Ongoing Clinical Trials of Vaccines to Fight against COVID-19 Pandemic

  • Coronavirus disease 2019 (COVID-19) has developed as a pandemic, and it created an outrageous effect on the current healthcare and economic system throughout the globe. To date, there is no appropriate therapeutics or vaccines against the disease. The entire human race is eagerly waiting for the development of new therapeutics or vaccines against severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). Efforts are being taken to develop vaccines at a rapid rate for fighting against the ongoing pandemic situation. Amongst the various vaccines under consideration, some are either in the preclinical stage or in the clinical stages of development (phase-I, -II, and -III). Even, phase-III trials are being conducted for some repurposed vaccines like Bacillus Calmette-Guérin, polio vaccine, and measles-mumps-rubella. We have highlighted the ongoing clinical trial landscape of the COVID-19 as well as repurposed vaccines. An insight into the current status of the available antigenic epitopes for SARS-CoV-2 and different types of vaccine platforms of COVID-19 vaccines has been discussed. These vaccines are highlighted throughout the world by different news agencies. Moreover, ongoing clinical trials for repurposed vaccines for COVID-19 and critical factors associated with the development of COVID-19 vaccines have also been described.

Clinical features of vaccination-associated thrombocytopenic purpura in children (예방접종과 관련된 소아 혈소판 감소 자반병의 임상적 특성)

  • Purpose : Idiopathic thrombocytopenic purpura (ITP) is a relatively common hematological disease in children. It generally occurs after exposure to a common viral infection episode; however, it may occasionally follow immunization with measles, measles-mumps-rubella (MMR), hepatitis B (HBV), influenza, diphtheria-tetanus-pertussis (DTP), or chickenpox vaccines. In this study, the incidence, clinical characteristics, and treatment outcome of vaccination-associated ITP were investigated and compared with non-vaccination-associated ITP. Methods : The admission records of 105 pediatric ITP patients between 0-14 years of age admitted to Department of Pediatrics, Wonkwang University Hospital from January 1994 to July 2007 were retrospecitively reviewed. Patients were grouped into a vaccination-associated group and a non-vaccination-associated group according to vaccination history within the previous 1 month, and various clinical features between the two groups were statistically analyzed. Results : Thirteen patients (12%) had a preceding vaccination. Eight had received DTP vaccination, 2 had received hepatitis B, and 1 each had received influenza, MMR, and Japanese B encephalitis vaccination. However, none of the patients had a recurrent thrombocytopenia after subsequent vaccinations. In the vaccination-associated group, the age was significantly lower, anemia was more common, and the risk period with blood platelet count $<20{\times}10^9/L$ was significantly shorter than for the in non-vaccination-associated group. Also, wet purpura was less prominent and the remission within 1month was more frequently achieved in the vaccination-associated ITP group. Conclusion : Vaccination-associated ITP patients showed mild symptoms with a more benign and shorter lasting course than non-vaccination-associated ITP patients. Moreover, platelet count assessment at the time of the next immunization may not be necessary.

