Interactions among Measles Virus Hemagglutinin, Fusion Protein and Cell Receptor Signaling Lymphocyte Activation Molecule (SLAM) Indicating a New Fusion-trimer Model

  • For measles viruses, fusion on the cell membrane is an important initial step in the entry into the infected cells. The recent research indicated that hemagglutinin firstly leads the conformational changes in the fusion protein then co-mediates the membrane fusion. In the work, we use the co-immunoprecipitation and pull-down techniques to identify the interactions among fusion protein, hemagglutinin and signaling lymphocyte activation molecule (SLAM), which reveal that the three proteins can form a functional complex to mediate the SLAM-dependent fusion. Moreover, under the confocal microscope, fusion protein and hemagglutinin protein can show the cocapping mediated by the SLAM. So fusion protein not only is involved in the fusion but also might directly interact with the SLAM to be a new fusion-trimer model, which might account for the infection mechanism of measles virus.

Life and Studies of Mongsoo Lee Heon gil (몽수 이헌길(李獻吉)의 생애와 학술 계통)

  • Lee Heon gil (a.k.a. Mongsoo), who was the pioneer of measles treatment in the 18th century, is the 14th generation of the Deokcheongun branch of the Lee family from Jeonju, living from August 25, 1738 (14th year of King Yeongjo) to April 29, 1784 (8th year of King Jeongjo). Under his teacher Lee Cheol hwan, the eldest grandson of Lee Yik, he learned medicine and studied it deeply. His medical service was most remarkable when measles was prevalent in 1775 (51st year of King Yeongjo) at his age of 38. It seems that he wrote books on measles based on his experiences at that time. Lee Heon gil's academic background can be inferred from his relation with Lee Cheol hwan and Jeong Yak yong. Associating with Lee Cheol hwan, he established close relationships with many scholars including the Lee family from Yeoju, who were the descendents of Seongho Lee Yik, and through this fellowship he was influenced directly and indirectly by Seongho Lee Yik. In addition, he maintained close contact with Jeong Yak yong while treating him several times. As Jeong Yak yong was also in close academic association with scholars in the Seongho School and Lee Yik's pupils, he had a philosophical consensus with Lee Heon gil. These academic backgrounds show that the philosophical flow of the Seongho School from Seongho Lee Yik to Jeong Yak yong also reached Lee Heongil.

  • Ma Gwa Hwe Tong, or 麻疹會通 is a general medicinal classic on measles which was an attempt to cope with the situation in the later Joseon Dynasty, when measles was prevailing all throughout the country. It was written in 1798 by Jung Yak Yong, who was a famous realist and a distinguished scholar, and in one of the chapters called Oh Gyun Pyun shows his thoughts. What he is trying to do through his writings, Ma Gwa Hwe Tong, was not reforming the current medicinal studies but to organize the classics on measles that he had access to with a system in order. The progressive thoughts shown in his other book, Eui-Ryung or 醫零, should be considered as his own thoughts only, and it is an overstatement to consider the book of Ma Gwa Hwe Tong to be his tryout in such thoughts.

Epidemiologic and Clinical Comparisons of Three Measles Outbreaks in Korea(1989~2001) (국내에서 1989년부터 2001년 사이에 세 번 돌발 유행한 홍역의 역학 및 임상적 비교)

  • Purpose : We evaluated and compared clinical and laboratory characteristics of patients with measles in three epidemics(1989~90, 1993~94, and 2000~01) in Daejeon, Korea. Methods : Retrospective analyses were performed using medical records of 520 patients with measles at the Catholic University of Korea, Daejeon St. Mary's Hospital during three epidemics. We divided the subjects into three groups, i.e., those who admitted during 1989~1990(group I, 116 patients), those during 1993~1994(group II, 127 patients), and those during 2000~2001(group III, 277 patients). We compared clinical, demographic and laboratory characteristics among these 3 groups. Results : In age distribution, ratios of under 2 years of age in three groups were 61%, 58% and 57%, respectively with no statistical differences. However there are differences in age distribution above 2 years of age, as 21% in 2~5 years of group I, 28% in 6~9 years of group II, and 21% above 10 years of group III. No statistical differences were present between 3 groups in the male to female ratio, MMR vaccination rate in above 2 years of age, duration of fever, incidence of hepatitis. Hospitalization days(P=0.019) and rate of complications(P=0.012) were longer and higher in group I than in group III. Conclusion : In three epidemics, the second peak age group(except 0~1 year) of children above 2 years of age who had mostly received MMR vaccination showed a trend for increased age with increasing time(statistical difference). This result suggest that secondary vaccine failure may have a role in each epidemics. So, if measles outbreaks is happened in the future, we will have consideration in this aspect.

