DEA를 이용한 한국과 일본 공공병원의 인적자원 효율성 평가 (A Comparative Study on the Human Resource Efficiency between the Korean and Japanese Municipal Hospitals using Data Envelopment Analysis)

  • This study applied Data Envelopment Analysis to a set of Korean Public Corporation Medical Centers and Japanese Municipal Hospitals to compare their relative human resource efficiencies. Based on the data provided on the inputs and outputs, the analysis showed Japanese municipal hospitals were relatively efficient than Korean hospitals. The result of analysing BCC model shows 5 hospitals in Japan and 7 in Korea with an efficiency rating of less than 1 as considered relatively inefficient. For the inefficient hospitals the manner in which inefficient hospitals may be made efficient were indicated by the managerial strategies based on dual variables. A subsequent analysis of Wilcoxon rank-sum test revealed that the medical revenue per medical expense, labor cost per value added revenue were statistically significant between efficient and inefficient Korean hospitals and medical revenue per medical expense, labor cost per value added revenue, bed occupancy rate, average length of stay, rate of personnel expenses per medical revenue were statistically significant between efficient and inefficient Japanese hospitals.

지방공사의료원 규모의 효율성이 수입과 비용에 미치는 영향에 관한 연구 (The Study for Influence of the Efficiency Score of Public Corporation Medical Center on Revenue and Expenses)

  • This study has two different objectives. First of all is to comparing results of size efficiency scoring on Public Corporation Medical Center(PCMC) by years of 1993, 1997 and 2003 using Data Envelopment Analysis (DEA). The second is to explore the relationship between revenue and PCMCs' efficiency score, and the relationship between expenses and the efficiency score in 2003. The average efficiency scores were significantly decreased by years of 1993, 1997 and 2003. The revenue per bed(revenue) in 2003 was smaller than the expenses per bed(expenses) in 2003, therefore PCMCs had deficits in 2003. The expenses was negatively related to the efficiency score. Therefore its means was that improving efficiency score decreased expenses. Contrarily, the revenue had any significant relation to the efficiency score. PCMC needs to various endeavors to improve their productivity and efficiency. One of the alternatives is reduce of work load through integration of PCMC and development of new performance index reflecting their situation and future direction.

병원의 진료비 청구 자체심사 과정과 이의신청 사례 (Hospital's Internal Review Procedure of Health Insurance Reimbursement)

  • The main purpose of this study is to examine the overall procedure of hospital's internal review of health insurance reimbursement, to present the case of protest against reimbursement cut, and hence to provide some information on hospital's management of medical revenue. The object of the case study is 'P' university medical center, possessing 5 different hospitals under its system. Presentation of the case of protest against reimbursement cut has following meanings: Firstly, to the hospitals that already have internal review departments, information on the details of the protest process and results can be exchanged. Secondly, to the Government and National Health Insurance Corporation, useful data are provided for the improvement of the rules and procedures of health insurance reimbursement. Thirdly, to the hospitals without internal review departments, fundamental materials on the internal review process are provided for the effective management of medical revenue.

종합병원의 전문의 수가 경영성과에 미치는 영향 (The Causality between the Number of Medical Specialists and the Managerial Performance in General Hospitals)

  • This study examines the statistical relationship between medical specialists and managerial performance, using regression analysis with the number of medical specialists per 100 beds as the independent variable and the managerial performance index as the dependent variable. Managerial performance index incorporated the number of out-patients per specialist, the number of in-patients per specialist, the volume of revenue per specialist, the number of beds per specialist, and the average length of stay. To compare different groups of hospitals, dummy variable was applied to five groups of hospitals according to size: 100-299 beds, 300-599 beds, 600-899 beds, 900-1199 beds, and more than 1200 beds. The data consisted of 181 general hospitals with more than 100 beds, which included 28 public hospitals, 73 corporate hospitals, 64 university hospitals and 16 private hospitals. Of those, 87 hospitals were located in big cities and 94 hospitals in medium to small cities. This study used hospitals from the Korean Hospital Association, and data published in 2004. The collected data sample was analyzed using the SPSSWIN 12.0 version, and the study hypothesis was tested using regression analysis. The findings of this study are summarized as follows: Hypothesis 1 predicting a negative effect of the number of medical specialists on the number of out-patients per specialist was supported with statistical significance. The analysis of dummy variable showed causality in all the hospital groups larger than the group of 100-299 beds. Hypothesis 2 predicting a negative effect of the number of medical specialists on the number of in-patients per specialist was supported with statistical significance. The analysis of dummy variable showed causality in the hospital group of 300-599 beds when compared to the group of 100-299 beds. Hypothesis 3 predicting a negative effect of the number of medical specialists on the volume of revenue per specialist was not supported. However, the analysis of dummy variable showed that the volume of revenue per specialist increased in the hospital groups of 600-899 beds, 900-1199 beds, and over 1200 beds, when compared to the group of 100-299 beds. Hypothesis 4 predicting a negative effect of the number of medical specialists on the average length of stay was supported with statistical significance. The analysis of dummy variable showed causality in the hospital group of 300-599 beds, when compared to the group of 100-299 beds. Results of this study show that the number of the medical specialists per 100 beds is an important factor in hospital managerial performance. Most hospitals have tried to retain as many medical specialists as possible to keep the number of patients stable, to ensure adequate revenue, and to maintain efficient managerial performance. Especially, the big hospitals with greater number of beds and medical specialists have shown greater revenue per medical specialist despite the smaller number of patients per medical specialist. Findings of this study explains why hospitals in Korea are getting bigger.

