Golden proportion assessment between maxillary and mandibular teeth on Indian population

  • PURPOSE. This study evaluated the existence of golden proportion between the widths of the maxillary and mandibular anterior teeth in Indian population. MATERIALS AND METHODS. The clinical tooth width measurements were recorded with the digital vernier calipers on 576 patients of both sexes in the age group of 21 - 30 years. Flexible ruler was used to determine the width of maxillary and mandibular anterior teeth on the patients by the same operator. The data obtained was statistically analyzed using paired student t-test (${\alpha}$=.05). RESULTS. The golden proportion was not found between the width of the right central and lateral incisors in 53% of women and 47% of men. The results revealed the golden percentage was rather inconstant in terms of relative tooth width. CONCLUSION. The golden proportion is an inappropriate method to relate the successive widths of the maxillary anterior teeth in Indian population.

미맹출 영구견치 및 소구치의 폭경 예측 (PREDICTION OF THE SUM OF MESIODISTAL WIDTHS OF THE UNERUPTED PERMANENT CANINES AND PREMOLARS)

  • 제1 대구치까지 맹출완료된 정상군(109명 : 남자65명, 여자44명)과 밀집군 (85명 : 남자 42명, 여자43명)의 치열모형상에서 치아폭경을 계측하여 남녀간, 그리고 밀집군과 정상군간에 상악 및 하악의 영구 절치의 근원심 폭경합과 상악 및 하악의 편측 영구 견치 및 소구치의 근원심 폭경 합의 관계를 연구하여 다음과 같은 결론을 얻었다. 정상군과 밀집군에서 상악 및 하악의 영구 절치의 근원심 폭경 합에 대한 상악 및 하악의 편측 영구 견치 및 소구치의 근원심 폭경합의 비율은 남여간에 유의한 차가 없었다. 하악 영구 절치의 근원심 폭경합에 대한 하악의 편측 영구 견치 및 소구치의 근원심 폭경합의 비율은 정상군과 밀집군간에 유의한 차가 없으나 상악의 편측 영구 견치 및 소구치의 근원심 폭경합의 비율은 두 군간에 유의한 차가 있었다. 상악 영구 절치의 근원심 폭경합에 대한 상악 및 하악의 편측 영구 견치 및 소구치의 근원심 폭경합의 비율은 정상군과 밀집군간에 유의한 차가 있었다. 정상군과 밀집군에서 상악 및 하악의 영구 절치의 근원심 폭경합을 이용하여 상악 및 하악의 편측 영구 견치 및 소구치의 근원심 폭경합에 대한 최소자승법에 의한 회귀방정식을 산출하고 각의 백분률표를 작성하였다.

Dentofacial transverse development in Koreans according to skeletal maturation: A cross-sectional study

  • Objective: The aim of this study was to establish the normative data of dentofacial transverse dimensions according to the skeletal maturation stage in Korean adolescents with good occlusion, assess gender differences and determine correlations between transverse variables. Methods: A total of 577 Korean subjects between ages 7 to 19 years and exhibiting skeletal Class I occlusion were categorized by skeletal maturation index (SMI) of Fishman using hand-wrist radiographs. Dentofacial transverse dimensions were assessed using posteroanterior cephalograms. Independent two-sample t-tests were used to analyze differences between genders. Pearson correlation coefficient was used to determine the correlation between transverse measurements. Results: Dentofacial transverse norms relevant to skeletal maturation stages were established. The average maxillomandibular width difference and ratio at growth completion was 22.16 mm and 77.01% for males; 23.70 mm and 74.06% for females, respectively. Males had greater facial, maxillary and mandibular widths compared to females at every SMI stage. The maxillary and mandibular intermolar widths showed the strongest correlation for both sexes (r = 0.826 for males, r = 0.725 for females). Conclusions: Dentofacial transverse norms of Korean adolescents were established according to developmental stage. All dentofacial widths were greater in males at growth completion. Maxillary and mandibular intermolar widths were strongly correlated. This study may serve as a guideline for the assessment of dentofacial transverse growth according to skeletal maturation stage in Korean adolescents with good occlusion.

전치폭경이 전치부 교합형태에 미치는 영향 (THE EFFECT OF MESIODISTAL CROWN WIDTHS OF ANTERIOR TEETH ON THE INCISOR RELATIONSHIP)

  • This study was intended to investigate the effect of mesiodistal crown widths of the anterior teeth on the incisor relationship and to determine whether incisor width ratio and anterior width ratio could be used to assess interarch tooth width harmony. From the casts taken from 63 subjects, 26 of open bite, 18 of deep bite and 19 of normal over bite with age of 17-20, mesiodistal crown widths of maxillary and mandibular 6 anterior teeth were measured with Boley gauge. On the basis of tooth measurements, anterior and incisor width ratio were calculated. The cephalograms were taken from same subjects to measure the degree of over bite and over jet and to compare the craniofacial bony structures between open bite, deep bite and normal over bite. Correlations among the anterior width ratio, incisor width ratio, over bite and over jet were calculated. The results were as follows. 1. There were no differences in mesiodistal widths of anterior teeth, incisor width ratio and anterior width ratio between open bite, deep bite and normal over bite. 2. The incisor width ratio and anterior width ratio can be useful in the assessment of tooth width harmony because the incisor width ratio and anterior width ratio were stable statistically and significantly correlated with each other. 3. Over bite and over jet were not correlated with incisor width ratio and anterior width ratio.

