Chuna Manual Therapy for Rheumatoid Arthritis : A Systematic Review and Meta-analysis (류마티스 관절염에 대한 추나요법의 효과 : 체계적 문헌고찰과 메타분석)

  • Objectives: To evaluate the evidence supporting the effectiveness of Chuna manual therapy for rheumatoid arthritis. Methods: We conducted a search across 9 electronic databases to find all randomized controlled clinical trials (RCTs) that used Chuna manual therapy as a treatment for rheumatoid arthritis. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results: Our inclusion criteria were met by 5 RCTs. The meta-analysis showed positive results for the use of Chuna manual therapy combined with oriental usual care (UC) in terms of the efficacy rate, pain, and duration of morning stiffness when compared to western UC. Positive results were also obtained in terms of the efficacy rate, when Chuna manual therapy combined with medication was compared to medication treatments alone. Conclusions: Our systematic review found encouraging, but limited evidence of Chuna manual therapy for rheumatoid arthritis. However, to obtain stronger evidence without the drawbacks of trial design and the quality of studies, we recommend a comparative research to test the effectiveness of Chuna manual therapy.

Review of Nursing Research on Psychotropic Drugs in Korea (정신약물 복용과 관련된 국내 간호연구의 고찰)

  • Purpose: This study critically reviewed nursing research psychotropic drugs that has been published in Korean journals. Another aim of this study was to identify trends in nursing research on psychotropic drugs and make suggestions for further study in Korea. Methods: Data were collected from degree theses and original articles on psychotropic drugs published in Korean journals from 1992 to 2013. Thirty-four articles were analyzed of which at least one nursing author participated in the study. Search keywords were "psychotropic drug" and "mentally ill patient & medication". Results: For the research design, quasi-experimental study was 58.8%, descriptive study was 17.7%, descriptive correlational study was 8.8%, qualitative study was 8.8% and model development research was 5.9%. Variables measured were knowledge of medication & symptom management, knowledge of disease, side effects, drug attitude, medication pattern, diet & activity, quality of life, and self-care. Conclusion: Despite recent increased interest in psychiatric medication, research on psychotropic drugs remains very limited, particularly regarding findings from a nurse's perspective. More research project should be designed to develop programs for the treatment of side effects from a nursing view-point.

Review of Experimental Studies on Estradiol Valerate-induced Polycystic Ovarian Syndrome in Herbal Medicine (Estradiol Valerate로 유발된 다낭성 난소 증후군의 한약치료에 대한 국내 실험연구 고찰)

  • Objectives: To observe the effects of Herb medication on Polycystic Ovary (PCO) in rats. Methods: Riss (Research Information Sharing Service) was searched for Oriental medicine about PCO on 2000~2014. So, Experimental studies using PCO model induced by Estradiol Valerate (EV) was selected to analyze how effects they were. Consequently, 10 studies published on 2000~2014 were selected to analyze about weight of model and both ovaries, the number aries, the number of follicle and corpus of follicle and corpus luteum, the level of serum Androstenedione (ADD) and total estrogen, the level of Nerve Growth Factor (NGF) and Corticotropin Releasing Factor (CRF), the crossover rate and the number of implantation. Results: Among them, there was significantly difference between treatment group and control group in weight of both ovaries, the number of follicle and corpora lutea, the crossover rate and the number of implantation. Conclusions: The effects of Herb medication was observed on making weight of ovaries with PCO to promote. The study about effect of Herb medication on NGF and CRF with PCOS had to be conducted. The further research have to focus on treatment of subfertility and infertility, that is field showing the advantage of Oriental medicine.

Medication Prescribing Status in Pediatric Patients with Migraine (소아청소년에서 편두통 치료 약물 처방 현황)

