Therapeutic Assessment of Primaquine for Radical Cure of Plasmodium vivax Malaria at Primary and Tertiary Care Centres in Southwestern India

  • Acquaintance is scanty on primaquine (PQ) efficacy and Plasmodium vivax recurrence in Udupi district, Karnataka, India. We assessed the efficacy of 14 days PQ regimen (0.25 mg/kg/day) to prevent P. vivax recurrence. Microscopically, aparasitemic adults (${\geq}18years$) after acute vivax malaria on day 28 were re-enrolled into 15 months' long follow-up study. A peripheral blood smear examination was performed with participants at every 1-2 month interval. A nested PCR test was performed to confirm the mono-infection with P. vivax. Of 114 participants, 28 (24.6%) recurred subsequently. The median (IQR) duration of the first recurrence was 3.1 (2.2-5.8) months which ranged from 1.2 to 15.1 months, including initial 28 days. Participants with history of vivax malaria had significantly higher risk of recurrence, with hazard ratio (HR) (95% CI) of 2.62 (1.24-5.54) (P=0.012). Severity of disease (11.4%, 13/114) was not associated (P=1.00) with recurrence. Of 28 recurrence cases, the nPCR proved that P. vivax mono-infection recurrence rate was at least 72.7% (16/22) at first recurrence. In Udupi district, PQ dose of 0.25 mg/kg/day over 14 days seems inadequate to prevent recurrence in substantial proportion of vivax malaria. Patients with a history of vivax malaria are at high risk of recurrences.

Enhancement of Laccase Production from Wood-Rotting Fungus by Co-Culture with Trichoderma longibrachiatum

  • This work aimed to evaluate the influence of culture conditions on laccase production in the co-culture of wood-rotting fungus with Trichoderma sp. The effects of infection extent, infection time, and culture filtrate of Trichoderma sp. on the laccase production by wood-rotting fungus in co-culture were examined. T. rubrum LKY-7 and T. longibrachiatum were selected as fungi which are effective in co-culture for laccase production. A significant increase in laccase activity was observed when T. rubrum LKY-7 was co-cultured with T. longibrachiatum in glucose-peptone liquid medium, yielding an increase of more than 5 times in laccase activity, as compared with control. Laccase production by T. rubrum LKY-7 during co-culturing was significantly influenced by the infection extent and the infection time of T. longibrachiatum. Maximal laccase activity was obtained when T. rubrum LKY-7 culture was infected by T. longibrachiatum after 3 days of cultivation at an inoculum size ratio of 0.5 to 1. The addition of culture filtrate or autoclaved mycelium of T. longibrachiatum to T. rubrum LKY-7 culture did not significantly enhance laccase production by T. rubrum LKY-7 as compared with control (mono cultures of T. rubrum LKY-7).

