A Hybrid Intervention for Post-infarction Papillary Muscle Rupture with Severe Mitral Regurgitation: A Case Report

  • Papillary muscle rupture with severe acute mitral regurgitation is a rare complication of acute myocardial infarction (AMI) that causes pulmonary congestion and cardiogenic shock. Moreover, it has a poor prognosis. Surgical intervention, including revascularization, is indicated; however, surgical mortality remains high. We report the case of an 85-year-old woman with cardiogenic shock from severe acute mitral regurgitation, in whom a hybrid intervention, combining percutaneous coronary intervention with mitral valve replacement via minithoracotomy, was performed after post-infarction papillary muscle rupture. She was discharged in a favorable clinical condition. We describe a novel hybrid intervention for treating a rare complication of AMI, which could minimize surgical invasion in elderly patients, prevent disuse syndrome after the intervention, and improve prognosis. However, mitral valve surgery via minithoracotomy for emergency cases requires technical proficiency, as well as collaboration with other healthcare professionals, and the choice to perform this procedure requires careful consideration.

승모판막 교련절개술의 임상적 연구 (Clinical Evaluation of Open Mitral Commissurotomy)

  • A clinical analysis was performed on 48 cases with mitral stenosis who received open mitral commissurotomy from December 1983 to June 1991 at the Department of Thoracic & Cardiovascular Surgery, Chungnam National University Hospital. Fifteen patients were men and 33 were women. the mean age was 35.6 years with the range of 16 to 61 years. The distribution of preoperative NYHA Functional Classes was as follows; class II, 25 patients; class III, 20 patients; class IV, 3 patients. Three patients had cerebral emboli preoperatively, all of whom were in atrial fibrillation. The preoperative electrocardiographic studies revealed that 32 patients had sinus rhythms and 16 had atrial fibrillations. Twenty-six patients had open mitral commissurotomy alone and 22 patients had additional cardiac procedures. Intraoperatively, there were 6 cases of left atrial thrombosis. There was no perioperative death but early postoperative complications were found in 3 cases. The patients were followed up from 2 to 99 months[mean 33.7 months . There was one case of late unexplained sudden death. There was a case of late postoperative cerebral infarction, 5 cases of mitral restenosis and 3 cases of congestive heart failure and/or arrhythmia. Mitral valve replacements were required in 3 patients.But, it is evident that open mitral commissurotomy has many advantages beyond mitral valve replacement concerning the results of the mitral valve replacement underwent during the same period at the Department of Thoracic & Cardiovascular Surgery, Chungnam National University Hospital.

연합판막질환의 판치환수술 (Double Valve Replacement: report of 5 cases)

  • Mitral and aortic valve replacement with tricuspid annuloplasty was undertaken in 5 patients out of 38 valvular surgery between the period from Jan. 1977 to May 1979 in the Dept. of Thoracic and Cardiovascular Surgery in Korea University Hospital. All were male patients with age ranging from 18 to 42 years, and preoperative evaluation revealed one case in Class IV, and four cases in Class III according to the classification of NYHA. Preoperative diagnosis was confirmed by routine cardiac study including retrograde aorto- and left ventriculography, and there were two cases with MSi+ASi+Ti, two cases with MSi+Ai+Ti, and one case with Mi+Ai+Ti. Double valve replacement was performed under the hypothermic cardiopulmonary bypass with total pump time of 247 min. in average ranging from 206 min. to 268 min. During aortic valve replacement, left coronary perfusion was done in the first two cases, and cardiac arrest with cardioplegic solution proposed by Bretschneider was applied in the remained three cases. Starr-Edwards, Bjork-Shiley prosthetic valves and Carpentier-Edwards tissue valve were replaced in the aortic area, and Carpentier-Edwards and Angell-Shiley tissue valves were replaced in the mitral area with each individual combination [three prosthetic and two tissue valves in the aortic, and five tissue valves in the mitral area respectively]. Postoperative recovery was uneventful in all cases except one case with hemopericardium, which was managed with pericardiectomy on the postoperative 10th day in good result. Follow-up after double valve replacement of the all five cases for the period from 6 months to 33 months revealed satisfactory adaptation in social activity and occupation with cardiac function of Class I according to the classification of NYHA In all five cases.

