발달장애 아동 어머니의 가정치료 참여도 (Involvement of Mothers of Developmentally Delayed Children in Home Treatment)

  • For the purpose of clarifying to what degree the mothers of developmentally delayed children are involved in treating their child at home. 193 mothers were sampled from 220 mothers of developmentally delayed children below 12 years of age who have visited one of four institutions: the Rehabilitation Hospital of Yonsei Medical Center, Inchon Severance Hospital, Disabled Welfare Center in Myongil-dong, and Nambu Disabled Welfare Hall. The study period was from Mar. 25, 1995 through Apr. 15, 1995. A questionnaire survey was conducted listing the characteristics of the developmentally delayed children, their mothers, mother's satisfaction with their therapists, and the actual conditions of the home treatment. 1. The mothers who treat their child at home for more than 31 minutes a day show a high involvement score, while the mothers of those who give treatment for less than 30 minutes a. day show a low involvement score. That is, the longer the treatment, the greater the involvement score. This indicates a statistically significant result(p<0.01). 2. In cases where a child's father is involved in the home treatment, his/her mother discloses a statistically high involvement score(p<0.001). 3. The result of analysis of cases where other family members, relatives or friends (fathers excepted) reveals a statistically significant high involvement score(p<0.05) for the mother. 4. Mothers in general represent a statistically significant high involvement in home treatment. In the meantime, the mothers in a nuclear family show a higher involvement home treatment than mothers in an extended family(p<0.01). 5. Among those respondents who think that home treatment is helpful and that mothers' involvement in home treatment is helpful, the mothers record a statistically significant high involvement score(p<0.05). When seen from the above perspectives, it seems of much significance that fathers and other relatives or family members play an important role in enhancing the involvement of mothers in home treatment. One point to note here is that providing a long home treatment time is crucial. Therefore, it is recommended that family members have access to rehabilitation treatment for training developmentally delayed children or their care giver; and moreover, we needed to carry out family training or at least arrange for meetings between the family members and medical personnel involved in the child's rehabilitation.

유치원 아동과 어머니의 아동도서 접근관점의 관계성에 대한 사례 연구 (A case study of kindergarten children's reading stances and responses with relation to those of their mothers)

  • The case study describes kindergarten children's reading stances and responses with relation to those of their mothers'. Rosenbaltt's theory (1978) of aesthetic and efferent stances in used to characterize the relationship between the reader and the text. mother's reading stances are examined by observing each case study child's reading experience with his/her mother at home. In most cases differences in some children's preferred stances seemed to reflect differences in their mothers' stances. As a result, children took a different type of stance from the teacher's efferent stance and were perceived by the teacher to be slower with regard to literacy development, even though they had formed their own reading worlds. Interestingly enough, children with an aesthetic reading stance were welcomed among their peers during free play time. In contrast, children who took a stance congruent with the teacher's efferent stnace were perceived by the teacher to be on the right track with regard to literacy development, although their reading worlds seened to be limited. Interestingly, the children with efferent reading stances were not engaged in collaborative meaning negotiation processes with peers. The case study children with mothers who did not take an extreme type of stance, I.e., neither a very efferent nor a very aesthetic stance, seemed to choose a type of stance depending on a context.

유아의 기질과 정서조절능력 간의 관계: 어머니 양육효능감의 매개적 역할 (The Relationship Between Young Children's Temperament and Emotional Regulation: The Mediating Role of Parenting Sense of Competence)

  • Objective: The purpose of this study was to investigate the relationships between young children's temperament, emotional regulation ability and mother's parenting sense of competence. Methods: The participants were comprised of 315 young children who attend a kindergarten or day care center and their mothers from Seoul, Inchon, and Gyeonggi-do Province. They completed questionnaires on temperament, emotion regulation ability, and parenting sense of competence. The data were analyzed by frequency analysis, Cronbach's ${\alpha}$, Pearson's correlation coefficients, and multiple regression analysis. Results: It was observed that young children's regularity/adaptability was positively correlated with sense of competence as a parent and young children's emotional regulation. Young children's regularity/adaptability was negatively correlated with dissatisfaction as a parent and young children's emotional instability/negativity. Sense of competence as a parent was positively correlated with young children's emotional regulation and negatively correlated with young children's emotional instability/negativity. In addition, it was further found that the mother's parenting sense of competence tended to play a perfectly/partially mediating role between young children's temperament and young children's emotional regulation ability respectively. Conclusion/Implications: These results clearly indicated that parenting sense of competence plays a crucial role between young children's temperament and emotional regulation.