  • A 371 agricultural households from 26 different communities in South Korea was subjected on a study of food taboos in January of 1966. To the pregnant women, those to whom a high protein diet is particurally important, as many as 14 different kinds of foods, mostly portein rich foods, were avoided to eat. It is believed that if duck is eaten while pregnant her baby may walk like a duck in later life. Some mother have a strong aversion to the rabbit meat that her unborn baby must be a harelip. It is feared to eat chicken, shark or carp by the pregnant mother for her baby may get a gooseflesh appearance, or fish scale-like skin in later life. It is thought that if mother eats soup made of meat borns, especially chicken bones, a disfigured baby may be born. Some area informed that if mother eats crab meat her future baby will always bubble. To the child-bearing mothers 13 different kinds of foods were avoided to eat. Some believe that if raddish kimchi, soybean curd, squash are eaten while dilivery that mother may get dental decay or to lose all her teeth. Other think that highly spiced raddish kimchi cause delivery difficult. To the lactating mothers 7 different items of foods were not recommended to eat. It is a common belief that eating green vegetables, especially fresh lettuce, are restricted that her baby may stool greenish. It is said that eating ginsen-chicken soup, or ginsen tea during lactating reduces breast milk secretion. To the weaning babies 7 different kinds of foods were prohibited to fee. Eggs are not eaten because mothers think her babies will start to talk very late. Eight different items of foods in cases of gastro-intestinal diseases, 5 items for liver disease, 7 items for high blood pressure as well as for paralysis were respectively restricted. It is said that meats including pork, beef, and chicken are neither desirable for the patients of high blood pressure nor those of paralysis. To the measles children 10 varieties of foods were restricted. Especially soybean products and meats were not encouraged to use for avoiding asecond attack of measles. For the common cold 8 different kinds of foods were aversed and men think that eating of soup of undria delays a recovery. For the tuberculosis 4 kinds of foods were prohibited to eat. It is said that wine, red pepper and ginsen will stimulate lung bleeding. Many mothers had a strong aversion to fermented shrimp and fish in case of style. and 5 different items of foods were restricted. In case of menstration not so many foods were restricted as other cases, but meat soup is not eaten in this condition in some areas. Majority of food taboos in Korean villages are neither based on tribal nor religious factors. But no one knows how, since what ages, from where, these food taboos have been transmitted and spread over the country. This survey found a great variety of food taboos, aversions, traditional beliefs and prohibitions latent unknown reseasons, or non-scientific conceptions, or completely different ideas from the modern medical aspect, or somewhat fallacious and superstitious beliefs. For the vascular disease contrasting approach were found between modern the oritical therapy and popular remedy among the rural populations who largely depend on the eastern medication. Further scientific study on either side should be done to lead the patient proper way. Many restricted foods such as rabbit, duck, chicken and fish are best resources of protein rich foods which are available in the village. Emphasis should be laid upon breaking down fallacious and supersititious food taboos through the extended nutrition education activities in order to improve food habit and good eating pattern for healthier and stronger generations of Korea.

Up-to-date or Complete Immunization Coverage and Their Related Factors (영유아의 예방접종 및 그 관련요인)

  • Objectives: Understanding the predictor of immunization status in childhood is critical issue to improve National Immunization Program (NIP). The aims of this study were to verify the status of up-to-date or complete immunization coverage and to investigate its related factors. Methods: As of 2005, according to local residence registry data, there were 2,188 children who aged 12 to 35 months in Nonsan city, Korea. We conducted household survey for aged 12 to 35 months children, using questionnaires to obtain data on the status of immunization such as BCG, DTaP (diphtheria, tetanus, and pertussis), Polio, and MMR (mump, measles, rubella). Finally 1,472 participated in the survey. The operating definitions used in this study were following; "Complete immunization rate" refers to the rate of children who received all immunization within recommended age intervals fully "on-time"; "The 4:3:1 series" means status of receiving the fourth diphtheria-tetanus-pertussis (4 DTP), the third Polio (3 Polio), and the first measles-mumps-rubella (1 MMR) doses. Multivariate logistic regression analyses were used to determine factors affecting complete vaccination coverage of children. Results: Immunization rates of vaccine based on the vaccination card were from 92.7% to 96.4% except 4th DTaP (79.3%). Complete immunization rate of Korea NIP was 74.0% and that of the 4:3:1 series was 77.1%. A parent as primary caregiver (OR 0.59, 95% CI 0.39-0.87 at 19-35 months of children's age) and first-born children (OR 1.79, 95% CI 1.05-3.03 at 24-35 months of children's age) were significantly related to complete immunization coverage of Korea NIP. And a parent as primary care giver (OR 0.58, 95% CI 0.38-0.88 at 19-35 months of children's age) and first-born children (OR 1.94, 95% CI 1.21-3.14 at 19-35 months, OR 2.23, 95% CI 1.27-3.91 at 24-35 months of children's age) were significantly related to complete immunization rate of 4:3:1 series. Conclusions: Government should take actions to increase complete immunization rate. In particular, intervention on the secondary caregiver and non-first-born children should be needed.