성인에서 Hyrax 장치로 완속상악확장하여 폭경부조화를 개선한 증례 (Correction of Transverse Discrepancy with Slowly Maxillary Expansion by Hyrax type expander in Adult Patient)

  • Approximately 30% of adult patients who want orthodontic treatment have transverse discrepancy with insufficient width of the maxilla. Particularly, in Class III patients requiring orthognathic surgery, the frequency of insufficient width of the maxillary arch related to respiratory problems is high. We report a case of non-surgical maxillary expansion using a Hyrax type expander with an orthognathic surgery, based on the reports that the ratio of non-fused midpalatal suture is not high in adults. A 30 years and 2 months old woman with a long face showed an Angle Class III with a vertical growth pattern. Class III molar and canine relation, anterior edge bite, and mandibular incisor compensatory lingual inclination were observed. The posterior buccal overjet seemed to be appropriate, but I diagnosed that there was a transverse discrepancy, for the following reasons. The inter-canine and inter-molar widths were sufficient but excessive lingual inclination of the mandibular molars was observed when assessing the bucco-lingual inclination based on the center of resistance of the maxillary and mandibular first molar. For this reason, it was expected that intercuspal interference would occur during orthodontic decompensation. Therefore, slow maxillary expansion using Hyrax type expander was performed and 2-jaw rotation surgery was performed to improve aesthetic and occlusion. Adults can also improve width discrepancy by non-surgical methods, which can avoid SARPE requiring additional surgery or segmental surgery lacking stability and predictability.

한국인 정상 교합자의 정모 두부 방사선 사진을 이용한 안모비율에 관한 연구 (A Posteroanterior Cephalometric Study on Craniofacial Proportions of Koreans with Normal Occlusion)

  • 두개 악안면 기형 환자의 종합적인 치료를 위해서는 입체적인 평가와 진단이 중요하다. 그러나 흔히 전후방적인 골격 평가에는 많은 분석 방법을 시행하고 있으나, 안모 비대칭과 상 하악골 횡적인 부조화의 진단을 위해 필요한 횡적 평가방법은 충분하지 못한 실정이다. 횡적인 부조화의 적절한 진단과 치료는 안정된 기능교합의 형성에 있어서 전후방 또는 수직적인 부조화보다 중요한 역할을 하며, 장기간의 안정성에도 영향을 미친다. 그리고 상하악의 횡적인 부조화에 따른 연조직의 변화가 제한되어 있기 때문에 시진을 통한 임상적 방법으로는 간과되기 쉬우며 더욱이 특징적인 안모 소견을 보이는 전후방 또는 수직적인 부조화와 동반된 경우에는 감별되기 어렵다. 그로 인해 정확한 진단과 치료를 위해서는 정상 안모에 관한 정모두부 방사선 계측학적 안모 비율의 평가가 필요하게 되었다. 본 연구는 1996학년도 연세대학교 신입생중에서 협조 가능한 정상 안모와 거의 정상 교합을 갖은 남녀 76명을 선정하여 안모 비대칭과 횡적인 부조화가 있는 악안면 기형 환자의 진단과 치료계획에 도움을 주는 자료를 얻고자 정모두부 방사선 계측사진을 이용하여 수평 및 수직 계측항목과 그 비율을 통계처리하여 다음과 같은 결과를 얻었다. 1. 정모의 수평 및 수직적인 계측항목과 그 비율의 평균값과 표준편차를 구하였다. 2. 정모의 폭경에 대한 고경의 비율은 0.837(남자: 0.836, 여자:841)이었다. 3. 상악골과 하악골간의 폭경비율은 0.747(남자: 0.745, 여자:0.752)이며, 높은 연관성을 나타내었다. 4. 두개 안면부 폭경들간에는 서로 연관성이 나타났다. (두개폭경, 관골전두봉합폭경, 안면폭경, 상악폭경, 하악폭경, 상악과 하악 구치부폭경) 5. 정모의 폭경이 증가할수록 안모고경과 다른 길이 계측치도 연관되어 증가하였지만 각도 계측값 ($Bj\ddot{o}rk$의 합, 하악평면각)의 감소와, 후전 안면고경 비율 증가로 인하여, Brachycephalic facial type의 안모 경향을 보였다. 이상의 결과는 악안면 기형 환자의 교정치료 및 악교정 수술치료의 계획에 기준치로써 사용될 수 있을 것이다.

Evaluation of Tooth Movement and Arch Dimension Change in the Mandible Using a New Three-dimensional Indirect Superimposition Method

  • Purpose: To analyze the amount and pattern of tooth movement and the changes in arch dimension of mandibular dentition after orthodontic treatment using a new three-dimensional (3D)-indirect superimposition method. Materials and Methods: The samples consisted of fifteen adult patients with class I bialveolar protrusion and minimal anterior crowding, treated by extraction of four first premolars with conventional sliding mechanics. After superimposition of 3D-virtual maxillary models before and after treatment using best-fit method, 3D-virtual mandibular model at each stage was placed into a common coordinate of superimposition using 3D-bite information, which resulted in 3D-indirect superimposition for mandibular dentition. The changes in mandibular dental and arch dimensional variables were measured with Rapidform 2006 (INUS Technology). Paired t-test was used for statistical analysis. Result: The anterior teeth moved backward, displaced laterally, and inclined lingually. The posterior teeth showed statistically significant contraction toward midsagittal plane. The amounts of backward movement of anterior teeth and forward movement of posterior teeth showed a ratio of 6 : 1. Although the inter-canine width increased slightly (0.8 mm, P<0.05), the inter-second premolar, inter-first molar, and inter-second molar widths decreased significantly with similar amounts (2.2 mm, P<0.05; 2.3 mm, P<0.01; 2.3 mm, P<0.001). The molar depth decreased (6.7 mm, P<0.001) but canine depth did not change. Conclusion: A new 3D-indirect superimposition of the mandibular dentitions using best-fit method and 3D-bite information can present a guideline for virtual treatment planning in terms of tooth position and arch dimension.