A Study on the status of Routine-Immunization in a Rural Area (한 농촌 면단위지역 영아의 예방접종실태에 관한 조사)

  • In order to find out the status of routine-immunization in a rural area, this study was performed, through analyzing the data which was obtained from the immunization register of infants who was born at Su-Dong myun in 1996 and 1997, managed by Su-dong Myun health subcenter. The results are as follows. 1. B.C.G immunization rate was the highest such as 52.2% within 1 month and next order such 34.8% at 2 month in 1996. In 1997, the highest such as 73.8%, almost all, within 1 month. 2. D.P.T immunization rate in 1996 showed, almost all, the highest at 3 month(79.4%) for 1st dose and at 5 month(78.4%) for 2nd dose. However, the rate for 3rd dose showed the highest at 7 month(51.4%), and next order at 8 month(35.1%) and at 6 month(13.5%). D.P.T immunization rate in 1997, similarly showed the highest at 3 month(81.8%) for 1st dose, at 5 month(71.2%) for 2nd dose and at 7 month(71.4%) for 3rd dose. 3. Hepatitis B immunization rate showed the highest at birth at once or within one week(87.0%) for 1st dose in 1996 and (94.7%) in 1997. The rate for 2nd dose showed the highest at 2 month(51.7%) in 1996 and (50.0%) in 1997, and next order at 1 month(44.8%) in 1996 and (34.4%) in 1997. The rate for 3rd dose showed the highest at 3 month(54.8%) in 1996 and 5 month(54.8%) in 1997, and next order at 5 month(25.8%) in 1996 and at 3 month(26.0%) in 1997. 4. Measles immunization rate was 76.1% in 1996. The rate(76.1%) by the kind of vaccine was the highest with measles-MMR(34.8%), and with MMR(32.6%) and next order with measles(8.7%). The rate by measles immunization time(month) was the highest such as 35.0% at 9 month and 10 month respectively and the rate by MMR was the highest at 16 month(35.5%), and 15 month(22.5%), 13 month (12.9%) and 14 month(12.9%) in next order.

Trend in Measles Seroprevalence in the Western Pacific Region: A Systematic Review

  • Despite improvements in vaccine coverage, a resurgence of measles has been reported, especially in the infant and adult populations in recent years. We conducted a systematic review of seroprevalence studies conducted in the Western Pacific Region (WPR) to provide insights into seropositivity trends in different countries. This systematic review aimed to collect data from all available measles seroprevalence studies to characterize the differences in population immunity against measles in different countries. We searched the online databases PubMed and Embase to identify: 1) observational studies that investigated seroprevalence in all age groups, and 2) results reported as antibody levels. The following variables were extracted from different study arms: paper identification (title, first author, publication year), inclusion and exclusion criteria, study site, age of subjects, number of subjects, country/area, population, methods, and seropositivity (%). The search yielded a total of 69 studies included in the review. Among the 1-6-year-old group, seropositivity remained relatively high, at 81-100% in China, 86-94% in Korea, and 77-91% in Australia. In adolescents aged 7-18-years old, seropositivity was relatively constant in China and Australia over time; however, a decreasing trend was noted in Korea in 2011 (66%), 2014 (69%), and 2014 (50%) in this age group. A similar downward trend was observed among Korean adults aged 19-39 years in 2011 (74%), 2019 (71%), and 2019 (64%). Children are likely to be protected by universal vaccination programs in WPR countries and regions. However, susceptible individuals with waned immunity may be present among the adult population.

The Characteristics of the 2000-2001 Measles Epidemic in the Seoul Metropolitan Area (2000-2001년 수도권 지역 홍역 유행의 양상)

  • Purpose : To assess the clinical characteristics of the 2000-2001 measles outbreak in the Seoul metropolitan area, Korea, the clinical data of measles inpatients were analyzed. Methods : Three hundred and five children diagnosed with measles by clinical manifestation from July, 2000 to February, 2001, in Seoul, Ilsan and Ansan City were grouped according to their age and investigated for clinical manifestations, vaccination history and measles-specific IgM/IgG antibody positivity. Results : Thirty eight point seven percent of the subjects were less than 12 months of age, 27.5 % were 12-47 months of age and 33.8% were 48 months of age or older. There was no significant sexual difference(male : female=1.2 : 1). This epidemic started in June, 2000 and the number of patients increased abruptly in October, peaked in December and finally decreased after February, 2001. It started from the older age group and moved to the younger. Sixty five point two percent had a history of more than 1-dose vaccination and 13.6% of the patients equal or more than 48 months of age had a history of 2-dose vaccination. Primary vaccine failure rate was 59.4%(107/180) and secondary vaccine failure rate was 3.9%(7/180) in 1 dose vaccinees. Sixty one point six percent showed more than one complication and 38.4% had no complication. The most common complication was pneumonia(31.8 %), followed by bronchitis(11.5%) and acute otitis media(4.6%). Vaccination and dose were not related significantly with the occurrence of complications. Conclusion : Compared with previous outbreaks in Korea, clinical features showed no specific change in the 2000-2001 measles epidemic. However, primary vaccine failure rate was so high that the second vaccination at four to six years of age must be emphasized in Korea.