A Study on the Lineage of Medicine in the Middle Period of the Chosun Dynasty (조선중기(朝鮮中期) 의학(醫學)의 계통(系統)에 관(關)한 연구(硏究))

  • The Korean Medicine in the middle period of the Chosun Dynasty, with its clear independence, has made an excellent display of originality and superiority in the world medicine. By pinpointing its merits, classifying its lineal descent, and examining its relation with present Korean Medicine we can draw the following conclusions. First, the study on the history of Korean Medicine should, in terms of time, grasp the generational stream and, in terms of space, examine the relation with medicines of neighbouring countries, with both an inner and outer way of observing. Secondly, it is but a manifestation of the colonial view of history to classify the 460 years from Tae-jo(太祖) to Cheol-jong(哲宗) as Mordern Period of Korean Medicine. Therefore, the 250-year period between the mid-16th century of Myung-jong(明宗) to the late-18th century of later Jeong-jo(正祖), which saw a pivotal development of the national medicine as a practical medicine based on experiences, should be established as the period of Korean Medicine in the mid-Chosun dynasty. It is required from both nationalistic view of history and history as a positive science. Thirdly, Korean Medicine in the mid-Chosun period was developed with an emphasis on an independent development and creative succession of the national medicine, which was succeeded to the late-Chosun period. Fourthly, medical thought in the mid-Chosun period was much influenced by, and based on the practicality of, Shil-hak(or practical science : 實學) as was the current of the times. Fifthly, though medicine in the mid-Chosun period was generally developed, the recipes for the measles, epidemic and smallpox were especially developed owing to rage of epidemics, and the development of the acupunture as a military medicine was the most c lear because of the two major wars against the Japanese and Chinese, respectively in 1592 and 1636. Sixthly, Whang doh-yeon(黃道淵)'s Bangyak-happyeon(方藥合編), Lee je-mah(李濟馬)'s Tongui-susebawon(東醫壽世保元) and Lee kyu-jun(李奎晙)'s Uigam-jungma(醫鑑重磨) are representative medical books, in the late-Chosun period, which creatively succeeded national medicine. Seventhly, the lineage of national medicine flows from Hyangyak-jipseongbang(鄕樂集成方) Uibangryuchui(醫方類聚) Gugeupbang(救急方) to Uirimchualyo(醫林撮要) Tonguibogam(東醫寶鑑) Uimunbogam(醫門寶鑑) Jejung-sinpyeon(濟衆新編) and to Bangyak-happyeon(方藥合編) Tongui-susebawon(東醫壽世保元) Uigam-jungma(醫鑑重磨). These books are representative medical books in the early middle and late Chosun period respectively. On the basis of the above facts, it is clear that the orthodoxy of national medicine is in the direction of a creative succession and that is the best way to keep the spirit of Korean medicine today.

Recent Mortality Trends in Korea (최근(最近) 한국인(韓國人)의 사망력(死亡力) 경향(傾向)에 관(關)한 고찰(考察))

  • A review has been made of mortality trends in Korea from 1958 to 1967 analyzing the data by sex, age and cause of death. The crude death rates and age specific death rates were estimated by the model of N. Keyfitz life table which had been developed by the data of the 1960's national census. The cause specific death rates shown in this article are based on the following: all deaths occurring in the death-registration are expressed as a numberator, while the denominator was estimated from the regular national census data by interpolation method. It is estimated that only an average of about 40% of deaths which occurred during a year were registered during 1958 to 1967. The validity and the reliability of the diagnosis of causes of death seem to be extremely poor in this country. Therefore the cause specific death rates in this article are aimed to reveal trends of causes of registered death ana not for the actual level of death rates. For 10 years very interesing mortality trends were observed : 1. The trend in the crude death rates was downward slowly. 2. The estimated death rate for the infant in 1960 was still high up to 100 per 1,000. 3. The rates for mortality attributed to such infectious diseases as pneumonia, bronchitis, gastroenteritis and measles decreased an average 40-60%. 4. The death rates for over-all tuberculosis decreased only 9.8%. 90% of the decrease was contributed by those in the less-than-15 year age group. 5. The death rates for chronic diseases, such as vascular diseases affecting the central nervous system, malignant neoplasm, major heart diseases and all accidents rose about 40-60%. 6. The rank order of the 10 leading causes of death showed large changes over the years, except for pneumonia and tuberculosis which occupyed 1st and 2nd places respectively. Vascular diseases affecting the central nervous system moved from 5th to 3rd place and malignant neoplasm from 6th to 4th place, The major heart diseases moved from 10th to 6th place and all accidents from 10th to 7th place. On tile other hand, gastroenteritis moved from 3rd to 5th place and influenja from 4th to 8th place.

