Fundamental Idea and Actuality of the Medical Dispute Mediation Act (의료분쟁조정법의 기본이념과 현실)

  • Medical treatment has great potential for conflict. Even the best-trained doctors can commit medical malpractice that result in continuing physical or mental disabilities or even death. Medical conflicts have been increasing over years. The medical conflicts between patient and medical professionals that result from medical professionals' mistakes are often fueled by a violation on the pretext of the injuries form medical malpractice and can lead to litigation. The litigation usually cost a lot of money and time. The extension of the litigation period as well as expensive cost and lack of medical knowledge placing a great burden on patients. Alternative Dispute Resolution(ADR) is more efficient than litigation. In 1988, the medical dispute mediation system has been introduces as the Act on Remedy for Damage from Medical Accident and Medical Dispute Mediation by Korean Medical Association came into effect after 23 years of enactment efforts. Medical Dispute Mediation Act(hereinafter referred to as the "MDMA") has finally entered into force from 8 April 2012. The purpose of the MDMA is to promptly and fairly redress injuries caused by medical malpractice and create a stable environment for medical services of public health or medical professionals by providing for matters regarding the mediation and arbitration of medical disputes(MDMA ${\S}1$). In an effort to secure the fair, speedy and inexpensive resolution of every malpractice case, the Korea Medical Dispute Mediation and Arbitration Agency(hereinafter referred to as the "K-MEDI") was established. Following the MDMA, the K-MEDI shall endeavor to ensure the medical dispute mediation or arbitration proceedings are conducted in a prompt, fair, and efficient manner, and patients and medical professionals shall attend proceedings in good faith with mutual trust and understanding when they participate in medical dispute mediation or arbitration proceedings.

Clinical and Neuropsychological Characteristics of Adult Alcohol Use Disorder Patients with or without Attention-Deficit Hyperactivity Symptom (성인 알코올사용장애 환자에서 주의력결핍 과잉행동 증상 유무에 따른 임상적 및 신경심리학적 특성)

  • Objectives : Alcohol use disorder (AUD) is often comorbid with adult attention-deficit hyperactivity disorder (ADHD) and other psychiatric illnesses. When associated with other mental problems, the prognosis of the AUD can be more serious. This study shows research on the clinical and neuropsychological characteristics according to whether or not ADHD symptoms and AUD were comorbid. Methods : A total of 64 adult inpatients who completed AUD scales about adult ADHD, alcohol dependence, depression, anxiety, and impulsiveness. They also completed neuropsychological tests about attention and executive function. According to the Adult ADHD Self-Report Scale score, patients were categorized into two groups (ADHD symptom positive/negative group). Results : Fourteen among the 64 subjects were part of the ADHD symptom positive group (21.9%). They had statistically significant shorter abstinence periods and a higher rate of history of 'rule violation during school' and 'physical damage in childhood' compared to the ADHD symptom negative group. Conclusion : The comorbidity rate with ADHD symptom in adult AUD is higher than the general population. The ADHD symptom positive group suffered from more severe and refractory AUD. Considering the more frequent history of rule violation during school in the ADHD symptom positive group, the association between ADHD and AUD may be mediated by conduct disorder. Therefore we suggest the necessity of careful evaluation and intervention in children and adolescents with ADHD and conduct disorder.

A Study on Measures for Preventing Personal Information Leakage in Financial Corporations (금융사 개인정보 유출 방지 방안에 관한 연구)

  • Personal information leakage in financial corporations including three card corporations has occurred constantly this year. It is due to incomplete encryption system and negligent personal security. Solicitors are known as a cause of information leakage because they operate with leaked information. Information leakage can cause secondary damage with mental demage to person and result in a drop in reliability as well as an operating loss in financial corporations. Also because it can destroy a base of credit society, prevention of recurrence is badly needed. The government finally announced 'general measures for prevention of information leakage in the field of finance' with sanctions reinforcement and restriction to collect, possess, provide personal information as the main agenda. And a related law revision is going in the National Assembly. In this paper, effectiveness of government measures is weighed with the cause analysis of information leakage and countermeasure for prevention of information leakage is found.

