일개 지역 요양병원 간병인의 감염관리수행도 관련성 연구 (A Study on the Relationship of Infection Control Performance in a Long Term Care Hospital Caregivers)

  • 본 연구의 목적은 일개 지역 요양병원 간병인의 일반적 특성, 감염관리 특성, 감염예방지식과 감염관리환경을 확인하고 감염관리 수행도에 미치는 요인을 확인하는데 있다. 연구대상자는 일개 지역 8개 요양병원의 간병인 197명이며, 자료수집은 자가기입 설문지를 이용하였으며, 2018년 8월에 진행되었다. 자료분석은 SPSS WIin 20.0을 이용하여 t-test, ANOVA, pearson's correlation coefficient, hierarchial multiple stepwise로 분석하였다. 연구결과 감염 예방지식은 감염관리수행도와 유의한 상관관계(p<.001)가 있었다. 요양병원 간병인의 감염관리수행도 영향요인은 70세 이상(t=2.50, p=.013), 8시간 근무(t=-2.62, p=.010), 1등급(t=2.48, p=.014), 감염예방지식(t=2.96 p=.003)이었으며, 제 변수들은 요양병원 간병인 감염관리수행도의 12.9%를 설명(F=6.70, p<.001)하였다. 본 연구결과는 요양병원 간병인의 감염관리수행도 영향요인을 확인함으로써 추후 간병인의 감염관리수행도 향상을 위한 교육프로그램 개발과 요양병원의 감염관리 수준을 향상시키는데 기초자료가 될 수 있다.

간호학생의 병원감염관리에 대한 지식, 태도 및 자신감 (Knowledge, Attitude and Self-Confidence of Student Nurses Regarding Nosocomial Infection Control)

  • Purpose: This study was conducted to identify the knowledge, attitude and self-confidence of student nurses regarding control of nosocomial infections and to provide data for preparation of an efficient policy and education program regarding nosocomial infections. Method: A descriptive survey design was utilized, and, using a questionnaire, which included 15 items for knowledge, 81 items for attitude, 81 items for self-confidence, data were collected from 520 student nurses. Descriptive statistic, t-test, one-way ANOVA and Pearson's correlation coefficients, and Stepwise multiple regression analysis were used with SPSS win 12.0 to analyze the data. Results: The mean score for knowledge was 0.63, for attitude, 4.38 and for self-confidence, 3.50. There was a significant positive correlation between knowledge, attitude and self-confidence in control of nosocomial infections. Attitude, practical attitude, and education were significant factors affecting self-confidence regarding nosocomial infection control. Conclusion: These research findings should be useful in promoting continuous and repeated educational programs on nosocomial infection for student nurses.

Rhinovirus and childhood asthma: an update

  • Asthma is recognized as a complex disease resulting from interactions between multiple genetic and environmental factors. Accumulating evidence suggests that respiratory viral infections in early life constitute a major environmental risk factor for the development of childhood asthma. Respiratory viral infections have also been recognized as the most common cause of asthma exacerbation. The advent of molecular diagnostics to detect respiratory viruses has provided new insights into the role of human rhinovirus (HRV) infections in the pathogenesis of asthma. However, it is still unclear whether HRV infections cause asthma or if wheezing with HRV infection is simply a predictor of childhood asthma. Recent clinical and experimental studies have identified plausible pathways by which HRV infection could cause asthma, particularly in a susceptible host, and exacerbate disease. Airway epithelial cells, the primary site of infection and replication of HRV, play a key role in these processes. Details regarding the role of genetic factors, including ORMDL3, are beginning to emerge. This review discusses recent clinical and experimental evidence for the role of HRV infection in the development and exacerbation of childhood asthma and the potential underlying mechanisms that have been proposed.

  • Purpose: This study aimed to identify the effects of nursing students' ethical awareness, ethical decision-making and attitude toward performance on intention about performance of infection control guidelines in pandemic infectious diseases. Methods: The survey was performed on 163 nursing students in three universities. Data were collected using a structured questionnaires and analyzed with t-test, analysis of variance (ANOVA), Pearson's correlation and multiple regression analysis. Results: Perceived health status, ethical awareness and attitude toward performance of infection control guidelines about major commitment were significant predictive variables. These variables accounted for 48.1% of the variance in major commitment. Conclusion: The findings indicate the necessity of developing educational programs to enhance nursing students's ethical awareness, and increase performance of control guidelines to prepare for the pandemic infectious diseases.

  • Stenotrophomonas maltophilia, a well-established opportunistic bacterium, primarily impacts healthcare settings. Infection of the musculoskeletal system with this bacterium is rare. We report on the first known case of hip periprosthetic joint infection (PJI) caused by S. maltophilia. The potential for development of a PJI caused by this pathogen should be considered by orthopaedic surgeons, particularly in patients with multiple severe comorbidities.

