A patient suffered from chronic ulcer due to recalcitrant fungal infection for 3.5 years. Five antifungal agents and 40 times of debridement—all failed. Finally, radical microscopic debridement was performed for eradication of fungal conidiospores. Since then, there was no recurrence at 2 years of follow-up. Scopulariopsis brevicaulis is one of the rarest pathogens of cutaneous fungal infections, for which multidrug resistance increased the complexity and difficulty of treatment. Radical excision, especially microscopic debridement, was the key for eradication of fungal conidiospores in this case.
The purpose of this study is to investigate the appropriate management of severe radionecrotic wounds of the anterior chest wall associated with infection of the soft tissues and ribs and exposure of vital structures(heart and lung), and present our strategies for reconstruction of these complicated patients. 9 patients have undergone radical debridement and immediate microsurgical reconstruction for severe radionecrotic wounds of the anterior chest wall over last 7 years. All patients had extensive osteomyelitis of the ribs or sternum, and chronic infection or cutaneous fistulae. 2 patients had pericardial effusions due to longstanding inflammation, and 6 patients had pleural effusions. 2 patients had ipsilateral lung collapse. 10 free flaps were performed for coverage of the huge defects. One patient required 2 free flaps to control the inflammation. 8 free TRAM flaps were used for coverage of the defects and in addition, the rectus abdominis muscle was packed into any deep cavity. 1 patients underwent latissimus dorsi muscle free flap because of previous abdominal surgery. After extensive debridement of the infected, radionecrotic wounds, all 10 free flaps were successful. All the extensive radionecrotic defects of the anterior chest wall were completely healed. Free flaps successfully covered the exposed vital structures of the heart and lungs. Patients with severe radionecrotic defects of the anterior chest wall after ablative breast cancer surgery and radiotherapy were successfully treated by radical debridement and immediate free flap surgery. The TRAM flap together with the rectus muscle is the treatment of choice for these huge defects. The latissimus dorsi muscle flap was the second choice in patients with previous abdominal surgery. The recipient vessel should be carefully selected because of possible radiation damage and inflammation.
Purpose: Scalp avulsion is a life-threatening injury that may cause trauma to the forehead, eyebrows, and periauricular tissue. It is difficult to treat scalp avulsion as it may lead to severe bleeding. Therefore, emergency scalp replantation surgery is necessary, and we must consider the function, aesthetics, and psychology of the patients. A case of scalp avulsion leading to massive bleeding was encountered by these authors, which led to a failure to achieve the proper operation conditions in an adequate time period. Methods: A 49-year-old female was hospitalized due to having had her head caught in a rotatory machine, causing complete scalp avulsion which included the dorsum of the nose, both eyebrows, and ears. Emergent microsurgical replantation was performed, where a superficial temporal artery and a vein were anastomosed, but the patient's vital signs were too unstable for further operation due to excessive blood loss. Three days after the microanastomosis, venous congestion developed at the replanted scalp, and a medicinal leech was used. Leech therapy resolved the venous congestion. A demarcation then developed between the vitalized scalp tissue and the necrotized area. Debridement was performed 2 times on the necrotized scalp area. Finally, split-thickness skin graft with a dermal acellular matrix ($Matriderm^{(R)}$) was performed on the defective areas, which included the left temporal area, the occipital area, and both eyebrows. Results: The forehead, vertex, right temporal area, and half of the occipital area were successfully replanted, and the hair at the replanted scalp was preserved. As stated above, two-thirds of the scalp survived; the patient could cover the skin graft area with her hair, and could wear a wig. Conclusion: Complete scalp avulsion needs emergent replantation with microsurgical revascularization, but it often leads to serious vital conditions. We report a case with acceptable results, although the microanastomosed vessel was minimal due to the patient's unstable vital signs.
전북대학교병원 정형외과에서 1993년 12월부터 1998년 9월까지 하지의 만성 골수염 7례에 대하여 유리 근 피판 이식술을 시행하고 최소 1년 2개월부터 최장 5년 3개월까지 추시하여 다음과 같은 임상적 결과를 얻었다. 1. 만성 골수염의 발생 부위는 경골이 4례, 종골 2례 그리고 대퇴골이 1례였다. 2. 만성 골수염의 지속 기간은 평균 31.6년이었다. 3. 전체 7례 중 1례에서 편평 상피암이 발병되었다. 4. 만성 골수염은 4례에서 혈행성 감염으로 초래되었고, 3례는 외상력이 있었는데 2례는 교통사고 그리고 1례는 경미한 외상이었다. 5. 치료는 부골 제거술과 유리 근 이식술을 시행하였던 예가 2례, 부골 제거술없이 유리 근 이식술을 시행한 예가 5례였다. 6. 전체 7례 중 6례에서 유리 근 이식술을 시행하였고 1례에서 유리 근피판 이식술을 시행하였는데 복직 근이 4례였고, 광배 피판, 광배 근피판 그리고 박근이 각각 1례씩이었으며 7례 중 6례(85.7%)에서 생존하였다. 7. 대퇴부에 시행하였던 광배 근피판 1례는 정맥이식술을 통한 단측 문합술을 시행하였으나 술 후 2일째부터 허혈성 변화를 일으켜 실패하였으며, 외상으로 인한 종골 1례에서는 복직근 이식술이 성공하였으나, 술 후 심한 외상성 족관절염으로 인한 극심한 통증으로 슬관절 하부 절단술이 시행되었다.
