대체의학(代體醫學)을 이용(利用)한 비만치료(肥滿治療)의 실제(實際) (A Case of Alternative Therapy for obese patient)

  • This case study was done on a 37 year old female patient at the department for rehabilitation in Bundang Cha Oriental hospital who was treated for 70 days by traditional Acupuncture and Herbal medicine therapy as well as fasting with saengshig(生食), air bath(風浴) and cold-hot-bath(冷溫浴) for skin revitalisation, furthermore by Doinangyo and exercise for improvement of general condition and colon-hydrocleansing. The results are as following: 1. The body weigth was reduced by 14.5kg from 87.4kg to 72.9kg 2. The obesity degree reduced from 159% to 133% 3. The BMI reduced from 33.3 to 27.8 4. The WHR reduced from 0.96 to 0.88 5. The amount of body fat reduced by 7.7kg from 27.2kg to 19.5kg 6. The fat distribution of the abdomen reduced from 0.92 to 0.85 7. The percent body fat was reduced from 31.1% to 26.7 % 8. Except of a mild systemic pruritus the clinical symtoms like kneepain, fatigue, chilling, headache, indigestion, menorrhalaia improved significantly.

  • Hybrid Electric Vehicles (HEVs) utilize electric power as well as a mechanical engine for propulsion; therefore the performance of HEV s can be directly influenced by the characteristics of the Energy Storage System (ESS). The ESS for HEVs generally requires high power performance, long cycle life and reliability, as well as cost effectiveness. So the Hybrid Energy Storage System (HESS), which combines different kinds of storage devices, has been considered to fulfill both performance and cost requirements. To improve operating efficiency, cycle life, and cold cranking of the HESS, an advanced dynamic control regime with which pertinent storage devices in the HESS can be selectively operated based on their status was presented. Verification tests were performed to confirm the degree of improvement in energy efficiency. In this paper, an advanced HESS with improved an Battery Management System (BMS), which has optimal switching control function based on the estimated State of Charge (SOC), has been developed and verified.

  • Coronavirus is an enveloped virus with positive-sense single-stranded RNA. Coronavirus infection in humans mainly affects the upper respiratory tract and to a lesser extent the gastrointestinal tract. Clinical symptoms of coronavirus infections can range from relatively mild (similar to the common cold) to severe (bronchitis, pneumonia, and renal involvement). The disease caused by the 2019 novel coronavirus (2019-nCoV) was called Covid-19 by the World Health Organization in February 2020. Face-to-face communication and consistent exposure to body fluids such as blood and saliva predispose dental care workers at serious risk for 2019-nCoV infection. As demonstrated by the recent coronavirus outbreak, information is not enough. During dental practice, blood and saliva can be scattered. Accordingly, dental practice can be a potential risk for dental staff, and there is a high risk of cross-infection. This article addresses all information collected to date on the virus, in accordance with the guidelines of international health care institutions, and provides a comprehensive protocol for managing possible exposure to patients or those suspected of having coronavirus.

HCoV-IMDB: Database for the Analysis of Interactions between HCoV and Host Immune Proteins

  • Coronaviruses are known respiratory pathogens. In the past, most human coronaviruses were thought to cause mild symptoms such as cold. However recently, as seen in the Severe Acute Respiratory Syndrome (SARS) and the Middle East Respiratory Syndrome (MERS), infectious diseases with severe pulmonary disease and respiratory symptoms are caused by coronaviruses, making research on coronaviruses become important. Considering previous studies, we constructed 'HCoV-IMDB (Human Corona Virus Immune Database)' to systematically provide genetic information on human coronavirus and host immune information, which can be used to analyze the interaction between human coronavirus and host immune proteins. The 'HCoV-IMDB' constructed in the study can be used to search for genetic information on human coronavirus and host immune protein and to download data. A BLAST search specific to the human coronavirus, one of the database functions, can be used to infer genetic information and evolutionary relationship about the query sequence.

담미(淡味)의 음양(陰陽) 특성에 대한 고찰(考察) (A Study on the Yin-Yang Characteristics of Bland Taste)

  • Objectives : This paper aims to examine the Yin-Yang characteristics of bland taste. Methods : Medicinals including bland taste were searched in herbology textbooks and mainly in the Bencaogangmu, of which major discourses were analyzed to examine the Yin-Yang characteristics of bland taste. Results & Conclusions : Most medicinals with bland taste were sweet-bland-cold, holding properties of cooling heat, stimulating urination and detoxification. These properties could be explained in relation to the Triple Burner's metabolism of Qi and fluids, as they stimulate perspiration and urination, improving the flow of fluids which in turn supports production of Original Qi, by working on the TB's Qi circulation system. In the bland taste discourse, it was understood that bland taste moves the Qi system and produces fluids, by first ascending then descending. Zhudanxi and the Neijing have opposing views on whether bland taste is Yin or Yang, but both saw blandness to have both Yin and Yang qualities. Characteristics of the bland taste could be better understood in relation to the Triple Burner. The phrase 'bland is attached to sweet taste[淡附于甘]' refers to the likeliness between the two tastes in that bland taste has the Central Earth qualities, integrating others and concealing itself in its blandness, its taste unspecific, mild and unstimulating thus communicating the Qi within the entire Triple Burner from Upper to Middle to Lower, as does sweet taste.

