A case of post-operative chylous ascites after a splenorenal shunt operation in a child with congenital hepatic fibrosis (선천성 간섬유화증에서 비-신장 문합수술 후에 발생한 소아의 유미성 복수증 1례)

  • Chylous ascites is a rare condition caused by various diseases and conditions that interfere with the abdominal or retroperitoneal lymphatics, and uncommonly it can manifest as a post-operative complication after abdominal, retroperitoneal or mediastinal surgery. Chylous ascites can be diagnosed by a high triglyceride content in ascites. The authors experienced a 5-year-old girl with congenital hepatic fibrosis who presented with chylous ascites after a splenorenal shunt operation, who was successfully managed by fasting and total parenteral nutrition, followed by a lipid-free diet with medium chain triglyceride supplementation. Here, the authors report this case of post-operative chylous ascites after a splenorenal shunt (Warren shunt) operation with a review of the pertinent literature.

Methanolic Extract of Turmeric (Curcuma longa L.) Enhanced the Lipolysis by Up-regulation of Lipase mRNA Expression in Differentiated 3T3-L1 Adipocytes

  • Effects of methanol extract from turmeric (Curcuma longa L.) (CME) on underlying mechanisms of lipolysis were investigated in 3T3-L1 adipocytes. Compared to the control, lipid accumulation with 72 hr treatment of CME at the concentration $20\;{\mu}g/mL$ was significantly decreased by 19.9% as quantified by Oil red O dye. Intracellular triglyceride (TG) content was also lowered by 19.3%. To determine the mechanism for TG content reduction, glycerol release level was measured. Incubation of 3T3-L1 adipocytes with 15 and $20\;{\mu}g/mL$ of CME significantly elevated the level of free glycerol released into the cultured medium by 20.4 and 28.6%, respectively. In subsequent measurements using quantitative real-time polymerase chain reaction (PCR), mRNA levels of hormone-sensitive lipase (HSL) and adipose triglyceride lipase (ATGL) were significantly increased by 21.2 and 24.9%, respectively, at the concentration $20\;{\mu}g/mL$. Results indicated that CME stimulated lipolysis through induction of HSL and ATGL mRNA expressions, resulting in increased glycerol release.

Energy Utilization of Growing Chicks in Various Nutritional Conditions

  • For the last two decades, energy utilization of growing chicks has been studied more and more. This paper focuses on the energy utilization estimated by the metabolizable energy (ME) values and the efficiency at which ME is used for growth of chicks under various nutritional environment. Degree of saturation of dietary fats is responsible for nitrogen-corrected apparent metabolizable energy (AMEn) of fats. The effect of dietary fat sources on heat production depends on the kind of unsaturated fatty acids as well as the degree of saturation. Medium chain triglyceride shows lower AME and net energy than long chain triglyceride. Phytase as feed additives increases the AME values of the diet along with improvement of the phosphorous utilization. Ostriches have higher ability to metabolize the energy of fiber-rich foodstuffs than fowls. Their higher ability seems to be associated with fermentation of fiber in the hindgut. Proportions of macronutrients in the diets have influenced not only the gain of body protein and energy but also the oxidative phosphorylation of the chicken liver. Essential amino acids deficiency reduces ME/GE (energy metabolizability) little, if any. Growing chicks respond to a deficiency of single essential amino acids with the reduction of energy retained as protein and increased energy retained as fat. Thus, energy retention is proportional to ME intake despite deficiency, and efficiency of ME utilization is not affected by deficiency of amino acids. Effect of oral administration of clenbuterol, a beta-adrenergic agonist, on the utilization of ME varies with the dose of the agents. Although the heat production related to eating behavior has been estimated less than 5% of ME, tube-feeding diets decreases HI by about 30%.

The Functional Behaviors of Cosurfactant in Design of Self-nanoemulsifying Drug Delivery Systems

  • Nanoemulsions have been widely investigated for many years because of their attractive and unique characteristics. Nanoemulsions are composed of oil, surfactant, co-surfactant and water. Especially, cosurfactant plays a critical role in formation of nanoemulsions. In pharmaceutical area, a pre-concentrate form of nanoemulsions which is known as self-nanoemulsifying drug delivery systems (SNEDDS) was available for some water-insoluble drugs. In this study, we investigated the functional behaviors of cosurfactant in design of SNEDDS and nanoemulsions. Cremophor RH 40$^{(R)}$, Propylene carbonate and medium chain triglyceride were selected for surfactant, cosurfactant and oil, respectively. Cyclosporine was employed as a drug. Phase diagrams showed the area of isotropic o/w region which forms o/w nanoemulsions was not significantly affected by the compositional ratio of cosurfactant. But, drug solubilization capacity, droplet size of nanoemulsions and drug release rate were greatly affected by the cosurfactant.

