  • 저자들은 전신 부종을 주소로 내원한 6세 소아에서 장 조직 검사로 장 단백 소실을 유발하는 원발성 장 림프관 확장증을 확인하였고, 특별한 약물 치료 없이 medium chain triglyceride를 지방 대체 식이로 이용한 저지방, 고단백의 식이요법만으로 장 단백 소실의 호전을 보였고 증상 소실과 지속적인 성장 발달을 유지하였던 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

MCT를 경구투여(經口投與)한 흰쥐의 혈중지질(血中脂質) 및 효소활성(酵素活性)에 미치는 영향(影響) (The Effect of Oral Administered Medium Chain Triglyceride Diet on the Serum Lipids and Enzyme Activities of Rats)

  • The purpous of the study was to find an effect of oral administered medium chain triglyceride(MCT) diet on the serum and enzyme activities of Spraque-Dawly rats when they were oral fed with MCT, soybean oil and palm oil. The result was as follow. 1) The body weight gains in MCT diet group was lower than that of the all experimental group. 2) Total cholesterol levels in serum and liver of MCT diet group were very lower than that of the all experimental groups. 3) HDL-cholesterol level of MCT diet group was higher than that of the all experimental group except normal group and VLDL, LDL-cholesterol levels was very higher. 4) TG and PL levels of MCT diet group higher than that of the control group. 5) The activities of GOT with soybean oil diet group and of GPT with MCT diet group were lower than that of the all experimental group. 6) HDL-levels in the serum lipoprotein of MCT diet group was increased and LDL levels was decreased.

유지방 획분 첨가가 코코아 버터의 지방조성과 용융거동에 미치는 영향 (Effect of Addition of Fractionated Milk Fats on Fat Composition and Melting Behavior of Cocoa Butter)

  • 유지방 획분의 첨가가 코코아 버터의 지방조성 및 용융 특성에 미치는 영향을 검토하였다. 여과하여 유지부분인 무수 유지방을 얻었고 용융 결정법으로 유지방을 분획하여 고융점 획분, 중융점 획분1, 중융점 획분 2, 저융점 획분을 얻었고 이들의 융점은 각각 $33.3^{\circ}C,\;31.2^{\circ}C,\;40.6^{\circ}C,\;37.4^{\circ}C,\;33.5℃$$6.5^{\circ}C$이었다. 고급지방산$(C16{\sim}C18)$과 포화 지방산의 함량은 융점 감소 순서인 고융점 획분> 중융점 획분1> 중융점 획분2> 무수 유지방> 저융점 획분 순서로 적게 나타났다. 또한 저급지방산$(C4{\sim}C10)$ 및 중급지방산$(C12{\sim}C14)$은 중융점 획분2> 무수 유지방> 고융점 획분> 중융점 획분1> 저융점 획분 순서로 적게 나타났다. 유지방 획분을 코코아 버터에 첨가한 결과 고급 지방산 함량은 유지방 획분의 첨가량에 증가함에 따라 감소하는 경향을 보였다. 포화 지방산 함량은 저융점 획분을 첨가한 경우에만 감소하였다. 저융점의 유지방 획분일수록 중성지방 조성 중 고급 중성지방의 함량은 적게 나타났다. 유지방 획분이 코코아 버터에 첨가될 때 구성 중성지방 조성을 비교한 결과, 획분의 첨가량이 증가함에 따라 고급 중성지방의 함량은 감소되었다. 여러가지 온도에서 측정된 무수 유지방과 유지방의 획분의 고체지방의 함량은 고융점 획분 > 중융점 획분 1> 중융점 획분 2> 무수 유지방> 저융점 획분 순서인 융점감소의 순서로 감소하였고, 코코아 버터에 비해 매우 완만하게 감소하는 경향을 보였다. 유지방 획분을 코코아 버터에 첨가시켜 온도별로 고체지방의 함량을 측정한 결과 무수 유지방과 유지방 획분들의 첨가량이 증가할수록 각 측정 온도의 고체지방의 함량은 일정한 비율로 감소되는 경향을 보였다. 이러한 결과는 무수 유지방과 유지방 획분들이 코코아 버터에 혼합하여 사용 할수 있다는 것을 의미한다.

