Reconstruction of Soft Tissue Defect with Free Flap in Pediatric Patients (소아 환자에서 유리피판술을 이용한 연부 조직 결손의 재건)

  • Purpose: Free flap reconstruction in the pediatric population is difficult. However, microsurgery has had remarkable success rates in children. The aim of study is to present our clinical experience using free flap for reconstruction of soft tissue defects in children and to describe long-term follow-up results. Methods: Between June 2002 and July 2010, 30 cases of pediatric reconstruction were performed with free flap. The authors analyzed several items, such as the kind of flap, associated complications, and growth problems. Results: Among the 30 cases, 21 cases were due to traffic accidents, 5 to cancer, and 4 to falls and other soft tissue defects. The lower leg and foot were the most common sites of the lesion. In the free flap operations we have done, 20 cases involved an anterolateral thigh perforator free flap, 6 a superficial circumflex iliac perforator free flap, and 4 an upper medial thigh perforator free flap. In early postoperative complications, partial necrosis was seen in 2 cases, infection in 1 case, and the hematoma in 1 case. A satisfactory success rate and functional results were achieved. Conclusion: Free flap reconstruction in children allows satisfactory function with no significant effect on growth. Free flaps are regarded as the primary choice for selective pediatric reconstructive cases.

Studies on the Clostridium perfringens type C infection of pig in Korea (국내(國內) 돼지의 Clostridium perfringens type C 감염증에 관한 연구)

  • Thirteen strains of Clostridium perfringens were isolated from the pigs with hemorrhagic enteritis. The characteristics of the outbreaks, clinical signs and lesions were examined. The biochemical properties, type of toxin and susceptibility to antimicrobial agents of the isolates were investigated. The results could be summarized as follows ; 1. Almost of the pigs affected with hemorrhagic enteritis, 17 cases examined from 1989 to 1992, were piglets less than 7 day old. 2. The average mortality rate of piglet less than 7 day old affected with hemorragic and necrotic enteritis was 48.5%. 3. The clinical signs of pigs with hemorrhagic enteritis were depression, hemorrhagic diarrhea, anemia and dehydration. Necropsy of the infected pigs showed typical hemorrhage of upper intestine and necrosis of mucosal membrane. 4. The characteristic biochemical properties of the isolates were 2-band hemolysis, positive reaction of reverse CAMP test and formation of LV precipitate in egg yolk medium. 5. The toxin type of the 13 isolates, investigated by mouse inoculation test, was all type C strains of Clostridium perfringens. 6. In susceptibility test to antimicrobial agents, 13 isolates of Clostridium perfringens were highly sensitive to ampicillin, enrofloxacin(Baytril), cephalothin, penicillin and trimethoprim-sulfamethoxazole.

Clinical Analysis of Cervical Necrotizing Fasciitis (경부 괴사성 근막염의 임상적 고찰)

  • Background and Objectives Cervical necrotizing fasciitis is a fulminant disease associated with necrosis of connective tissue, spread along the fascial plane, and high mortality. We analyzed the clinical characteristics and treatment outcome of this rare fatal disease. Materials and Methods We retrospectively reviewed the medical records of 19 patients treated for cervical necrotizing fasciitis from January 1999 to January 2009. Mean age was 53.7 years. Results The most common predisposing illness was tonsillitis (36.8%), followed by odontogenic infection (15.7%). Diabetes mellitus was most common underlying disease. Liver cirrhosis and chronic renal failure were found in 2 patients each. All patients were treated with combination of parenteral antibiotics and wide surgical debridement by transcervical and/or thoracotomy approach. Multiple surgical debridements were performed in 7 patients. Tracheotomy was performed in most of the patients (88.8%). Period of total hospitalization and Intensive care unit was 23 days and 10.1 days. Two patients died of disease and overall survival rate was 89.4%. Conclusion Early surgical management and care in intensive care unit are essential for cervical necrotizing fasciitis. Possible complications such as respiratory failure, mediastinitis or sepsis should be carefully evaluated.

