The Correlation Between Forward Head Position and Neck Flexor Thickness During Cranio-Cervical Flexion Exercise (머리-목굽힘 운동 시 앞쪽머리자세와 목굽힘근 근두께 간의 상관성 연구)

  • Background: As the increase of forward head position, we studied the change of thickness of deep cervical flexor (DCF) compared of with sternocleidomastoid (SCM). we measured and analysed the change of thickness of the neck flexor for forward head position and cranio-cervical flexion exercise (CCFE). Methods: Using a cross-sectional design, we conducted the study selecting 35 healthy adults (12 males, 23 females). We measured the craniovertebral angle (CVA), instructed them to perform the CCFE, and measured the DCF and SCM using ultrasonography during the contraction and relaxation period. Results: Intra-reliability of SCM is .96, longus capitis is .92 and longus coli is .97. we compared according to the change of forward head position, Correlation of DCF is .841, and DCF/SCM is .754 by significant positive correlation. At the comparison of CCFE and Resting muscle thickness, SCM and DCF is .00, DCF/SCM is .68. Conclusion: There is a strong positive correlation between the change amount of DCF and DCF/SCM as the increase of CVA.

Effects of Cervical Sensorimotor Control Training on Pain, Function and Psychosocial Status in Patients With Chronic Neck Pain (목뼈 부위 감각운동 조절 훈련이 만성 목 통증 환자의 통증과 기능, 심리사회적 수준에 미치는 영향)

  • Background: It is reported that the proprioceptive sensation of patients with neck pain is reduced, and neck sensory-motor control training using visual feedback is reported to be effective. Objects: The purpose of this study is to investigate how sensorimotor control training for the cervical spine affects pain, function, and psychosocial status in patients with chronic cervical pain. Methods: The subjects consisted of 36 adults (male: 15, female: 21) who had experienced cervical spine pain for more than 6 weeks. An exercise program composed of cervical stabilization exercise (10 minutes), electrotherapy (10 minutes), manual therapy (10 minutes), and cervical sensorimotor control training (10 minutes) was implemented for both the experimental and the control groups. The cervical range of motion (CROM) and head repositioning accuracy were assessed using a CROM device. In the experimental group, the subjects wore a laser device on the head to provide visual feedback while following pictures in front of their eyes; whereas, in the control group, the subjects had the same training of following pictures without the laser device. Results: There were no statistically significant differences between the two groups in pain, dysfunction, range of motion, or psychosocial status; however, post-test results showed significant decreases after 2 weeks and 4 weeks compared to baseline (p < 0.01), and after 4 weeks compared to after 2 weeks (p < 0.01). The cervical joint position sense differed significantly between the two groups (p < 0.05). Conclusion: In this study, visual feedback enhanced proprioception in the cervical spine, resulting in improved cervical joint position sense. On the other hand, there were no significant effects on pain, dysfunction, range of motion, or psychosocial status.

Modern Interpretation on Kinesiology of Yangsaeng-Doinbub Presented in [Jebyungwonhuron·Huhrobyungjehu] - Dealing with Knee Joint, Nuchal Area and Upper Extremity (「제병원후론(諸病源候論)·허로병제후(虛勞病諸候)」에서 제시된 양생방도인법과 현대 운동치료의 비교 연구 - 슬관절과 경부/상지부 중심으로)

  • Objectives The objective of this study is to interpretate Yangsaeng-Doinbub presented in [Jebyungwonhuron Huhrobyungjehu] in a modern kinesiologic approach. Methods Based on the interpretation of [Jebyungwonhuron Huhrobyungjehu] and implementation of its kinesiology, this study presents similar kinesiologies and their purposed, with the reference to various documents on modern kinesiology. Results 1) Exercises in Yangsaeng-Doinbub presented in [Jebyungwonhuron Huhrobyungjehu], which are similar to resistance exercise, can be used for isometric exercise of knee joint extensor and cervical extensor. 2) Exercises in Yangsaeng-Doinbub presented in [Jebyungwonhuron Huhrobyungjehu], which are similar to stretching exercise, has its purpose to stretch semitendinosus m., semimembranous m., gastrocnemius m., quadriceps femoris m., upper trapezius m., levator scapulae m., serratus m., and splenius capitis m. 3) Exercises in Yangsaeng-Doinbub presented in [Jebyungwonhuron Huhrobyungjehu], which are similar to active exercise, can be used for strengthen quadriceps femoris m.. 4) Yangsaeng-Doinibub also describes breathing methods, which help normalization of breathing exercised and increase the efficiency of spine and joint exercises. Conclusions The modern interpretation on kinesiology of Yangsaeng-Doinbub presented in [Jebyungwonhuron Huhrobyungjehu] leads to a conclusion that Yangsaeng-Doinbub consists of various exercises for knee joint, neck and upper extremity.

