shahid Sadoughi Hospital and Ramazanzadeh Radiotherapy Center, Yazd, Iran between 2004 and 2012. Survival differences were determined by Kaplan-Meier analysis. Multivariate analysis was performed using the Cox regression method. A p<0.05 value was considered statistically significant. Results: There were 36 patients with ovarian carcinomas, 4 with borderline ovarian tumors and 46 with endometrial carcinomas. The mean age of the patients was $53.8{\pm}15.2years$. In patients with ovarian carcinoma the overall survival in the negative cytology group was better than the patients with positive cytology although this difference failed to reach statistical significance (p=0.30). At 0 to 50 months the overall survival was better in patients with endometrial adenocarcinoma and negative cytology than the patients with positive cytology but then it decreased (p=0.85). At 15 to 60 months patients with FIGO 2009 stage IA-II endometrial andocarcinoma and negative peritoneal cytology had a superior survival rate compared to 1988 IIIA and positive cytology only, although this difference failed to reach statistical significance(p=0.94). Multivariate analysis using Cox proportional hazards model showed that stage and peritoneal cytology were predictors of death. Conclusions: Our results show good correlation of peritoneal cytology with prognosis in patients with ovarian carcinoma. In endometrial carcinoma it had prognostic importance. Additional research is warranted.

Influence of Ethnicity on Survival of Breast Cancer Patients in Turkey

  • Background: Kurdish women with breast cancer have more unfavorable prognostic factors than their Turkish and Arab counterparts. However, the effects of these factors on breast cancer survival among these ethnic groups remain unclear. We therefore investigated the impact of ethnicity on survival in breast cancer patients in Turkey. Materials and Methods: Ethnicity, age, stage at diagnosis, tumor characteristics, treatments given (surgery, chemotherapy, radiotherapy and hormone therapy), and survival times were recorded. Kaplan-Meier analysis was used to estimate the overall survival times and survival plots. Log-rank test was used to compare the survival curves.Results: Of the 723 breast cancer patients included in the study, 496 (68.7%) were Turkish, 189 (26.2%) were Kurdish, 37 (5.1%) were Arabic and 1 was Armenian. Kurdish women with breast cancer had larger tumor sizes and higher rates of hormone receptor negative tumors than Turkish and Arab patients. Mean follow-up time was 118.4 [95% Confidence Interval (CI): 95.4-141.3] months, and it was 129.9 (95% CI: 93.7-166.2), 124.2 (95% CI: 108.4-140.1) and 103.1 (95% CI: 85.9-120.4) months for Turkish, Arabic and Kurdish patients, respectively. Conclusions: Kurdish ethnicity is associated with higher rates of hormone receptor negative and triple-negative tumors and with worse survival. Clinical and epidemiological research is warranted to elucidate reasons underlying overall survival, variations in tumor biology, differences in treatment responsiveness, and effects of social factors among ethnic groups in Turkey.

Relationships Between Clinical Behavior of Laryngeal Squamous Cell Carcinomas and Expression of VEGF, MMP-9 and E-Cadherin

  • The biological mechanisms of cancer and associations with behavior of tumours need to be studied to understand progression and determine appropriate treatments. Here we investigated expression of VEGF, MMP-9 and E-cadherin in laryngeal SCCs and their relations with clinical behavior. This prospective study was based on 38 surgical specimens from patients with primary laryngeal SCC and data recorded in their cards. Expression of the three factors in tumor tissue was examined using immunohistochemistry and correlations with clinical parameters of primary tumors, regional lymph node metastases, stage of disease, histopathologic differentiation, and vascular/cartilage invasion were investigated. Regarding the cases with positive MMP-9 expression, the difference between well and moderately/poorly differentiated tumors was statistically significant. However, differences between early stage (stage I and II) and late-stage (stage III and IV) tumours, and between positive and negative for pLN metastasis were not. No significant relationship between positive VEGF and tumor differentiation or stage was apparent, but E-cadherin levels significantly differed between well and moderately/poorly differentiated tumours and with the presence of pLN metastasis. E-cadherin staining did not vary between MMP-9 positive and negative cases. In conclusion, MMP-9 may be a negative predictor of differentiation in laryngeal SCC, while E-cadherin is a predictor of differentiation and nodal metastases. Even if the difference between VEGF expression and tumor stage was not statistically significant, it seems that there exists some relationship, which might be clarified with a greater number of cases.

  • The management of malignant tumors of the oral cavity requires that the resection should be performed with adequate negative margins. To this end, the surgeon is sometimes required to increase the access to the oral cavity to resect with clear margins although most of our resections can be carried via the transoral route. This article highlights some of the most common surgical accesses to the oral cavity: Peroral approach, mandibulotomy and mandibular swing approach, visor flap approach, upper & lower cheek flap approach, pull-through technique. Also, various modes of management are grossly described according to primary site, size, and stage of the tumors.