Hepatoprotective and a Potential Antiviral Effect of Biphenyl Dimethyl Dicarboxylate/Amantadine for an Acute Viral Hepatitis Induced by MHV-2 in ICR Mice (마우스 간염바이러스(MHV-2)에 의해 유발된 전격성 바이러스간염에 대한 비페닐메칠디카르복실레이트/아만타딘제제의 간보호 및 잠재적 항바이러스효과)

  • The mouse hepatitis virus (MHV-2) induces broad collapses, focal necrosis and cytolysis of hepatocytes, and leads to death after three to five days of intraperitoneal injection in mice. The present study investigated whether the combinatorial treatment of dimethyl dicarboxylate/amantadine (2:1) showed hepatoprotective and/or antiviral properties in MHV-2 infected ICR mice. In the study, we found that dimethyl dicarboxylate/amantadine group (VDDBA) increased the survival rate (30.8%) when compared to positive control, VL (7.7%) and that VDDBA lengthened the survival time (4.2 d)after MHV-2 infection. In addition, ALT and AST were well regulated when treated with VDDBA (p<0.01). Finally, we concluded that those results were probably from the inhibition of viral replication and at least antiproliferative effect on MHV-2.

  • Necrotizing lymphadenitis, or Kikuchi's disease, a newly recognized disease of unknown origin, occurred usually in young woman and mostly resolved spontaneously without treatment within a few months. Clinically, characteristic symptoms and sings were local lymph node enlargement, sometimes accompanied by tenderness, fever, weightless, leukopenia, and elevated erythrocyte sedimentation rate, and so necrotizing lymphadenitis can be confused with malignant lymphoma and tuberculosis. The histologic features of necrotizing lymphadenitis are distinctive. : lymph node biopsy reveals areas with frank cellular necrosis, karyorrhexis, and absence of plasma cell. We present nineteen cases of necrotizing lymphadenitis during from March 1990 to January 1993 and discuss their exact diagnosis and proper treatment.

Primary Segmental Volvulus of the Small Bowel -Report of 2 Cases- (소장의 국한적 원발성염전 2례 보고)

  • Primary segmental volvulus of the small intestine is not associated with malrotation, malfixation of the midgut, nor other primary small bowel lesions such as small bowel tumors. This entity is known to be more prevalent in adult and in certain global areas associated with particular diet habits. There have been very few reports in neonates, but not in this country so far. The author reports two cases of primary segmental volvulus. Case 1 was a septic 4-day-old girl with hematochezia due to jejunal volvulus with partial necrosis and panperitonitis. Resection of the segment and Bishop-Koop enterostomy were successful. Case 2 was a 3-day-old boy, who had ileal volvulus with ultra-short length of ileal atresia, probably due to intrauterine segmental volvulus. Limited resection of the atresia and spreading of the mesenteric base were enough to recovery. The rarity of the pathognomonic findings and limitation of the diagnostic workup due to rapid progression limit early diagnosis and good survival rate in this particular condition.

  • In other to understand the effect of yellow soil to mortality of Korean scallops, P. yessoensis, We investigated its mortality at indoor tanks. The environmental conditions such as water temperature, Salinity, Do and pH were continued constantly during the experimental periods. The 100% of survival rate showed in two experiments groups such as 0.1% and 0.4% of concentration of yellow soil and the other groups as 0.05% and 0.2% of concentration of yellow soil was appeared one dead scallop at each group for 8 days of the experiment periods. the gills of scallop in high concentration of yellow soil (0.2% and 0.4% groups) were covered by yellow soil particles so that this group's scallop should be got a high stress from yellow soil. I think this situation will be more continued for long time the scallop will become to dead. The results of bacteriological analysis did not isolated from haemolymph of scallops and no Perkinsus infectious disease in scallops and the scallops showed necrosis and degeneration on digestive grand and gills of scallop.

