만성 간 질환자의 피로, 우울 및 수면장애 (A study of Fatigue, Depression and Sleep Disorders in Patients with Chronic Liver Disease)

  • The purpose of this study is to investigate the level of fatigue, depression and sleep disorders and to find the correlation between them in patients with chronic liver disease. Methods: The participants consisted of 130 patients with chronic liver disease who visited the C University Hospital in Daegu, Korea from July to August, 2011. Data were collected by a self-reporting questionnaire of the Revised Piper Fatigue Scale, Depression Inventory (BDI) and Sleep Scale. Collected data were analyzed by using PASW Statistics 18.0 program for descriptive statistics, T-test, one-way ANOVA and Correlation Analysis. Results: There were significant differences in fatigue scores (t=8.415, $p$=.004) and depression scores (t=10.08, $p$=.002) between subjects with symptoms of liver disease and those with no symptoms. There was no significant difference in sleep disorder scores. In addition, there is a significant correlation between fatigue and depression (r=.641), linking fatigue and sleep disorders (r=.578), and between depression and sleep disorders (r=.572). Conclusion: It is necessary to develop multidisciplinary intervention programs to relieve patientsuffering.

Development of Patients Environmental Sustainability Performance in Healthcare Sector: A Conceptual Framework and Further Research Directions

  • Purpose - The purpose of this development the business model of the healthcare sector in order to promote patents satisfaction towards medical sector services improvement for the medical business model innovation to possess a competitive advantage in the medical and pharmaceutical industry. Research design, data, and methodology - Safety standard protocol from existing multidisciplinary literature is a process of theorization, which uses grounded theory methodology rather than a description of the data and the targeted phenomenon by using Jabareen (2009). The first task is to map the spectrum of food safety literature regarding the phenomenon in safety management. This process includes developing the implementation factors and other sources such as existing business models and practices into the protocol design. Results - The study suggests the conceptual framework to improve the safety management for patients' environmental sustainability performance. Conclusion - The business model may support the beneficial aspect to healthcare government's policymakers, hospital employees, and medical specialist who can apply the practical perspective of its value regarding an educational protocol. Originality/value - This study contributes to and extends our understanding of environmental sustainability performance, identifying the rationale for safety standards performance in the healthcare industry with suggested hybrid safety standards market consumer interconnector.

노인간호 교육프로그램이 간호사의 노인에 대한 지식, 태도 및 간호수행도에 미치는 효과 (The Effect of Elderly Nursing Care Education Programs on Nurses' Knowledge, Attitudes and Nursing Performance Toward the Elderly)

  • Purpose: The purposes of this study were to develop an elderly care nursing education program and evaluate its effect on knowledge, attitudes and nursing performance of nurses toward older adults. Methods: A quasi-experimental study with a non-equivalent control group pretest-posttest design was used. Participants were recruited from two geriatric hospitals. The sample consisted of an intervention group (n=29) that participated in the educational program and a control group (n=30). The data were collected prior to and 6 weeks after the intervention using self-administered questionnaires. Data analyses utilized $x^2$-test, Fisher's exact probability test, and t-test. Results: After the intervention, knowledge and nursing performance about the elderly care increased significantly in the experimental group compared to the control group. However, there was no statistically significant difference between the groups in their attitudes toward the elderly. Conclusion: Findings indicate that to provide better nursing care to the elderly, nurses need to be able to practically apply the knowledge they have acquired from the elder-care nursing education programs in hands-on situations. To achieve this, nurses should work to change their personal attitudes about the elderly. Also, evidence-based guidelines for elderly nursing care, administrative support of institutions, and multidisciplinary approaches are required.

