Purpose: The purpose of this study was to investigate the effects of muscle activity and muscle strength according to verbal command volume during isokinetic and isometric quadricep exercises. Methods: To measure muscle activity and muscle strength, surface electrodes were attached to the participants, as they sat on a Biodex chair. The isometric exercise was performed three times, with maximum exercise at $30^{\circ}$ bending angle, based on a maximum extension state of the knee at $0^{\circ}$. The average holding time was unified to three seconds. In addition, the isokinetic exercise was performed three times, at $60^{\circ}/sec$. The verbal command ranged between 0∾60 dB and 0∾75 dB. Muscle activity was measured using surface electromyography (4D-MT, Relive, Gimhae, Korea). The Biodex System 4 was used to measure the isometric and isokinetic strength of the nodal line, and 4D-MT was used to measure muscle activity. Results: There were significant improvements in the maximal and relative muscle strengths, when the 0∾ 60 dB and 0∾75 dB verbal commands were applied with isokinetic extension/flexion (p<.05). The isokinetic exercise (0∾75 dB) group showed a significant difference in the vastus medialis oblique muscle activity change (p<.05), while the isometric exercise (0∾75 dB) group showed a significant difference in the rectus femoris muscle activity change (p<.05). Conclusions: Our results reveal that verbal commands effectively improve muscle activity and muscle strength during isokinetic and isometric quadricep exercises.
The purpose of this study were to compare the differences of isometric lumber extension strength and subjective pain degrees between obesity patients group and normal body fat group in disc surgical operated patients. The research purposed to indicate how isometric lumbar extension exercise for 12 weeks affected to lumbar strength and visual analogue scale of patients suffered by chronic back pain. The subjects were 65 low back pain patients(male 30, female 35)who had disease on lumbar in W hospital. The lumbar extension strength was measured at seven degrees of angles, which were $0^{\circ}$$12^{\circ}$$24^{\circ}$$36^{\circ}$$48^{\circ}$$60^{\circ}$ and $72^{\circ}$ before and after the exercise program. We got the results of subjective pain degree using the modified visual analogue scale(VAS) of Lawlis et al(1989) and measured the maximal isometric lumbar strength of all subjects using MedX lumbar extension machine. Results were as follows; After the exercise, the lumbar extension strength of normal body fat patients groups included males and female were greater than that of the obesity patients groups in all angles(p<.05). The visual analogue scale of chronic back pain patients was decreased significantly after the exercise(p<.05). The results showed the significance between the lumbar extension strength and the visual analogue scale of chronic back pain patients and showed that the isometric lumbar extension exercise decreased the subjective pain degrees of visual analogue scale with and increased lumbar extension strength. The correlation between the visual analogue scale and the %body fat of chronic back pain patients was no significant after exercise. Therefore, the lumbar extension strength exercise is needed for improvement of back strength, decrease of %body fat.
The purpose of this study was to determine the effect of muscle fatigue by neuromuscular electrical stimulation(NMES). Using Biodex System 3PRO(Biodex Medical Systems Inc, USA), experiment was conducted as to the normal group(I) composed of fifteen adults and the patient group(II) composed of fifteen patients with spastic hemiplegia. As to each group, maximal tolerated intensity(MTI) and maximal tolerated isometric contraction(MTIC) in electric currents yielded by low rate(20 pps) and high rate(100 pps) neuromuscular electrical stimulation and the aspects of decrease and restoration of the isometric contraction were examined, and their strength decrement index(SDI) and strength recovery index(SRI) were also calculated. 1. As for MTI in NMES, the MTI of the group II was higher than that of the group I in both low rate and high rate NMES. In comparison within group, MTI of group II was significantly higher in high rate NMES rather than in low rate NMES(p<0.05). 2. In comparison of MTIC between groups, the group I showed higher in both low rate and high rate NMES. In comparison within group, MTIC of group II was significantly higher in high rate NMES rather than in low rate NMES(p<0.01). 3. As for SDI, both groups showed highest SDI in high rate NMES, but no significant differences could be observed. 4. As for SRI, both groups showed significantly low SRI in low rate NMES(p<0.01, p<0.05), and comparison between groups showed no significant differences could be observed. These result lead us to the conclusion that muscle fatigue was influenced by frequency, high rate NMES was lower at SDI and higher at SRI on compare to low rate NMES, therefor, a further studies concerning electrical stimulation should consider differences each frequency in response to treatment.
