The Effect of Motion Recognition Occupational Therapy on the Physical Self-efficacy, and Visual-motor Integration, Interactive Peer Play of Children with Neurodevelopmental Disorders (모션 인식 활용 작업치료가 신경발달장애 아동의 신체적 자기효능감 및 시각-운동통합 능력, 놀이기술에 미치는 영향)

  • Purpose : The purpose of this study was to examine the effects of applying occupational therapy that uses motion recognition on the physical self-efficacy, visual-motor integration ability, and play skills of children who have neurodevelopmental disorder before and after treatment. Methods : This The study chose 16 children with neurodevelopmental disorder as research subjects who were randomly and evenly allocated into an experimental group and a control group. The experiment followed a pretest-posttest design. As an intervention, the experimental group received motion recognition-based occupational therapy and a separate sensory integration program. The control group only participated in the separate sensory integration program. The eight-week experiment duration included 24 intervention sessions where the a 50-minute session was implemented three times a week for eight weeks. To compare the physical self-efficacy, visual-motor integration ability, and play skills before and after the intervention, measurement tools including the Physical self efficacy, Beery VMI-6, and Penn interactive peer play scale were used. All measured variables were analyzed and expressed as mean, standard deviation and percentage. Results : The motion recognition-based occupational therapy demonstrated a significant effect on improving the physical self-efficacy, visual-motor integration ability, and play skills of the experimental group. The intervention also caused a significant difference between the experimental group and control group in terms of the physical self-efficacy, visual-motor integration ability, and play skills. Conclusion : We confirmed the possibility motion recognition-based occupational therapy could be effective in improving the physical self-efficacy, visual-motor integration ability, and play skills for patients who have neurodevelopmental disorder. Based on the study result, further future studies are expected based on this study result that prove the application effect of the motion recognition-based occupational therapy using disabled and non- disabled children as subjects are expected in the future.

Effect of Pharmacological Treatment for Attention-Deficit Hyperactivity Disorder on Motor Coordination: Open Label Study (주의력결핍 과잉행동장애 아동의 약물치료가 운동협응력에 미치는 효과: 개방연구)

  • Objectives: The objective of this study was to investigate the effect of pharmacological treatments for attention-deficit hyperactivity disorder (ADHD) on motor coordination, using the Developmental Coordination Disorder Questionnaire (DCDQ). Methods: The participants were recruited from April 2015 to November 2016 from the Department of Psychiatry of Asan Medical Center and were treated for 3 months with methylphenidate or atomoxetine. The illness severity at baseline and 3 months were scored using the ADHD Rating Scale (ARS), Clinical Global Impression-Severity Scale (CGI-S) and/or Clinical Global Impression-Improvement Scale (CGI-I). A total of 39 children with ADHD (age $8.0{\pm}1.4years$, 36 boys) completed the Advanced Test of Attention (ATA) and their parents completed the DCDQ at baseline and 3 months. The paired t-test, mixed between-within analysis of variance and correlation analysis were used. Results: The CGI-S (p<0.001), ARS (p<0.001), and fine motor/hand writing (p=0.005) on the DCDQ were significantly changed between pre-treatment and post-treatment. When the participants were divided into those who were suspected of having developmental coordination disorder (DCD) (n=23) and those who probably did not (n=16), the control during movement, fine motor/hand writing and general coordination scores on the DCDQ showed the main effects for group (p<0.001, p<0.001 and p<0.001, respectively). The fine motor/hand writing on the DCDQ has a significant main effect for time [F(1,37)=7.31, p=0.010, ${\eta}^2=0.405$] and the interaction effect between group and time was also significant [F(1,37)=4.63, p=0.038, ${\eta}^2=0.111$]. The baseline visual commission error on the ATA is significantly correlated with the changes in the DCDQ total scores (r=0.330, p=0.040). Conclusion: Our results provide preliminary evidence that pharmacological treatment for ADHD improves not only the core symptoms of ADHD, but also the motor coordination. Further studies are needed to confirm the effect of the pharmacological treatment for ADHD on the motor coordination.