Influences of an Experimental Exposure to Excavator Noise on the Cardiac Factors and Cerebral Hemodynamics

  • Noise may cause damage of the auditory system, hypertension, and cardiovascular disease. However, we haven't the data enough to be available for understanding various effects of noise on the human body. The current study was prospectively designed to investigate the changes of the cardiac factors and cerebral hemodynamics following a transient exposure to noise in young people. 80 subjects (mean aged $23.45\pm2.40$ years) participated in this experiment and were exposed to excavator-noise with 90 decibels for 15 minutes using ear-phone. Cardiac factors such as heart rate (HR), blood pressure (BP) and heart rate-systolic pressure product (RPP), and cerebral hemodynamics such as mean blood flow velocities (Vm), pulsatility indexes (PI), resistance indexes (RI) and mean blood flow velocities at breathing-hold (Vh) in the middle (MCA), anterior (ACA) and posterior cerebral arteries (PCA) were measured before (baseline) and during the noise-exposure. Although there were individual differences in above mentioned parameters, HR, systolic and diastolic BP, RPP, MCA-Vm, MCA-PI, MCA-RI, ACA-Vm, ACA-PI, ACA-RI, PCA-Vm, PCA-PI, and PCA-RI during the noise-exposure decreased compared with the baselines (P<0.05 or P<0.01), The findings of the present study suggest that a transient exposure to excavator-noise at rest causes changes in the cardiac factors and cerebral hemodynamics with individual differences. Further studies need to be carried out for clarifying the effects of longer exposure and combined mental activity with noise exposure.

  • Besides from medical data, the patients who were previously called as attention disorder, mental instability, moral imbeciles, or moral defectives, can also be identified by exploring literatures and historical figures. In the past, as we can notice from the titles, they were recognized as a 'moral defect group'. And rather than treating them, separation from the society was the main solution. After the endemic encephalitis from 1917 to late 1920s, however, many survivors suffered from behavioral problems similar to those of the previous 'moral defect group' and studies on the relationship between brain damage and behavior problems were started henceforth. After being known as the 'minimal brain dysfunction', it was developed into the current attention-deficit/hyperactivity disorder. While the disease concept changed and developed over time, after numerous trials and errors, treatment medication starting from central nervous system stimulants such as amphetamine and methylphenidate is used for treatment in children and adult patients with ADHD, and most recently non-stimulants such as atomoxetine has become the one of the first line treatment options. Although we went through a thorough verification process of the safety and efficacy of the medication by contemplating the historical development process, we believe that adjustment is needed for remaining concerns on medication abuse and slight differences in disease paradigm and therapeutic philosophy depending on cultures.

Protective effect of Shenqi-wan and its fractions on N-methyl-D-aspartate-induced excitotoxicity in rat hippocampus

  • Shenqi-wan, Oriental herbal medicine formulation, has traditionally been used for the treatment of delayed mental and physical development in children, complications of diabetes, and glomerulonephritis. In the present study, we investigated the protective effect of the aqueous extract of Shenqi-wan and its fractions against N-methyl-D-aspartate (NMDA)-induced exitotoxicity in rat hippocampal CA1 neurons. Fractions were elucidated at 0 - 10 min, 11 - 20 min, and 21 - 30 min by using gravity column chromatography method. In the present results, treatment with NMDA on cultured hippocampal slices induced neuronal death in the hippocampal CA1 region. Pretreatment with the Shenqi-wan did not exerted protective effect, however its fractions suppressed NMDA-induced neuronal damage. The fraction elucidated at 11 - 20 min showed the most potent protective effect. These results revealed that effective substances of the Shenqi-wan against NMDA-induced excitotoxicity may exist mainly in the fraction elucidated at 11 - 20 min.

A Case Study of Oriental Medicine Treatment on Peripheral Facial Palsy (말초성 안면마비 환자의 한방치료 치험례)

  • Objective: Facial nerve palsy is caused by damage to the 7th cranial nerve. It is the main symptom of facial muscle paralysis on the affected side. Usually, recovery from this disease begins 2-3 weeks after onset and most patients recover in 4-8 weeks. If the patients cannot receive proper treatment, severe permanent impairments, both physical and mental, may remain, so this disease should be treated appropriately. In this study, a patient with facial nerve palsy was admitted to the Korean medicine hospital for treatment. We report on the patient's progress and the effects of treatment. Methods: We cured the patient with herbal medicines, acupuncture, herbal acupuncture therapy, and physical therapy. We used a numerical rating scale, the House Brackmann grading system, and a weighted regional grading system to assess symptom changes. Result: The patient with facial nerve palsy was hospitalized for 23 days and recovered from symptoms without significant problems on the face or in motor function.