반응성 애착 장애의 치료 (TREATMENT OF REACTIVE ATTACHMENT DISORDER)

  • 반응성 애착 장애에 관한 미국과 영국으로부터의 대부분 논문들은 국외에서 입양한 탈억제형(Disinhibited Type) 반응성 애착장애를 보고하였다. 터키와 한국에서만 반응성 애착장애의 억제형(Inhibited Type) 아동들에 대한 보고를 하고 있는데 대부분 친부모에 의해 양육되고 있으며 그들의 증세는 자폐장애와 매우 유사하다. 반응성 애착장애의 치료, 특히 억제형의 치료에 대한 논문은 매우 드문 상태이기 때문에 저자는 1987년부터 한국의 반응성 애착장애 아동들을 치료해 온 경험을 이 논문에 자세히 기술하였다. 반응성 애착장애 환자와 그 부모들을 치료하기 위해서는 세가지 중요한 영역이 반드시 포함되어야 한다:1) 반응성 애착장애 아동에게 치료적인 환경을 조성해 주어야 한다. 2)개별 놀이치료, 엄마 놀이치료, 형제 놀이치료, 또래 놀이치료, 집단 놀이치료를 통해 반응성 애착장애 아동과 그 엄마와의 사이에 애착을 형성시켜 주어야 한다. 3) 언어치료, 인지치료, 치료교육등을 통하여 뒤진 발달을 촉진시켜 주어야 한다. 이 치료방법은 미국이나 영국을 포함한 서구에서의 다른 치료 방법보다 우리나라 환자들에게 좀더 쉽게 좀더 효과적으로 적용될 수 있을 것이다.

자폐아동의 조기 선별을 위한 상호작용행동체크리스트 개발 (Development of an Interaction Behaviors Checklist for Early Detection of Autistic Children)

  • Purpose: This study was conducted to develop a behavioral checklist to predict an autistic disorder and to identify the earliest detecting time. Method: One hundred and fifty eight children including normal, autistic, institutionalized normal, and retarded were assessed using critical interaction behavioral markers from literature review. Data was collected by semi-structured mother-child interaction by videotape recording and analyzed byfactor analysis, Cronbach a, Kappa, $x^2$, and Duncan. Result: Ten behavioral markers were sorted into 2 factors; joint-attention and synchronized behavior. Autistic children were impaired in pretend play, odeclarative pointing, proimperative pointing, gaze-monitoring, referential looking, showing, joint-attention, rhythmical vocal exchange, and synchronized laughing. The sychronized behavior was also a critical marker to predict the autistic disorder. However, it was difficult to differentiate autistic disorder from mental retardation. In addition, the appropriate detecting time was around 18 months after birth. Conclusion: This checklist should be behavior markers to predict autistic disorder and could be useful as educational material at children's clinics, parents class, and for caregivers in the health center. In addition, early detection should lead to treatment being started as soon after 18 months of age as possible.

영아의 기질과 어머니의 정서적 가용성 및 영아의 정서적 반응성과 참여 간의 관련성 (The Relationship between Infant Temperament, Maternal Emotional Availability, Infant Emotional Responsiveness and Involvement)

  • The main purpose of this study is to examine the relationship between the variables of infant temperament, maternal emotional availability, infant emotional responsiveness and involvement. A total of 60 infants and their mothers participated in this study. Mother-infant dyads were observed and mothers then completed questionnaires. Data was analyzed by means of descriptive statistics, t-tests, Pearson's correlations and hierarchical regressions. In terms of infant emotional responsiveness, maternal sensitivity and non-hostility, these were revealed to have had an effect on infant emotional responsiveness. Moreover, infants who have mothers who had the ability to read their infants' emotional signals accurately, responded in appropriate ways and interacted with affection, showed a higher level of responsiveness. In terms of infant emotional involvement, maternal structuring had an effect on infant emotional involvement. That is, infants who have mothers willing to support their infants through appropriate guidance and facilitate their interaction were more emotionally involved during the play sessions. In conclusion, maternal sensitivity, non-hostility and structuring were all related to infant emotional responsiveness and involvement, however, the degree of the effects of these variables differed.

아버지의 영아 돌봄에 관한 문헌연구 (A Study of father's care giving in infancy)