  • 연구배경 : Rifampicin (RFP)은 결핵단기요법의 근간이 되는 중요한 약제이다. RFP내성 결핵균은 대부분 isoniazid (INH)에도 내성이며, 따라서 RFP내성은 다제약제내성의 지표이다. 그러나 최근 서구에서는 드문 것으로 알려진 RFP단독내성결핵이, 특히 HIV감염과 연관되어 많은 빈도로 보고되었다. 따라서 서구와는 다른 환경을 가진 국내에서 RFP단독내성결핵의 빈도, 가능한 원인들, 및 임상상을 알아보고자 하였다. 대상 및 방법 : 1990년 1월부터 1997년 8월까지 서울중앙병원에서 대한결핵협회에 의뢰하여 결핵균 감수성검사 결과가 확인된 699명(921검체)중 INH감수성, RFP내성결과를 보인 총 18명(31검체)을 대상으로 의무기록을 검토하였다. 과거결핵의 병력, 약제복용력, 동반된 전신질환 유무, HIV검사 여부, 위장질환 여부, 감수성 결과의 변화 양상, 그리고 RFP단독내성 확인후 약제의 변경등 임상 경과를 고찰하였다. 또한 보관된 6 균주는 염기서열결정법으로 rpoB유전자의 돌연변이 양상을 확인하였다. 결 과 : 18명의 평균나이는 $43{\pm}14$세였고 남녀비는 12 : 6이었다. 18명중 16명(89%)에서 과거 결핵을 앓은 병력이 있었고, 이 중 약제복용력이 확인된 12명중 11명은 불규칙하게 약제를 복용하였다. 특기할 만한 위장질환을 진단받은 병력은 없었으며, RFP을 단독으로 복용한 병력도 없었다. 기저질환으로 당뇨병이 4예에서 있었다. HIV검사가 시행된 2명 모두 음성이었다. 11명에서 감수성검사가 2회 이상 반복 시행되었는데, 6명에서는 RFP단독내성을 유지하였고 5명에서는 다제내성으로 바뀌었다. 8명(44%)은 완치되었으며, 6명(33%)은 치료실패하였고 3명(17%)은 추적검사에서 소실되었으며, 1명은 아직 치료중이다. rpoB 유전자 염기서열결정법을 시행한 6균주중 5예에서 rpoB유전자 돌연변이가 발견되었으나 새로운 형태의 돌연변이는 없었다. 결 론 : HIV감염율이 서구에 비하여 낮은 국내에서 RFP단독내성 폐결핵의 임상상은 다제내성결핵과 유사하였다. 그러나 일부 RFP단독내성결핵은 RFP을 포함한 1차약제만으로도 완치가 가능하였고, 이는 RFP 단독내성이 다제내성결핵과는 다른 부류일 가능성도 시사하였다.

Tall Fescue의 초지형과 Endophyte 감염이 한우의 가축행동에 미치는 영향 (Effect of pasture Types and Endophyte Infection on the Animal Behavior of Korean native Cattle)

  • This experiment was carried out to investigate the effect of pasture types(mixture and mono tall fescue pasture) and endophyte infection on the animal behavior of Korean native cattle at the Experimental Field of Grassland and Forage Crops Division, National Livestock Research Institute, Suweon, in 1994. The results obtained are sumunarized as follows: DM intake of Korean native cattle was increased with mixture and endophyte-free tall fescue for both pasture types. Mixture and endophyte-free tall fescue showed good animal preference. Daily grazing time was increased with mixture and endophyte-free tall fescue in monoculture, while annual grazing period was increased with monoculture and endophyte-infect tall fescue. Amount of drinking water of Korean native cattle was increased with tall fescue monoculture rather than mixture, however the amount of drinking water was higher with endophyte-infect tall fescue in monoculture. Salt intake of Korean native cattle was increased with mixture and endophyte-6ee tall fescue in monoculture. Rectal temperature of Korean native cattle was not influenced by pasture types and endophyte infection. Based on the results of this experiment, DM intake of Korean native cattle and daily grazing time were increased by mixture and endophyte-free tall fescue with good quality.

  • 좌측 대퇴골 원위부 골육종 진단 받은 9세 여아로 광범위 절제수술과 재활용 자가골 이식술을 받은 후 하지 부동과 성장판 손상이 있어 확장형 종양 대치물로 치환수술을 시행하였다. 그 후 종양 대치물에 심부 감염 발생하여 종양 대치물 제거 후 감염을 조절하기 위한 시멘트와 골수내정을 이용한 한시적 공간 대치물을 삽입하였다. 감염이 조절된 후 외고정 장치를 이용한 하지 부동(10 cm)에 대한 교정을 시행하였고, 마지막으로 종양 대치물 재삽입을 통한 관절 기능 보존을 시행한 1례를 보고 하고자 한다.

