Pain Relieving Effect of Intraoral Sucrose Replacement in Neonates (신생아의 통증완화를 위한 자당(sucrose) 경구투여 효과)

  • The ability of neonates to perceive and react to pain, has been acknowledged recently. Recent researches have been shown that even short term pain can have lasting negative effects. We know that most of the anatomical pathway and neurotrans-mitter function necessary for pain perception are fully or nearly fully developed in the neonatal period. Many people are still reluctant to believe that pain felt by neonates may be as severe as that felt by older children or adults yet. The objective of the study is to assess and compare the analgesic effects of orally administered sucrose and pacifiers. And to determine the synergistic analgesic effects of sucrose and pacifiers. The tools for this study is Lawrence's NIPS (neonatal infant pain scale) with behavioural pain responses and index for pain physiological reponses as heart rate, respiratory rate and degree of SaO₂. The participants are 96 healthy infants of neonatal age 1-7days and randomised to receive heel prick before 2minutes to blood sampling and physiological test in Nursery of K Medical University Hospital on May-July, 2000. The experimental group assigned to one of three treatment groups : no treatment; a pacifier; placebo(2ml 25% sucrose). Collected data were analyzed with the SAS program using X²-test, ANOVA and Duncan's multiple range test as post hoc. The results were as follows. 1) Pain behaviour responses: The pain score of placebo group(2ml 25% sucrose) is lower than no treatment group and pacifier group significantly(P=.000). Placebo group is different from no treatment and pacifier group with Duncan's multiple range test. 2) Pain physiologic responses (1) heart rate: The heart rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group significantly(P=.000). The heart rate change of placebo group is less than the other groups. (2) respiratory rate: The respiratory rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.2340). But, the respiratory rate of placebo group is lower than the other groups. (3) SaO₂: The SaO₂ score of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.3265). But, the change of SaO₂ score of placebo group is less than the other groups. In conclusion, the sucrose placebo showed pain relief effect in behavior responses and less physiological responses. Accordingly, the sucrose placebo should be applied nursing intervention for simple pain management as heel prick.

Treatment Outcome and Prognostic Factors of Cushing's Disease in Adults (성인 쿠싱병의 치료성적과 예후인자)

  • Objectives : The authors analyzed the surgical series of Cushing's disease to evaluate the proper treatment policy and to verify the possible prognostic factors. Material and Methods : Of 50 patients diagnosed as Cushing's disease and operated at Department of Neurosurgery of our institute between 1988 and 1999, 48 patients with available medical records were analyzed retrospectively. Mean follow-up period was 48 months(3 to 109 months). Preoperative diagnosis was made after evaluating the patients with multiple-stage endocrinological studies and 31 selective patients were evaluated with inferior petrosal sinus sampling(IPSS). Magnetic resonance imaging(MRI) and/or high resolution computerized tomography(CT) was done in all patients. A total of 51 transsphenoidal adenomectomy(TSA) were performed including 3 revision for initial surgical failure cases. Remission was decided on the basis of both endocrinological criteria and clinical status. Radiation and/or ketoconazole therapy were applied to failed cases. For the verification of prognostic factors, the authors evaluated the statistical significance of multiple variables over remission rate by chi-square test. Result : Sensitivity of IPSS for central localization was 93.5% which was better than that of MRI(87.5%). But for lateralization, it was 72.4% for IPSS versus MRI 90.5%. Success rate of TSA was 82%(42/51) and recurrence rate was 9%(4/48). When including adjuvant treatments for surgically failed cases, overall success rate was 89.6% and all of 3 reoperated cases(TSA) due to recurrence were successful. Significant complication occurred in 7.8%(4/51) after TSA including hypopituitarism, diabetes insipidus, and visual loss. Non-existence of tumor in MRI and prolonged symptom duration(>3 years) were significant prognostic factors. Conclusion : TSA can be considered as initial treatment for Cushing's disease. In surgically failed cases, multiple treatment modality may improve the overall outcome and repeated TSA for recurrent cases seem to provide similar success.

  • Objectives Advanced researches on the relationship between obesity and heart rate variability (HRV), heretofore, focused on characteristics of HRV depending on the state of obesity. However, the previous researches have not quantified predictive power of HRV toward the obesity-related variables, which is rather more meaningful for clinicians who regularly treat obese patients. Hence, we designed a research to investigate whether HRV could predict serum levels of obesity-related metabolites. Methods Ninety obese premenopausal women meeting the inclusion criteria were recruited. The HRV test, blood sampling, and measurement of physical traits were conducted. Multiple regression analysis of the measurement data was carried out, putting obesity-related metabolites (insulin, glucose, triglyceride, hs-CRP, HDL, LDL, total cholesterol) as outcome variables and the others as predictors. To select appropriate predictive variables, the Akaike's Information Criterion (AIC) was applied. Normality and homoskedasticity of residuals for each model were tested to identify if there were any violations of the regression analysis's basic assumption. Logarithm transformation was used for the values of the concentration of metabolites and the HRV. Results The regression model including Total Power (TP) value and BMI had significant predictive power for serum insulin concentration (F(2, 88)=835.7, p<0.001, $R^2=0.95$). The regression coefficient of ln (TP) was -0.1002. However, it was not sure if the HRV could predict concentrations of other metabolites. Conclusions The results suggest that the Total Power (TP) value of the HRV can predict the level of serum insulin. If the BMI could be assumed as being constant, when the TP value is multiplied by n, the predicted change of insulin could be drawn by multiplying $n^{-0.1002}$. The uncertainty of this model can be assumed as approximately 5%.