The Necessity of Vaccine Education for Pharmacists and Development of a Vaccine Leaflet for Patient Counseling (약사의 백신에 대한 교육의 필요성 및 환자 상담을 위한 소책자 개발)

  • Vaccines are products for immunization which can provoke antibodies by eliciting immune reponses without causing disease and have played an important role in preventing fatal and contagious diseases as well as H1N1 influenza. They are classified by two following categories; lived attenuated vaccine and killed vaccine and currently commonly using vaccines are BCG, diphtheria, tetanus, mumps, measles, rubella, polio, Haemophilus influenza type b, hepatitis B, influenza etc. All vaccines must be used correctly to reach optimal therapeutic goals and also informed well to patients to decrease potential problems. In order to do, pharmacists must have good knowledge of vaccines. The purpose of this study is to evaluate the necessity of vaccine education for pharmacists and develop a vaccine leaflet for patient counseling. We have performed a survey with questionnaire for a total of 176 pharmacists and nurses(hospital pharmacists, n=65; community pharmacists, n=50; hospital nurses, n=61) from January 27th to March 12th, 2010. The questionnaire includes items about vaccine education and counseling and 12 quizzes to evaluate responders' knowledge of vaccines. We used the SPSS(Version 12. for windows) program to analyze the data. In results, 94.9% of all responders said they had not been educated on vaccines. And only 1.1% of all responders said they know about vaccines enough to counsel patients. Pharmacists who have an experience recommending vaccines to other people are 21.7%. On the other hand, nurses who have an experience recommending vaccines to other people are 55.7%(p=0.000). The mean number of correct answers at the 12 quizzes are followings; hospital pharmacist, 8.1; community pharmacist, 6.1, hospital nurses, 6.2(p=0.000). A vaccine leaflet for patient counseling is developed with several references. In conclusion, due to no opportunity of vaccine education, pharmacists have no confidence to counsel patients and lack of knowledge of vaccine. But importance of vaccine's role is increasing, pharmacists should counsel patients in vaccination. So they need vaccine education and a vaccine leaflet will be helpful for their counseling.

The Medical Study on the Development of Pediatrics in Myeong(明) Dynasty (명대(明代)의 소아과학(小兒科學)에 관(關)한 연구(硏究))

  • Myeong(明) Dynasty Period put together clinically various medical theory in Song(宋) Dynasty and Geum-Won(金元) age, organized medical system. They have been developed in theory, which thought of Byeonjeungronchi(辨證論治) more important, and clinical part which included diagnosis, therapy, prevention. In that age reactional medical thought was in fashion because of affect of Ihak(理學), therefore pediatrics have been affected from them. Especially, looking at the symptom of Byeonjeungronchi doctors, Manjeon(萬全), Nobaeksa(魯伯嗣), Wangdaeryun(王大綸), Wanggeungdang(王肯堂), Nuyeong(樓英) had succeed to Jeoneul(錢乙)' the five viscera Byeonjeung(辨證) system. Manjeon advocated Samyuyeosabujokron(三有餘四不足論) about Jangbu(臟腑) and organized the curative principle about Ojangheosil(五臟虛實). Gupyeong(寇平), commented on diagnosis of five viscera and classification of disease of five viscera. Wangdaeryun in the close of Myeong Dynasty Age explained main pulse, pulse of illness in detail according to weakness or strength of five viscera, pathological or physiological features of five viscera and Saenggeuk(生克) relation of Ohaeng(五行) in the book of "Yeongdongryuchwe(嬰童類萃)". Wanggeungdang and Nuyeong had organized system of argument which classified disease as a result of symptom of five viscera. "Yugwajeungchijunseung(幼科證治準繩)" and "Uihakgangmok(醫學綱目) Soabu(小兒部)" had been written by this principle. Nobaeksa had arranged the principle of cure about five viscera and explained method of common use about each organ's disease. Besides, Seolgi(薛己), Janggyeong-ak(張景岳), insisted about Myeongmun(命門) because he thought of Bisin(脾腎) of children and vigor by nature importance. Seolgi had applied and used very well Bojung-ikgitang(補中益氣湯) based on Idongwon(李東垣)'s Biwiseol(脾胃說) and controled and helped spleen and stomach. At the same time, he took a serious view about supplementing children's Sin-gi(腎氣) according to so many spleen and stomach disease was fallen because they couldn't make warm the spirit of Jungju(中州), result of weakening Hwa(火) of Myeongmun. Also Janggyeong-ak took a serious view strengthen of Bisin, so he assorted and used Insam(人蔘) and Buja(附子) to supplement children's weaken energy in kidney Jeonggi(精氣).

