Characteristics and outcomes of patients with septic shock who transferred to the emergency department in tertiary referral center: multicenter, retrospective, observational study (상급종합병원 및 종합병원 응급실로 전원된 패혈성 쇼크 환자의 특성과 예후: 다기관 후향적 관찰연구)

  • Objective: We evaluated the clinical characteristics and prognoses of patients with septic shock who transferred to the emergency department (ED) in a tertiary referral center. Methods: This study was performed using a prospective, multi-center registry of septic shock, with the participation of 11 tertiary referral centers in the Korean Shock Society between October 2015 and February 2017. We classified the patients as a transferred group who transferred from other hospitals after meeting the inclusion criteria upon ED arrival and a non-transferred group who presented directly to the ED. Primary outcome was hospital mortality. We conducted multiple logistic regression analysis to assess variables related to in-hospital mortality. Results: A total of 2,098 patients were included, and we assigned 717 patients to the transferred group and 1,381 patients to the non-transferred group. The initial Sequential Organ Failure Assessment score was higher in the transferred group than the non-transferred group (6; interquartile range [IQR], 4-9 vs. 6; IQR, 4-8; P<0.001). Mechanical ventilator (29% vs. 21%, P<0.001) and renal replacement therapy (12% vs. 9%, P=0.034) within 24 hours after ED arrival were more frequently applied in the transferred group than the non-transferred group. Overall hospital mortality was 22% and there was no significant difference between transferred and non-transferred groups (23% vs. 22%, P=0.820). Multivariable analysis showed an odds ratio for in-hospital mortality of 1.00 (95% confidence interval, 0.78-1.28; P=0.999) for the transferred group compared with the non-transferred group. Conclusion: The transferred group showed higher severity and needed more organ support procedures than the non-transferred group. However, inter-hospital transfer did not affect in-hospital mortality.

  • Creativity is a social byproduct formed in a specific sociocultural context. Although creativity differs according to the level of culture ranging from supranational, national, professional to organizational levels, research studies documented in literature have mainly dealt with creativity in the (super)national level. For example, differences in personal creativity in the East and the West led by collectivism and individualism, respectively have been dominating in the studies about cultural differences in creativity. Consequently, the domain specificity of creativity and the effects of a more micro level of culture, such as domains, occupations, and organizations, on creative outcomes have been dismissed. Understanding the characteristics of domains, and sociocultural and environmental factors leading to creative products is worth investigating considering that the conception of creativity today highly relies on the nature of domains valued in a certain sociocultural environment. This study reviewed a wide range of literature regarding creativity and sociocultural and environmental factors in order to explore how culture and/or cultural factors inspired creative thinking and generating ideas for creative performance and products. Cultural factors were identified either as a value or a practice (custom) and examined on the supernational, national, professional, and organizational levels. For supernational and national cultures, issues on individualism, collectivism, artistic social atmosphere, and multi-cultural experiences were discussed, while the professional and organizational cultures delved topics on the domain of architecture in which both originality and functionality are greatly valued for creativity. The authors concluded that the impact of culture on creative thinking and idea generation varied according to the levels of culture although issues on multifactorial aspects of culture, differences between personal and collective creativity, and the identification of culture either as a value or a practice (custom) should be discussed further for future studies.

Path Analysis of the Self-Reported Driving Abilities of Elderly Drivers (고령운전자의 자가보고식 운전능력에 대한 경로분석)

  • Objective : This study aims to identify the self-reported driving abilities of elderly drivers and their correlations to the demographic factors that influence them, and to verify the adequacy of the hypothetical model, constructed based on vision, auditory, cognition, motor, and psychological factors, in order to present a path model on the self-reported driving abilities of elderly drivers. Methods : The participants in this study were 122 elderly drivers aged 65 years or older residing in the community. This study evaluated the following factors of the participants: Vision and hearing, motor ability, cognitive ability, depression, self-reported driving abilities. Results : The results of this study are as follows. In the case of men, the self-reported driving ability score was higher than for women, and those driving 6-7 days per week had higher scores than those driving 3 days or less. The period of holding a driver's license and driving experience positively correlated with self-reported driving abilities. The final model of factors influencing the self-reported driving abilities of elderly drivers had a p value (.911) exceeding .05; TLI (1.202), NFI (.949), and CFI (1.000) of over .90; and RMSEA (.000) of lower than 0.1, indicating that the hypothesis model fit the data well. First, the directly influential factors on the self-reported driving abilities of elderly drivers were depression, decreased hearing, and grip strength. Second, age was found to have a direct influence on depression and grip strength; moreover, depression and grip strength as a mediator indirectly influenced their self-reported driving abilities. Third, depression was found to have a direct influence on their delayed cognitive processing and grip strength. Conclusion : The significance of this study is in the identification of direct and indirect factors influencing the self-reported driving abilities of elderly drivers in regional communities, and in the verification of multi-dimensional effects of diverse factors influencing such abilities.