3세 이하 Mycoplasma pneumoniae 폐렴환자의 임상적 고찰 (Clinical Findings of Mycoplasma pneumoniae pneumonia under 3 Year-Old Children)

  • 목 적 : 3세 이하에서 Mycoplasma pneumoniae 폐렴은 드문 것으로 알려져 있으나 최근 영유아에서도 Mycoplasma pneumoniae 폐렴이 많이 발생되고 있어서 3세 이하에서의 Mycoplasma pneumoniae 폐렴의 임상적 특성을 알아보고자 하였다. 방 법 : 1994년 1월부터 1997년 12월까지 4년간 경희대학교 부속병원 소아과에 입원한 환아 중 Mycoplasma pneumoniae 폐렴으로 진단된 3세 이하 환아 30명을 대상으로 의무기록 을 후향적으로 조사하여 임상양상, 혈청학적 소견, 방사선 소견, 합병증 및 치료 효과를 비교 분석 하였다. 결 과 : 1) 3세 이하에서 폐렴으로 입원한 환아 235명중 30명(12.7%)이 Mycoplasma pneumoniae 폐렴이었고 남녀비는 1.3 : 1로 남아에서 다소 높았다. 2) 발생시기는 여름 및 가을, 초겨울에 걸쳐 넓게 분포하였고 1994년과 1997년에 호발하였다. 3) 임상증상은 기침, 객담, 발열 순이었고, 그 외 콧물, 구토 증세도 나타났으며, 청진소견 상수포음이 26례(86.7%)로 가장 많았고, 그 외에 천명, 호흡음 감소가 나타났다. 4) 방사선 소견상 24례(80.0%)에서 폐침윤을 보였고, 일측성이 13례(43.3%), 양측성이 10 례(33.4%)였고, 흉막삼출이 있었던 경우는 1례(3.3%)였으며, 무기폐를 보인 경우는 없었고, 정상 소견이 6례(20.0%)였다. 6) 혈액검사당 백혈구 수는 정상인 경우가 24례(80.0%)였고, 호중구 증가를 보인 경우는 1례(3.3%)에 불과했으며, 호산구 증가는 10례(33.3%)에서 보였으며, 혈침속도 증가는 56.7%, CRP 양성률은 60.0%였다. 7) 냉응집소치가 양성인 경우는 19례(63.3%)였으며, Mycoplasma 항체치가 양성인 경우는 27례(90.0%)로 Mycoplasma 항체치가 더 높은 양성률을 보였으며 두 검사간 상관관계는 없었다. 또한 13례를 대상으로 시행한 Mycoplasma PCR 검사상 12례(92.3%)에서 양성반응을 보여 Mycoplasma pneumoniae 폐렴의 진단에 유용한 검사로 생각되나 이에 대한 임상적용 은 앞으로 더 연구되어야 할 것으로 사료된다. 8) 합병증으로는 피부발진 3례(10.0%)와 흉막삼출 1례(3.3%), 관절통 1례(3.3%)가 발생되었으나 경증이었고 후유증 없이 모두 완쾌되었다. 결 론 : Mycoplasma pneumoniae 폐렴은 3세 이하에서도 드물지 않게 발생되고 있으며, 임상양상의 특징으로는 방사선상 폐침윤이 없는 경우가 많고 합병증 발생이 적으며 증세가 경미하여 회복이 빠른 것 등이다. 3세 이하 폐렴 환아 중 진단되지 않고 누락된 환아가 다수 있을 것으로 생각되며 이 환아들을 포함하면 실제 발생률은 더 높을 것으로 사료된다.