Common Disease Codes in Pediatric Inpatients (1997-2008) (소아청소년과 입원환자의 흔한 질병 코드들(1997-2008): 감염질환 중심의 단일기관 연구)

  • Purpose : In this study, we listed common diseases in pediatric inpatients and evaluated the distribution of diseases by period and age group, in order to estimate the epidemiologic trend. Methods : Patients who were admitted to the Department of Pediatrics between 1997 and 2008 were included. Demographic characteristics, date of admission, and International Classification of Diseases (ICD) code of patients were indentified. Study period was divided into two; early (1997-2002) and late (2003-2008), and age of patients were grouped into four; infancy, early childhood, late childhood, and adolescence. Results : A total of 33,513 patients were admitted for 12 years. In the list of ICD code, Pneumonia (J12-J18; 21.2%) was the most prevalent, followed by gastroenteritis (A00-A09; 17.8%), bronchiolitis (J21; 11.9%), and so on. Common diseases ranked from 1 to 10 comprised the majority (79.1%) of all the inpatients. There was increase in the number of inpatients with respiratory infectious disease (bronchiolitis, otitis media, and sinusitis), enlarged lymph node, or impetigo/cellulitis, but decrease in the number of inpatients with aseptic meningitis, intussusceptions, measles, or nephritic/nephrotic syndrome. The distribution of diseases also showed age group-specific difference. Conclusion : The distribution of diseases by period and age group was different. The epidemiologic trend should be considered in developing the management of strategy for the Department of Pediatrics.

A Study on Knowledge, Attitudes, and Practice of Health Care of Housewives in Rural Area (with Established Viliage Voluntary Health Worker System) (일부(一部) 농촌지역(農村地域) 주부(主婦)의 보건의료(保健醫療)에 대한 지식(知識).태도(態度) 및 실천도(實踐度)에 관한 조사(調査) -마을보건임원조직(保健任員組織) 활용지역(活用地域) 중심(中心)-)

  • In order to determine the knowledge of, attitudes to, and practice of housewives toward health care in a rural area, a survey with questionnaire was carried out with 87 housewives who were sampled randomly from 6 villages in Sudong Myun, from April 16th to 21st, 1979. The following results were obtained. 1. Of the housewives studied, 61.5% knew that B.C.G. is a vaccine for T.B prevention and 12.3% knew that D.P.T. is a vaccine for diphtheria, pertussis, and tetanus. 2. The vaccination rate of the children under six-year of the housewives studied was: polio 83.1%, B.C.G. 75.4%, D.P.T. 66.2%, and measles 55.4% respectively. 3. The vaccination rate was higher in children in the area near from the health subcenter than in there of the area further away. 4. Out of 87 respondants, 87.5% knew one or more methods of contraception for spacing children. These were: loop 69.0%, oral pill 66.7% and condom 14.9% respectively. 5. Out of 87 respondants, 82.2% knew the methods of contraception for sterilization. These were: laparascopy 87.5% and vasectomy 16.9%. 6. Out of 87 respondants those who had experience using contraceptive methods were 70.1% and present users were 47.1%. 7. Contraception practice rate was higher in the group of housewives having middle school education or above than those having primary school education or less. 8. Functions of the health subcenter listed by respondants were: patients care 72.4%, family planning 31.0%, vaccination 23.0%, T.B. control 3.4%, health education 3.4%, infant birth delivery assistance 1.1% respectively. 9. Housewives who knew that there is a village health voluntary worker in their own village were 63.2%(55), and 58.2% of those who knew appreciated her activities. 10. Purposes of expenditure of Myun community health development funds listed by respondants were: aid for patient care 34.5%, aid for health subcenter operation 16.1%, and aid for Myun health development 6.9% respectively. 11. It seems that both of the distance from the health subcenter and the utility rate level of the village health voluntary worker are co-related to the B.C.G. vaccination rate of children. 12. It seems that both of the distance from the health subcenter and the utility rate level of the village health voluntary worker are not co-related to the rate of contraception practice.