심장 다판치환술의 임상적 고찰 (Clinical Experience of Multiple Valve Replacement)

  • From April 1982 to December, 1988, multiple valve replacement was performed in 49 patients. Mitral and aortic valve replacement were done in 42 patients, 4 underwent mitral and tricuspid valve replacement and 3 patients underwent triple valve replacement. Of the valve implanted, 50 were Duromedics, 21 St. Jude, 13 Bjork-Shiley, 9 Carpentier-Edwards, 6 Ionescu-Shiley, and 2 Medtronic. The hospital mortality rate was 28.5 % [14 patients] and the late mortality rate was 6.1 % [3 patients], the mortality rate was high in early operative period but decreased with time. [20% at 1986, 18.2 % at 1987, 9.5% at 1988] The causes of death were low cardiac output in 8, congestive heart failure in 2, multiple organ failure in 1, LV rupture in 1, intracerebral hemorrhage in 1 and sudden death in l. The actuarial survival rate excluding operative death was 77% at 7 years.

승모판막 치환의 조기 성적;158 임상보고례 (Early Results of Mitral Valve Replacement - Clinical analysis of 158 cases -)

  • The results of the clinical observations on the 158 cases of the mitral valvular heart disease treated at the Department of Thoracic and Cardiovascular Surgery, Masan Koryo General Hospital from June, 1986 to December, 1991 were as follows; 1. There were 50 men and 108 women with sex ratio 1: 2.2. 2. The age of patient varied widely from 13 years to 65years. 3. The preoperative functional level accoridng to the NYHA classification were class II, III, IV in 18%, 67%, 15%. 4. All 158 patients were operated on under direct vision using extracorporeal circulation, mitral valve replacement in 118 cases, mitral and aortic valve replacement 40 cases. 5. The follow up period was between 2 months and 65 months postoperatively. [mean 30.1 months] 6. The early operative motality was 5.7%[9 cases], late motality was 4%[6 cases]. 7. The cardiothoracic ratio in the chest X-ray decreased at the 6th month postoperation. [pre-op: 0.60$\pm$0.07, post-op: 0.56$\pm$0.06 p<0.01]

  • A 69-year-old patient with rupture of anterolateral papillary muscle following non-Q-wave myocardial infarction is described. Transesophageal echocardiography was useful in making the diagnosis. Mitral valve replacement was performed 22 days after the onset of acute myocardial infarction. The patient was discharged in his good condition and coronary angiography is scheduled.

Outcomes of Mitral Valve Repair: Quadrangular Resection versus Chordal Replacement

  • Background: Mitral valve repair for posterior mitral leaflet (PML) prolapse has been considered to be a standard treatment because of its high success rate and high level of patient satisfaction. The aim of this study was to evaluate the clinical results of two different techniques of PML prolapse, quadrangular resection (QR) and chordal replacement (CR). Materials and Methods: The subjects consisted of 56 patients who had undergone mitral valve repair for PML prolapse between November 1997 and December 2010. The patients were divided into two groups according to surgical technique. Among them, 31 patients underwent QR (group QR) and 25 patients had CR (group CR). We reviewed the medical records of the patients retrospectively to compare the clinical outcomes of both groups. Results: After mitral valve repair, the degree of mitral regurgitation (MR) in both groups decreased to the to a mild degree or less and the amount of remnant MR was slightly higher in the CR group but it was not statistically different. Three patients received mitral valve-related reoperation (2 in the QR group and 1 in the CR group). Freedom from mitral valve-related reoperation at 7 years was 93% for the QR group and 96% for the CR group and was not significantly different between the two groups. Conclusion: Both QR and CR showed excellent long-term results and were considered equally effective methods for PML prolapse.

