Development of a 2-DOF Ankle Mechanism for Gait Rehabilitation Robots (보행 재활 로봇을 위한 2자유도 족관절 기구 개발)

  • In this paper, we designed and tested an ankle joint mechanism for a gait rehabilitation robot. Gait rehabilitation programs are designed to improve the natural leg motion of patients who have lost their walking capabilities by accident or disease. Strengthening the muscles of the lower-limbs and stimulation of the nervous system corresponding to walking helps patients to walk again using gait assistive devices. It is an obvious requirement that the rehabilitation system's motion should be similar to and as natural as the normal gait. However, the system being used for gait rehabilitation does not pay much attention to ankle joints, which play an important role in correct walking as the motion of the ankle should reflect the movement of the center of gravity (COG) of the body. Consequently, we have designed an ankle mechanism that ensures the safety of the patient as well as efficient gait training. Also, even patients with low leg muscle strength are able to operate the ankle joint due to the direct-drive mechanism without a reducer. This safety feature prevents any possible adverse load on the human ankle. The additional degree of freedom for the roll motion achieves a gait pattern which is similar to the normal gait and with a greater degree of comfort.

  • Objectives & Methods : I investigated 45 literature of Oriental and Western medicine about the treatment of pain in shoulder and arm. Result and Conclusion : 1. The etiological causes of Pain in Shoulder and Arm based on literatures of Oriental medicine are attack of wind-heat on the lung, wind cold, damp-heat struggle between the vital energy and pathogenic factor and six pathogenic factors. And all these causes are the conception of blockage syndrome, Qi and blood stagnating in meridian system. 2. The treatment of Pain in Shoulder and Arm based on Oriental medicine is mainly composed of both medical therapy for Bi syndrome due to pathogenic wind, deficiency of both Qi and blood, consumption of the liver and the spleen, and also acupuncture and moxibustion treatment by selection for acupoint. And those treatments are for treating etiology. And also there are treatments using the meridian system and Twelve Muscle Region and Ashihyeol for the purpose of treating the symptoms. 3. The etiological causes of Pain in Shoulder and Arm based on literatures of Western medicine are degenerative cut of tendon and nerve symptoms caused by tendonitis, bursitis, calcification, ruptured cervical disc and thoracic outlet syndrome. 4. The treatment of Pain in Shoulder and Arm based on Western medicine is for alleviation of pain, such as giving an anodyne, steroid products, local anesthetic injection and stretching and strengthening the muscles.

A comparison of ankle function between adults with and without Down syndrome

  • Objective: The purpose of this study was to compare ankle function between adults with and without Down syndrome (DS). Design: Cross-sectional study. Methods: Ten adults with DS and 18 without participated in this study and underwent manual muscle test (MMT), range of motion (ROM) assessment, star excursion balance test (SEBT), and functional movement screen (FMS). The tests were demonstrated to increase their accuracy and the actual measurements were assessed after one or two demonstrations. To minimize the standby time and fatigue, the travelled distance and measuring order were adjusted. To remove the influence of shoes on the measurements, the shoes were taken off and only socks were worn. Results: Dorsal and plantar flexion MMTs of both ankles were significantly weaker and plantar flexion ROM of both ankles were significantly lower in adults with DS compared with those without (p<0.05). However, dorsal flexion ROM of both ankles were not significantly different between them. There were significant differences in distances measured in all the directions (anterior, anterolateral, lateral, posterolateral, posterior, posteromedial, medial, and anteromedial directions) of SEBT (p<0.05). Significant differences were also demonstrated in the scores of hurdle step, inline lunge, shoulder mobility, and rotary stability among the seven items of FMS (p<0.05). Conclusions: To enhance the dynamic stability of adults with DS, it is necessary to improve ankle stability by strengthening the ankle dorsal and plantar flexors.

  • The purpose of this study lies in finding out the effect that variation of pain and body deflection posture has an influence on the static spinal stabilization after having performed spinal stabilization exercise making degenerative disc disease patients an object over 8 weeks using $CENTAUR^{(R)}$, 3-D spinal stabilization training implement. Subjects : 61 of DDD patients were made as an object of this study (mean age: 45.46 years, SD: ${\pm}12.78$, range: 16-68), their average height was 161.87cm, average weight 60.70kg, 12 males and 49 females were involved. Methods: 8 various investigations were performed and varied values were compared with reinvestigation done after having exercised 8 weeks using 3-D $CENTAUR^{(R)}$. We used VAS(Visual Analog Scale) in order to see the variation of pain intensity, MOS(Modified Oswestry Scale) in order to see activities of daily life. Results VAS was lessened from 7.50 to 2.71, limitation of routine life(MOS) from 20.26 to 9.32, there were remarkable differences statistically(p<0.05). As a result of muscular investigation for static spinal stabilization by 8 variations of body deflection, muscular strength were all increased and there were remarkable differences statistically(p<0.05). Conclusions : It has been turned out that pain and limitation of daily life was lessened as a result of making 61 of degenerative disc disease patients exercised 8 weeks using $CENTAUR^{(R)}$, 3-D spinal stabilization training implement, deep muscular power was increased. Thus it has been turned out that 3-D lumbar stabilization exercise has an effect on the spinal muscles strengthening and alleviation of their pain for degenerative disc disease.