  • Genetic variance and covariance components of the linear traits and the ordered categorical traits, that are usually observed as dichotomous or polychotomous outcomes, were simultaneously estimated in a multivariate threshold animal model with concepts of arbitrary underlying liability scales with Bayesian inference via Gibbs sampling algorithms. A multivariate threshold animal model in this study can be allowed in any combination of missing traits with assuming correlation among the traits considered. Gibbs sampling algorithms as a hierarchical Bayesian inference were used to get reliable point estimates to which marginal posterior means of parameters were assumed. Main point of this study is that the underlying values for the observations on the categorical traits sampled at previous round of iteration and the observations on the continuous traits can be considered to sample the underlying values for categorical data and continuous data with missing at current cycle (see appendix). This study also showed that the underlying variables for missing categorical data should be generated with taking into account for the correlated traits to satisfy the fully conditional posterior distributions of parameters although some of papers (Wang et al., 1997; VanTassell et al., 1998) presented that only the residual effects of missing traits were generated in same situation. In present study, Gibbs samplers for making the fully Bayesian inferences for unknown parameters of interests are played rolls with methodologies to enable the any combinations of the linear and categorical traits with missing observations. Moreover, two kinds of constraints to guarantee identifiability for the arbitrary underlying variables are shown with keeping the fully conditional posterior distributions of those parameters. Numerical example for a threshold animal model included the maternal and permanent environmental effects on a multiple ordered categorical trait as calving ease, a binary trait as non-return rate, and the other normally distributed trait, birth weight, is provided with simulation study.

  • To assess quantitatively the association between benzene exposure and leukemia, we examined the mortality rate of a cohort with occupational exposure to benzene. Cumulative exposure for each cohort member was estimated from historical air-sampling data and, when no sampling data existed, from interpolation on the basis of existing data. The overall standardized mortality ratio (a measure of relative risk multiplied by 100) for leukemia was 337 (95 percent confidence interval, 154 to 641), and that for multiple myeloma was 409 (95 percent confidence interval, 110 to 1047). With stratification according to levels of cumulative exposure, the standardized mortality ratios for leukemia increased from 109 to 322, 1186, and 6637 with increases in cumulative benzene exposure from less than 40 parts per million-years (ppm-years), to 40 to 199, 200 to 399, and 400 or more. respectively. A cumulative benzene exposure of 400 ppm years is equivalent to a mean annual exposure of 10 ppm over a 40-year working lifetime; 10 ppm is the currently enforceable standard in the United States for occupational exposure to benzene. To examine the shape of the exposure-response relation, we performed a conditional logistic-regression analysis, in which 10 controls were matched to each cohort member with leukemia. From this model, it can be calculated that protection from benzene induced leukemia would increase exponentially with any reduction in the permissible exposure limit.

Eight Weeks Twenty Meters Walk Aerobic Exercise Improve Cardio-respiratory Fitness and Muscular Strength of Stroke Survivor Outpatients in Tertiary Hospitals in Osogbo, Nigeria

  • PURPOSE: This study examined the effects of an eight weeks twenty meters walk on the cardiorespiratory fitness and strength of the shoulder extensor, hip extensor, and dorsiflexor of stroke survivor outpatients in two tertiary hospitals in Osogbo, Nigeria. METHODS: A purposive sampling technique was used to select 21 registered right or left outpatient hemiplegic stroke survivors in a pre- and post-test experimental research design. The research questions were presented using the descriptive statistics of frequency, percentage, mean, and standard deviation. The differences between the mean of the cardio-respiratory indices and the muscle strength were tested by repeated measures analysis of variance followed by Bonferroni post-hoc test for multiple comparisons. RESULTS: No significant differences in heart rate were observed at week 0 compared to week 4. A significant decrease was recorded in the parameter at week 8, compared to week 4. Moreover, there were significant decreases in blood pressure and respiratory rate in week 0, compared to week 4, and in the respiratory rate, compared to week 8. In contrast, significant elevations in VO2 max were observed in week 0, compared to week 4, and in the week 4, compared to week 8. Furthermore, significant elevations in muscular strength were documented when comparisons were made at weeks 0, 4, and 8. CONCLUSION: Eight weeks of twenty meters walk aerobic exercise improve the cardio-respiratory fitness and muscular strength of stroke survivor outpatients.