  • This experiment was canied out to investigate the effect of pasture types(mixture and mono tall fescue pasture) and endophyte infection on the dry matter yield, forage quality and animal performance of pasture plants at the Experimental Field of Grassland and Forage Crops Division, National Livestock Research Institute, Suweon, in 1994. The results obtained are surmmarized as follows : DM yield of tall fescue monoculture was significantly higher(p<0.05) than that of mixture pasture, while DM yield of endophyte-infected pasture was significantly higher(p<0.05) than that of endophyte-free for two pasture types. Crude protein yield(CPY) of tall fescue monoculture was significantly higher(p<0.05) than that of mixture, and the higher CPY with endophyte-infect tall fescue for both pasture types resulted from higher DM yield of the pastures. NDF and ADF contents of pasture plants were not influenced by pasture types, and there was no difference for NDF and ADF contents between endophyte-infect and -free pasture mixture, however, those of pasture plants were increased with endophyte-infect tall fescue in monoculture. IVDMD of pasture plants was increased with mixture and endophyte-free tall fescue for both pasture types. IVDMD of pasture plants was greatly influenced by pasture types and endophyte infection. There were no significant differences in average daily gain of Korean native cattle between pasture types, but those of the cattle were significantly increased with endophyte-free tall fescue for both pasture types(p<0.05). Based on the results of this experiment, it is suggested that a slightly more forage yield could be obtained , from monoculture and endophyte-infected tall fescue. However, liveweight gain was increased by mixture and endophyte-free tall fescue with good quality.

감귤류 변패의 원인균인 Penicillium sp.-L4가 생성하는 식물세포벽 분해효소의 작용양상

  • Penicillium sp.-L4, a causative fungus of rot in citrus fruits, was isolated and its mode of hydrolytic enzyme production was investigated. Carboxymethylcellulase (CMCase), polygalacturonase(PGase), extra- & intra-cellular $\beta$-glucosidase and cellobiase were produced drastically by addition of substrates in minimal media. Production of the hydrolytic enzymes were induced efficiently by cellobiose and cellooligosaccharides which were the products of cellulose hydrolysis, but repressed by addition of mono-saccharide such as glucose, raffinose, galacturonic acid. The relative activity of p-nitrophenyl-$\beta$-D-glucopyranoside(PNPG) hydrolysis was higher than that of cellobiose hydrolysis in extracellular enzymes, and reverse is true in intracellular enzymes. Intact enzyme production of P. sp.-L4 on lemon peel lesion was sequential. $\beta$-Glucosidase and CMCase were produced first and followed by PGase. The enzyme productivities and pH in lesions were coincident with optimal pH of each enzyme activities.

Malaria Prevalence in a Low Transmission Area, Jazan District of Southwestern Saudi Arabia

  • Detailed description of malaria in low transmission areas is crucial for elimination. The current study aimed to provide a comprehensive description for malaria transmission in Jazan, a low transmission district, southwestern Saudi Arabia. Patients at a tertiary care hospital were recruited in our study between August 2016 and September 2018. Malaria diagnosis was performed through a species-specific nested polymerase chain reaction (nested PCR), microscopy and Paramax-$3^{TM}$ rapid detection test (RDT). Malaria was detected in 30 patients by the PCR, with point prevalence of 10.9%. Of these malaria infections, 80% was imported, 26.6% was asymptomatic and 23.3% was sub-microscopic. Malaria was reported throughout the year, with February/March and September/October peaks. Infection was significantly more in males than in females (P=0.01). Likewise, infections were detected more in febrile than in non-febrile patients (P=0.01). Adult aged 15-24 years, fever and travel were identified as high-risk factors. Malaria was primarily attributed to Plasmodium falciparum mono-infections, followed by P. vivax mono-infections and lastly to falciparum/vivax mixed infections accounting 76.6%, 16.6%, and 6.6% of PCR-confirmed malaria cases, respectively. The nested PCR was superior to the smear microscopy (sensitivity 76.6%; specificity 100%) and the RDT (sensitivity 83.3%, specificity 94.2%). The overall percent agreement between microscopy and the RDT was 92.7% (kappa=0.63). High proportion of imported malaria including sub-microscopic and sub-patent cases were described. We suggest that incorporation of molecular tool into the conventional malaria diagnosis is beneficial in Jazan district.