In vitro Anticancer Activity of Paclitaxel Incorporated in Low-melting Solid Lipid Nanoparticles

  • Triglyceride solid lipid with medium chain fatty acid, tricaprin (TC), was used as a core matrix of lipid nanoparticles (LN) to solubilize water-insoluble paclitaxel and enhance the stability of nanoparticles by immobilization of incorporated drug in the solid core during storage at low temperature. In the present study, TC-LN containing paclitaxel was prepared by hot melt homogenization method using TC as a core lipid and phospholipids as stabilizers. The particle size of TC-LN containing paclitaxel was less than 200 nm and its zeta potential was around -40 mV. Calorimetric analysis showed TC core could be solidified by freezing and thawing in the manufacturing process in which the hot dispersion should be prepared at elevated temperature and subsequently cooled to obtain solid lipid nanoparticles. The melting transition of TC core was observed at $27.5^{\circ}C$, which was lower than melting point of TC bulk. The particle size of TC-LN remained unchanged when kept at $4^{\circ}C$. Paclitaxel containing TC-LN showed comparable anticancer activity to the Cremophore ELbased paclitaxel formulation against human ovarian (OVCAR-3) and breast (MCF-7) cancer cell lines. Thus, lipid nanoparticles with medium chain solid lipid may have a potential as alternative delivery system for parenteral administration of paclitaxel.

Effect of Medium-chain Triglyceride (MCT) on Growth Performance, Nutrient Digestibility, Blood Characteristics in Weanling Pigs

  • One hundred and twenty weanling pigs in experiment 1 (Exp. 1) ($6.91{\pm}0.99kg$; 21 d of age) and Exp. 2 ($10.20{\pm}1.09kg$; 28 d of age) were used in two 42-d and 35-d experiments to evaluate the effect of medium-chain-triglyceride (MCT) on growth performance, apparent total tract digestibility (ATTD) of nutrients and blood profile. In both of Exp. 1 and Exp. 2, the same dietary treatments were utilized as follows : i) negative control (NC), ii) positive control (PC), NC+antibiotics (40 mg/kg Tiamulin, 110 mg/kg Tylosin, and 10 mg/kg Enramycin, iii) MCT3, NC+0.32% (phase 1, 2 and 3) MCT, and iv) MCT5, NC+0.55% (phase 1), 0.32% (phase 2 and 3) MCT. In Exp. 1, the pigs fed MCT5 diets had higher (p<0.05) ADG compared to NC treatment during the first 2 wk. From d 15 to 28, the ATTD of energy was improved (p<0.05) by MCT3 compared to the PC treatment. No effect has been observed on the blood profiles [red blood cell (RBC), white blood cell (WBC), immunoglobulin-G (IgG), lymphocyte concentration] measured in this study. In Exp. 2, the ADG were increased (p<0.05) by the MCT5 treatment than the PC treatment from d 0 to 14. Pigs fed PC treatment diet had lower ADFI (p<0.05) and better FCR (p<0.05) than NC treatment, whereas no differences were shown between MCT treatments and NC or PC treatment from d 15 to 35 and overall phase. The ATTD of DM and nitrogen were improved (p<0.05) by the effect of MCT5 related to the NC and PC treatment at the end of 2nd and 5th wk. The pigs fed MCT3 had higher (p<0.05) energy digestibility than PC treatment. No effects were seen in the blood profiles we measured (WBC, RBC, lymphocyte and immunoglobulin-G). In conclusion, the addition of MCT in the weanling pigs diet can improve the ADG and digestibility during the earlier period (first 2 wks), but had little effect on the blood characteristics.

Impact of Intravenous Omega-3-Enriched Lipid Emulsion on Liver Enzyme and Triglyceride Serum Levels of Children Undergoing Gastrointestinal Surgery

  • Purpose: To investigate the impact of omega-3-enriched lipid emulsion (LE) on liver enzyme (aspartate transaminase [AST] and alanine aminotransferase [ALT]) and triglyceride (TG) levels of children undergoing gastrointestinal surgery. Methods: This experimental randomized controlled group pretest-posttest design study included 14 children who underwent gastrointestinal surgery due to duodenal atresia, jejunal atresia, esophageal atresia, and need for parenteral nutrition for a minimum of 3 days at RSUD Dr. Soetomo Surabaya between August 2018 and January 2019. These children were divided into two groups, those who received standard intravenous LE (medium-chain triglyceride [MCT]/long-chain triglyceride [LCT]) and those who received intravenous omega-3-enriched LE. Differences in AST, ALT, and TG levels were measured before surgery and 3 days after the administration of parenteral nutrition. Results: Liver enzyme and TG levels in each group did not differ significantly before versus 3 days after surgery. However, TG levels were significantly lower in the omega-3-enriched intravenous LE group (p=0.041) at 3 days after surgery, and statistically significant difference in changes in TG levels was noted at 3 days after surgery between MCT/LCT intravenous LE group and the omega-3-enriched intravenous LE group (p=0.008). Conclusion: The intravenous omega-3-enriched LE had a better TG-lowering effect than the MCT/LCT intravenous LE in children undergoing gastrointestinal surgery.

