매복된 하악 제 3 대구치 발치시 항생제 투여의 효용성 (THE EFFICACY OF ANTIBIOTIC ADMINISTRATION IN THE IMPACTED MANDIBULAR THIRD MOLAR EXTRACTION)

  • To evaluate the efficacy of antibiotic administration in the impacted mandibular third molar extraction the author investigated 107 patients requiring extraction of the lower third molar who visited at the Department of oral and maxillofacial Surgery, Chosun Dental Hospital and were healthy without medical problems and had no signs and symptoms of infection around the lower third molar. The patients were divided into 4 groups according to the method of antibiotics administration: 13 patients without antibiotics administration(group 1), 30 patients with only intravenous injection of $Cefazoline^{(R)}$ 2.0g 30 minutes before the procedure(group 2), 39 patients with intravenous injection of $Cefazoline^{(R)}$ 2.0g 30 minutes before the procedure and oral administration of follow-up dosages of $Augmentin^{(R)}$ for 1 day(group 3), and 25 patients with intravenous injection of $Cefazoline^{(R)}$ 2.0g 30 minutes before the procedure and oral administration of follow-up dosages of $Augmentin^{(R)}$ for 3 day(group 4). Infection rates after extraction were calculated and compared according to gender, the age of the patients, the level of impaction, and also compared between four groups. The results were as follows. 1. The overall infection rate was 8.4%. 2. The infection rate was higher in male(11.9%) than in female(4.2%), but there were statistically no significant differences between them. 3. Infection rate was lower in patients under age 30(7.2%) than in patients over age 30(12.5%), but there were statistically no significant between them. 4. Infection rate was higher in patients with complete bony impacted tooth(11.1%) than in patients with partial bony impacted tooth(6.5%), but there were statistically no significant differences between them. 5. Infection rate was 7.7% in group 1, 10.0% in group 2, 10.3% in group 3, 4.0% in group 4 and there were statistically no significant differences between groups. In summary, it is more important to extract the impacted lower third molar under aseptic condition and to minimize the injury of tissue if possible than to administer the preventive antibiotics in reducing the infection rate in healthy patients without medical problems who had no signs and symptoms of infection around the lower third molar.

  • 영구 전치의 매복은 제 3 대구치나 견치에 비해서 드물게 나타나지만 학령기 아동에서 관찰할 수 있다. 매복치아의 원인은 외상, 공간 부족, 치아의 맹출 경로상에 위치하는 과잉치, 유치의 치근단 감염 등 다양하다. 매복된 치아는 부정교합과 인접치아의 치근 흡수, 낭종 등의 병적 변화를 유발할 수 있으므로 임상적 검사와 방사선학적 검사를 통해 정확한 위치를 확인하고, 적절한 치료계획을 통해 정상 맹출을 유도하여야 한다. 매복된 치아가 병적 변화를 일으킨 경우나 인접치에 손상을 주었을 경우에 발치를 시행하게 된다. 그러나 교정적 견인을 통해 기능과 심미성을 회복시켜 줄 수 있으므로 조기 발견과 발견시의 올바른 진단과 치료 계획을 수립하는 것이 중요하다. 교정적 견인시에는 매복치를 배열할 공간의 확보와 고정원에 대한 고려가 선행되어야 한다. 견인시 적절한 방향의 힘을 통해서 치근의 흡수 등의 부작용이 일어나지 않도록 해야 한다. 본 증례는 매복된 전치들을 forced eruption을 통해서 정상적인 맹출 방향으로 유도한 치험례이다.

우식치면과 치아발거의 상호관계에 대한 연구

  • The 800 extracted permanent teeth of first and second molar were observed and obtained the following results; Those teeth were extracted from the population of dentistless rural area. The cause of extraction of those teeth was only the dental caries. It was performed by the free dental service team from 1966 to 1972. The findings were as follows: 1. The most incidence of caries occurrence was on the occlusal surface and the least was lingual surface. 2. The distal surface caries rate was higher than the other surface except occlusals. 3. The average rate of decayed surface was higher in the second molar group than the first molar. 4. The average rate of dacayed surface was higher in the teeth of mandible than maxilla. 5. The average rate of decayed surface was higher in female than male. 6. The average rate decayed surface per tooth was 2.54±1.98.

Third molar extraction in middle-aged and elderly patient

  • Extraction of impacted third molars is a routine procedure performed by oral and maxillofacial surgeons. However, extractions in middle-aged or elderly individuals are not easy, and there are several factors that need to be considered. These factors include decreased healing potential and the risk of complications increasing with age. In addition, third molars can often be fully or deeply impacted in middle-aged individuals, and pathologic changes, such as cysts, caries, or periodontitis of the adjacent second molar, can develop. Furthermore, the rate of ankylosis and systemic disease increases after middle age. It is considered that these factors acting alone or in combination increase the difficulty of extraction.