  • Background: Migraine is one of the leading causes of poor quality of life and disability, and migraine incidences in pediatrics are increasing. Proper medication is important for the preventive and acute treatment of migraine. This study aimed to identify the current status of prescribed medication in pediatric patients with migraine. Methods: We used data from a sample of pediatric patients from the Health Insurance Review and Assessment Service (HIRA-PPS-2018) and analyzed the status of prescription drugs and frequency of visits to medical institutions with migraine diagnoses in pediatric patients. Results: A total of 12,228 pediatric patients diagnosed with migraine during 2018 were analyzed. Among these patients, 7,170 (58.64%) were girls and 9,510 (77.77%) were adolescents. Additionally, 9,157 patients (74.89%) received acute treatment, and 592 patients (4.84%) received combination therapy with analgesics and triptans. Acetaminophen for acute treatment and flunarizine for preventive treatment were the most commonly prescribed. In most children and adolescents, acute treatment drugs were prescribed for less than 14 days. Conclusion: Analgesics, such as acetaminophen or non-steroidal anti-inflammatory drugs, were prescribed frequently for acute treatment in pediatric patients with migraine. The drug prescription duration was within the recommended range, indicating a low risk of overdose. For preventive treatment, clinically studied medication for pediatric patients with migraine was used.

Pilot Study of Effect to the Growth after the Administration of Herbal Medicine to the Prepuberty Children (한약 투여가 사춘기 전 소아의 신장성장에 미치는 영향에 대한 예비 연구)

  • Objectives : The aim of this study was to evaluate the effect to the height-growth after the administration of herbal medicine to the prepuberty children Methods : This clinical study has been carried out with 18 children, who visited to the Department of Pediatrics, Dong-eui oriental medical center from January 2005 to August 2008. They had been treated for height-growth with herbal medicine more than 6 months. This study has been carried out by chart-review to see the effectiveness. Height percentile, BMI and GV(Growth Velocity) after medication was compared with the ones before medication. Results : 1. This clinical study has been carried out with 18 children, 12 boys(66.7%) and six girls(23.3%). Their average age was 10.30 years old. 2. Generally total children's average height percentile was increased compared to after medication. Especially eight of them has significantly increasd height percentile(44.4%). 3. Total children's average GV/6months was also increased after medication. Especially ten of them has significantly increasd GV/6months (55.6%). 4. Total children's average BMI also increased (17.01 to 17.85) after medication. Conclusions : Herbal medicine affects to the growth of pre-puberty children.

Weighting of Acute Myocardial Infarction Quality Indicators using Delphi Method (델파이기법을 이용한 급성심근경색증 질 평가지표 가중치 부여)

  • Purpose: Health Insurance Review & Assessment Service (HIRA) launched an Acute Myocardial Infarction(AMI) assessment for the Payment For Performance(Quality Incentives) Pilot Project from July 2007. Assessment measures of AMI were composed of five process measures and one outcome measure, and each measure was incorporated into one composite quality score to Pay for Performance. Method: For calculation of composite quality score, we considered weighting for the measures using the Delphi method. The questionnaire was composed of three measure groups, 'Reperfusion rate'(Fibrolytic therapy received within 60 minutes of hospital arrival, Primary Percutaneous Coronary Intervention within 120 minutes of hospital arrival), 'Medication prescription rate'(Aspirin at arrival, Aspirin prescribed at discharge, Beta-blocker prescribed at discharge) and 'Survival Index'(30-day mortality rate). Result: A panel composed of 18 and completed a questionnaire by allocation of 10 scores to the three above mentioned measure groups. The Delphi was carried out until three rounds of surveys. In conclusion, each measure group was weighted differently and the 10 scores were allocated as 4.5 to 'Reperfusion rate', 2.5 to 'Medication prescription rate', and 3.0 to 'Survival Index'. Conclusion: The results of this study proposed the calculation method for weighting of Acute Myocardial Infarction quality indicators.

A Case Report of Helicobacter Pylori-Related Iron Deficiency Anemia in a Child; Review of Western Literature (Helicobacter pylori 감염과 관련된 철결핍성 빈혈 환아 증례보고)

  • Objectives The purpose of this study is to report a case of iron deficiency anemia in a child with repetitive bleeding in the upper gastrointestinal tract caused by Helicobacter pylori infection, and summarize review of a western literature. Methods A 13-year-old patient, who were suffering from iron deficiency anemia from repetitive bleeding in the upper gastrointestinal tract, was administered by Ikwiseungyang-tang gamibang (益胃升陽湯加味方) and Samchulgeonbitang (蔘出健脾湯). After several tests, we realized that an iron deficiency anemia was related to Helicobacter pylori. Results By herb medication, the patient's symptoms were alleviated. Conclusions Helicobacter pylori infection is one of the reasons of refractory iron-deficiency anemia which is unresponsiveness to oral iron therapy. We report the findings of herb medication along with summarization of a western literature.