COVID-19와 두경부암 (Head and Neck Oncologic Care During the COVID-19 Pandemic)

  • It is well established that cancer patients are more susceptible to infection because of the immunosuppressive status caused by both disease itself and anticancer treatment, including surgery and chemoradiation. Head and neck cancer (HNC) patients are generally at high-risk for coronavirus disease 2019 (COVID-19) infection and serious adverse outcomes. Although there is an urgent need for guidance in the delivery of safe, quality oncologic care, no international consented recommendation addressed the management of HNC patients in COVID-19 due to limited data. In this review, we summarized the consideration for head and neck oncologic care in the context of the COVID-19 pandemic, based on the data and the very recent recommendations from the Korean Cancer Association and National Cancer Center. COVID-19 should be taken into consideration in the comprehensive management of HNC patients, and multidisciplinary evaluation of multilevel surgical-risks, discussion of optimized strategy, and shared-decision-making with the patient are needed to maximize both the safety from infectious pandemic and outcome of surgical and oncologic care.

명량해전 당일 울돌목 조류.조석 재현을 통한 해전 전개 재해석 (A Revisit to the Myungryang Naval Battle through Hindcasting Tidal Currents and Tides)

  • As a multidisciplinary study encompassing oceanography and history, we have attempted to reanalyze the course of a historical navel battle, Myungryang Naval Battle(September 16th, 1597 according to the lunar calendar) through hindcasting the paleo-tidal currents and -tides(PTC). Firstly, we conducted harmonic analysis using 6-month current data observed at Uldolmok and 1-year elevation data provided by Korea Ocean Research and Development Institute in order to understand their characteristics and to hindcast the PTC. Observation results show that Uldolmok, ~300m wide, relatively narrow channel, is characterized by a flood-dominant mixed mainly semidiurnal tidal regime induced by relatively-strong shallow water constituents, showing closely a standing wave type of tidal current. Further, we hindcasted PTC on the day of Myungryang Naval Battle. Our results were compared and discussed with results(time and speeds of maximum(flood and ebb) currents and high and low water times) of the previous studies estimated from different methods. Lastly, we reconstruct the course of the event of Myungryang Naval Battle recorded in the Admiral Sun-Sin Yi's War Diary(Nangjung Iigi in Korean) based on our hindcasting results.

우리나라 보건의료 발전을 위한 의료기술평가의 역할 (Roles of Health Technology Assessment for Better Health and Universal Health Coverage in Korea)

  • Health technology assessment (HTA) is defined as multidisciplinary policy analysis to look into the medical, economic, social, and ethical implications of the development, distribution, and use of health technology. Following the recent changes in the social environment, there are increasing needs to improve Korea's healthcare environment by, inter alia, assessing health technologies in an organized, timely manner in accordance with the government's strategies to ensure that citizens' medical expenses are kept at a stable level. Dedicated to HTA and research, the National Evidence-based Healthcare Collaborating Agency (NECA) analyzes and provides grounds on the clinical safety, efficacy, and economic feasibility of health technologies. HTA offers the most suitable grounds for decision making not only by healthcare professionals but also by policy makers and citizens as seen in a case in 2009 where research revealed that glucosamine lacked preventive and treatment effects for osteoarthritis and glucosamine was subsequently excluded from the National Health Insurance's benefit list to stop the insurance scheme from suffering financial losses and citizens from paying unnecessary medical expenses. For the development of HTA in Korea, the NECA will continue exerting itself to accomplish its mission of providing policy support by health technology reassessment, promoting the establishment and use of big data and HTA platforms for public interest, and developing a new value-based HTA system.

유도무기 통합최적설계를 위한 데이터베이스 설계 연구 (Database Schema Design for Integrated Missile Design and Optimization)

  • 본 논문에서는 유도무기 통합 설계 프레임워크를 위한 데이터베이스를 설계하였다. 실제 개발에 앞서 유도무기 설계에 필요한 데이터베이스를 위한 요구도를 분석하였으며, 데이터베이스 설계 개념을 정립하였다. 또한 해석도구 간의 데이터흐름 등을 파악하였으며, 이들 정보를 토대로 테이블들을 구성하고 각 객체관계를 정의하여 데이터베이스 스키마를 설계 및 구축하였다. 개발된 DB는 유도무기 통합 설계 프레임워크에 통합되었으며 프레임워크내의 데이터 저장, 검색 및 관리 등의 역할을 수행하며, 유도무기 설계에 적용하여 그 효용성을 검증하였다.