  • These days social and economical changes have influence on the structure of family and the role of family members. Working mothers and widowers with children are increasing because of economical difficulties. Support from relatives are decreasing because of the conspicuous trend toward nuclear families. According to these reasons androgynous fathers are required. Today's fathers in Korea socially and culturally have learned about traditional parenting, but they are changing their fathering styles to meet the demands of the times. However they don't have their own fathering models. Therefore nurses who hold an advantageous position to teach and support from clinic have to encourage them to care their infants. The purposes of this study were to define father's care giving in infancy, understand influencing factors on fathering, and the differences between fathering and mothering, then contribute to nursing implementation for supporting fathers. This study was designed to review references about father's care giving. The results were follows: Six aspects of parent participation were direct care. indirect care, play, decision-making concerning the child, amount of time of sole responsibility for the child and overall availability to the child. Direct care involved feeding, bathing, going to child if child awakens. dressing, putting child to bed, taking child to doctor, nurse, or dentist, transporting child to and from sitter, day care, or school, washing child's hair. Indirect care involved cleaning up after child, preparing child's food, fixing child's broken playthings, washing child's clothes, arranging baby-sitting, shopping for child's toys and clothes, transporting baby-sitter to and from your home. Young fathers were gradually participating in direct care like feeding, taking child to doctor. Father's care giving stimulated mothering and promoted parent-infant relationship. Influencing factors of fathering would be divided into father characteristics, surrounding factors, infant attributes. Father characteristics were age, role perception, relationship with parent. Surrounding factors were the opportunity of early contact, support system, spouse's expectation, marital adjustment, feeding type, past experience of care giving. Infant attributes were temperament, behavior, age, sex. The differences between fathering and mothering were reviewed. Fathers were poor at care giving. but their caring was similar to mother's. This subtle difference positively worked upon infant's growth and development. On the basis of these theoretical data, nurses can empower fathers to cooperate with mothers in caring infants.

반복적으로 머리를 때리는 전반성발달장애 환아 1례 (A CASE OF PERVASIVE DEVELOPMENTAL DISORDER NOS WITH REPETATIVE SELF-INJURIOUS BEHAVIOR)

  • 자해행동은 정신지체에서 자주 나타나며, 특히 자폐증에서 더욱 많이 나타난다. 자해행동은 질환이라기보다는 하나의 증상군으로 다루어져왔지만, 사망률에 직접적인 영향을 줄만큼 응급인 임상적 상황이다. 본 증례는 반복적으로 머리를 때리는 자해행위를 보이는 난치성 자폐증 장애 환아가 입원한 상태에서 약물요법과 행동치료를 병행하여 치료하였기에 임상경험을 문헌고찰과 함께 보고하는 바이다. 환아는 7세된 남아로 99년 4월 20일 자해행동을 주소로 OO대학교 어린이병원 소아정신과에 내원하였으며 7월 10일까지 12주간 입원치료를 받았다. 약물치료로는 입원 4주경부터 haloperidol 0.5mg에서 1.0mg으로 증량하였고, naltrexone을 $25{\sim}50mg$을 입원기간중에 병합 투여하였다. 행동치료로는 차등강화(Differential Reinforcement of Other behavior)를 이용하여 정규적인 놀이학습을 수행하였고, 초기에 사용했던 신체적 강박을 해제하기 위해서 머리 보호대와 팔거리를 이용하였다. 현재 외래 통원치료중이며 약물은 haloperidol 0.5mg 및 naltrexone 50mg을 유지하고 있고, 환아 모를 교육하여 집에서 놀이학습을 한시간씩 수행하고 있다. 퇴원당시 자해행동은 중등도 이상 감소되었으며 외래에서도 호전된 상태를 계속 유지하고 있다.

On second when take smoking as it is burning cagar to end. 8) Female smoker are no good and bring many trouble to the mother's body and fetal body(an unborn child). 9) Nicotine also, like drug and alcohol break out reliance, introspection, poisonous or obstinence syndrome 10) A counterplan about smoke should be settlement with propriety control of smoking and an antismoking(non-smoker).

만 2세 영아의 어머니가 가상/역할 놀이와 책읽기 맥락에서 사용하는 정신 상태 용어 (Mental-state Talks of Mothers with 2-year-olds in Pretense/Role-play and Book Reading Contexts)

  • 본 연구는 어머니가 만 2세 자녀와 함께 가상/역할 놀이와 책 읽기의 두 맥락에서 상호작용하는 동안 어머니가 사용하는 정신 상태 용어의 특성을 살펴보고 비교하는 데 그 목적이 있다. 연구대상은 서울시에 거주하는 36쌍의 어머니-만 2세 자녀 쌍이었다. 연구결과 어머니들은 책 읽기 맥락보다 가장/역할 놀이 맥락에서 정신 상태 용어를 더 많이 사용하였으나 세 가지 유형의 정신 상태용어를 사용하는 비율은 맥락에 따라 다르지 않았다. 두 맥락 모두에서 욕구 상태 언어를 정서 상태나 인지 상태 용어보다 자주 사용하였으나 어머니들이 사용하는 욕구 상태, 정서 상태, 인지 상태 용어는 개인 별로 편차가 매우 심했고, 하나의 맥락에서 정신 상태 용어를 많이 사용하는 어머니들은 다른 맥락에서도 정신 상태 용어를 많이 사용하였다. 본 연구의 결과는 자녀의 마음이론 발달에 기여하는 어머니 역할에 대한 실천적 시사점을 갖는다.