An Establishment of the Optimum Sowing Time for a Machine Harvest of Perilla for Seed (종실용 들깨의 기계수확에 적합한 최적 파종시기 설정)

  • In order to promote the mechanized cultivation of perilla for seed, which has been increasing in cultivation area and production recently as demand increases according to the health-functional effects, we carried out this experiment to determine the optimum sowing time of perilla to minimize the seed loss at harvest and increase the yield. We used two different types of perilla varieties, 'Sodam(small-branch)' and 'Deulsaem(multi-branch)', and the sowing time was June 15, June 30, July 15 and August 1. As the sowing time is late, days of growth from sowing to flowering were shortened, and they were shortened from 14, 26 and 31~32 days on June 30, July 15 and August 1 as compared with June 15, respectively. And, the stem length and culm diameter were shortened or tapered and the number of nodes tended to decrease. The number of effective branch was 82%, 61% and 56% on June 30, July 15 and August 1 as compared with June 15, respectively. Accordingly, it seems to make against in securing the yield from July 15. And, the lowest cluster height was generally shorter as the sowing time is late, and the height was below 15cm on July 15 and August 1. It seems that this may work against the machine harvest. There was a high degree of significance between the sowing time and the yield. Although, the total yield was not statistically significant among June 15, June 30 and July 15, the ratio of shattering seed at harvest was in order of July 15, August 1(30.3%)> June 15(15.3%)> June 30(13.5%). Therefore, the net yield except for shattered seed was higher in order of June 30${\geq}$ June 15> July 15> August 1. This tendency was characteristic regardless of variety and sowing method. And, the protein content in perilla seed increased as the sowing time was delayed, and the content was the highest on August 1. The content of crude fat was relatively high on June 15 and July 15 in 'Sodam', and June 30 and July 15 in 'Deulsaem', respectively. And, the content of linolenic acid was found to be the highest on August 1. As a result, the optimal sowing time for machine harvest of perilla for seed is about June 30. At this time, it is determined that the sowing time is the most suitable to be advantageous in increasing the yield of perilla seed, while minimizing the seed loss due to the shattering at harvest.

Verification of Genetic Process for the High-purity Limestone in Daegi Formation by Oxygen-carbon Stable Isotope Characteristics (산소-탄소 안정동위원소특성을 이용한 대기층 고품위 석회석의 생성기작 해석)

  • Two assertions about the process the formation of the high-purity limestone in the Taebaeksan Basin, categorized into syngenetic and epigenetic origin, are verified on the basis of its oxygen-carbon stable isotopic characteristics. The carbonate rocks sampled from the selective six high-purity limestone mines and several outcrops in the Daegi formation are featured by various colors such as the gray, light gray and dark gray. They show a wide range of oxygen stable isotope ratios (4.5 ~ 21.6 ‰), but a narrow range of carbon stable isotope ratios (-1.1 ~ 0.8 ‰, except for vein calcite), which means that they had not experienced strong hydrothermal alteration. In addition, there is no difference in the range of the oxygen stable isotope ratios by mine and color, and it is similar to the range from surrounding outcrop samples. These results indicate that the effect of the hydrothermal alteration were negligible in the generation of high-purity limestone in deposit scale. Whereas, the carbonate rocks can be divided texturally into two groups on the basis of an oxygen isotope ratio; the massive-textured or well-layered samples (>15 ‰), and the layer-disturbed (or layer-destructed) and showing over two colors in one sample (<15 ‰). In the multi-colored samples, the bright parts are characterized by the very low oxygen stable isotope ratios, compared to the dark parts, implying the increase in brightness of the carbonate rocks could be induced by the interaction between hydrothermal fluid and rock. However, these can be applied in a small scale such as one sample and are not suitable for interpretation of the generation of high-purity limestone as a deposit scale. In particular, the high oxygen isotope ratios from the recrystallized white limestone suggest that hydrothermal fluids are also rarely involved during recrystallization process. In addition, the occurrences of the high-purity limestone orebody strongly support the high-purity limestone in the area are syngenetic rather than epigenetic; the high-purity limestone layers in the area show continuous and almost horizontal shapes, and is intercalated between dolomite layers. Consequently, the overall reinterpretation based on the sequential stratigraphy over the Taebaeksan basin would play an important role to find additional reserves of the high-purity limestone.