Effects of Perceptual Motor Program on Visual Motor Integration Skill and Motor Skill of a Child With Attention Deficit Hyperactivity Disorder: Single Subject Research Design (지각운동프로그램이 주의력결핍과잉행동장애아동의 시각운동통합기술과 운동기술에 미치는 영향)

  • Objective : The purpose of this study is to explore the effectiveness of a perceptual motor program in promoting visual motor and motor control outcomes in a child with Attention-Deficit/Hyperactivity Disorder (ADHD) who is enrolled in an elementary school. Methods : The subject is 8-year-old boy who was diagnosed with ADHD but did not receive medication. Research design is a single-subject A-B-A(follow-up). Baseline and follow-up phase were 3 sessions and intervention phase were 8 sessions. After performing 40 minutes of free play on the baseline A and follow-up A, we used the copying subtest of the Korean Development Test of Visual Perception 2 (K-DTVP-2) for evaluating visual motor integration skill and the rope jumping subtest for motor skill. After providing the perceptual motor activity in intervention phase B, copying and ropejumping were performed. Results : The standard scores of the Copying for measuring visual motor integration skill were poor at an average of 4.7 points at baseline phase A, but maintained an average of 9.6 points at intervention phase B and an below average of 7.7 points at the follow-up phase A. In the Rope jumping for the motor skill, it was increased by 4.3 times in the baseline phase A and 5.9 times in the intervention phase B, but slightly decreased by 5 times in the follow-up phase A. Conclusion : This study suggests that perceptual motor program has a positive effect on visual motor and motor function of ADHD children.

  • Traumatic brain injury(TBI) used to be developed after a severe traumatic event. Therefore TBI usually induces acute or chronic stress reaction. I reviewed the concept, epidemiology, biological or psychosocial etiology, diagnosis and treatment of post-traumatic stress disorder (PTSD), and discussed about PTSD or stress reaction after TBI. Early evaluation and management of stress reaction after TBI are important.

The Clinical Effect of Botulinum Toxin in a Patient with Tourette's Syndrome: A Case Report and Review (뚜렛 증후군에서 보툴리눔 톡신의 임상 효과 : 증례보고 및 고찰)

  • Botulinum toxin, a neurotoxin, is known to be an inhibitor of cholinergic neuromuscular transmission. Recently, it was reported that the administration of botulinum toxin is effective for the treatment of focal neurological motor disorders such as cervical dystonia, blepharospasm, hemifacial spasm, spasmodic dysphonia, and writer's cramp. Several case studies reported that the botulinum toxin was administered for the treatment of motor tic or vocal tic. It was found that this toxin reduces the frequency and severity of the tic as well as the premonitory urge and symptoms. In our case study, a noticeable decrease of motor tic symptom was observed after an intramuscular injection of 300mg of botulinum toxin in an 18-year-old patient with Tourette's disorder who showed only a little improvement of motor tic and vocal tic symptoms after treatment with antipsychotic drugs for several years. This case is reported in our study and literature survey was undertaken for reviewing similar cases. In our study, an 18-year-old boy diagnosed with Tourette's disorder based on Diagnostic and Statistical Manual of Mental Disorders, fourth edition presented with the following scores : the Clinical Global Impression scale, Yale Global Tic Severity Scale (motor/vocal/severity), Premonitory Urge Score, Korean Attention-Deficit Hyperactivity Disorder Rating scale, and Kovac Depression scale which were performed prior to the treatment were 5, 21/5/50, 100, 17, and 18 points, respectively. Two weeks after the injection of botulinum toxin, the scores were 4, 17/5/40, 50, 16, and 19 points, respectively. Eight weeks after the injection of botulinum toxin, they had become 3, 15/5/30, 25, 16, and 20 points, respectively, which clearly indicates a noticeable decrease of motor tic symptom.

  • Objective : The purpose of this study is to report a case of tic-disorder child treated by Oriental medicine and Play therapy. Methods : We treated the Tic-disorder child with herbal medicine (Yangsintangkamibang), acupuncture and Play therapy. Then we evaluated tic disorder by Yale Global Tic Severity Scale(YGTSS) and observed the progress of tic disorder with the play therapy. Results : We treated a tic-disorder patient with oriental medicine, play therapy and had a good effect on a patient who had facial motor tic disorder. The patient's symptoms were improved and the YGTSS also decreased. Conclusions : This study shows that co-treatment of oriental medicine and play therapy can be an effective treatment for tic disorder. And the further study is needed with more cases and longer duration.