Upper limb nerve damage is a common condition, and evidence suggests that functional recovery may be limited following peripheral nerve repair in cases of delayed reconstruction or reconstruction of long nerve defects. A 26-year-old man presented with traumatic injury from a wide, blunt wound of the right forearm caused by broken glass, with soft tissue loss, complete transection of the radial and ulnar arteries, and a large median nerve gap. The patient underwent debridement and subsequent surgery with a microsurgical free radial fasciocutaneous flap to provide a direct blood supply to the hand; the cephalic vein within the flap was employed as a venous vascularized chamber to wrap the sural nerve graft and to repair the wide gap (14 cm) in the median nerve. During the postoperative period, the patient followed an intensive rehabilitation program and was monitored for functional performance over 5 years of follow-up. Our assessment demonstrated skin tropism and sufficient muscle power to act against strong resistance (M5) in the muscles previously affected by paralysis, as well as a good localization of stimuli in the median nerve region and an imperfect recovery of two-point discrimination (S3+). We propose a novel and efficient procedure to repair >10-cm peripheral nerve gap injuries related to upper limb trauma.
저자들은 1988년부터 1998년까지 시행한 252예의 유리피판술 중 9예에서 미세혈관 문합부의 혈전 등의 이유로 실패하였으며, 이들 중 두경부 및 하지의 7예에서는 첫 수술 4-16일 사이에 제 2의 유리피판술을 성공적으로 시행하여 환부를 치유시키고, 처음에 계획하였던 수술적 목표를 달성하였다. 수부의 2예는 환자 및 보호자와 충분한 상의하여 원거리 피판과 피부이식 등 고식적인 치료 방법을 선택하였다. 수부의 경우는 다른 부위보다 고식적 치료 방법이 덜 부담되었던 것으로 생각되었으며, 두경부와 하지는 환부의 특성상 제 2의 유리 피판술이 최선의 선택으로 생각되었다. 유리 피판술이 실패한 환부에서 계속된 제 2의 유리 피판술을 시행하는 것은, 처음의 수술보다 면밀한 술전 계획과 준비, 완벽한 수술, 수술후의 철저한 환자 관리가 요구되었으나, 이들 환부의 처치에 매우 효과적이고 환자의 기대에 부응할 수 있는 방법이라고 생각된다.
목적: 하지, 특히 정강이 앞 부분이나 발, 발목에서의 연부조직 결손은 피복에 어려움이 있다. 저자들은 하지의 개방성 골절 또는 폐쇄적 골절 후 수상부위에 발생한 다양한 연부조직 결손에 대하여 역행성 표재 비복동맥 피판술을 사용하여 피복을 시행한 예들을 수집, 분석하여 그 임상적 결과를 보고하고자 한다. 대상 및 방법: 2003년 8월부터 2018년 4월까지 하지의 골절 후 수상 부위에 발생한 연부조직 결손에 대하여 역행성 비복동맥 피판술을 시행받은 환자를 대상으로 하였다. 수술 후 최소 6개월 추시가 가능했던 16명의 환자를 대상으로 하였으며, 평균 추시기간은 18개월이었다. 8예는 개방성, 다른 8예는 폐쇄적 골절 후 발생한 합병증으로 연부조직 결손이 발생했다. 피판의 평균크기는 51.9 cm2였으며, 가장 큰 피판의 크기는 10×15 cm2였다. 결과 분석을 위해 수술 후 피판의 생존 여부와 추가 술식, 합병증 등을 조사하였다. 결과: 최종 추시 시 이식 피판은 모두 생존하였으며, 공여부에 별다른 합병증도 발생하지 않았다. 수술 후 단기 추시 당시 1예에서 경계부의 부분 괴사 소견이 발견되었으나 변연절제술과 1차 봉합을 통해 회복되었다. 다른 1예에서는 혈종이 발생하여 추가로 혈종제거술을 시행하였으며, 추시기간 내 피판은 성공적으로 생존하였다. 이식부의 미용적 목적으로 피판 축소수술이 3예에서 시행되었고, 피판부를 절개하고 접근하여 내고정물을 제거하거나 추가적 내고정술을 시행한 건이 3예였었다. 결론: 역행성 표재 비복동맥 피판술은 하지 골절과 동반된 연부조직 결손의 치료에 있어 적극적 적용을 고려할 만한 유용한 술기의 하나가 될 수 있을 것으로 생각된다.