Purpose: The purpose of this study was to investigate the differences in body composition, upper and lower limb muscle strength, and functional physical ability in urban-dwelling elderly women with or without obesity. Methods: All study participants were assigned to the normal weight group (n=8, BMI<25) and the obesity group (n=7, BMI>25) based on their obesity rate. Anthropometric measurement was conducted and body composition was measured. For the upper and lower limb strength, grip strength and maximal isometric knee extension and flexion were evaluated by a dynamometer. The senior fitness test was performed to measure functional ability. Data analysis was conducted by the independent t-test and the alpha level was set at 0.05. Results: The waist, hips, and thighs of obese elderly women were thicker than those of normal-weight elderly women. This physical difference resulted from body fat mass, not muscle mass. Despite a similar level of limb muscle mass between the two groups, the upper limb grip strength was higher (24.00% for left, 19.95% for right) in the normal-weight women than the obese women (p<0.05), but otherwise there was no difference in maximal knee flexion or extension isometric strength. Functional physical ability showed no difference in a 30-second chair sit and stand test and a six-minute walk test, but a 30-second arm-curl (11.00% for left, 14.81% for right), back stretch (8.54cm for left, 8.99cm for right), chair sit and reach (9.22cm for left, 6.24cm for right), and 2.44 meter round trip walk (0.62 sec, 9.39%) were faster in performance for normal-weight elderly women than obese elderly women (p<0.05). Conclusion: Taken together, despite similar levels of upper and lower extremity muscle mass, normal-weight elderly women showed higher performance in upper limb strength, flexibility, and agility than obese elderly women, but there was no difference in lower extremity functional muscle strength and cardiopulmonary endurance.
Objective: Physiologically, the vastus medialis muscle is the first muscle to undergo muscle atrophy, and it was thought that pain in patients with knee osteoarthritis could be reduced if this muscle could be strengthened and stabilized. The purpose of this study was to prove the effectiveness in knee osteoarthritis using polydioxanone sutures that have been tried in other musculoskeletal areas. Method: Forty knee osteoarthritis patients voluntarily participated in the study, and divided into 30 polydioxanone suture needle (MEST-B2375 produced by Ovmedi Co.) and 10 sham needle (without suture). And the needles were inserted into the vastus medialis muscle. In all patients, safety evaluation including blood tests and ultrasonography as well as efficacy evaluation including isometric maximal contractile strength of quadriceps muscle, weight bearing pain, impression of change, quadriceps angle, rescue drug intake were evaluated up to 30 weeks after the procedure. Results: Isometric maximal contractile strength showed a significant improvement at 4 weeks after the procedure in the polydioxanone suture group, and the weight-bearing pain showed a significant improvement at every visit in the polydioxanone suture group compared with baseline values. Patient global impression of change score showed significant improvement at 20 and 30 weeks, and clinical score showed improvement at every visit. Conclusion: Insertion of polydioxanone sutures showed improvement in muscle strength and knee pain by supporting and fixation of the vastus medialis muscle in patients with degenerative knee osteoarthritis. Insertion of polydioxanone sutures is considered to have a therapeutic effect in knee osteoarthritis patients.

The present study purposed to examine the effects of transcutaneous electrical nerve stimulation, self-stretching and functional massage on the recovery of muscle contraction force for muscle fatigue caused by sustained isotonic contraction. The subjects of this study were 45 healthy students. They were divided into transcutaneous electrical nerve stimulation group(n=15), self-stretching group(n=15) and functional massage group(n=15), and using Primus RS. We observed the pattern of changes in maximal voluntary isometric contraction force(MVIC) after causing muscle fatigue in quadriceps femoris muscle through sustained isotonic contraction. Maximal voluntary isometric contraction force(MVIC) were greatly increased after transcutaneous electrical nerve stimulation, self-stretching and functional massage. In the comparison of recovery rate of muscle contraction force for muscle fatigue caused by sustained isotonic contraction among the treatment groups, it did not show any significant differences. However, it showed that each treatment may be effective in recovery of muscle fatigue caused by sustained isotonic contraction.
Purpose: This study examines the effects of elastic band of resistance exercise for balance control of the elderly. Methods: Thirty of eighty participants in experiment subjects who demonstrated balance-impairment through the use of primary screening tests including the one leg standing test (OLST), functional reach test (FRT) and timed up and go (TUG) were selected as subjects. Fifteen subjects that underwent muscle-strengthening exercise using an elastic band were selected as the exercise group and fifteen subjects were selected as a control group. Subjects undertook a home-based exercise program three times per week for 9 weeks. Muscle strength, functional assessment and a balance test were quantitatively measured before and after the exercise regimen. Results: After muscle strengthening exercises, changes in maximal voluntary isometric contraction (MVIC) showed a significant increase in all of the lower extremity muscles of the exercise group subjects. There were statistically significant differences between the exercise and control groups for changes in the OLST, FRT and TUG, which are functional assessments of balance ability, and changes of the unit path length and circumference area, measurement items of quantitative analysis. In addition, from examining correlations between MVIC, balance ability, it was found that an increase of muscle strength in the hip joint group of muscles among the lower extremity muscles had greater improvement in correlation with balance ability in this elderly population. Conclusion: Resistance exercise using elastic bands had significant effects on muscle strengthening in elderly subjects, with a resultant increase of lower extremity muscle strength with significant improvement of balance ability.