  • Soft tissue tumors of the foot have a low incidence rate, and most of them are symptom free, thus it is difficult to diagnose accurately. Herein, we report a 15-year-old male patient who had swelling without pain on the lateral margin of both feet. We performed excisional biopsy of the abductor digiti minimi via subtotal resection, following radiograph and magnetic resonance imaging. According to the histological analysis, hypertrophy of abductor digiti minimi was positive, and other soft tissue tumors were negative. Six months after the operation, normal appearance of both feet was maintained and the patient was satisfied with the result.

종격동경 검사의 임상적 의의 (Mediastinoscopy; Its clinical significance.)

  • The mediastinoscopy was a well known useful diagnostic tool for detection of mediastinal lymph nodes invasion by bronchogenic carcinoma, and also useful means for histologic diagnosis of metastatic carcinoma and certain mediastinal tumors. 31 cases of mediastinoscopies were reviewed which were experienced at Kyung Hee University Hospital from July, 1979 to June, 1985. We experienced 20 cervical mediastinoscopies, 10 left anterior mediastinotomy, and 1 both procedures. Of the 31 cases, 22 cases were used for preoperative staging of bronchogenic carcinoma, 7 cases for mediastinal tumor diagnosis, and 2 cases for histologic diagnosis of metastatic carcinoma. In 22 mediastinoscopies which were used for preoperative staging, 10 cases were revealed positive mediastinal nodes, and could avoid meaningless thoracotomy. All 12 mediastinoscopy negative patients were received thoracotomy, and 10 of them were resectable. The resectability in bronchogenic carcinoma was 83%, on the contrary, the other series at premediastinoscopic era revealed only 65% resectability. Other mediastinal lesions such as tuberculous granuloma [4], sarcoidosis [2], malignant thymoma [1], and metastatic carcinoma [2] were also diagnosed successfully. Mediastinoscopy is very useful tool for determination of treating method of bronchogenic carcinoma, and for diagnosis of certain mediastinal tumors which, otherwise, need a thoracotomy for confirmatory diagnosis.

Distinctions Between Clinicopathological Factors and Prognosis of Alpha-fetoprotein Negative and Positive Hepatocelluar Carcinoma Patients

  • Serum alpha-fetoprotein (AFP) is a significant marker for clinical diagnosis and prognosis evaluation in hepatocellular carcinoma (HCC) patients. However, some proportion of liver cancer patients are AFP-negative (AFP ${\leq}$20ng/ml). In order to study the differences between clinicopathological factors and prognosis of alpha-fetoprotein negative and positive patients, a total of 114 cases (41 AFP-negative and 73 AFP-positive) were selected for our research. By systematically statistical analysis, the results demonstrated that compared with AFP-negative patients, AFP-positive examples were more likely to feature cirrhosis nodules, non-complete neoplasm capsules, and a poor Edmondson-steiner grade. Furthermore, AFP-negative patients demonstrated a favorable long-term prognosis. By univariate analysis and multivariate analysis with Cox's proportional hazards model, multiple tumors were found to be independent risk factors for worse survival of AFP negative patients; however, less tumor-free margins, multiple tumors and Edmondson-steiner grades III/IV, proved to be independent risk factors leading to a poor prognosis of AFP positive cases. Finally, we can infer that high levels of AFP signify a highly malignant tumor and unfavorable prognosis.

Overexpressions of Vimentin and Integrins in Human Metastatic Spine Tumors

  • Objective : To comparatively investigate the expression of several integrins in specimens of human bone metastases and degenerative bone tissue. Methods : Degenerative cancellous tissue was obtained from a sample of human degenerative spine. Thirteen human specimens were obtained from metastatic spine tumors, whose primary cancer was colon cancer (n=3), hepatocellular cancer (n=3), lung cancer (n=4), and breast cancer (n=3). The expression of vimentin and integrins ${\alpha}v$, ${\beta}1$, and ${\beta}3$ was assessed in metastatic and degenerative specimens by immunohistochemistry and real-time reverse transcription-polymerase chain reaction (qRT-PCR). Results : Immunohistochemical staining showed that vimentin and integrin ${\alpha}v$ was broadly expressed in all tissues examined. By contrast, integrin ${\beta}1$ was weakly expressed only in 38.4% (5/13) of tissues. Integrin ${\beta}3$ was consistently negative in all cases examined. qRT-PCR analysis showed that vimentin gene expression was higher in all metastatic specimens, as compared to degenerative bone. The gene expression of integrin ${\alpha}v$ in breast specimen was significantly higher than others (p=0.045). The gene expression of integrin ${\beta}1$ was also higher in all metastatic specimens than in degenerative bone tissue. The gene expression of integrin ${\beta}3$ was variable. Conclusion : Spinal metastatic tumors have mesenchymal characteristics such as increased expression of vimentin. The increased expression of integrin ${\alpha}v$ and ${\beta}1$ in spine metastatic tumors suggests that adhesive molecules such as integrin may have implications for the prevention of spine metastasis.