Development of a Reading Training Software offering Visual-Auditory Cue for Patients with Motor Speech Disorder (말운동장애인을 위한 시-청각 단서 제공 읽기 훈련 프로그램 개발)

  • In this paper, we developed a visual-auditory cue software for reading training of motor speech disorder patients. Motor speech disorder patients can use the visual and/or auditory cues for reading training and improving their symptom. The software provides some sentences with visual-auditory cues. Our sentences used for reading training are adequately comprised on modulation training according to a professional advice in speech therapy field. To ameliorate reading skills we developed two algorithms, first one is automatically searching the starting time of speech spoken by patients and the other one is removing auditory-cue from the recorded speech that recorded at the same time. The searching of speech starting time was experimented by 10 sentences per 6 subjects in four kinds of noisy environments thus the results is that $7.042{\pm}8.99[ms]$ error was detected. The experiment of the cancellation algorithm of auditory-cue was executed from 6 subjects with 1 syllable speech. The result takes improved the speech recognition rate $25{\pm}9.547[%]$ between before and after cancellation of auditory-cue in speech. User satisfaction index of the developed program was estimated as good.

  • ■Objectives This case study is to report the effectiveness of Korean medicine in Parkinsonism patient's treatment. ■Methods We used the acupuncture, electro-acupuncture, moxibustion, cupping therapy, herbal medicine, especially Palmulgunja-tang to the Parkinsonism patient with motor disorder such as Postural Instability and Gait Difficulty(PIGD) and aphonia. Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait pattern, voice dB and self-evaluation of speed and volume were used to assess the change of symptoms. ■Results ‌After treatment, the UPDRS score decreased in overall category and the walking pattern has improved. In addition, the improvement was observed in voice volume and in self assessment of the patient. ■Conclusion This case suggests the effect of Korean medical treatment on motor disorder and aphonia in Parkinsonism.

  • Method : Chronic motor tic or vocal tic disorders are characterized which have one of the two kinds of tics for more than a year. It is characterized chronic motor tic or vocal tic disorders that have one of the two kinds of tics for more than a year. But Tourette's disorder is a disease which has one or more motor tics and vocal tics for more than a year. Contemporary empirical studies have suggested the rate of comorbid attention-deficit hyperactivity disorder(ADHD) or obsessive compulsive disorder(OCD) in children with tic disorders be high. Objects : As society is confusing, tic disorders are increasing. Therefore, determining the treatment strategy in children with tic disorders is very important. So we studied the literatures of traditional chinese medicine about tic disorders and reported the results. Results : The principals for medical treatment were So-gan(疏肝), I-gi(理氣), Sik-pung(熄風), Gin-kyung(鎭痙), Chung-oel(淸熱) etc. Medicines treatment is used more than acupuncture treatment. The herbs are used Uncariae ramulus et Uncus(釣鉤藤), Paeoniae Radix Alba(白芍藥), Poria(白茯岺), Glycyrrhizae Radix(甘草), Bupleuri Radi.x(柴胡), Fossilia ossis Mastodi(龍骨) etc. in the order named. Acupuncture points were used Naegwan(PC6), Shinmuin(HT7), P'ungnyur4(ST40), Paek'oe. (GV20), Chung-wan(CV12), Samumgye(SP6), Kongson(SP4), T'aech'ung(LR3) and so on.

A On-site Monitoring Device of Work-related Musculoskeletal Disorder Risk Based on 3D-Camera (3D 카메라 기반 직업성 근골격계 부담 작업 모니터링 장치)

  • A 3D camera-based on-site work-related musculoskeletal disorder risk assessment(WMDs) tool has been developed. The device consists of Kinect a 3D camera manufactured by Microsoft, a servo-motor, and a mobile robot. To complement inherent narrow field of view(FOV) of Kinect, Kinect is rotated according to PID servo-control algorithm by a servo-motor attached underneath, to track movement of a subject, producing skeleton-based motion data. With servo-control, full 360 degrees tracking of a test subject is possible by single Kinect. It was found from experimental tests that the proposed device can be successfully employed for on-site WMDs risk assessing tool.