Cardioplegic Solution의 심근보호 효과에 관한 실험적 연구 (An Experimental Study on the Myocardial Protection Effects of the Cardioplegic Solution)

  • The increasing use of cardioplegic solution for the reduction of ischemic tissue injury requires that all cardiplegic solution be carefully assessed for any protective or damaging properties. This study describes functional, enzymatic and structural assessment of the efficiency of three cardioplegic solutions (Young & GIK, Bretschneider, and $K^{+}$ Albumin solution) in a Modified Isolated Rat Heart Model of cardiopulmonary bypass and ischemic arrest. Isolated rat heart were subjected to a 2-minute period of coronary infusion with a cold cardioplegic or a noncardioplegic solution immediately before and also at the midpoint of a 60-minute period of hypothermic ($10{\pm}1$. C) ischemic cardiac arrest. The results of this study were as follow: 1. Spontaneous heart beat after ischemic arrest occured 16 seconds later after Langendorff reperfusion in the Young & GIK group (n=6), and 40 second later in the Bretschneider group (n=6) and 6 minute later in the $K^{+}$ Albumin group (n=6), and 16 minute later in the control group (non-cardioplegia). A good recovery state of spontaneous heart beat was shown in the Young & GIK and Bretschneider groups. 2. The percentage of recorveries of heart function at 30 minute after postischemic working heart perfusion were : heart rate $91.6{\pm}3.1$% (P<0.01)m oeaj airtuc oressyre $83{\pm}3$% (P<0.01), coronary flow $70{\pm}8$% (P<0.05) and aortic flow flow rate $39{\pm}9.3$% (P<0.05) in the Young & GIK group. This percentage of recoveries of the Young & GIK group was significantly greater than the control group. In the Bretschneider group, the percentage of recoveries were : heart rate $87.8{\pm}7.5$%(P<0.05), peak aortic pressure $71{\pm}2.3$% (P<0.05) and aortic flow rate $33.2{\pm}6.6$%(P<0.05). hte percentage of recoveries were significantly greater than in the control group. In the $K^{+}$ Albumin group, recoveries of heart function were poor. 3. Total CPK leakage was $131.2{\pm}12.75$IU/30 min/gm. dry weight in the control group, $50.65{\pm}12.75$IU in the Young & GIK gruop, $69.40{\pm}32.21$Iu in Bretschneider group, and $103.65{\pm}15.47$IU in the $K^{+}$ Albumin group during the 30 minute postischemic Langendorff reperfusion. Total CPK leakage was significantly less (P<0.001) in the Young & GIK group, than in the control group. 4. Direct correlatin between percentage recovery of aortic flow rate and total amount of CPK leakage from Myocardium was noticed.(Correlation Coefficient r = 0.76, P<0.001). 5. Mild perivascular edema was the only finding of light microscopic study of myocardium after 60 minute ischemic arrest with cold cardioplegic solutions and hypothermla.

동양의 전통적 환경인식과 환경관 (Thre Traditional Perception and Viewpoint to Natural Environment in the Orient)

  • In the orient, ancestor's perception to natural environment was related on the change of climate. Man had been a essence of the warm blood animal, which had suited in mild climate for living and acting. The weather of hot or cold was not only comfort in human life but influences negatively on human behavior. The most people of oriental was particularly interrelated to seasonal change of climate in earlier time. Because, there are elements such as temperature, humidity and wind in the category of climate, these elements differentiated seasonal change. The main methods of perception of natural environment were observed and classified. Although these methods were in the primitive stage, these coincided the tendency of modern science. For example, confucian was recognized the law of vertical air current that warm air rised and cold air sinked. And they could classify all elements nature based on the principles of 'Yin and Yang(陰陽論)' such as male and female, the sun and moon. shade and light etc. Thus results of the observation and the classification concerned with physical environment can be utilize a wisdom for progressive life of inhabitants. It was a origin of the education in natural environment. Commonness in the viewpoint of environment in the orient if recognized the law of circulation. Buddhism, originated in india, realized that all of phenomena in the world was changed slowly through the principles of cycle(輪廻說) such as male and female, the sun and moon, shade and light etc. Thus results of the observation and the classification concerned with physical environment can be utilized a wisdom for progressive life of inhabitants. Then Taoism emphasized the pure nature without the artificiality. Because complexual environment was composed of several elements, It was maintained through the artificiality. Because complexual environment was composed of several elements, It was maintained through the interaction of cause and effects. The solution of environmental problem is maintenanced the harmony between cause and effects by a philosophical concept.

통증어휘를 이용한 통증비율척도의 개발연구 (A Study for Development of Ratio Beale Measuring Pain Using Korean Pain Tersm)