Effects of Sling Exercise on Spinal Flexibility and Dynamic Balance in Adolescent Scoliosis Patients (청소년기 척추측만증 환자에 대한 슬링운동이 척추유연성과 동적 균형에 미치는 영향)

  • Background: The purpose of this study was compare and analyze the flexibility and dynamic balance in adolescents with idiopathic scoliosis and provide scientific basis for effectively treating idiopathic scoliosis. Methods: The subjects of this study were 20 members of adolescences. To measure the flexibility of the spine, a left buckling instrument was used, and it was measured using a modified sit and reach test. They were randomly divided into core exercise with sling program group (n=10) and general exercise program group (n=10), each consisting 50 minutes each for 4 times per week for 8 weeks. Results: The results of the study are as follows. First, Both groups were increased statistically significantly in spinal flexibility but no significant differences have been found between the two groups after 8 weeks of exercise. Second, Both groups were increased statistically significantly in balance ability of dynamic foot pleasure ratio but no significant differences have been found between the two groups after 8 weeks of exercise. Conclusion: In summarizing the results of this study, sling exercise with core exercise was effective in the decrease of flexibility and stability than general scoliosis exercise in adolescents with idiopathic scoliosis. It is also believed to be applicable to spinal diseases caused by muscular weakness since it is effective in strengthening core muscle strength.

Effect on Thoracic Exercise Programs in Employees With Chronic Low Back Pain (만성 요통근로자의 흉추운동프로그램 효과)

  • The objectives of this study were to examine the relative efficacy of three active exercise programs for work-related, chronic low back pain, and to observe to what extent the programs affected the mechanical stability of the lumbar region. The subjects were 64 employees who were randomly divided into three groups to match the three active exercise programs which were performed 3 times a week for 6 months. All subjects were assessed with the same measurements at a pre-study examination, and then were reassessed at 2 weeks, 3months and 6 months after the study. The pain intensity didn't show any significant difference among the three groups. However, the Oswestry Disability Index showed significant differences among the three groups at 6 months and the lumbar and thoracic exercise groups showed significant decreases compared to the general physiotherapy group (p<.05). Maximal stretching with both hands in the overhead direction showed a significant difference among the three groups at 3 months and 6 months, and the thoracic exercise group at 6 months showed a significant increase in overhead stretching compared to the lumbar exercise and general physiotherapy groups (p<.05). The group that performed maximal stretching with both hands in the overhead direction showed the most significant among the 3 months and 6 months. At 6 months, the thoracic exercise group showed a significant increase in overhead stretching compared to the lumbar exercise and general physiotherapy groups (p<.05). The lumbar region angle of inclination showed significant differences among the three groups at 2 weeks 3 months, and 6 months, with the thoracic exercise group being decreased more significantly at 6 months than the lumbar exercise and general physiotherapy groups (p<.05). Exercise aimed at increasing thoracic mobility has an effect on lumbar stability. Furthermore, it is far more effective for lumbar stabilization than general physiotherapy and deep muscle strengthening lumbar exercise.

Influence of Hip Abduction Velocity and Position on the Onset Times of Gluteus Medius and Tensor Fascia Latae Relative to Quadratus Lumborum in Healthy Subject: A Pilot Study

  • PURPOSE: The aim of this study was to investigate the influence of hip abduction velocity and position change on the relative onset times of the gluteus medius, the tensor fascia latae, and the quadratus lumborum in healthy subjects. METHODS: For this study, 15 healthy young adults were recruited. The subjects were asked to move their hip joints up to 35 degrees of abduction at a speed of $70^{\circ}/sec$ and $17.5^{\circ}/sec$ in the supine and side-lying positions. Electromyography data was collected for the gluteus medius, tensor fascia latae, and quadratus lumborum to determine the onset times. RESULTS: There were significant differences between the fast speed ($70^{\circ}/sec$) and the slow speed ($17.5^{\circ}/sec$) in hip abduction in a supine position and in a side-lying position, relatively. The onset time of the gluteus medius was faster than that of the tensor fascia latae and the quadratus lumborum in the side-lying position at the speed of $70^{\circ}/sec$ and $17.5^{\circ}/sec$. CONCLUSION: The findings of this study indicated that hip abduction in a side-lying position is an effective method to recruit the gluteus medius earlier than the tensor fascia latae and the quadratus lumborum. Thus, the exercise position is considered necessory in the purpose of rehabilitation for gluteus medius muscle strengthening program.