참굴큰입흡충(Gymnophalloides seoi)의 제2중간숙주로서 참굴의 역할 (Oysters, Crassostrea gigas, as the second intermediate host of Gymnophalloides seoi (Gymnophallidae))

  • 우리 나라 고유의 인체 기생 장흡충 Gwmopholloides seoi Lee, Chai and Hong, 1993은 급성 췌장염 환자에서 처음 발견되었고. 신안군 도서지방에 큰 유행지가 있음이 밝혀져 의학적 중요성이 점차 높아지고 있으나 지금까지 인체 감염원이 되는 제2중간숙주가 알려져 있지 않았다. 이번 연구에서는 자연산 참굴(Crossoskrea gigas)이 제2중간숙주의 하나일 것으로 추정하고 신안군 유행지에서 참굴을 채집한 후 gniulophallid metacercariae에 감염되어 있는지 조사하였다. 조사한 참굴 50마리 모두에서 glrnnophallid metacercariae가 발견되어 100%의 감염률을 보였고. 참굴 1마리 당 평균 610개(개체당 2-4.792개)의 감염량을 보였다 충체의 종은 추후 마우스 감염 실험을 통하여 G. seoi로 동정할 수 있었다. 유충(metacercariae)은 낭을 가지고 있지 않았고, 구흡반으로 참굴의 외투막(mantle surface)을 흡착하고 있었으며, 참굴의 절편 표본에서 참굴큰입흡충 속의 특징 중 하나인 복측흡(ventral pit)이 관찰되며. 복흡반 전방에 근육질이 아닌 생식공이 있고 저정낭으로 연결되어 있었다. 저정낭은 대부녁의 충체에서 단낭성(mono-sac)이어서 유일한 다른 종인 G. toiensis가 이낭성(bipartite) 저정낭을 가진 것과는 차이를 보였다. 이 연구를 통하여 참굴이 G. seoi의 제2중간숙주이며 동시에 가장 중요한 인체 감염원일 것으로 판단하였고, 이 흡충의 우리말 이름을 참굴큰입흡충으로 제안한다.

High Frequency of Enteric Protozoan, Viral, and Bacterial Potential Pathogens in Community-Acquired Acute Diarrheal Episodes: Evidence Based on Results of Luminex Gastrointestinal Pathogen Panel Assay

  • Infectious diarrhea is endemic in most developing countries. We aimed to investigate the protozoan, viral, and bacterial causes of acute diarrhea in Taif, Saudi Arabia. A cross-sectional prospective 1-year study was conducted on 163 diarrheal patients of various ages. Stool samples were collected, 1 per patient, and tested for 3 protozoa, 3 viruses, and 9 bacteria with the Luminex Gastrointestinal Pathogen Panel. Overall, 53.4% (87/163) of samples were positives (20.8% protozoa, 19.6% viruses, 2.8% bacteria, and 9.8% mixed). Rotavirus (19.6%), Giardia duodenalis (16.5%), and Cryptosporidium spp. (8.5%) were the mostly detected pathogens. Adenovirus 40/41 (4.2%), Salmonella (3%), Shiga toxin-producing Escherichia coli (3%), and Entamoeba histolytica (2.4%) were also detected. Norovirus GI/II, Vibrio cholerae, Yersinia enterocolitica, and Clostridium difficile toxin A/B were not detected in any patients. All pathogens were involved in coinfections except E. histolytica. Giardia (5.5%) and rotavirus (3%) were the most commonly detected in co-infections. Enterotoxigenic E. coli (2.4%), Campylobacter spp. (2.4%), E. coli 0157 (1.8%), and Shigella spp. (1.2%) were detected in patients only as co-infections. Infections were more in children 0-4 years, less in adults <40 years, and least >40 years, with statistically significant differences in risk across age groups observed with rotavirus (P<0.001), Giardia (P=0.006), and Cryptosporidium (P=0.036) infections. Lastly, infections were not significantly more in the spring. This report demonstrates the high burden of various enteropathogens in the setting. Further studies are needed to define the impact of these findings on the clinical course of the disease.
