Ultrastructural Alterations in the Gastric Mucous Epithelial Cells of Mouse Inoculated with Ehrlich Carcinoma Cells, Induced by 5-Fluorouracil, Mitomycin C or Acriflavine-Guanosine Compound (AG60) (5-Fluorouracil, Mitomycin C 및 Acriflavine-Guanosine 복합제가 Ehrlich 암세포를 이식한 생쥐 위점막 점액상피세포의 미세구조에 미치는 영향)

  • This experiment was performed to evaluate the morphological responses of the gastric epithelial cells of the mouse, inoculated with Ehrlich carcinoma cells in the inguinal area, following administration of 5-fluorouracil, mitomycin C or Acriflavine-Guanosine compound (AG60). In this study, each mouse was inoculated with $1{\times}10^7$ Ehrlich carcinoma cells subcutaneously in the inguinal area. From next day after inoculations, 0.2 mL of saline, 5-fluorouracil (30 mg/kg), mitomycin C ($400{\mu}g/kg$) or AG60 (30 mg/kg) were injected to the animals every other day, respectively. Each animals were sacrificed after 7th injection and tissue were taken from the gastric mucosa. Thereafter, the ultrathin sections were stained with uranyl acetate and lead citrate. In the 5-fluorouracil-, mitomycin C- or AG60-treated mice, myelin figures and multivesicular bodies within the gastric mucous epithelial cells were observed more frequently than those of the normal control. In the 5-fluorouracil-treated mice, membrane structures containing a few mucous granules in the luminal space were observed. Indeed, bulging cytoplasmic process containing mucous granules protruding into the gastric lumen were observed in the mitomycin Ctreated mice. Therefore, this study suggested that AG60 as compared with 5-fluorourail and mitomycin C may effective medicine without damage to the secretion ability of gastric mucous epithelial cells.

IN VIVO EVALUATION OF ACCURACY OF TWO ELECTRONIC APEX LOCATORS (전자근관장 측정기의 정확도에 관한 연구)

  • It is necessary to measure the length of a root canal in order to attain a satisfactory prognosis after root canal therapy. There are several methods for determining root canal length, such as tactile sensation by the dental practitioner, the utilization of x-ray film, and electronic root canal measurement. Among these, the electrical measurement methods, in which the impedence between the oral mucous membrane and periodontal membrane is determined, have advantages of simplicity and accuracy. During root canal treatment, the root canal contains a solution of high electrical conductivity such as pus, blood, sodium hypochlorite and so on. Recently a new electronic root canal measurement device of frequency-dependent type has been developed, which is capable of measuring the length of root canal under moist conditions. Endex and Root ZX, which are frequency-dependent type, were evaluated for accuracy of measuring root canal length in vivo by stereomicroscope. The result were as follows ; 1. 82.5% of Endex and 87.5% of Root ZX measured in the range of ${\pm}0.5$ mm from the apical foramen and both showed 57.5 % in the range of 0.1 mm to 0.5 mm. 2. Endex showed significantly higher accuracy in vital teeth than nonvital teeth(p<0.05). But in case of Root ZX, there was no significant difference between vital and nonvital teeth. 3. As a result of this study, there was no significant difference in accuracy between Endex and Root ZX, and both devices showed file passes the apical foramen in more than half of the cases, and it is thought that this must be considered clinically.

Uterine Cervix Metastasized from Signet Ring Cell Adenocarcinoma - 1 Case Report - (자궁경부에 전이된 반지세포암종 - 1례 보고-)