A Study on the Transmission of 'Soeburi-Song' in Ulsan (울산쇠부리소리의 전승 양상)

  • Ulsan Soeburi song was reenacted in the 1980s based on the testimony and songs of late Choi Jae man (1987 death), the last blacksmith of the iron production plant at Dalcheon dong, Ulsan in August 1981. The purpose of this study is to analyze Soeburi song from the musical perspective based on 13 kinds of data including video in 1981, and confirm the changing patterns in the tradition process. The derived results are summarized as follows. In the results of examining Soeburi Song data in 2017, the percussion instruments consist of kkwaenggwari 2 (leading small gong 1, follow small gong 1), jing 2 (large gong 2), buk 4 (drum 4), janggu 4 (double headed drum 4), taepyongso 1 (Korean shawm 1), and Jangdan (rhythm) consists of five such as Jilgut, Jajinmori, Dadeuraegi, Deotbaegi, Jajin Deotbaegi. The vocal songs are sung accompanied by the Deotbaegi Jajin Deotbaegi (beat) of quarter small triplet time, or without accompaniment. The scale is mostly Mi La do's third note or Mi La do re's fourth note, and the range does not exceed one octave. All the cadence tones are the same as La. From the results of observing Soeburi song performance until today after the excavation in 1981, it is found that there are four major changes. First, the composition of the music is differentiated into 'long Jajin (slow fast)', and , , and are added. Second, the singing method is based on 'single cantor + multi post singers' since 1980's reenactment, and a single post singer was also specified from time to time. In addition, , which was performed in 2013, became the foundation of . Third, a melodic change of was observed. All beat structures are quarter small triplet time, but the speed gets slow, Mi La do's three notes are skeletonized to be corrected with high re and low sol, and then the characteristics of Menari tori (the mode appeared in the eastern province of the Korean peninsula) are to be clear. Lastly, the four percussion instruments such as kkwaenggwari, jing, janggu, and buk are frequently used, and depending on the performance, sogo (hand drum), taepyongso, yoryeong (bell) are also added. Jangdan played Jajinmori, Dadeuraegi, Deotbaegi and Jajin Deotbaegi from the beginning, and thereafter, the Jilgut Jangdan was added. Through these results as above, it is confirmed that at the time of the first excavation, a simple form of such as has been changed into a male labor song, the purpose of which has changed, and that the playability has become stronger and changed into a performing arts.

Base Study Related with Development of Natural Bio-Adhesives Using Seaweeds (해초류를 이용한 천연 바이오 접착제 개발 기반 연구)

  • In this study, in a bid to develop natural bioadhesives for paper craft, the hanji industry, and preserving cultural assets, complex polysaccharides were extracted from brown and red algae and used as an ingredient in adhesives. Brown algae include sea trumpet, kelp, sea oak, and sea mustard, whereas red algae include Pachymeniopsis elliptica agar-agar weed, Gloiopeltis tenax, and hunori. The polysaccharides were extracted after transforming them from non-aqueous Ca complexes contained in each of the brown and red algae into water-soluble polysaccharides containing alkali metals with a solubility level of 1. and extracted Subsequently, only the polysaccharides were extracted using alcohol precipitation. The adhesion tensile strengths of kelp, a brown algae, and Pachymeniopsis elliptica, a red algae, were 21.58 and 32.99 kgf, respectively. They thus demonstrated better adhesion than that of solid glue products such as water plants (18.45 kgf) and glue sticks (20.45 kgf). The extraction yield of these polysaccharides is supposed to be determined according to their extracted environments; however, no difference in adhesion strength was seen. Further, it was found that the shapes of polysaccharides were determined by their growing environment instead of extraction environment. Use of multi-step alcohol precipitation method during extraction enabled the removal of the constituents except protein and other polysaccharides, thereby demonstrating a stable outcome without cultivation of mold. Furthermore, there was no occurrence of mold even after production of the adhesives by the simple solution method, which demonstrates the adhesive's potential as an environment-friendly adhesive material.

Evaluation of Commercial Korean Honey Quality and Correlation Analysis of the Quality Parameters (국내 시판 벌꿀의 품질 평가 및 품질인자간 상관관계 분석)

  • Honey is made from flower nectar by honey bees. In this study, 120 honeys from various flowers and across eight different provinces in Korea were collected and their components, antioxidants, and hemolytic activities against red blood cell were evaluated. Our results show that total polyphenol (TP) varied widely across the samples, with chestnut honey showing the highest TP ($77.1{\pm}8.4mg/100g$), protein content ($25.9{\pm}0.9mg/100g$), and absorbance at 400 nm ($A_{400}$ : $0.156{\pm}0.036$). In contrast, the acacia honey and sugar honey had a TP of 9.5~30 mg, 12~15 mg/100g of, and the lowest $A_{400}$ of $0.06{\pm}0.02$. High amounts of total flavonoid were quantified in the jujube and chestnut honeys at $8.73{\pm}7.31$ and $8.39{\pm}3.02mg/100g$, respectively. No samples demonstrated hemolytic activity up to 1 mg/ml. Antioxidant activities determined by DPPH, ABTS, and nitrite scavenging placed the chestnut honey highest, followed by jujube, styrax, multi-floral, citrus, acacia and sugar honey. Analysis of parameter correlations indicated that the components and bioactivity of the honey are dependent on the origin of the flower rather than on bee-farming regions. A positive correlation between TP content and antioxidant activity was identified. The correlation coefficients between $A_{400}$ and the TP, ABTS scavenging, and reducing power values were 0.804, 0.772 and 0.741, respectively. We therefore suggest that $A_{400}$ could be used as a noble, economic and simple factor for honey quality evaluation. Our results can potentially be used to develop functional honey for the food and pharmaceutical industries.

