Usefulness of Gracilis Flap for Perineal Area Reconstruction (회음부 재건에 있어서 박근피판의 유용성)

  • Purpose: Due to the closed and humid condition of the perineal area, wound problems occurring at this site are sometimes complicated, especially following postoperative radiation therapy. Moreover, the anal sphincter is a very important functional structure but reconstruction of the anal sphincter after severe trauma poses a challenging problem to plastic surgeons. In this article, we demonstrate the usefulness of the pedicled gracilis flap in the reconstruction of the perineal area. Methods: From September 2008 to November 2009, 6 patients, 4 males and 2 females, underwent surgery of the perineal area. The age of the patients ranged from 21 to 62 years (mean age was 48). The mean follow up period was 14 months. In 4 cases, the patient presented with wound problems after postoperative radiation therapy for anal cancer. In 2 cases, the patient presented with traumatic sphincter damage. Only the gracilis muscle was used in the 4 cases and a musculocutaneous flap was used in the 2 cases involving skin defects, respectively. Results: Among the 6 patients, 1 patient underwent hematoma evacuation of the donor site, and 1 patient presented with prolapse of the vaginal mucosa which recovered spontaneously. There were no report of other complications and there were no wound recurrences. Minimal incontinence was observed in all patients who underwent sphincter reconstruction, but all were satisfied with the overall results. Conclusion: The gracilis flap is useful in the reconstruction of the perineal area, such as in cases of radiotherapy induced wound problems and sphincter damage following severe trauma, due to its easy accessibility, rich vascularity, and minimal donor site morbidity.

Alimentary Tract Duplication in Pediatric Patients: Its Distinct Clinical Features and Managements

  • Purpose: Alimentary tract duplication (ATD) is a rare congenital condition that may occur throughout the intestinal tract. Clinical symptoms are generally related to the involved site, size of duplication, or associated ectopic mucosa. This study aimed to identify clinical implications by anatomical locations and age group and then suggest a relevant management according to its distinct features. Methods: We retrospectively reviewed the clinical data of pediatric patients who received a surgical management due to ATD. Furthermore, data including patients' demographics, anatomical distribution of the duplication, clinical features according to anatomical variants, and outcomes were compared. Results: A total of 25 patients were included in this study. ATD developed most commonly in the midgut, especially at the ileocecal region. The most common clinical presentation was abdominal pain, a sign resulting from intestinal obstruction, gastrointestinal bleeding, and intussusception. The non-communicating cystic type was the most common pathological feature in all age groups. Clinically, prenatal detection was relatively low; however, it usually manifested before the infantile period. A laparoscopic procedure was performed in most cases (18/25, 72.0%), significantly in the midgut lesion (p=0.012). Conclusion: ATD occurs most commonly at the ileocecal region, and a symptomatic one may usually be detected before the early childhood period. Surgical management should be considered whether symptom or not regarding its symptomatic progression, and a minimal invasive procedure is the preferred method, especially for the midgut lesion.

An Online Forklift Dispatching Algorithm Based on Minimal Cost Assignment Approach (최소 비용할당 기반 온라인 지게차 운영 알고리즘)

  • Forklifts in a shipyard lift and transport heavy objects. Tasks occur dynamically and the rate of the task occurrence changes over time. Especially, the rate of the task occurrence is high immediately after morning and afternoon business hours. The weight of objects varies according to task characteristic, and a forklift also has the workable or allowable weight limit. In this study, we propose an online forklift dispatching algorithm based on nearest-neighbor dispatching rule using minimal cost assignment approach in order to attain the efficient operations. The proposed algorithm considers various types of forklift and multiple jobs at the same time to determine the dispatch plan. We generate dummy forklifts and dummy tasks to handle unbalance in the numbers of forklifts and tasks by taking their capacity limits and weights. In addition, a method of systematic forklift selection is also devised considering the condition of the forklift. The performance indicator is the total travel distance and the average task waiting time. We validate our approach against the priority rule-based method of the previous study by discrete-event simulation.