  • The main purpose of this study is to develop a ratio scale measuring level of pain using Korean pain terms. The specific purposes of this study are to identify the degree of pain of each pain term in each subclass: to classify each subclass in terms of dimensions of pain; and to analyze factors of the Korean pain ratio scale clustering together. One hundred an4 fifty eight pain terms which were originally identified as representative terms and their synonyms were used for data collection. Fifty eight nursing professors ana sixty one medical doctors who have contacted with patients having pain were asked to rate the weight of each pain term on a visual analogue scale. Subclasses in which ranks of pain terms were same f s findings in two previous studies were 1) thermal 3 am 2) cavity pressure, 3) single stimulating pain, 4) radiation pain. and 5) chemical pain. Subclasses in which ranks of pain terms were confused were 1) incisive pressure, and 2) cold pain. Subclasses in which one new pain term was added were 1) inflammatory-repeated pain, 2) punctuate pressure, 3) constrictive pressure, 4) fatigue-related pressure, and 5) suffering-relate4 pain. Subclasses in which two new pain terms were added were 1) traction pressure, 2) peripheral nerve pain, 3) dull pain, 4) pulsation-related pain, 5) digestion-related pain, 6) tract pain, and 7) punishment-related pain. Subclass in which 3 new pain terms were included was fear-related pain. Rating scores of 5 words in 4 subclasses were significantly different between the normal group and the extreme group of subjects in terms of subjective rating. Only one word among 6 words was that newly added to the scale. Rating scores of 12 words in 9 subclasses were significantly different between doctor group and nursing professor group. Among these 12 words, only 3 were those newly added to the scale. In comparison of these 12 words, mean scores of the nursing professors were always 7 to 16 points higher than those of the medical doctors. In the analysis of judgement of subjects in terms of dimensions of pain terms, subclasses of dull pain, cavity pressure, tract pain and cold pain were suggested to be included in the miscellaneous dimension. As a result of factor analysis of the ratings given to 96 pain words using principal components analysis without iteration and with varimax rotation limiting the number of factors to 4, factors of severe pain (factor I) mild-moderate pain (factor II) , causative pain (factor III) and temperature-related pain(factor IV) were extracted with the factor loading above 0.388. When the pain words were re-arranged on the bases of factor loading above 0.368, number of factors decreased to only first two factors. Maximum score of pain word in factor II was 46.17 and the minimum score of the factor I was 45.36. Further studies are needed to identify the validity, reliability, sensitivity and practicability of this ratio scale using patients having various sources of pain.

700MPa급 용착금속의 미세조직에 따른 저온균열 감수성 (Cold Crack Susceptibility of 700 MPa welding Consumable According Microstructure)

  • 과거 고강도강 용접부에서 발생하는 저온균열은 주로 용접열영향부에서 발생하였는데, 이러한 문제점을 해결하기 위하여 강재 메이커들은 고강도강의 용접성을 향상시키고자 노력하였다. 이러한 노력의 결과로 TMCP, HSLA 강 등이 개발되었고 이들 강재는 예열온도를 저하시킬 수 있다는 장점 때문에 보편화되어 사용되었다. 이러한 강재는 모재 예열온도를 기준으로 적용하게 되면 용착금속에서 저온균열이 발생하는 경우가 있다. 따라서 이제는 용접재료의 용접성, 즉 용접재료의 저온균열 저항성을 평가 할 수 있는 기법이 요구된다. 본 연구의 목적은 용착금속의 저온균열 저항성을 평가하는 것인데, 저온균열 저항성은 용착금속의 미세조직에 따라 다르게 나타날 수 있다. 용착금속의 합금조성은 기본적으로 용착금속에 요구되는 최저 강도와 충격인성을 만족할 수 있도록 설계한다. 하지만 유사한 강도의 유사한 합금조성이더라도 일부 합금 성분에 의해 용착금속의 미세조직들은 상이하게 나타날 수 있는데, 미세조직 특성에 의하여 용착금속의 강도와 저온인성이 결정된다. 용착금속의 저온균열 저항성을 평가하기위하여 Gapped Bead-on-Groove(G-BOG) 시험에 사용된 모재는 50mm 두께의 mild steel을 사용하였으며, 모재의 희석을 방지하기위해 15mm 깊이로 V-groove 가공 후 buttering 용접 하였다. 용접된 시편은 다시 5mm 깊이로 V-groove로 2차 가공 후 Ar + 20% $Co_2$ gas를 사용하여 용접하였다. 용접재료는 ER-100S-G grade로 비슷한 합금조성을 갖는 2 종류를 사용하였다. A용접재료는 Ti 이 0.1% 함유 되었으며, B용접재료는 Ti 함유되지 않은 것을 사용하였다. 또한 예열 온도에 따라 저온균열 감수성을 평가하기위하여 모재의 예열온도를 각각 상온, $50^{\circ}C,\;75^{\circ}C,\;100^{\circ}C$로 하여 실험을 진행하였다. 용착금속의 미세조직을 확인해본 결과 Ti 함유된 A 용착금속 미세조직은 대부분 침상형페라이트로 나타났으며, Ti 함유되지 않은 B 용착금속 미세조직은 대부분 베이나이트로 나타났다. G-BOG 시험 결과 Ti 함유된 A 시편이 Ti 함유되지 않은 B 시편보다 저온균열 발생량이 적었다. 이는 용착금속의 미세조직분포 및 특성에 따라 저온균열감수성이 다르다는 것을 나타낸다.