Analgesic efficiency of dexketoprofen trometamol in third molar surgery: a systematic review and meta-analysis

  • This study aims to compare the analgesic efficacy of dexketoprofen trometamol (DT) with other analgesic drugs for pain relief after third molar surgery. The PubMed, Scopus, and Web of Science databases were searched to identify randomized controlled trials comparing DT with other analgesics for third molar surgery. The outcome measures were the sum of pain intensity differences (SPID), total pain relief (TOTPAR) at the 6th and 8th postoperative hours, time to rescue medication, and tolerability. In total, four studies met our inclusion criteria. A total of 660 third molar surgeries were performed: 365 in the DT group and 295 in the active control group. Compared to other analgesics, DT produced significantly better pain relief at the 6th postoperative hour: SPID (MD, 0.33; P = 0.01) and TOTPAR (MD, 0.41; P = 0.02). However, there were no statistically significant differences in the efficiency of pain relief at the 8th postoperative hour, time to rescue medication, or tolerability. Overall, a 25 mg dose produced the best results for pain relief. In conclusion, DT (25 mg) is a viable alternative to contemporary analgesics for pain relief after third molar surgery, particularly during the early postoperative period.

Safety and Effectiveness of Food Allergen Immunotherapy (Oral): A Systematic Literature Review and Meta-analysis

  • Purpose: Food Allergen Immunotherapy (Oral) is a form of immunotherapy administered to patients who are allergic to foods such as egg, milk, and peanut. The food allergen is orally administered to the patient in an escalating dose for desensitization or tolerance development. The safety and effectiveness of the therapy were assessed using a systematic literature review and meta-analysis. Methods: For a literature search, 8 national databases and a number of international databases including Ovid-MEDLINE, Ovid-EMBASE, and Cochrane Library were used; and 13 articles (all from international databases) were selected. The target of Food Allergen Immunotherapy (Oral) included patients with food allergy, and the intervention was food allergen immunotherapy without limiting the food type. The safety and effectiveness of Food Allergen Immunotherapy (Oral) were assessed by reviewing all the articles reporting on the therapy. The control group received standard therapies including aversion therapy, no treatment, anti-histamine treatment, and placebo. Safety was assessed through the incidence of complication and emergency medication. Effectiveness was assessed based on therapy success rate, symptomatic improvement, and quality of life. Results: Although Food Allergen Immunotherapy (Oral) was shown to have successful desensitization in patients with food allergy, the safety of the technique has not yet reached an acceptable level; the possible reason is due to the high rate of complication and frequency of emergency medication. Also, each study employed varying protocols while relying on a small number of participants and a short monitoring period. Conclusion: The results of assessment suggest that the level of evidence from current literature review is low and further research is necessitated for the verification of the safety and effectiveness of the therapy (Grade of Recommendation: A; Level of Technology: II-b).

Review On Concept and Clinical Application Of Yin-Deficiency·Blood-Deficiency in Donguibogam (『동의보감(東醫寶鑑)』의 음허(陰虛)·혈허(血虛) 개념 및 임상활용 고찰)

  • The aim of this study to distinguish Yin-Deficiency and Blood-Deficiency in Donguibogam To Investigate the Donguibogam Hu-Ro Sector "Yin-Deficiency Medication" and other parts and associating Herb(Bencao) In Donguibogam Hu-Ro Sector "Yin-Deficiency Medication", Yin-Deficiency is divided Blood-Deficiency and Em Hu Hwa Dong. But Si Wu Tang treating Blood-Deficiency be used at Em Hu Hwa Dong and Da Bu Yin Wan treating Em Hu Hwa Dong be used at Blood-Deficiency. But in the use of each Herb(Bencao), Yin-Deficiency Medicine and Blood-Deficiency Medicine are distinguised. One can understand that Donguibogam's use of prescription might not distinguish Yin-Deficiency and Blood-Deficiency, But each Herb(Bencao) distinguish those conceptions. Because each view is both meaningful, choosing of view in accordance with the purpose of treatment is necessary for successful clinical trial.