한의과대학 부속 한방병원의 재무비율 분석 -본원과 분원의 비교를 중심으로- (Financial Ratio Analysis of Oriental Medicine Hospital affiliated with Universities)

  • This study was conducted to analyze if there is a difference between the head hospital and branch hospital by comparing the profitability and operating expenses to patient revenue of oriental medicine hospitals affiliated with universities in order to find whether opening branch hospitals is an appropriate method to increase profitability. Profit indices used for the comparison of head hospital and branch hospital include ratio of operating profit on medical revenue, net-income on medical revenue, net profit to total assets, and operating profit to total assets; and cost indices included ratio of labor costs, material costs and administrative costs. In comparison of profit indices of head hospitals and branch hospitals, head hospitals displayed negative(-) in all four profit index averages while branch hospitals displayed positive(+), showing that branch hospitals have higher profitability. In particular, in the case of head hospitals, ratio of net profit to total assets was -13.6%, while that of branch hospitals was 12.9%, which was higher than 3.1%, the average of Korean oriental medicine hospitals in 2011. As a result of difference analysis between groups of head hospitals and branch hospitals, profit indices of ratio of operating profit on medical revenue, net-income on medical revenue, and ratio of net profit to total assets were found to vary by hospitals, but there was no statistically significant difference between head hospitals and branch hospitals(p<0.1). Only the ratio of operating profit to total assets of head hospitals and branch hospitals indicated significant difference between the two groups, showing that ratio of operating profit to total assets of branch hospitals is larger than that of head hospitals. Meanwhile, the cost indices of ratio of labor costs, material costs and administrative costs in the difference test results did not show significant difference between the head hospital and branch hospital(p<0.1). Thus, it cannot be said that a certain oriental medicine hospital's profitability is high or low depending on whether it is head hospital or a branch as profitability varies depending on the management environment of the hospital. Therefore, oriental medicine hospitals affiliated with universities would need to make efforts to increase their profitability as an individual hospital rather than focusing on whether they are head hospital or a branch.

지방공사의료원의 인적자원 효율성평가 (A Study of Human Resource Efficiency in Public Corporation Medical Centers)

  • This study applied Data Envelopment Analysis(DEA) and Ratio Analysis and Regression Analysis to a set of Korean Public Corporation Medical Centers to evaluate their relative human resource efficiencies. The output measure used in this study was based on health insurance system which was used in both in-patient departments and out-patient departments. Inputs included working time of the doctors, nurses, technicians, and managerial department staff. Based on the data provided on the inputs and outputs, the analysis showed 23 of the 34 hospitals to be relatively inefficient. Each hospital with an efficiency rating of less than 1 was considered relatively inefficient. In addition, managerial strategies based on dual variables were constructed to indicate the manner In which inefficient hospitals may be made efficient. A subsequent analysis of t-test revealed that the bed occupancy rate, medical revenue per 100beds, value added revenue per staff, medical revenue per staff were statistically significant. The results of this study suggest the DEA is a promising tool for evaluating relative human resource efficiency in hospitals which have multiple inputs and outputs and where the efficient production function is not specifiable with any precision. But it is considered that efficiency evaluations may be most effective]y accomplished by Incorporating a combination of methodologies such as ratio analysis and regression analysis.

대학병원의 재무성과 비교 분석 (Comparative Analysis of Financial Performance in University Hospital)

  • Objectives: The purpose of this study is to compare analysis of financial performance in university hospitals. Methods: Data from 2005 to 2017 were collected from income statement, balance sheet, and annual reports in 23 university hospitals. The dependent variables are used financial performance, namely, medical profit to total assets, medical profit to medical revenue, and net profit to medical revenue. The independent variables are establishment type, hospital province, bed, open liquidity, stability, and activity. Results: From 2005 to 2007, university hospitals steadily increased medical revenues, nonmedical revenues, medical profit, net profit, and reserve fund for essential business by investing fixed assets using financial leverage. From 2015 to 2017, the debt ratio was minimized based on existing management performance. Results showed that university hospitals maintained high profitability by actively investing in medical equipment, medical environment, and facilities using reserve fund for essential business. Conclusions: Results suggest that this will be the basic data for efficient management of university hospitals.