  • Patellofemoral pain syndrome is the most common problem involving the knee, accounting for 25% of knee injuries. Repetitive, overuse activities cause increased force at the patellofemoral joint, resulting in pain during flexion and extension activities. Most research have been conducted in exploring the patellofemoral compressive force in gait, squat and lunges, even though in real cases, possibilities in landing exist. The purpose of this study was to compare the differences in patellofemoral compressive force according to two different height. Sixteen collegiate male students(age: 22.25 ${\pm}$ 3.30 yrs, height: 177.25 ${\pm}$ 4.44 cm, weight: 77.50 ${\pm}$ 8.18 kg) were chosen. The subjects performed drop landings in 45 cm, 60 cm. The findings demonstrated that higher height showed peak knee extension moment, quadriceps contraction force, patellofemoral compressive force with increased VGRF. Regarding the patellofemoral joint compressive force, it increased by quadriceps contraction force with knee flexion during landing, yet, it showed no difference in maximal knee flexion. To minimize patellofemoral joint stress and reduce the likelihood of developing PFPS, we recommend that predesigned quadriceps and hip muscle group strengthening are needed during conditioning and training.

Changes in Grip Strength and Associations with Grip Strength in Breast Cancer Survivors Treated with Adjuvant Chemotherapy

  • Purpose: The aim of this study was to identify the change in hand grip strength (HGS) for consecutive 6 months in breast cancer survivors (BCS) undergoing postoperative adjuvant chemotherapy (adjCTX) and to determine the factors relevant to HGS after chemotherapy completion Methods: This study was a retrospective observational cohort study. BCS (N=38) who underwent breast cancer surgery and subsequent adjCTX were enrolled. The HGS of both sides was measured before adjCTX and 3 weeks, 2 months and 6 months after adjCTX. And we also collected body composition, anthropometric measurements, social demographics and clinical data. Then we analyzed the changes in HGS over time, and correlation of collected data with HGS at 6 months. Results: A significant increase in HGS of the unaffected side was observed within the first 3 weeks of adjCTX and maintained up to 6 months. In correlation analysis, HGS of both sides was positively correlated with the ipsilateral HGS at 3 weeks, and contralateral HGS at 6 months. And skeletal muscle mass was also positively correlated with the HGS of the unaffected side. In addition, subjects treated with targeted therapy had lower HGS of the unaffected side. However, HGS of the affected side was significantly lower in subjects with breast reconstruction and treated without radiotherapy. Conclusions: To maintain the early recovery of HGS, active and continuous exercise intervention for strengthening might be necessary.

The Effects of Epimedii Herba on a Hypothyroidism Rat Model induced by PTU(6-Propyl, 2-thiouracil) (음양곽(淫羊藿)이 실험적으로 유발된 갑상선 기능저하증 동물모델에 미치는 영향과 안전성)

  • Objectives: The symptoms of hypothyroidism are fatigue, cold intolerance, arthralgia, muscle cramps, dry skin and etc. Although hypothyroidism is a relatively common endocrinical disease, we do have many difficulties treating it effectively. The symptoms of hypothyroidism are similar to those of Yang-deficiency in Oriental medicine. Epimedii Herba is a popular herb that has the effect of tonifying the kidney and strengthening Yang in Oriental medicine. In this study, we investigated the therapeutic effects of Epimedii Herba on a hypothyroidism rat model induced by PTU(6-Propyl, 2-thiouracil). Methods: 24 two-month-old Spargue-Dawley(SD) rats were divided into 4 groups: 1) normal(n=6), 2) PTU-induced hypothyroidism control(n=6), 3) hypothyroidism rat treated with Epimedii Herba(n=6), 4) hypothyroidism rat treated with levothyroxine(n=6). PTU was administered for 4 weeks, Epimedii Herba and levothyroxine was administered for 2 weeks after PTU was initiated for a total duration of 2 week. At the end of the experiment, blood samples from all the rats were taken from their hearts and were analyzed. Results: In comparison with normal group, the PTU-induced control group significantly showed hypothyroidism with low T3, T4 and high TSH. In Epimedii Herba group, T4 was significantly increased(p<0.05). There was no significant difference in TSH between the Epimedii Herba treatment group and the control group. And no significant differences were observed in biochemical labs and weight between the Epimedii Herba group and the control group. Conclusions: These results suggest that Epimedii Herba could help thyroid cells to produce thyroid hormones. And no significant side effects related with Epimedii Herba were found, suggesting that it is safe to administer. According to these results, Epimedii Herba may be a safe alternative medicine for hypothyroidism.
