Study on the distribution in major disease category and frequency of clinical usage of national health insurance herbal prescription based on analysis on KCD8 disease code of indications (적응증의 KCD8 상병코드 분석 결과를 기반으로 한 보험한약제제의 질병 대분류 분포 분석과 사용 빈도 연구)

  • Objectives: National health insurance herbal prescription of Korean medicine has been serving important role in public healthcare in spite of continuous demand on revision of system. However, the categories of insurance herbal prescriptions are not equally distributed throughout the KCD-based major disease categories. We analyzed statistical database of claimed national health insurance classified as major disease categories by years. We classified all 56 herbal prescriptions as per their total medical indications into 22 major disease categories to analyze their distribution. Significant increase of M and S-T code claims were found, whereas decrease of U code claims by years. We figured out that the 56 prescriptions were unequally distributed along with enrichment of certain codes such as K and J. Meanwhile, the insurance claim of each prescription was positively correlated with number of code types of their indications. As a result, we believe that the reform of national health insurance herbal prescription list is necessary to promote use of it in clinic.

Tasks for Insuring the Composite Herbal Preparations of Sasang Constitutional Medicine in the National Health Insurance (사상체질처방 복합제제의 보험급여화 추진 과제)

  • Reforming the insurance scheme for herbal drugs in the National Health Insurance is a long-cherished desire for Korean Medicine doctors. Because most Korean Medicine doctors distrust the quality of existing insured herbal drugs, which are powdered mixes of each herbal extract, the use and the expenditure of insured herbal drugs have been decreased in the last ten years. To address this, it has been demanded to insure the composite type of herbal preparation, which is the extracted powder of the whole prescription, to the benefit coverage for herbal drugs. Many stake holders, however, have so far been unable to reach an agreement on this. In this situation, Sasang Constitutional prescriptions are expected to make a breach of insuring the composite herbal preparations, because some of them were approved as prescription drugs in 1999. In this review, I discussed the problems of insured herbal drugs, the necessity of insuring the composite herbal preparations and Sasang Constitutional prescriptions, and the tasks of Sasang Constitutional Society to insure them.

A survey on additional needs for herbal preparations in national health insurance service (보험급여 한약제제 기준처방 개선방안 설문조사 연구)

  • Objectives : In order to increase the utilization rate of insurance covered herbal medicines, it is necessary to improve the standard prescription lists of current national health insurance service. Methods : Previous research and discussions have been summarized, the cases of overseas insurance prescription were investigated, and compared with the insurance prescription of Korea. We conducted two questionnaire surveys. From the first email questionnaire survey, response rate was very low. So once again, from december 4, 2014 to december 17, we emailed a questionnaire survey to 17,275 members of the Korean medicine doctors and 890 peoples(5.1%) responded for two weeks. We analyzed prescriptions from previous studies, overseas cases, and surveys. Results : With overseas herbal medicine health insurance lists, prescriptions recommended by the experts committee, the prescriptions results of the first and second surveys, and the prescriptions presented in previous studies, we compared and analyzed all the results and we derived 202 prescriptions for NHIS. Conclusion: We suggest 127 Single extracts and 202 prescriptions to be added for NHI.

Herbal medicine prescription analyses of bronchiectasis patients with claim data during 5 years (2013~2017) (최근 5개년 (2013~2017)간 기관지확장증(J47) 환자에게 처방한 급여한약제제 현황 분석 - 건강보험청구자료 중심으로)