Primary xanthoma inferior to the right mandibular third molar and intraoral vertical ramus osteotomy

  • Intraosseous xanthoma of the mandible is a rare benign disorder. A 17-year-old male patient presented with a suspected abscess in the right mandibular third molar, detected on a panoramic radiograph. The patient had no history of systemic or lipid-related metabolic diseases and complained of no specific symptoms or pain. A radiographic examination revealed a heterogeneous radiolucency extending from the apical to the distal aspect of the right mandibular third molar tooth germ. The lesion measured 9 × 16 × 24 mm (antero-posterior × mediolateral × supero-inferior) and showed a relatively well-defined, multilocular, foamy appearance with hyperostotic borders spreading to the inferior alveolar nerve canal. After excisional biopsy, a diagnosis of central xanthoma was made. The lesion recurred, and intraoral vertical ramus osteotomy was done near the lesion. For the treatment of xanthoma of the mandible, extensive and delicate surgical treatment under general anesthesia should be considered.

치아귀열에 관한 임상례

  • This is a case report on split tooth which the author experienced. Fine hairline fracture of right 1st molar was detected by means of staining dye stoff under illumination. The patients complained sensitive to temperature change and to biting force.

구강구조모델과 워터쉐드를 이용한 치아영역 분할 (Tooth Region Segmentation by Oral Cavity Model and Watershed Algorithm)

  • 본 논문에서는 치아에 대한 컬러영상에서 개별적인 치아영역을 분할하기 위한 새로운 방법을 제안하였다. 제안하는 알고리듬은 치아의 구조적 특징을 이용한 구강구조모델과 워터쉐드 알고리듬의 새로운 경계선 설정방법 등이 사용되었다. 먼저, 컬러영상으로부터 치아영역이 강조된 회색레벨 영상을 획득하고 치아영역 분할시 문제가 될 수 있는 불필요한 부분을 영상에서 제거하였다. 다음으로 제안한 구강구조모델을 이용한 치아영상의 영상향상을 실행하였고, 향상된 영상을 워터쉐드 알고리즘을 이용하여 개별적 치아영역을 분할하였다. 워터쉐드 알고리즘에 필요한 경계선과 시드는 최소 문턱치를 이용한 이진영상의 경계선과 국부 최대값을 적용하였다. 제안한 방법의 성능을 평가하기 위하여 기존의 방법과 제안한 방법에 대하여 비교 실험을 수행하였다. 실험 결과, 제안한 방법이 기존의 방법에 비하여 대구치영역의 검출율이 향상됨을 확인하였으며 치아를 포함한 구강 내 영역의 중복검출 등의 문제를 방지하여 치아영역 검출 성능이 향상되었음을 확인하였다.

Prediction of optimal bending angles of a running loop to achieve bodily protraction of a molar using the finite element method

  • Objective: The purpose of this study was to predict the optimal bending angles of a running loop for bodily protraction of the mandibular first molars and to clarify the mechanics of molar tipping and rotation. Methods: A three-dimensional finite element model was developed for predicting tooth movement, and a mechanical model based on the beam theory was constructed for clarifying force systems. Results: When a running loop without bends was used, the molar tipped mesially by $9.6^{\circ}$ and rotated counterclockwise by $5.4^{\circ}$. These angles were almost similar to those predicted by the beam theory. When the amount of tip-back and toe-in angles were $11.5^{\circ}$ and $9.9^{\circ}$, respectively, bodily movement of the molar was achieved. When the bend angles were increased to $14.2^{\circ}$ and $18.7^{\circ}$, the molar tipped distally by $4.9^{\circ}$ and rotated clockwise by $1.5^{\circ}$. Conclusions: Bodily movement of a mandibular first molar was achieved during protraction by controlling the tip-back and toe-in angles with the use of a running loop. The beam theory was effective for understanding the mechanics of molar tipping and rotation, as well as for predicting the optimal bending angles.

Characteristics of bony changes and tooth displacement in the mandibular cystic lesion involving the impacted third molar

  • Objectives: The purpose of this retrospective study is to find the differentiating characteristics of cystic and cystic-appearing lesions that involve the impacted mandibular third molar by analyzing panoramic radiographs and computed tomography images, and to aid the preoperative diagnosis. Materials and Methods: Eighty-one patients who had a mandibular cystic or cystic-appearing lesion that involved impacted mandibular third molar and underwent cyst enucleation were included in the study. The preoperative panoramic radiograph and computed tomography findings were analyzed in accordance to the histopathologic type. Results: Most of the cystic lesions containing the mandibular third molar were diagnosed as a dentigerous cyst (77.8%). The occurrence of mesio-distal displacement of the third molar was more frequent in the odontogenic keratocyst (71.4%) and in the ameloblastoma (85.7%) than in the dentigerous cyst (19.1%). Downward displacement was primarily observed in each group. Odontogenic keratocyst and ameloblastoma showed more aggressive growth pattern with higher rate of bony discontinuity and cortical bone expansion than in dentigerous cyst. Conclusion: When evaluating mandibular cystic lesions involving the impacted mandibular third molar, dentigerous cyst should first be suspected. However, when the third molar displacement and cortical bone absorption are observed, then odontogenic keratocyst or ameloblastoma should be considered.