Selection of Multienzyme Complex-Producing Bacteria Under Aerobic Cultivation

  • The selection of multienzyme complex-producing bacteria under aerobic condition was conducted for improving the degradation of lignocellulosic substances. The criteria for selection were cellulase and xylanase enzyme production, the presence of cellulose-binding domains and/or xylan-binding domains in enzymes to bind to insoluble substances, the adhesion of bacterial cells to insoluble substances, and the production of multiple cellulases and xylanases in a form of a high molecular weight complex. Among the six Bacillus strains, isolated from various sources and deposited in our laboratory, Paenibacillus curdlanolyticus B-6 strain was the best producer of cellulase and xylanase enzymes, which have both cellulose-binding factors (CBFs) and xylan-binding factors (XBFs). Moreover, multiple carboxymethyl cellulases (CMCases) and xylanases were produced by the strain B-6. The zymograms analysis showed at least 9 types of xylanases and 6 types of CMCases associated in a protein band of xylanase and cellulase with high molecular weight. These cells also enabled to adhere to both avicel and insoluble xylan, which were analyzed by scanning electron microscopy. The results indicated that the strain B-6 produced the multienzyme complex, which may be cellulosome or xylanosome. Thus, P. curdlanolyticus B-6 was selected to study the role and interaction between the enzymes and their substrates and the cooperation of multiple enzymes to enhance the hydrolysis due to the complex structure for efficient cellulases and xylanases degradation of insoluble polysaccharides.

Multiple liver infection by Sphingomonas sp. and Bacillus sp. in a wild rodent (Apodemus agrarius): a case report

  • The first case of liver infection caused by Sphingomonas sp. and Bacillus sp. in a wild rodent is reported. A captured wild rodent, Apodemus agrarius (A. agrarius), presented with multiple liver abscess-like nodules (diameter 0.7~2.4 mm) in which Gram-positive and Gram-negative bacilli were detected simultaneously. These were grown in aerobic and anaerobic cultures, respectively, and were identified as Sphingomonas sp. and Bacillus sp., respectively, according to 16S rRNA sequencing.

PRECEDE 모형에 기반한 119구급대원의 감염관리 수행 관련 요인 분석 (Analysis of the factors related to the infection control practice of 119 emergency medical service providers based on the PRECEDE model)

  • Purpose: Emergency medical service (EMS) personnel are at high risk of spreading infection. In this study, we used the PRECEDE model to identify the knowledge, status, and barriers to infection control among Korean paramedics to provide basic infection control data. Methods: A total of 164 respondents were analyzed for the study. A questionnaire was administered and collected through an online self-response platform. Descriptive analysis, t-test, ANOVA, multiple regression, and logistic regression analyses were performed to determine infection control practices and associated factors using SAS 9.4. To identify the pathways and direct, indirect, total effects based on the PRECEDE model, we used AMOS 26.0. Results: Highly rated self-efficacy (OR 8.82, 95% CI: 3.23-24.09), awareness (OR 6.05, 95% CI: 2.06-17.72), and enabling factors (OR 3.23, 95% CI: 1.18-8.78) led to superior infection control. As a result of the structural model analysis, the highly rated enabling factors and awareness led to superior practice patterns. Conclusion: Practice is related to self-efficacy, awareness, and enabling factors; however, further research is needed to develop strategies for infection control. In particular, institutional arrangements are needed to improve the enabling factors. Improving infection control performance may lead to better infection control and enhanced protection of EMS personnel and patients against infection risks.

  • Recently, the hospital infection has emerged as a major concern in the media. In everyday life, but not good if the infection refers to infection with the population living in the hospital. The case with which the equipment is used in a hospital if the person mediating and mediated, patients sometimes with the pathogen causing the disease to their patients. disease in our country for some time, and Mers is finished, large hospitals are taking place is a change in hygiene and operating systems. The Mers is spreading visits and influenced the patient or patient care. And the hygiene to limit the visiting hours for hospital infection prevention and stabilization of the patient. The infection, especially infection among patients and patients but a number of factors, from the emergency room or intensive care unit of frequent hospital contacts between patients with infection and of course, furniture for storing multiple people to use in the ward also become a source of infection. Hospital, another patient is a Cabinet that used to accept the next patient will cause a secondary infection and the role of infectious agents. Therefore, the general public will have a nervousness in the hospital, also medical care needed to avoid the problem occurred. This study examines such issues for the Cabinet of the hospital with a secondary infection is concerned, eco-friendly and hygienic storage furniture and one-off development that will help to improve the hospital environment for research purposes.

치과위생사의 감염관리 인지 및 실천도 연구 :인증치과병원과 비인증치과병원의 비교 (Awareness and practice of infection control in dental hygienists: A comparison between accredited and non-accredited dental hospitals)

  • Objectives: The purpose of this study was to examine the perception of dental hospital accreditation and the awareness and practice of infection control in dental hygienists. Methods: A self-reported questionnaire was completed by 238 dental hygienists working at dental hospitals in Gwangju Jeonnam and analyzed from October 24, 2016 to September 22, 2017. Data were analyzed with the independent t-test, one-way analysis of variance, Pearson's correlation analysis, and multiple regression analysis using the Statistical Package for the Social Sciences software, version 21.0. Results: Compared to non-accredited dental hospitals, all three variables were high for accredited dental hospitals. In accredited dental hospitals, healthcare accreditation expectancy effects correlated to awareness (r=0.407) and practice (r=0.533) of infection control, and awareness of infection control correlated to its practice (r=0.725). In non-accredited dental hospitals, healthcare accreditation expectancy effects correlated to awareness of infection control (r=0.239), and awareness of infection control correlated to its practice (r=0.481). Accredited dental hospitals showed healthcare accreditation expectancy effects (${\beta}=0.258$) and awareness of infection control (${\beta}=0.556$), and non-accredited dental hospitals were influenced by the number of employees (${\beta}=0.567$) and awareness of infection control (${\beta}=0.376$). Conclusions: It is necessary to develop efficient and systematic infection control programs to improve the awareness and practice of infection control in dental hygienists and patient's safety in the clinical field.