  • Objectives : Korean national health insurance data is a useful real-world data representing whole medical bills submitted to Health Insurance Review Agency. This study aims to understand recent benefit trend of insurance herbal preparations for treating bronchiectasis(disease code J47) utilizing insurance data. Methods : We reviewed national health insurance claims data from 2013 to 2017 which have main diagnosis or sub diagnosis code of J47 and with the record of prescribing insurance herbal medication. Frequency analysis was performed to analyze the most frequently prescribed prescription. Results & Conclusions : Both the number of claims statement(770 to 1,746cases) and patients(266 to 484) of insurance herbal preparations increased considerably from 2013 to 2017. Top 10 preparations based on the number of claims statement were 'Samso-eum', 'Yeonkyopaedok-san', 'Socheongryong-tang', 'Bojungikgi-tang', 'Hyangsapyungwi-san', 'Yijin-tang', 'Saengmaek-san', 'Jaeumganghwa-tang', 'Ojeok-san' and 'Gungha-tang'. Top 10 preparations based on the number of patients were 'Samso-eum', 'Socheongryong-Tang', 'Saengmaek-san', 'Yeonkyopaedok-san', 'Haengso-tang', 'Hyangsapyungwi-san', Yijin-tang', 'Jaeumganghwa-tang', 'Bojungikgi-tang' and 'Hyeonggaeyeongyo-tang' in respectiv order. Claims of top 10 frequent preparations occupied more than 60% of total claims. We hope this finding to be utilized as basic data for future research of evidence-based bronchiectasis treatment utilizing Korean traditional medicine.

The Analysis of Main Diseases and Herbal Preparations in Herbal Health Insurance (한약제제 보험급여 주상병과 처방분석)

  • Objectives : Recently, the total medical expenses of the korean oriental medical service in national health insurance is on the increase every year. Herbal medicines are one of the major methods of the medical treatment. But the expenses of these herbal preparations that can receive benefits from insurance system are decreasing. Methods : In this research, we obtained statistical data of the benefit states of herbal preparations in herbal heath insurance during year 2001-2003 from Health Insurance Review Agency. We analyzed top twenty main diseases in herbal health insurance and mainly used prescription in these diseases. Results : There were wide differences in the application of prescriptions among diseases. For example, musculoskeletal diseases occupied an important position and Ojucksan took more than 50 percentage. Conclusion : We hope that this study could be a basic data for improving the benefit system of herbal health insurance and further studies should be carried out subsequently.

Survey on National Health Insurance Coverage for Herbal Decoctions (비급여 한약의 보험급여에 대한 한의사의 인식도 조사)

  • Objective : This research aimed to investigate and analyze the opinions of Oriental Medical Doctors (O.M.D.) on covering herbal medicines into National Health Insurance scheme. Methods : Structured questionnaires were e-mailed to the O.M.D.s listed on member's DB of the Association of Korean Oriental Medicine. Collected data were analyzed with the SPSS 12.0 program. Results : The insurance coverage for herbal decoctions was supported by 80.8% of the responders, the median reasonable price for daily herbal decoctions was \9,517, the minimum price median of daily herbal decoctions considering its costs was \8,080, and the daily technical fee median was \4,379. On health insurance coverage for herbal decoctions, O.M.D.s were specially considering the standardization of herbal medicines. Conclusion : In this study, majority of O.M.D.s assented to health insurance coverage for herbal decoctions, but considered standardization of herbal medicines and optional prescription fee. It needs to prepare plan for insurance coverage of herbal decoctions that reflect the character of oriental medicine and oriental medical institutions.

Analysis of the current status of quantitative literature evidence for the prescription of 56 herbal medicines covered by health insurance (건강보험 급여 한약제제 56종 처방의 계량적 문헌 근거 현황 분석)

  • Objectives: The purpose of this study is to analyze the current state of quantitative literature evidence for the prescription of 56 herbal medicines covered by health insurance that have been studied in Korea for the past 30 years, to evaluate the reliability of the evidence, and to find out the research direction of herbal medicine prescription in the future. Methods: 56 kinds of herbal medicine prescriptions were searched in domestic literature search databases OASIS, DBpia, and overseas PubMed, classified into chemistry, toxicity, cells, animals, clinical cases, and clinical trial studies, and built into an EBM pyramid structure. Results: When classified according to research contents, there were 61 cases (7.5%) of physicochemical analysis to identify constituent substances, 80 cases (9.8%) of toxicity evaluation, and 672 cases (82.7%) of efficacy evaluation. The efficacy evidence was classified according to the evidence-based medical pyramid structure: 196 cell trials (29.1%), 372 animal trials (55.4%), 89 case and case reporting series (13.3%), 7 comparative case studies (1.1%), and 8 randomized control clinical trials (1.2%). In the pyramid composition, the basis for the validity of 56 kinds of herbal medicines prescribed was 568 cases (84.5%) in cell and animal units, which could not be said to be highly reliable. There was no relationship between the ranking of quantitative literature evidence for herbal medicine prescriptions and the ranking of salary administration. Conclusions: In an era that continues to require scientific evidence for herbal medicine, traditional herbal medicine should secure the basis for safety validity even for the 10th most frequent prescription among 56 herbal medicine prescriptions for consumers. In particular, traditional herbal medicine should increase the quantitative and qualitative level of case reports on related herbal medicine prescriptions, focusing on each clinical society, and move toward comparative case studies and randomized clinical trial so that traditional herbal medicine is positioned as Evidence-based medicine.