Effects of Semi-Occluded Vocal Tract Exercise in Patients with Functional Aphonia (반폐쇄성도훈련이 기능적 실성증 환자의 음성 개선에 미치는 효과)

  • Background and Objectives : Functional aphonia is characterized by incomplete closure of the vocal folds. Semi-occluded vocal tract exercise (SOVTE) allows smoothly vocal folds collision without damage to the vocal folds tissues to produce normal vocal intensity. The purpose of this study is to report the effect of SOVTE in patients with functional aphonia. Materials and Method : Seven patients diagnosed with functional aphonia were treated with 1-3 voice therapy sessions using voiced lip-trill, humming, Lax Vox in SOVTE. To assess the effectiveness of semi-occluded vocal tract exercise, cepstral analysis and auditory perceptual assessment were performed before and after voice therapy. Results : F0 (fundamental frequency), CPP (cepstral peak prominence) and L/H ratio (low/high spectral ratio) were significantly increased, while CPP Standard deviation, L/H ratio Standard deviation were decreased. In addition, 'Grade', 'Breathiness' and 'Asthenia' were significantly decreased in the GRBAS scale after SOVTE (p<0.05). Conclusion : In our study, SOVTE seemed to be effective to elicit voice quickly and promote vocal folds vibration without muscular effort in patients with functional aphonia.

Effects of Facial Exercise for Facial Muscle Strengthening and Rejuvenation: Systematic Review

  • Purpose: The mass of facial muscles can be increased through exercise, as is also the case for muscles in the extremities. This systematic review was conducted to investigate the effect of facial exercises on facial muscle strengthening and facial rejuvenation, focusing on recent studies. Methods: A literature search was performed using the PubMed, ScienceDirect, and Web of Science databases. The quality of the trials was evaluated according to the PEDro scale. In total, 11 studies were included in this review: four studies on facial exercise for facial rejuvenation and seven studies on strengthening the muscles of the face. Results: Facial exercises for facial rejuvenation increased the mechanical properties and elasticity of the skin of the face and neck, the thickness and cross-sectional area of the facial muscles, and the fullness of the upper and lower cheeks. Conclusion: A study aimed at strengthening facial muscles showed improvements in labial closure strength and tongue elevation strength. Despite the positive results for facial rejuvenation and muscle strengthening, the level of evidence was low. Therefore, in future research, it will be necessary to investigate the effects of facial exercise in a thoroughly controlled experiment with a sufficient sample size to increase the level of evidence.

For example, it can be used to correct improper low back and neck exercise patterns. 6) Yangsaeng-Doinbub also describes breathing methods, which help normalization of breathing exercises and increase the efficiency of spine exercises. Conclusions The modern interpretation on kinesiology of Yangsaeng-Doinbub presented in "Zhu-Bing-Yuan-Hou-Lun Yao-Bei-Bing-Zhu-Hou" leads to a conclusion that Yangsaeng-Doinbub consists of numourous exercises for various body parts. In particular, breathing methods increase efficiency of such exercises. Plus, the exercises in Yangsaeng-Doinbub can be applied to various uses by doctors.

The Effect of Joint Mobilization and McKenzie exercise on the Cervical range of motion and Tenderness (관절가동술과 멕켄지 운동이 경부 가동범위와 압통에 미치는 영향)

  • Purpose : To evaluate the effects joint mobilization and McKenzie exercise on the cervical range of motion and tenderness through cervical range of motion(CROM), algometer. Methods : The subjects consisted of thirty five patients. Eighteen underwent McKenzie exercise, seventeen did Joint Mobilization. The joint mobilization group received joint mobilization exercise for about 15 minutes, and McKenzie exercise group received extension, side-flexion, flexion, elevation, exercise for 15 minutes respectively. The test period of each group took place three times a week during 6 weeks. All measurements for each the subject took the following treatment: pre-treatment, treatment in 2 weeks, treatment in 6 weeks, post-treatment in 2 weeks. Results : The flexion, extension and side-flexion CROM of both groups were statistically significant increase within treatment period(p<.05), and also a significant difference within pre-treatment and post-treatment in a 2 week period(p<.05), within treatment in 2 weeks and post-treatment in 2 weeks by checking the extent of effect in the treatment period(p<.05). The trapezius tenderness threshold of both groups were statistically significant increase within treatment period(p<.05), and also significant difference within pre-treatment and post-treatment, within treatment in 2 weeks and post-treatment by checking the extent of effect in the treatment period. Conclusion : Joint mobilization and Mckenzie exercise improved cervical range of motion and tenderness threshold.

Proposal of Applying the Exercise Program for the Prevention of Work-related Chronic Low Back Pain

  • Objective: The purpose of this research is to provide exercise programs for the prevention of work related chronic back pain. Background: In order to prevent musculoskeletal disease, including proper medical care health promotion programs are needed. Method: This is a research of musculoskeletal disease looking at 618 workers working at a car engine manufacturing factory from April to July of 2008. Through questionnaire specific areas of musculoskeletal diseases experienced by the workers were identified and preventative exercise program for chronic low back pain was recommended. Result: Research showed that of the musculoskeletal disease experienced by the workers, 197 presented with low back pain, 171 presented with shoulder pain, 64 presented with neck pain and 44 presented with knee pain. The symptoms of low back pain included stiffness(143), twinge and burning sensation(24) and absence of sensation(19). Using this result 4 types of exercise programs were recommended for prevention of chronic low back pain. Conclusion: Preventative exercise programs recommended for the workers in this research is easily accessible for the workers. Use of the suggested exercise programs will inevitably decrease work related low back pain. Also 2 other recommendations were made: 1) Internal structural change may be necessary using ergonomics. 2) More exercise programs to be used to increase adaptation and tolerance of joints and muscles that are constantly used for repetitive work. Application: This study can be used to provide for the prevention of work-related Chronic Low Back pain.