  • In this paper we analyze the CA formed by combining the uniform 102 CA $\mathbb{C}_u$ and the m-cell 90/150 hybrid CA $\mathbb{C}_h$ whose characteristic polynomial is $(x+1)^m$. We analyze cycle structures of complemented group CA derived from $\mathbb{C}_u$ and propose a condition of complemented CA dividing the entire state space into smaller cycles of equal lengths. And we analyze the cycle structure of complemented group CA $\mathbb{C}^{\prime}$ derived from the CA $\mathbb{C}$ formed by combining $\mathbb{C}_u$ and $\mathbb{C}_h$ with complement vector F such that $(T+I)^{q-1}F{\neq}0$ where $(x+1)^q$ is the minimal polynomial of $\mathbb{C}$.

A Study on Long-term Monitoring of Seaweed Flora and Community Structure at Hakampo, Western Coast of Korea (한국 서해안 학암포 해조상 및 군집구조의 장기모니터링 연구)

  • Macroalgal community structure was seasonally examined at Hakampo (Taean) in western coast of Korea from February 2007 to October 2010. Also, the effects of "Hebei Spirit" oil spill on the seaweed community structure were evaluated. A total of 101 macroalgal species were identified, comprising 12 green, 18 brown and 71 red algae. Species richness ranged 58-65 species with maximal in 2008 and minimal in 2009. Seaweed biomass ranged $75.81-102.06g\;dry\;wt./m^2$ (mean, $88.78g/m^2$) with maximal in 2008 and minimal in 2010. Vertical distribution from the high to low intertidal zone was Neorhodomela aculeata and Polyopes affinis; Corallina pilulifera and Chondrus ocellatus; Sargassum thunbergii and Ulva australis. Coarsely-branched seaweeds comprised the highest proportion of biomass ($37.17g/m^2$, or 41.86% of the total biomass) and ecological state group I (ESG I) seaweed biomass was between 81.67-85.44%. Also, ephemeral macroalgae including Ulva species sharply increased in species number and biomass within 1-2 year from the "Hebei Sprit" oil spill in the mid and low intertidal zone. Hakampo rocky shore is still good condition as evaluated based on macroalgal species number, biomass, and composition in functional form and ESG I seaweeds.

Endoscopie Removal of Adult Esophageal Foreign Bodies (성인 식도이물의 내시경적 치료)

  • The majority of esophageal Foreign body ingestions occur in the pediatric population. In adults, true foreign object ingestion occurs more commonly among those with psychiatric disorders, mental impairment. The management of esophageal foreign bodies is influenced by the age, clinical condition of ingested material, anatomic location and technical abilities of the endoscopist. Recently the therapeutic endoscopy is becoming wider and more rational in application. We evaluated the role of endoscopy for removal of esophageal foreign bodies during the period of 4 years from January 2000 to December 2003 at the Department of Otolaryngology and Gastroenterology, Seonam University Hospital. The results were as follow, 1) The age ranged from 21 to 74 years old (mean 50.5), most frequent age group was between 61-70 years old and male to female ratio was 1:1.4. 2) Fish bone was the most frequent foreign body in the esophagus ($47.1\%$), food material ($23.5\%$) and meats ($17.6\%$) were next frequent foreign bodies. The most frequent site of lodgement was the first ($78.4\%$), second ($17.6\%$) and third narrowing ($3.9\%$) in order. 3) The most common symptom was foreign body sensation (28.6%). the next common symptoms were chest discomfort($23.8\%$) and dysphagia($19\%$). 4) In duration of lodgement, 49cases ($96.1\%$) were lodged for less than one day. 5) The foreign bodies of esophagus were removed successfully by flexible endoscope with basket, snare, forceps, overtube and endoscopic variceal ligation cap. There were only 3 cases of minimal complications, esophageal mucosal tearing. In conclusion, endoscopic esophageal foreign body removal is useful and safe with minimal or no complications.

Construction and Characterization of Novel Expression Vectors for Genetic Adipose Tissue Ablation