St. Jude 승모판막의 장기 임상성적 (Long-Term Clinical Results of the St. Jude Medical Valve in Mitral Position)

  • A total of 217 patients underwent single mitral valve replacement [MVR] with the St. Jude Medical valve between September 1984 and the end of 1992. There were 86 males and 131 females with a mean age of 34.6 $\pm$ 14.4 years[range 5 months-61 years]. A previous valve replacement had been performed in 46 patients [21.2 %]. An early mortality rate was 7.4 % [5.2 % in primary MVR; and 15.2 %in re-replacement MVR]. Early survivors of 201 patients were followed up for a total of 934.5 patient-years[mean 4.7 $\pm$ 2.1years]. A late mortality rate was 2.5 % or 0.54%/patient-year. The linearized rates of thromboembolism, valve thrombosis and anticoagulation-related bleeding were 1.301 %, 0.214 % and 0.428 %/patient-year, respectively. The actuarial survival including operative mortality was 89.9 % $\pm$ 2.1% at postoperative 10 years. The freedom from thromboemolism was 91.3 %$\pm$ 2.5% and the actuarial estimate of incidence free from late deaths and all complications were 80.9 % $\pm$ 3.8 % at 10 years. There were no mechanical failures. In summary, the St. Jude Medical prosthesis performed satisfactorily with an acceptable rate of late complications.

개심술 치험 60례 보 (Clinical analysis of 60 cases of open heart surgery)

  • Sixty cases of open heart surgery were performed in the Department of Thoracic and Cardiovascular Surgery of Chonbuk National University Hospital from July, 1983 to June, 1984. The patients were consisted of 40 [66%] congenital anomalies containing 26 [43%] patients of acyanotic group and 4 [23%] of cyanotic group, and 20 [34%] acquired heart diseases which involved one or more cardiac valves. The male patients were 42 and the female 18. In 20 valvular heart diseases, open mitral commissurotomy was done in 5 patients, mitral valvular replacement with tissue valve in 6, mitral valvular replacement with mechanical valve in 5, mitral valvular replacement with tricuspid annuloplasty in 2, mitral annuloplasty in 1, and mitral and aortic valvular replacements with mechanical valves in 1. The most frequency complication was low cardiac output syndrome occurred in 9, and the next was urethral stenosis, ARDS, and postoperative bleeding, etc. The perioperative mortality was 21% in congenital cyanotic heart disease, 12% in congenital acyanotic heart disease, and 5% in acquired heart disease.

소아에서의 승모판치환술 (Mitral Valve Replacement in Children Less Than 16 years of Age)

  • 연세대학교 심장혈관센터에서는 1980년 2월부터 1995년 9월까지 승모판질환을 가진 2개월부터 15세 가지의 소아환자 31명에게 승모판치환술을 시행하였다. 이중 추적조사가 가능했던 환자는 28명이었으며, 남아는 10명, 여아는 18명이었고, 체중은 4.9kg부터 5.6kg까지 였다. 조기사망은 2례(7.1%)에서 발생하였고, 이들은 모두 1세이하의 영아였으나, 각 연령군사이의 사망률은 통계학적으로 의미 있는 차이는 없었다(p=0.13). 5세이하의 소아에서 판막과 관련된 합병증률은 57.1%였으며, 다른 연령군에 비해 승모판치환술후 높은 합병증률을 보여주었다(p<0.05). 조직 판막을 사용하였던 소아환자는 모두 7명이며, 그들의 판막실패 없는 5년 생존률은 50%이었다. 기계 판막을 사용하였던 나머지 21명의 환자에서 아직 판막실패는 발생하지 않았다. 조직 판막의 높은 재치환률을 생각해볼 때, 기계판막은 항응고제가 필요하고, 출혈과 혈전색 전증 가능성 이 있지만, 내구성 이 짧은 조직판막보다는 좋다. 그러나, 자라나는 심장내에 장기간 삽입된 기계판막은 성장함에 따라 재치 환술을 필요로 할 것으로 생각된다. 소아에서의 승모판치환술은 만족할 만한 장기\ulcorner적을 보여주고 있지만, 사망률과 합병증률에 영향을 미치는 중요한 요소는 소아의 연령이다. 특히 5세이하의 환아에서는 승모판치환술시 세심한 주의를 요한다.