Correlation with Daily Life, Physical Activity, Pain, and Degree of Disability of Office Workers with Non-Specific Chronic Neck Pain

  • Objective: The purpose of this study was to prepare evidence for the prevention and management of nonspecific chronic neck pain (NSCNP) by examining the correlation between activity of daily living and physical activities of office workers with NSCNP. Design: Crossed-sectional study Methods: 89 patients with NSCNP were recruited for this study. But 2 subjects met the exclusion criteria and were dropped out, and 86 subjects finally participated. Numerical pain rating scale (NPRS) and neck disability index (NDI) were used to check the pain intensity and disability of patients with neck pain, respectively. For the activity of daily living, computer use time, sleep time, and driving time were used. To find out the physical activities of the subjects, International Physical Activity Questionnaires (IPAQ-SF) was used. Correlation analysis was performed to find out the correlation of each variable. Results: A clear positive correlation was established between computer use time and pain (p<0.05), and a clear positive correlation was established between computer use time and disability index (p<0.05). The correlation between NPRS and NDI and physical activity total time, high intensity activity score, moderate intensity activity score, and walking score were not statistically significant (p>0.05). Conclusions: In treating patients with NSCNP, it is necessary to reduce the computer usage time as a professional factor or to educate the proper posture. In addition, rather than emphasizing physically comprehensive physical activity, grafting therapeutic exercise directly related to neck pain could have a more positive effect on NSCNP patients.

A study on characteristics and physiological variables of chest pain induced by exercise test in angina suspected patients (협심증이 의심되는 환자에서 운동부하검사로 유발되는 흉통의 양상과 생리적 변인에 관한 연구)

  • The purpose of this study was to identify the characteristics and physiological variables of chest pain induced by exercise test in angina suspected patients. The subjects of this study consisted of 28 inpatients and outpatients aged between 40 and 75 who underwent treadmill test at exercise testing laboratory of S-University from January 2000 to June 2000. Subjects were interviewed with questionnaire regarding sociodemography, the past health history and history related to chest pain before the exercise test. Subjects were interviewed with questionnaire concerning quality, intensity, duration of chest pain induced by walking on the treadmill(Marquette, U.S.A. 1992) according to Bruce protocol following exercise test. Systolic and diastolic blood pressure were measured before, during and after the test, heart rate was determined by ECG. The results of this study were as follows ; 1) Quality of chest pain induced by exercise test were feeling stiffness 19(67.9%), heavy 10(36.0%), exploded 9(32.1%), crushing, suffocating, tight 8(28.6%), stuffy, prickly 7(25.0%), burning 6(21.4%), clasp 5(17.9%), cleaved, tensed, piercing 3(10.7%), perfectly fitting, sore 2(7.1%), tearing, tingling, ticklish, heartburn 1(3.6%). 2) Mean score of VAS(intensity of pain) following exercise test was $5.79{\pm}2.27$ and mean duration of chest pain after the test was $7.83{\pm}5.31$ minutes. 3) Sites of chest-pain induced by exercise test were middle site 11(39.3%), left-chest 10(35.6%), right-chest 6(21.5%). Radiation site of chest-pain was neck(18.0%), right flank site 1(3.6%), left shoulder & arm 2(7.1%) and back 1(3.6%). 4) Symptoms other than chest-pain induced by exercise test were dyspnea 21(75.6%), perspiration 14(50.4%), fatigue 12(43.2%), leg-pain 11(39.6%), dizziness 7(25.2%) anxiety toward chest-pain 3(10.8%), thirst 2(7.1%), and palpation, headache and tingling sensation of hand and leg 1(3.6%). 5) Mean MET(intensity of exercise) during the exercise test was $7.64{\pm}2.57$ and mean RPE(rating of perceived exertion) was $15.89{\pm}2.36$. Mean duration of exercise was $6.79{\pm}2.88$. 6) correlation coefficients between RPE and VAS was 0.500(p=0.003), those between MET and VAS was 0.287(p=0.069) and those between either depression or elevation of ST segment and VAS was 0.236(p=0.114). 7) There was a significant difference in mean systolic pressure between before and after the test as $146.29{\pm}28.18mmHg$ and $177.96{\pm}28.82mmHg$(t=-5.640, p=0.000), a significant difference in mean diastolic blood pressure between before and after the test as $84.85{\pm}15.07mmHg$ and $88.89{\pm}13.72mmHg$(t=-2.082, p=0.047), and there was a significant difference in mean heart-rate between before and after the test as $81.89{\pm}12.22/min$ and $160.68{\pm}21.77/min$(t=-21.255, p=0.000).