Effect of Medium and Long Chain Triglyceride Diet on the Serum Lipids of Rats Fed with Cholesterol (중쇄(中鎖)와 장쇄지방(長鎖脂肪) 식이(食餌)가 Cholesterol급여(給與) 흰쥐의 혈청지질(血淸脂質)에 미치는 영향(影響))

  • Because of its more complete hydrolysis and rapid absorption, MCT is expected its usefulness in the diets of patients with malabsorption syndrom. Also, several authors reported that serum cholesterol level was lower after MCT feedings. In this study, rate of each experimental group were fed for 4weeks with 20% MCT, 20% corn oil, mixed diet of 10% MCT and 10% corn oil, mixed diet of 17% corn oil and 3% shortening. After experimental diet, it was measured to growth rate and serum cholesterol, triglyceride and phospholipid level. The results were as follows; 1. The body weight gain was the lowest in MCT group and others were lower than control group. 2. Serum total cholesterol level was the lowest in corn oil group and others were significantly lower than control group. Free cholesterol level was lower in all experimental groups than control group too. 3. Serum HDL-cholesterol level was significantly higher in all experimental groups than control group. 4. The ratio of VLDL, LDL-Cholesterol to HDL-Cholesterol was significantly lower in all experimental groups than control group. 5. Serum triglyceride level was higher in all experimental groups than control group. Serum phospholipid level was significantly in only mixed diet group of corn oil and shortening than control group. 6. The ratio of total cholesterol to phospholipid was significantly lower in corn oil diet group than control group.

Self-Emulsifying Drug Delivery System Containing Ibuprofen for Oral Use (이부프로펜을 함유하는 경구용 자가유화 약물 송달시스템(SEDDS))

  • Self-Emulsifying System(SES), an isotropic mixture of oil and surfactant which forms oil-in-water emulsion, is expected to improve in vitro drug dissolution and enhance in vivo drug absorption. A poorly water soluble drug, ibu-profen(IBP) was incorporated into the SES to improve absorption, and enhance bioavailability of drug. Medium chain triglyceride, glyceryl tricaprylate(GTC) as an oil, and Tween 85 as a surfactant were used to formulate SES. To characterize SESs with various concentrations of Tween 85, the phase separation and solubility of IBP-SEDDS containing IBP as a function of Tween 85 concentration were conducted, and the particle size was measured using photon correlation spectroscopic method. The SES with optimal concentration of Tween 85(35%(w/w)) was selected based on its high drug loading, small particle size and low surfactant concentration. After an oral administration of IBP-SEDDS and IBP suspension in methyl cellulose equivalent to 40.0 mg/kg to rats, the pharmacokinetic parameters were compared. The $C_{max}(163.17\;vs\;88.82\;{\mu}g/ml)$, $AUC(12897.01\;vs\;8751.13\;{\mu}g\;min/ml)$ and Bioavailability(86.44 vs 58.65%) significantly increased but $T_max(10\;vs\;20\;min)$ was significantly advanced. The current SEDDS containing IBP provide an alternative to improve an oral bio-availability of IBP.

Effects of Fractions from Benincasa hispida on Inhibition of Adipogenesis in 3T3-L1 Preadipocytes (동과 분획물이 3T3-L1 지방세포 분화 억제에 미치는 영향)

  • The effects of three fractions, hexane (BHHH), chloroform (BHHC), and ethyl acetate (BHHE), from water extract of Benincasa hispida on the underlying mechanisms of adipogenesis were investigated in 3T3-L1 cells. Intracellular lipid droplets were stained with Oil Red O dye and quantified. Compared to control, lipid accumulation significantly decreased by 11% and 13% upon treatment with BHHC and BHHE, respectively at a concentration of 50 ${\mu}g/mL$. Intracellular triglyceride (TG) levels were also reduced by 21% and 16%, respectively, at the same concentration. To determine the mechanism behind the reductions in TG content and lipid accumulation, glycerol release and expression levels of adipogenic marker genes were measured. The levels of free glycerol released into culture medium increased by 13% and 17% upon treatment with BHHC and BHHE, respectively. In subsequent measurements using real-time polymerization chain reaction, the mRNA levels of $PPAR{\gamma}$, C/$EBP{\alpha}$, and leptin significantly decreased upon treatment with BHHE (45%, 67%, and 35%) in comparison with non-treated control. These results suggest that BHHE inhibits adipocyte differentiation by blocking $PPAR{\gamma}$, C/$EBP{\alpha}$, and leptin gene expression in 3T3-L1 cells, resulting in reduced lipid accumulation, increased glycerol release, and intracellular triglycerides.