  • This study is a report about a specific patient whose primary stomach adenocarcinoma metastasized to uterine cervix adenocarcinoma. A thirty-nine year old female patient was initially diagnosed as having metastatic adenocarcinoma in the supraclavicular lymph node. Upon further examination, she was diagnosed with stomach adenocarcinoma. 8 months later, a cervix punch biopsy was performed. The stains used for examination were H&E stain, PAS stain, Alcian blue stain, Mucicarmine stain, Papanicolaou's (Pap.) stain, and as immunohistochemical stains, cytokeratin 7 and 20 were done. In the H&E stain, the tumor cells showed prominent and eccentric nuclei, thin nuclear membrane in abundant mucous cytoplasm, and cylinder shape. In the PAS stain, intracytoplasmic mucin vacuoles were stained with pink, and in Alcian blue and Mucicarmine stains, intracytoplasmic mucin vacuoles were stained with blue and red. As in the above results, she was diagnosed with undifferentiated adenocarcinoma. As found on the cytologic smear preparation of the uterine cervix stained by Papanicolaou's stains, the background was relatively clear, the number of malignant cells was relatively low, and large and eccentric nuclei in abundant cytoplasm were observed. Upon observing the tissue preparation of the uterine cervix biopsy by H&E stain, a clear background, large and eccentric nuclei, and a signet ring cell types were observed, and the number of malignant cells were fewer than in the primary uterine cervix adenocarcinoma. The vacuoles in cytoplasm were observed. The nuclear membrane and chromatin were thick and very rough, and upon observation by cytokeratin 7 and 20 of immunohistochemical stain, the tumor cells indicated a positive rate of 70% and 20%, respectively. According to these results, also she was diagnosed with metastasized uterine cervix adenocarcinoma. In summary of the results of pathologic findings on stomach biopsy and cytologic, histopathologic, and immunohistochemical finding on uterine cervix biopsy, the adenocarcinoma of her uterine cervix could assert the adenocarcinoma of signet ring cell type that was metastasized from the primary undifferentiated adenocarcinoma in stomach.

The Healing of Exposed Implant Surfaces in A Maxillary Sinus Cavity (상악동 내 노출된 임플란트의 치유양상)

  • 1. Objective. The aim of this study was to investigate whether dental implant exposure to the maxillary sinus cavity increases the risk of maxillary sinus complications. 2. Study design. An implant was placed bilaterally in the maxillary sinus of eight adult female mongrel dogs in a way that it penetrated the bone and mucous membrane of the maxillary sinus floor to the extent of 2 mm, 4 mm, or 8 mm. The implants were left in place for six months. 3. Results. Radiographic and histologic examinations did not show any signs of pathologic findings in the maxillary sinus of the eight dogs. 4. Conclusion. This study indicates that implant protrusion into the maxillary sinus cavity is not related to the development of sinus complications.

A Case of Atrial Septal Defect (ASD) in a Miniature Schunauzer Dog (Miniature Schunauzer Dog에서 발생한 심방중격 결손 증례)

  • An atrial septal defect (ASD) is congenital heart disease with a communication between the atria, which allows blood to shoot from the atrium with pressure. A 3-month-old female Miniature Schunauzer was referred to the Veterinary Teaching Hospital of Konkuk University for the evaluation of systolic heart murmur. At presentation, the mucous membrane was cyanotic. On physical examination, an ejection-type systolic murmur was auscultated at the pulmonic area. In addition, thoracic radiography showed enlargement of main pulmonary artery and right atrial/ventricular enlargement. Echocardiography revealed dilated right atrium and atrial septal defect. However, mitral and tricuspid valve were still intact and well tolerating. The presence of an ASD was confirmed by identifying flow across the defect with color Doppler imaging. Doppler echocardiography provides a means of non-invasive documentation and quantification of ASD. Complete blood count and serum chemistry were not remarkable. Although large defect was confirmed between the two atrium, the patient did not show any obvious clinical signs of heart failure at this time.

  • Dyskeratosis congenita is a rare congenital disorder characterized by a triad of reticular pigmentation of the skin, dystrophic nails, and leukoplakia of the mucous membrane. Sometimes it is associated with bone marrow failure, secondary malignancy and interstitial lung disease. Though it is rare, Dyskeratosis congenita is diagnosed relatively easily when clinicians suspect it. It can be diagnosed just by gross inspection with care. Dyskeratosis congenita should be considered as one cause associated with interstitial lung disease. In Korea, interstitial lung disease with dyskeratosis congenita has not been reported. We report a case and review the literature.