A Study of Quality Improvement Methods of Archival Contents Service - With as the Central Figure of Cases among Korea and England and Japan - (기록정보콘텐츠의 품질향상 방안 연구 - 한국·영국·일본의 사례비교를 중심으로 -)

  • Unlike in the past, a new paradigm has been presented which the use of records is much more important than the preservation of those in the present. It will be necessary for users to signalizes the value of records and to make themselves easily accessible to records in order that records should be more effectively used. To meet the needs of the times, it is the very 'Contents' that starts to attract public attention. National Archives in England produces and provides contents utilizing multi-media by using digital technology on various sorts of archives which England has. In addition to England, Asian Historical Records center in Japan in Japan makes Archival Contents Service reflecting users' needs and continues to update the latest contents. What is more, National Archives of Korea has recently promoted the introduction of digital archive by the change of archival paradigm in records management, and it is giving an impetus to the development of contents in the digital archive. In fact, it is crucial to keep as many contents as possible and to give service to public, but it will not be possible to get positive response from public and to offer much higher level of archival information service until the quality of contents is highly improved. Accordingly, this manuscript analyzed the feature and type of contents which National Archives of Korea provides with cases of both TNA in England and Asian Historical Records center in Japan, each of which has different characteristics. Also, it dealt with several methods of the usage of contents in those organizations. Furthermore, this study explained what kind of contents and feedback are given to users. Moreover, it divided the components of contents of three institutions as mentioned into three by information provided and evaluated the quality of contents by establishing the details of contents. In addition, there were implications for archives with regard to reference for building contents.

A Study on History and Archetype Technology of Goli-su in Korea (한국 고리수의 역사와 원형기술의 복원 연구)

  • Goli-su is the innovative special kind of the embroidery technique, which combines twining and interlacing skill with metal technology and makes the loops woven to each other with a strand. The loops floating on the space of the ground look like floating veins of sculpture and give people the feeling of the openwork. This kind of characteristic has some similarities with the lacework craft of Western Europe in texture and technique style, but it has its own features different from that of Western Europe. It mainly represents the splendid gloss with metallic materials in the Embroidered cloth, such as gold foil or wire. In the 10th century, early days of Goryo, we can see the basic Goli-su structure form of its initial period in the boy motif embroidery purse unearthed from the first level of Octagonal Nine-storied Pagoda of Woljeong-sa. In the Middle period of Joseon, there are several pieces of Goli-su embroidered relic called "Battle Flag of Goryo", which was taken by the Japanese in 1592 and is now in the Japanese temple. This piece is now converted into altar-table covers. In 18~19th century, two pairs of embroidered pillows in Joseon palace were kept intact, whose time and source are very accurate. The frame of the pillows was embroidered with Goli-su veins, and some gold foil papers were inserted into the inside. The triangle motif with silk was embroidered on the pillow. The stitch in the Needle-Looped embroidery is divided into three kinds according to comprehensive classification: 1. Goli-su ; 2. Goli-Kamgi-su ; 3. Goli-Saegim-su. From the 10th century newly establishing stage to the 13th century, Goli-su has appeared variational stitches and employed 2~3 dimensional color schemes gradually. According to the research of this thesis, we can still see this stitch in the embroidery pillow, which proves that Goli-suwas still kept in Korea in the 19th century. And in terms of the research achievement of this thesis, Archetype technology of Goli-su was restored. Han Sang-soo, Important Intangible Cultural Heritage No. 80 and Master of Embroidery already recreated the Korean relics of Goli-su in Joseon Dynasty. The Needle-Looped embriodery is the overall technological result of ancestral outstanding Metal craft, Twining and Interlacing craft, and Embroidery art. We should inherit, create, and seek the new direction in modern multi-dimensional and international industry societyon the basis of these research results. We can inherit the long history of embroidering, weaving, fiber processing, and expand the applications of other craft industries, and develop new advanced additional values of new dress material, fashion technology, ornament craft and artistic design. Thus, other crafts assist each other and broaden the expressive field to pursue more diversified formative beauty and beautify our life abundantly together.