종합병원의 직·간접보조금이 수익성에 미치는 영향 (Effect of Direct and Indirect Subsidies on Profitability in General Hospitals)

  • 본 연구는 우리나라 종합병원을 대상으로 직·간접보조금이 수익성에 미치는 영향을 살펴보고자 하였다. 자료는 의료기관 회계정보공시시스템을 이용하여 2016년부터 2018년까지 270개 종합병원 회계정보를 수집하였다. 분석지표는 수익성으로 사업수익에 대한 순이익, 보조금지표는 사업수익에 대한 보조금(시설투자 목적의 국고보조금, 고유목적사업 관련 비용, 연구수익, 기부금수익) 비율을 사용하였다. 연구결과, 직접보조금인 국고보조금 비율은 공립기관이 57.30%로 매우 높았다. 간접보조금으로 세금감면 효과인 고유목적사업 관련 비용 비율은 사립대학교병원이 6.69%로 가장 높았으며, 결손 또는 운영보조 목적인 연구 수익 비율은 국립대학교병원이 2.8%로 가장 높았고 기부금수익 비율은 공립기관이 36.4%로 가장 높았다. 보조금이 수익성에 미치는 영향을 살펴본 결과, 국립대학교병원은 사업수익 대비 고유목적사업 관련 비용의 비율과 연구수익 비율이 낮을수록 사업수익 순이익율이 높았으며, 의료법인과 재단법인은 사업수익 대비 고유목적사업 관련 비용 비율이 높을수록 사업수익 순이익율이 높았다는 것을 알 수 있었다. 이러한 결과는 고유목적사업과 관련된 자금의 활용에 따라 수익성이 변동될 수 있다는 것을 의미한다.

인공수정체 보험급여 전.후 진료양상의 변화 (Changes in Medical Practice Pattern before and after Covering Intraocular Lens in the Health Insurance)

  • This study is to find out changes in medical practice at a university hospital before and after covering intraocular lens (IOL) from the health insurance benefit. The coverage started on March 1, 1993 and a total of 596 cases who were discharged from July 1 to December 31, 1992 and 580 cases who were discharged from July 1 to December 31, 1993 were analyzed. Since the standard reimbursement scheme was changed from March 1, 1993, the charges for 1992 were transformed into 1993 scheme. Major findings are as follows: Average length of stay was statistically significantly decreased from 8.24 days in 1992 to 6.86 days in 1993. Charges except IOL has been statistically significantly decreased from 501,000 Won in 1992 to 444,000 Won in 1993. Charges for drugs and injection have been reduced. However, charge per day for them was not much different. This is due to decrease in length of stay. Charges for laboratory tests and radiologic examination were quite the same. Charges which are not covered by the insurance remained the same. The revenue of the hospital was reduced as expected. However, the hospital reduced the length of stay and increase the turnover rate In order to compensate the potential loss of revenue due to the difference of reimbursement between the out-of-pocket expense and the insurance coverage. By introducing the IOL benefit in the insurance, the insured pays less, hospital generates more revenue through shortening the hospital stay, and the total medical care cost becomes less nationwidely.

지역사회 의료공급자의 지불보상체계상의 특징이 지역사회 주민의 의료이용에 미치는 영향: 미국사례분석 (The Effect of Payment Method of Community Medical Provider on Medical Care Use of Community Residents)

  • Due to the existence of asymmetry of information between doctor and patient, it has been believed that doctor might affect patient's decision making process of purchasing medical care. Based on this notion, doctor's reimbursement method has been suggested as an effective policy device of improving efficiency of patient's medical care use by way of its affecting doctor's practice pattern. By using the Community Tracking Study (CTS) household and physician data set, which includes not only various information on patient's medical care use, but doctor's practice arrangements and sources of practice revenue, this paper investigates the effect of community doctor's characteristics of reimbursement method on community patient's medical care use under the control of patient's socio-demographic characteristics and community doctor's practice type. In the process of estimating econometric model, the endogeneity problem of individual health insurance purchase was corrected by using 2818. And due to the existence of sample selection problem, Heckman's two-step estimation method was used for strengthen the robustness of estimation which was adversely affected by sample selection problem The empirical results show that as the average value of community doctor's portion of practice revenue determined by prospective method out of total revenue increases, the community patient's total out-of-pocket medical cost decreases. This results suggest, as doctor's practice revenues are mainly determined by prospective method, such as capitation, doctors would be more conscious about practice cost, which might affect doctor's practice pattern and by which his/her patient's use of medical care would decrease.