  • Infant formula is classified into standard cow's milk-based and special formulas. This review aimed at summarizing the types of special milk formulas currently sold in Korea, and the appropriate indications for the use of these formulas; lactose free formula, soy-based formula, protein hydrolysate formula, amino acid-based formula, preterm formula, medium chain triglyceride formula, low-phosphorus formula, protein-energy-enriched formula, and formulas for inborn errors of metabolism.

Medium- and long-chain triglyceride propofol reduces the activity of acetyl-coenzyme A carboxylase in hepatic lipid metabolism in HepG2 and Huh7 cells

  • Medium- and long-chain triglyceride (MCT/LCT) propofol is widely used as an intravenous anesthetic, especially in the intensive care unit. The present study aimed to assess whether MCT/LCT propofol is safe in the hyperlipidemic population for long-term use. Free fatty acids (FFAs) were used to establish high-fat stimulation of HepG2 and Huh7 cells. Subsequently, these cells were treated with propofol at the concentration of 0, 4, or 8 ㎍/ml for 24 and 48 h. The results indicated that the cell viability was notably decreased when the cells were stimulated with 2 mmol/L FFAs and treated with 12 ㎍/ml MCT/LCT propofol. Accordingly, we chose 2 mmol/L FFAs along with 4 and 8 ㎍/ml MCT/LCT propofol for the subsequent experiments. Four and 8 ㎍/ml MCT/LCT propofol inhibited FFA-induced lipid accumulation in the cells and significantly reversed acetyl coenzyme A carboxylase (ACC) activity. In addition, MCT/LCT propofol not only significantly promoted the phosphorylation of AMPK and ACC, but also reversed the FFA-induced decreased phosphorylation of AMPK and ACC. In conclusion, MCT/LCT propofol reverses the negative effects caused by FFAs in HepG2 and Huh7 cells, indicating that MCT/LCT propofol might positively regulate lipid metabolism.

Application of Microencapsulated Isoflavone into Milk

  • This study was designed to develop a microencapsulated, water-soluble isoflavone for application into milk and to examine the hypocholesterolemic effect of such a milk product in a rat diet. The coating material was medium-chain triglyceride (MCT) and the core material was watersoluble isoflavone. The microencapsulation efficiency was 70.2% when the ratio (w/w) of coating material to core material was 15:1. The isoflavone release from the microcapsules was 8% after 3-day storage at $4^{\circ}C$. In in vitro study, 4.0-9.3% of water-soluble isoflavone in simulated gastric fluid was released in the pH range of 2 to 5 after 60 min incubation; however, in simulated intestinal fluid at pH 8, 87.6% of isoflavone was released from the capsules after 40 min incubation time. In sensory analysis, the scores of bitterness, astringency, and off-taste in the encapsulated isoflavone-added milk were slightly, but not significantly, different from those in uncapsulated, isoflavone-added milk. In blood analysis, total cholesterol was significantly decreased in the isoflavone-added group compared with that in the control after 6-week feeding. Therefore, this study confirmed the acceptability of MCT as a coating material in the microencapsulation of water-soluble isoflavone for application into milk, although a slight adverse effect was found in terms of sensory attributes. In addition, blood total cholesterol was lowered in rats which had been fed a cholesterol-reduced and microencapsulated, isoflavoneadded milk for 6 weeks.

오일 성분의 고형 자가 유화 시스템을 위한 규산칼슘 함유 분말의 제조 및 평가 (Fabrication and Evaluation of Powders Containing Calcium Silicate for Solid Self-emulsifying System of Oil)

  • The objective of this study is to assess the impact of spray drying conditions on medium-chain triglyceride (MCT) loading, solubility, and release of an MCT-loaded solid self-emulsifying system in a water-insoluble oily substance. MCT-loaded solid self-emulsifying systems are prepared by spray drying with SDS and calcium silicate. The effects of inlet temperature (60, 80, or 100℃) and feed solution composition (0, 10, 50, 90, or 100% ethanol) on physicochemical properties of MCT-loaded solid self-emulsifying systems are studied. The inlet temperature significantly affects the water solubility of MCT. Moreover, the feed solution composition significantly affects water solubility, release rate, and MCT loading. The MCT-loaded solid self-emulsifying system obtained at 60℃ using 90% ethanol feed solution shows the best physicochemical properties among the synthesized products and exhibits better water solubility (4.43 ± 0.44 vs. 0 ㎍/mL) and release (94.4 ± 1.6 vs. 32.8 ± 7.4%, 60 min) than a commercial product. Furthermore, the MCT-loaded solid self-emulsifying system shows an excellent emulsion droplet size (approximately 230 nm).