A Design Procedure for Safety Simulation System Using Virtual Reality

  • One of the objectives of any task design is to provide a safe and helpful workplace for the employees. The safety and health module may include means for confronting the design with safety and health regulations and standards as well as tools for obstacles and collisions detection (such as error models and simulators). Virtual Reality is a leading edge technology which has only very recently become available on platforms and at prices accessible to the majority of simulation engineers. The design of an automated manufacturing system is a complicated, multidisciplinary task that requires involvement of several specialists. In this paper, a design procedure that facilitates the safety and ergonomic considerations of an automated manufacturing system are described. The procedure consists of the following major steps: Data collection and analysis of the data, creation of a three-dimensional simulation model of the work environment, simulation for safety analysis and risk assessment, development of safety solutions, selection of the preferred solutions, implementation of the selected solutions, reporting, and training When improving the safety of an existing system the three-dimensional simulation model helps the designer to perceive the work from operators point of view objectively and safely without the exposure to hazards of the actual system.

Health Personnel's Knowledge, Attitudes, and Self-Efficacy Related to Providing Palliative Care in Persons with Chronic Diseases

  • Purpose: The purpose of this study was to examine the relationships of knowledge, attitudes, and self-efficacy related to palliative care among health care providers (doctors and nurses) in order to provide a basis to develop a training program for health care providers. Methods: A correlational and descriptive study design was used. Participants were recruited from a university-affiliated hospital located in Daejeon and an e-nurse community. After IRB approval, data were collected from July 12, 2018, to September 30, 2018. A total of 169 responses were finally analyzed using version SPSS 24. The data were analyzed in terms of descriptive statistics (frequency and percentage or mean and standard deviation, as appropriate), the t-test, analysis of variance (with the Duncan post hoc test), and Pearson correlation coefficients. Results: Knowledge, attitudes, and self-efficacy were significantly higher in those who had received palliative care training or had been exposed to awareness-raising initiatives. There were positive relationships among knowledge, attitudes, and self-efficacy, with small to moderate effect sizes. Conclusion: Palliative care training for health care professionals is necessary to meet patients' needs. Such programs should take into account not only knowledge about palliative care, but also ways to improve empathy and resolve ethical dilemmas. Interprofessional training would be an excellent option to share therapeutic goals and develop communication skills among multidisciplinary team members.

Factors Affecting Nurses' Performance of Cancer Pain Management in a Tertiary Hospital

  • Purpose: More than 60% of patients with advanced cancer experience pain, and uncontrolled pain reduces the quality of life. Nurses are the closest healthcare providers to the patient and are suitable for managing cancer pain using pharmacological and non-pharmacological interventions. This study aimed to identify factors affecting the performance of cancer pain management among nurses. Methods: This study was conducted among 155 participating nurses working at a tertiary hospital who had experience with cancer pain management. Data collection was performed between October 18, 2021 and October 25, 2021. Data analysis was conducted using descriptive statistics, the independent-sample t-test, one-way analysis of variance, and hierarchical regression analysis. Results: There were 110 subjects (71.0%) who had no experience of cancer pain management education. The results of regression analysis indicated that barriers included medical staff, patients, and the hospital system for cancer pain management (𝛽=0.28, P<0.001). The performance of cancer pain management was also affected by experience of cancer pain management training (𝛽=0.22, P=0.007), and cancer pain management knowledge (𝛽=0.21, P=0.006). The explanatory power of the variable was 16.6%. Conclusion: It is crucial to assess system-related obstacles, as well as patients and medical staff, in order to improve nurses' cancer pain management performance. A systematic approach incorporating multidisciplinary interventions from interprofessional teams is required for effective pain management. Furthermore, pain management education is required both for cancer ward nurses and nurses in other wards.
