Immediate loading of mandibular single implant by using surgical guide and modeless digital prosthesis: a case report (수술용 가이드와 modeless 디지털 보철물을 이용한 하악 구치부 단일 임플란트 즉시 하중 증례)

  • In this case report, immediate loading of an implant-supported single-tooth prosthesis through complete digital workflow. A patient presented for restoration of missing a single tooth in the mandibular first molar. The digital impression was made with intraoral scanner and implant was placed using surgical guide pre-fabricated with pre-operative computed tomography (CT) and scan data. After 1 week later, prefabricated customized abutment and provisional restoration were connected for immediate loading. After 8 weeks later, abutment level impression was taken by intraoral scanner. At 3 months later from implant installation, monolithic zirconia crown were fabricated. This clinical report presents satisfying result in accuracy and patient satisfaction. A completely modeless digital procedure met expectations regarding precision, esthetics, and functionality.

Full mouth implant rehabilitation with double scanning of provisional restoration (임시치아 double scanning을 이용한 전악 임플란트 수복 증례)

  • It is important to produce a provisional restoration reflecting the patient's jaw relation, occlusal plane, lip support, shape of teeth, and occlusion type for fully edentulous patients before making a definite prosthesis. The patient introduced in this study showed bad prognosis of remained tooth after severe periodontal diseases. Therefore, remaining teeth were extracted and replaced with dental implants. Provisional restorations were fabricated and the the patient's vertical and horizontal jaw relationship, occlusal plane, amount of overjet and overbite, size of teeth, and length of anterior tooth were recorded. Provisional restorations were scanned and CAD/CAM techniques were used to fabricate a monolithic zirconia bridge, which contour is identical with the provisional restorations. The patient was satisfied with the treatment results on functional, esthetic aspects and the prosthesis retained stable during the four-month clinical observation period.

Success rate of all-ceramic FPDs depending on the time of restoration between 2011 and 2023

  • PURPOSE. Studies about success of FPDs (fixed partial dentures) mostly include restorations built by different clinicians. This results in limited comparability of the data. The aim of this study was to evaluate complications of all-ceramic FPDs built by 1 dentist between 2011 to 2023. MATERIALS AND METHODS. 342 all-ceramic FPDs were observed during follow-up care. 48 patients received 262 single crowns, 59 bridges and 21 veneers. Because of the different lengths of the bridges, units were defined as restored or replaced tooth. 465 units performed by the same dentist from Nov 2011 to Nov 2022 were included. Influencing factors "restoration", "construction", "abutment", "localization", "vitality" and "application period" were evaluated using Kaplan-Meier Analysis and Log-Rank Tests. RESULTS. 406 units (87.3 %) showed no complication. 7 correctable chippings (1.5 %) and 10 recementable decementations (2.1 %) occurred. Six decemented units got lost (1.3 %). 21 units failed due to fatal fracture (4.5 %). Crown margin complications, such as secondary caries, occurred in 15 units (3.2 %). Comparing the influencing factors resulted in higher complication rates of veneers (P < .001), of monolithic ceramics (P ≤ .050) and of molar-restorations (P = .047). The application period had no influence on the success and survival rate. CONCLUSION. Overall, all-ceramic FPDs showed good clinical results. Although less complications were observed with modern restorations, these more often led to complete failure. To generate evidence-based recommendations, further studies are needed to evaluate the mid- and short-term success and survival of current all-ceramic restorations.

Full-mouth rehabilitation of skeletal anterior open bite with severely decayed dentition: A case report (심한 우식을 동반한 골격성 전치부 개방 교합 환자의 전악 수복 증례)

  • The open bite malocclusion is a common clinical entity and has multifactorial causes. Development of effective treatment plan and management is dependent on proper diagnosis. The skeletal open bite patient requires a coordinated orthodontic and orthognathic surgical approach to achieve stable occlusion, acceptable esthetics, and improved function. But in case of open bite with severely decayed dentition, restoration in the entire dentition is necessary. Using the facial analysis and diagnostic wax-up, the most effective treatment was prosthetic rehabilitation. The provisional restorations were fabricated to satisfy esthetic and functional requirements, which result in the uniformly distributed occlusal force, anterior and canine guidance. The inter-arch relationship, labio-dental harmony, and the soft tissue aspect, which is important to estimate the longevity were evaluated. Definitive restorations of monolithic zirconia were made by replicating provisional restorations by using the latest CAD/CAM technology. They were delivered to the patient and clinical follow-up observation was satisfactory.

Influence of crown-to-implant ratio of short vs long implants on implant stability and marginal bone loss in the mandibular single molar implant (하악 구치 단일임플란트 수복에서 임플란트 길이에 따른 치관-임플란트 비율이 임플란트안정성 및 변연골소실에 주는 영향)

  • Purpose: The purpose of this randomized clinical trial is to determine whether implant length and the crown-to-implant (C/I) ratio influence implant stability and peri-implant marginal bone loss (MBL). Materials and Methods: 46 patients with single tooth missing in the posterior molar region of the mandible were included in this study. 19 implants (CMI IS-III $active^{(R)}$ long implant) of 5.0 mm diameter and 10 mm length were installed for the control group, while 27 implants (CMI IS-III $active^{(R)}$ short implant) of 5.5 mm diameter and 6.6, 7.3 or 8.5 mm length were placed for the experimental group. Each implant was inserted and immediately loaded using the digitally pre-fabricated surgical template and provisional restoration. The CAD-CAM monolithic zirconia crown was fabricated at 3 months after the surgery as a definitive restoration. The ISQ value and the MBL was measured at 48 weeks after the surgery. The correlation between the C/I ratio, MBL, and secondary implant stability was analyzed. Results: Successful results in terms of ISQ and MBL were achieved with both groups. There was no significant difference between the groups in terms of ISQ values and MBL at 48 weeks after the surgery (P > 0.05). No significant correlation was found between the C/I ratio and secondary stability as well as the C/I ratio and the MBL (P > 0.05). Conclusion: The influence of C/I ratio in both groups was not shown on the stability nor the marginal bone loss in implants supporting single crown of the mandible. Short implant could be a preferable alternative option in the reduced bone height mandible under the limited condition despite its higher C/I ratio.