The neuroprotective effects of Nokyongdaebo-tang(Lurongdabutang) treatment in pathological Alzheimer's disease model of neural tissues (Alzheimer's Disease 병태모델에서 녹용대보탕(鹿茸大補湯)의 신경세포 보호효과)

  • Objectives : Alzheimer's disease(AD) is the most common form of dementia, which is characterized by progressive deterioration of memory and higher cortical functions that ultimately results in total degradation of intellectual and mental activities. Nokyongdaebo-tang(Lurongdabutang) has been usually used for the treatment for the deficiency syndrome dementia and amnesia. This experiment was designed to investigate the effect of the Nokyongdaebo-tang(Lurongdabutang) hot water extract on pathological AD model. Methods : The effects of the Nokyongdaebo-tang(Lurongdabutang) hot water extract on cultured spinal cord cells induced by ${\beta}$-amyloid were investigated. The effects of the Nokyongdaebo-tan(Lurongdabutang) hot water extract on the memory deficit mice induced by scopolamine were investigated. Results : 1. ${\beta}$-amyloid treatment on cultured spinal cord cells increased both GFAP-staining intensity of astrocytes and caspase 3 immunoreactivity on cultured cells. Then, Nokyongdaebo-tang(Lurongdabutang) treatment reduced the labeling intensity for both GFAP and caspase 3 proteins in culture cells. 2. Scopolamine treatment into mice increased levels of GFAP-positive astrocytes and caspase 3-labeled cells of the hippocampal subfields dentate hilar region, CA3 and CA1 area. In vivo administration of Nokyongdaebo-tang(Lurongdabutang) attenuated labeling intensity for those two proteins in the same hippocampal areas. Similar effects were observed by the treatment of galanthamine, an inhibitor of acetylcholinesterase. Conclusions : This experiment shows that the Nokyongdaebo-tang(Lurongdabutang) may play a protective role in damaged neural tissues. Since neuronal damage seen in degenerative brains such as AD are largely unknown, the current data may provide possible insight into therapeutic strategies for AD treatments. Nokyongdaebo-tang(Lurongdabutang) might be effective for the prevention and treatment of AD.

  • Objectives : This study is designed to investigate the pathology, acupuncture and moxibustion therapy applicable of the dementia through literature of oriental medicine. Results : The findings of this study are as follows ; 1. The cause of dementia is internal damage of seven emotions(七情內傷), deficiency of the kidney essence(腎精不足), retention of dampness in the mental(痰迷心竅). 2. The acupuncture meridians used often for dementia are $Tok-maek(GV),\;Chok-t'aeyang\;Pang-gwang-Ky\check{o}ng(BL),\;Chok-yangm\check{o}yng\;Wi-Ky\check{o}ng(ST),\;Su-gwor\check{u}m\;Shimp'o-Ky\check{o}ng(PC),\;Su-so\check{u}m\;Shim-Ky\check{o}ng(HT)$. 3. The spots on the body suiTable for acupuncture used for therapy of the dementia are 98 points and extra points are 6 points, total therapeutic acupunture points are 104 points. And acupunture points are frequently used Shinmun(神門-HT7, 24), Paek'oe(百會-GV20, 19), Chok-samni(足三里-ST36, 18), Sugu(水溝-GV26, 17), P'ungnyung(豊隆-ST40, 16), Taech'u(大椎-GV14, 14), $Sam\check{u}mgyo(三陰交-SP6,\;14)$, Shimsu(心兪-BL15, 14), $Taen\check{u}ng(大陵-PC7,\;11)$, Nogung(勞宮-PC8, 11), Sosang(少商-LU11, 10), Kansa(間使-PC5, 10), Naegwan(內關-PC6, 10). 4. The acupoints used often on moxibustion are Paek'oe(百會-GV20), Chang-gang(長强-GV1), Taech'u(大椎-GV14), Kansa(間使-PC5). Conclusion : It is recommended that further study of many sided investigations in the future.

Clinical Outcomes of Diffuse Axonal Injury According to Radiological Grade

  • Purpose: Patients with diffuse axonal injury experience various disabilities and have a high cost of treatment. Recent researches have revealed the underlying mechanism and pathogenesis of diffuse axonal injury. This study aimed to investigate the correlation between the radiological grading of diffuse axonal injury and the clinical outcomes of patients. Methods: From January 2011 to December 2016, among 294 patients with traumatic brain injury, 44 patients underwent magnetic resonance imaging (MRI). A total of 18 patients were enrolled in this study except for other cerebral injuries, such as cerebral hemorrhage or hypoxic brain damage. Demographic data, clinical data, and radiological findings were retrospectively reviewed. The grading of diffuse axonal injury was analyzed based on patient's MRI findings. Results: For the most severe diffuse axonal injury patients, prolonged intensive care unit (ICU) stay (p=0.035), hospital stay (p=0.012), and prolonged mechanical ventilation (p=0.030) were observed. However, there was no significant difference in healthcare-associated infection rates between MRI grading (p=0.123). Massive transfusion, initial hemoglobin and lactate levels, and MRI gradings were found to be highly significant in predicting the duration of unconsciousness. Conclusions: This study showed that patients with high grade diffuse axonal injury have prolonged ICU stays and significantly longer hospital stays. Deteriorated mental patients with high energy injuries should be evaluated to identify diffuse axonal injuries by using an appropriate imaging tool, such as MRI. It will be important to predict the duration of consciousness recovery using MRI scans.