A Study of Ways of Paying Insurance Benefits and the Range of Insurance Coverage of Health Insurance for Packed Herbal Medicine (첩약의 건강보험 급여형태 및 급여범위에 대한 연구)

  • Objectives : The purpose of this study was to provide lawmakers with basic data for the purpose of establishing a policy on health insurance for packed herbal medicine. Methods : To properly execute an insurance system for packed herbal medicine, we surveyed the following questions from May 1st to June 15th, 2006: (1)the way to pay the insurance benefits (2)specific impression (disease) names as related to insurance (3)the range of insurance coverage. We obtained answers from 304 Korea traditional doctors. Results : 89.4% respondents were agreed on execution of health insurance for packed herbal medicine. 93.86% respondents answered that the way to pay insurance benefits and the range of insurance coverage have to be changed properly. Most respondents answered that the coverage should be based on a specific disease being treated, not the current symptoms method which relates to the prescription. The execution of health insurance for packed herbal medicine is expected to increase the number of patients and make the Korean medical system more popular. Conclusion : The findings are expected to make it easier for the general public to get treatment by providing basic data with policy makers that will cover packed herbal medicine in insurance. Also, a proper Korea traditional health insurance program should be set up as soon as possible to widen the choice of medicine for the general public.

Study on Standard Herb Prescriptions Based on the Specific Names of the Sick and Wounded for the Application of Oriental Herbal Medicine Health Insurance (한약 건강 보험 시행을 위한 특정 상병명에 따른 기준처방 조사)

  • Objectives: To study the standard herb prescriptions based on the specific names of the sick and wounded for the application of Eastern Medicine Health Insurance. Methods: We selected 678 herb prescriptions from the eleven Eastern medical books recognized by the Ministry for Health of Korea and then researched the concrete opinions of Korean Oriental Medicine Societies (KOMS) on those herb prescriptions from July 31, 2006 to August 31, 2006. Results and Conclusions: KOMS said that 80 herb prescriptions must be included and 26 herb prescriptions must be excluded. They also said that the proper numbers of the specific names of the sick and wounded for the application of Eastern Herbal Medicine Health Insurance were 20-29 and the proper range of herb prescription change was within 30 percent.

  • Objectives : This Study aims to extract the actual prescriptions used frequently in the clinical settings and the frequently used prescription list of textbook on herbal formulae by comparing the prescriptions recorded in the textbook on herbal formulae, and to examine the range of the efficacies of the corresponding prescription on the basis of the records of the Electronic Medical Record (EMR). Methods : By making comparison of the herb weight ratios of the prescriptions recorded in the textbook on herbal formulae with those recorded in EMR, the frequency is measured on the basis of the textbook on herbal formulae prescription and the frequency indication is computed for the clinical prescriptions with lower level of differences. Results & Conclusions : On the basis of the details of the clinical prescriptions used at the P Hospital, Yugmijihwang-tang, Samlyeongbaegchul-san, Jugyeobseoggo-tang, Maegmundong-tang, Olyeong-san and Baegho-tang, among the similar prescriptions for which the title prescriptions of the textbook on herbal formulae and the herbal composition coincide by more than 80%, are not included in the list of prescriptions covered under the national health insurance system even though they are frequently used prescriptions.