Anisomycin protects against sepsis by attenuating IκB kinase-dependent NF-κB activation and inflammatory gene expression

  • Anisomycin is known to inhibit eukaryotic protein synthesis and has been established as an antibiotic and anticancer drug. However, the molecular targets of anisomycin and its mechanism of action have not been explained in macrophages. Here, we demonstrated the anti-inflammatory effects of anisomycin both in vivo and in vitro. We found that anisomycin decreased the mortality rate of macrophages in cecal ligation and puncture (CLP)- and lipopolysaccharide (LPS)-induced acute sepsis. It also declined the gene expression of proinflammatory mediators such as inducible nitric oxide synthase, tumor necrosis factor-α, and interleukin-1β as well as the nitric oxide and proinflammatory cytokines production in macrophages subjected to LPS-induced acute sepsis. Furthermore, anisomycin attenuated nuclear factor (NF)-κB activation in LPS-induced macrophages, which correlated with the inhibition of phosphorylation of NF-κB-inducing kinase and IκB kinase, phosphorylation and IκBα proteolytic degradation, and NF-κB p65 subunit nuclear translocation. These results suggest that anisomycin prevented acute inflammation by inhibiting NF-κB-related inflammatory gene expression and could be a potential therapeutic candidate for sepsis.

The Antiulcer Effects of Alove vera on the Stomach ulcer Induced by Stress in rats (흰쥐의 스트레스성 위궤양에 대한 Alove vera의 항궤양작용)

  • This was performed to investigate the antiulcer effects of Aloe vera on the stomach ulcer induced by restraint and water-immersion stress in rats. For this experiment, 60 male Sprague-Dawley strain were used. The experimental groups were divided into five: a control(CA) and 4 aloe treatment groups. Each dose of aloe was 50 mg/kg BW(AA), 100mg/kg BW(AB), 200 mg/kg BW(AC), and 400 mg/kg BW(AD). The rats were allocated to each group by 12 and observed for 4 weeks. The results are as following. 1. The stomach surface pH in each group showed no significant difference, byt the values of aloe treatment groups were higher than the value of the control group. 2. The gastric wall mucus was significantly increased in all aloe treatment groups(p<0.05) compared with the control group. Especially in AC and AD group the differences were higher(p<0.01). 3. At shear rate rate 11.25, 45.0, 90.0, 225.0 sec-1, whole blood viscosity and plasma viscosity were measured. Most of the values of aloe treatment groups were significantly low compared with those of the control group(P<0.05). 4. The ulcer index of aloe treatment groups were significantly low campared with control group(p<0.05). Especially in AC and AD group the differences were more significant(p<0.01). 5. Less severe ulcers were observed in AA and AB group than in the control group. Tissues of AC and AD group had only slight ulcers and necrosis of tissue was not observed in these groups. Especially in AD group, there was more mucus than other groups and it seemed that alove vera stimulated the epithelial regeneration. From the results of this study, it can be concluded that the oral administration of Alove vera results in protection of stomach ulcer by stimulating the secretion of gastric mucus and the circulation of blood.

Clinical experiences of finger replantation in pediatric patients (소아에서의 미세수지접합술의 경험)

  • Purpose: Owing to improvement of microscope, microsurgery implements, and microsuture, finger replantation has shown much development. With high success rate of microsurgery in children, positive results have been reported ever from distal amputation. Here, we report the patients demographics, methods, and results of the microsurgery performed in children in our hospital for the last 8 years. Methods: From the medical records of 21 patients who had given the treatment in our hospital from January 2000 to December 2007, we analyzed patients' sex, age, operative method, and complication retrospectively. Results: The number of male patients was twice as many as female, where most patients belong to the ages of five to ten years. Operative methods performed in this study included end - to - end anastomosis of artery and vein, vein graft, and epineurial suture. As a result, 19 out of 21 cases were successfully accomplished, and four of them went through the debridement of necrotic tissue due to the partial necrosis of the lesion. A one - year follow - up observation was done after surgery and most of them were almost fully recovered like in their previous state. Conclusion: The success rate of finger replantaion in children is continuously improving despite the difficulty of vessel anastomotic procedure, rehabilitation treatment and management after surgery. We report the satisfactory results of pediatric finger replantation technically and aesthetically.