Enhancing the Moisturizing Ability of the Skin Softener using Nanoemulsion Based on Phospholipid Liposome

  • In this paper, we present the improvement in low moisturizing ability and stability that existing skin softeners have due to the low oil content, by developing skin softener using nanoemulsion of phospholipid liposome, based on the properties of nanoemulsion in cosmetic formulation. In this study, two types of oil; dimethicone (DC 200/6cs) or medium chain triglyceride (MCT), and two kinds of lecithin; unsaturated or saturated were respectively applied to produce nanoemulsion. In the particle size analysis of nanoemulsion, the droplet size of nanoemulsion containing DC200/6cs and unsaturated lecithin was the smallest, and all nanoemulsion showed high stability in the measurement of zeta potential. Therefore, with the smallest particle size and high stability, moisture contents and trans epidermal water loss(TEWL) were measured using the skin softener of DC200/6cs and unsaturated lecithin contained nanoemulsion, and the measurement was compared with the non-oil skin softener and the skin softener with only small amount of oil. The results showed that the moisture content of the skin softener using nanoemulsion increased greatly than other two skin softeners, showing high hydration ability and water retention capacity, and TEWL decreased greatly, therefore preventing the evaporation of moisture from the skin. As a result, the oil content and stability of the skin softener was improved by utilizing nanoemulson based of phospholipid liposome, and it is expected to be used in various ways in cosmetic industry.

A study on the formation and Ostwald ripening stability of nanoemulsion with various emulsifiers (유화제 종류에 따른 nanoemulsion의 형성과 Ostwald ripening에 관한 연구)

  • This study aimed to investigate the effect of various emulsifiers on the formation of nanoemulsions and their stability properties. MCT (medium chain triglyceride) nanoemulsions were prepared (10 wt% oil, 10 wt% emulsifiers, 20 mM bis-tris, pH 7) with emulsifier such as Tween 20 (Polyoxyethylene(20) sorbitan monolaurate), Almax 3800 (Sorbitan monooleate), soy lecithin, and SSL (sodium stearoyl lactylate) and changes in fat globule size with respect to storage period and stability properties by Turbiscan were investigated. In case of control nanoemulsion with 10 wt% Tween 20, the initial fat globule size was 89.0 nm and 113.4 nm after 28 day of storage and this large increase (ca. 24 nm) was thought to be caused by Ostwald ripening. When Tween 20 was partially replaced with Almax 3800, lecithin and SSL in nanoemulsions, their physicochemical properties (i.e., fat globule size and stability) were changed accordingly. In general, the intial fat globule size was decreased with increasing the concentration of the emulsifiers and the stability against Ostwald ripening increased. The most stable nanoemulsions against Ostwald ripening could be prepared with emulsifiers of Tween 20 and Almax 3800 or lecithin in the ratio of 6:4 (wt%), which was verified with Ostwald ripening rate (${\omega}$). In addition, the emulsion stability by Turbiscan was observed to be consistent with results of changes in fat globule size with storage period.

Microemulsion Fomulation for Enhanced Topical Absorption of Root Extract of Angelica gigas (당귀 추출물의 피부 흡수 증가를 위한 마이크로에멀젼 조성)

  • Angelica gigas is one of the most widely used herbal medicines in Asia. Root extract of Angelica gigas is known to have anti-oxidant activity and skin whitening effect. The aim of this study was to prepare microemulsion system of root extracts of Angelica gigas for topical delivery. Microemulsion was successfully prepared by using MCT (medium chain triglyceride) as an oil phase, Labrasol as a surfactant, and the mixture of propyleneglycol and phosphatidylcholine (4 : 1) as a cosurfactant. In vitro and in vivo skin permeation and deposition of decursin, as a marker, was determined using hairless mouse. Microemulsion significantly increased the in vitro skin permeation of decursin for up to 12 hours and was significantly higher than the control (water). Moreover, microemulsion formulation showed significantly higher skin deposition of decursin compared to the control in both in vitro and in vivo studies. Thus, microemulsion could be a useful vehicle for topical application of root extracts of Angelica gigas.