TREATMENT OF HEAVY MANDIBULAR BUCCAL FRENUM USING APICALLY POSITIONED FLAP UNDER DEEP SEDATION IN CHILDREN (소아환자의 깊은 진정요법 하에서 근단변위 판막술을 이용한 거대협소대의 치료)

  • The mandibular buccal frenum is defined as a fold of mucous membrane at the posterior labial vestibule and attaches the lips and the cheeks to the alveolar mucosa, gingiva, and underlying periosteum. The buccal frenum becomes a problem when its attachment is too close to the marginal gingiva. It may then pull on healthy gingiva, encourage plaque formation and interfere with tooth brushing. Especially, heavy buccal frenum mucogingivally results in insufficent attached gingiva, inadequate vestibular depth and high frenum attachment and also difficulty in eruption of mandibular premolar. Frenotomy, frenectomy and mucogingival surgery are used in treating heavy buccal frenum. Frenotomy with autogenous free gingival graft has been used popularly because of its stable result. But, it is difficult in younger children because of inadequate donor site, difficulty in making recipient site and behavior management. Frenotomy with apically positioned flap is considered as more efficient way for a very young child with heavy buccal frenum. Additionally, modified deep sedation with $N_2O-O_2$ can be used as an adjunct for the effective treatment outcome. Decrease in muscle pull, adequate width of attached gingiva and increased vestibular depth can be expected from this treatment approach.

Effects of Meatal Care in Reduction of Catheter-associated Urinary Tract Infection for Elderly Patients in the ICU (외요도구 간호가 노인중환자의 유치도뇨관 관련 요로감염에 미치는 효과)

  • Purpose: The purpose of this study was to compare the effects of meatal care with 10% betadine or with normal saline on the incidence of urinary tract infection (UTI) for elderly patients with indwelling urinary catheter in the ICU. Method: A quasi experimental design with non-equivalent control group was used. The 37 patients who participated in this study were 65-year-old or older. Patients in the saline group (n=20) received meatal care with normal saline and those in the betadine group (n=17) received meatal care with 10% betadine once a day for 6 days. Urine cultures were done on the 7th day for both groups to detect UTIs. Results: No difference was observed in the incidence of urinary tract infection between the two groups, regardless of patients' gender, ability to communicate or history of operation. Conclusion: The results indicated that use of saline which is cheap and does not irritate the mucous membrane is effective in preventing UTI within the first 7 days, and can be used instead of betadine for meatal care for elderly patients with indwelling urinary catheter in the ICU.

  • The authors observed the ultrastructure of oral leukoplakia simplex of gingiva, buccal mucosa, tongue and alveolar ridge. For the purpose of clearly defining the lesions under investigation in this study, leukoplakias were cinsidered to be any white patches on the oral mucous membranes that could not be removed by rubbing and could not be classified clinically or microscopically as another diagnosable disease. The tissue to be examined were embedded in paraffin for light microscopic study. The tissue to be examined under the electronomicroscope were fixed in 2.5% glutaraldehyde in 0.1M cacodylate buffer and 1% osmic acid in 0.1M cacodylate buffer, dehydrated with guaded alchol, and treated with propylene oxide, and embedded in Epon.Ultrathin sections were obtained by LKB III ultrotome, stained with uranyl acetate/lead citrate, and examined with Corinth 500EM. The results were as follows : 1. Epithelium of leukoplakia consisted of stratum basale, stratum spinosum, stratum granulosum and stratum corneum. 2. There was hyperorthokerotosis or hyperparakeratosis. 3. Granular cells contained a lot number of membrane coating granule showing lamellar structure, clearing ot codensation, and a lot of keratohyaline granule varied in size. 4. An increased concentration of tonofilaments and an increased number of desmosomes were found in the stratum spinosum. 5. Basal lamina generally showed its continuity, but in some locatoins, its interreption and multiplication appeared.

  • The epidermal tissue of Korean planaria Phagocata vivida(Ijima et Kaburaki) is composed of the simple columnar epithelium. The ventral epidermis of this animals is thinner than the dorsal epidermis and has a furrow in the median line in which dark cell is observed. The clear cells which are electron-lucent are located either side of the dark cells. Those are compactly covered with long cilia. The free surface of the latero-ventral epidermis is tightly contacted with the earth and this epidermal free surface has a great number of short cilia, and a lot of C-type of basophilic granule cell are migrated into the cytoplasm of epithelium from mesenchyme passing through the basement membrane and then this granules are put out of latero-ventral free surface. Dorsal epidermis is thickest among the whole epidermis of these animals and the rhabdite granules are more distributed in dorsal epidermis than in ventral epidermis. According to the cytochemical and ultrastructural research, composed epidermis of this planaria are divided into nine type cells, that is, ciliated columnar epithelium, dark cell, mucous cell, rhabdite-forming cell, sunk round cell, A type, B type, C type and D type of basophilic granule cell.