Background: The hamstring muscles in the lower extremity are highly important for knee joint stability and can be classified into medial and lateral hamstrings according to the anatomical position, which have some different functions. To measure the strength of the individual hamstring muscles, manual muscle testing is clinically performed by dividing rotation postures into internal and external postures. However, this has no sufficient scientific background. Objects: This study aimed to test the difference that the tibial rotation would cause in the muscle activity of the medial and lateral hamstrings. Methods: The muscle activities of the biceps femoris, semitendinosus, and semimembranosus were measured in a total of three different postures (neutral position and internal and external rotations) with 3 replications. During the maximal isometric contraction, resistance was constantly provided by the string attached to the strap, not by manual resistance of the examiner. Before and after electromyography measurements, the participants underwent hamstring flexibility measurement using the active knee extension test in the supine position on the treatment table. Results: The semitendinosus had a 12.56% reduction in muscle activity in external rotation as compared with that in neutral position. The biceps femoris and semimembranosus showed reduced muscle activities in both external and internal rotations as compared with those in neutral position. Only the women showed significant decreases in the comparison between pre and post-active knee extension. Conclusion: Only the semitendinosus muscle was consistent with the anatomical speculation. However, the reduction in the muscle activity of the semitendinosus as compared with that in neutral position was only 12.56%, the clinical value of which may be difficult to justify.
The purpose of this dissertation was to analyze the effect of trunk extension strengthening exercise on muscle performance of the upper limb in adolescent baseball player. The twenty people were studied : experimental group(10), comparative group(10). The experimental group has done trunk extension strengthening exercise for 8 weeks. The study analyzes isometric maximal strength of shoulder internal rotation. shoulder external rotation, elbow flexion, elbow extension, forearm pronation, forearm supination and ball speed. All of subjects were tested for 3 times ; pre, mid, post. The results were as follows; 1. Maximal isometric strength of upper limb, during trunk extension strengthening exercise in experimental group, shoulder internal rotation and external rotation showed it has slightly increased and comparative group showed it has no change, but not significant elbow flexion and extension significantly(p<0.05) increased after exercise either for 4 or 8 weeks compared with that of control group. Forearm pronation showed not significantly changed in both group, but significantly different between group either for 4 or 8 weeks. Forearm supination, significantly((p<0.05) increased after 8 weeks in experimental group. 2. Ball speed showed slightly increased but not significantly in experimental group. These results it may expect improvement of upper limb muscle performance of upper limb in adolescent baseball player. However, in case of shoulder a point of view of bunk extension strengthening exercise of this study hasn't a significant influence. More experimental studies are needs, hereafter which will use more experimental subjects and various methods of exercise and new application of treatment term to define significant change.
PURPOSE: This study examined the effects of Robot Tilt-table Training (RTT) on the lower extremity strength, balance, gait, and satisfaction with rehabilitation, in patients with subacute stroke (less than six months after stroke onset), and requiring intensive rehabilitation. METHODS: A total of 29 subacute stroke patients were divided into an RTT group (n = 14) and a Body Weight Support Treadmill Training (BWSTT) group (n = 15). The mean age of patients was 62 years. RTT and BWSTT were performed for four weeks, three times a week, for 30 minutes. Isometric strength of the lower extremities before and after intervention was compared by measuring the maximal voluntary isometric contraction of the lower extremity muscles. To compare the balance function, the center of pressure (COP) path-length and COP velocity were measured. Timed Up & Go test (TUG) and 10 Meter Walking Test (10 MWT) were evaluated to compare the gait function. A satisfaction with rehabilitation survey was conducted for subjective evaluation of the subject's satisfaction with the rehabilitation training imparted. RESULTS: In the intra-group comparison, both groups showed significant improvement in lower extremity strength, balance, gait, and satisfaction with rehabilitation, by comparing the parameters before and after the intervention (p < .05). Comparison of the amount of change between groups revealed significant improvement for all parameters in the RTT group, except for the 10 MWT (p < .05). CONCLUSION: Both groups are effective for all variables, but the RTT group showed enhanced efficacy for variables such as lower extremity strength, balance, gait, and satisfaction with rehabilitation, as compared to the BWSTT group.