Analysis of Sexual Behaviors among Adults in Korea: Results from the "Korean National Survey on Sexual Consciousness"

  • Purpose This study aimed to understand the characteristics of sexual behaviors among Korean adults to facilitate the development of strategies and policies focused on sexual health in groups categorized by sociodemographic characteristics. Materials and Methods A nationally representative probability sample of 2,500 individuals (1,273 men and 1,227 women) aged 18-69 years obtained using a stratified multiple-stage sampling method based on Statistics Korea (KOSTAT) participated in a cross-sectional online survey. The survey consisted of structured questionnaires comprising questions on demographic information, lifetime sexual behavior, and sexual behavior in the previous 12 months. The mean age at first sexual intercourse was lower in men than in women (21.9±4.4 vs. 24.1±4.4 years, p=0.001). The overall prevalence rate of sexual events with casual partners within previous 12 months was 13.1% (95% confidence interval [CI], 11.6%-14.5%). It was found to more commonly exist among lower age groups and men. The overall regular condom use rates with relationship partner and casual partner were 14.8% (95% CI, 13.2%-16.4%) and 39.6% (95% CI, 33.9%-45.3%), respectively. Condom use rate with casual partners among 20s and 30s men was 51.2%. Overall, only 10.4% of the respondents had received sexual education about sexually transmitted infections. This study provided contemporary sexual behaviors in Korean adults, and identified socio-demographic factors that seem to influence sexual behaviors. Low condom use rates and low rate of receiving sexual education were concerns. The result of this study would be useful to health professionals to formulate policies and strategies related to sexual health.

Development of Rapid Detection Method for Volatilized Formaldehyde from Wood (목재 폼알데하이드 신속검출 공정개발)

  • We designed a new rapid detection method for volatilized formaldehyde from wood. The process was installed with volatilizing and collecting parts in an incubator. For rapid sampling of formaldehyde from wood, we pulverized the wood to sawdust, and used 0.15-2.0 mm particles for the tests. The highest sampling rate (94.8%) was obtained at 40 mL/min flow rate and $100^{\circ}C$. Under the optimized condition, we could collect the volatilized formaldehyde with good recovery rate. The developed method was applied to the monitoring of the formaldehyde from wood, and the measured concentrations were 0.7-4.6 ${\mu}g/g$ from natural wood, 5.9-12.3 ${\mu}g/g$ from preserved wood, and 5.9-211.5 ${\mu}g/g$ from chemical adhesive processed wood. From the results, we identified natural wood sawdust and chemically processed wood (medium density fiberboard, high density fiberboard, particle board) by the formaldehyde contents except preserved wood.

Dementia Knowledge and Related Factors in Middle-aged Adults (중년 성인의 치매지식과 관련요인)

  • Purpose: The purpose of this study was to identify the level of knowledge about dementia and to find the related factors in middle-aged adults. Method: The research design was a descriptive study by using a constructive self-report questionnaire with 34 items. Data were collected from 397 middle-aged adults using structured questionnaire. Data analysis was done by SPSS/WIN Programs using frequency, range from minimum to maximum, percentage, mean, SD, ${\chi}^2-test$, t-test, ANOVA, and Duncan's Multiple Range test. Results: The rate of correct answer of middle-aged adults was 68.7%, showing the mid-level of knowledge about dementia. There were significant differences according to age, education, marital status, job, income, information and information sources for dementia in middle-aged adults. Conclusion: These results indicate that Korean middle-aged adults need more education and counseling on dementia. Further studies are needed to overcome regionalism in sampling of this study and to develop standard measuring tools for the level of knowledge about dementia.

  • Purpose: Identify the factors that influence following up control and reuse intention of patient who used dental health care institution after dental heath care service and in order to prepare the method which improve the quality of dental health service which dental heath care institution service afford. Methods: Data were collected through random sampling from June 20th to August 20th 2010 (for 60 days). Once we explained the purpose of our survey to people who experienced the dental service within one year, we distributed the questionnaires to someone who volunteered to respond and they answered all questions by themselves based on the actual experience of dental health care organizations. Even if the survey was conducted for 610 people, only 585 properly answered questionnaires were analyzed because responses which had many unanswered questions and had errors in responsive way were excluded. Results: Result of multiple regression analysis, the value of dental clinic service, the following up control after dental treatment, the technique and kindness of dentist, the environment of treatment, type of dental service and the kindness of dental staff is significant main cause to intention of reuse dental clinic. Conclusion: In order to increase the rate of patient reuse, enhance the value of service with following up control after health treatment and the high quality of dental health service.