  • Obesity, one of the most common metabolic diseases in industrial countries is characterized by an increase in the number or size of adipocytes. In an effort to create transgenic mouse models for the study of obesity we developed a novel technique in which adipose tissue can be ablated genetically at will, at any specific developmental stage and/or physiological condition, by the treatment of ganciclovir. We made a series of adipocytespecific expression vectors using minimal regulatory regions of brown adipocyte-specific uncoupling protein (UCP-1) gene and adipocyte-specific aP2 gene, and then analyzed their expression characteristics in cultured cell lines. When both constructs pUCP-LacZ and paP2-LacZ were transfected transiently into differentiating 3T3-L1 (pre-while adipocytes) and HIB-1B (pre-brown adipocytes) cell lines in vitro and then monitored by X-gal staining of cells, these regulatory regions were sufficient to show proper differentiation stage-specific expression in adipocvtes. To confirm that adipocytes expressing HSV-TK controlled by these minimal requlatory elements are sufficient to kill themselves with ganciclovir treatment pUCP-TK and paP2-TK expression constructs were transfected stably into HIB-1B and 3T3-L1 cells, respectively, and their ganciclovir sensitivities were tested during in vitro differentiation of cells. As expected more than 80% of cells were dead by the 7th day of treatment with ganciclovir while negative control cells were not affected at all. The data suqqest that the constructed vectors are suitable for obtaining novel obese transqenic models based on a conditional genetic tissue ablation method.

Influence of Heart Rate and Innovative Motion-Correction Algorithm on Coronary Artery Image Quality and Measurement Accuracy Using 256-Detector Row Computed Tomography Scanner: Phantom Study

  • Objective: To investigate the efficacy of motion-correction algorithm (MCA) in improving coronary artery image quality and measurement accuracy using an anthropomorphic dynamic heart phantom and 256-detector row computed tomography (CT) scanner. Materials and Methods: An anthropomorphic dynamic heart phantom was scanned under a static condition and under heart rate (HR) simulation of 50-120 beats per minute (bpm), and the obtained images were reconstructed using conventional algorithm (CA) and MCA. We compared the subjective image quality of coronary arteries using a four-point scale (1, excellent; 2, good; 3, fair; 4, poor) and measurement accuracy using measurement errors of the minimal luminal diameter (MLD) and minimal luminal area (MLA). Results: Compared with CA, MCA significantly improved the subjective image quality at HRs of 110 bpm (1.3 ± 0.3 vs. 1.9 ± 0.8, p = 0.003) and 120 bpm (1.7 ± 0.7 vs. 2.3 ± 0.6, p = 0.006). The measurement error of MLD significantly decreased on using MCA at 110 bpm (11.7 ± 5.9% vs. 18.4 ± 9.4%, p = 0.013) and 120 bpm (10.0 ± 7.3% vs. 25.0 ± 16.5%, p = 0.013). The measurement error of the MLA was also reduced using MCA at 110 bpm (19.2 ± 28.1% vs. 26.4 ± 21.6%, p = 0.028) and 120 bpm (17.9 ± 17.7% vs. 34.8 ± 19.6%, p = 0.018). Conclusion: Motion-correction algorithm can improve the coronary artery image quality and measurement accuracy at a high HR using an anthropomorphic dynamic heart phantom and 256-detector row CT scanner.

Effect of Visual and Somatosensory Information Inputs on Postural Sway in Patients With Stroke Using Tri-Axial Accelerometer Measurement

  • Background: Posture balance control is the ability to maintain the body's center of gravity in the minimal postural sway state on a supportive surface. This ability is obtained through a complicated process of sensing the movements of the human body through sensory organs and then integrating the information into the central nervous system and reacting to the musculoskeletal system and the support action of the musculoskeletal system. Motor function, including coordination, motor, and vision, vestibular sense, and sensory function, including proprioception, should act in an integrated way. However, more than half of stroke patients have motor, sensory, cognitive, and emotional disorders for a long time. Motor and sensory disorders cause the greatest difficulty in postural control among stroke patients. Objects: The purpose of this study is to determine the effect of visual and somatosensory information on postural sway in stroke patients and carrying out a kinematic analysis using a tri-axial accelerometer and a quantitative assessment. Methods: Thirty-four subjects posed four stance condition was accepted various sensory information for counterbalance. This experiment referred to the computerized dynamic posturography assessments and was redesigned four condition blocking visual and somatosensory information. To measure the postural sway of the subjects' trunk, a wireless tri-axial accelerometer was used by signal vector magnitude value. Ony-way measure analysis of variance was performed among four condition. Results: There were significant differences when somatosensory information input blocked (p<.05). Conclusion: The sensory significantly affecting the balance ability of stroke patients is somatosensory, and the amount of actual movement of the trunk could be objectively compared and analyzed through quantitative figures using a tri-axial accelerometer for balance ability.
