Masticatory Performance Measured with a Chewing Gum Containing Spherical Resinous Microparticles

  • Objective: This study aimed to investigate the factors associated with masticatory performance, as measured with a chewing gum containing spherical resinous microparticles, and to evaluate the method by examining the relationship with self-reported masticatory status. Methods: The participants in this study comprised 903 industrial workers (mean age, $42.2{\pm}11.6years$). A questionnaire was administered to assess self-reported masticatory status. The masticatory performance score was calculated by counting the number of particles in the chewing gum. Clinical oral examinations were administered. Multiple linear regression analysis was conducted on the masticatory performance scores to examine the related factors. Analysis of covariance was conducted to investigate the association between the masticatory performance score and the self-reported masticatory status. Results: Significant predictors of the masticatory performance score were sex (p<0.001), age (p<0.001), decayed teeth (p=0.009), total-functional tooth units (p<0.001), periodontitis (p=0.003), and malocclusion (p=0.011). The relationship between the masticatory performance score and the self-reported masticatory status was attenuated after controlling for confounding factors. Conclusion: The masticatory performance increased with age and decreased as the oral health status worsened. Using this chewing gum method partly, but not comprehensively, reflects masticatory performance. Therefore, various masticatory performance-related indexes should be employed to measure masticatory performance accurately.

  • This investigation was designed to determine the effectiveness of the posterior occlusal schemes on masticatory activity during mastication in complete denture. Twelve edentulous subjects were selected for this study. All subjects had no past history and no functional abnormality on masticatory system and TMjoint. And, they had residual ridge of favorable morphology, firm mucosa and Class I skeletal jaw relationship, Twelve experimental denture with interchangeable occlusions(0-degree teeth, 30-degree teeth, Levin teeth and S-A teeth) were constructed for this study. The masticatory performance was analyzed by means of standard sieve(10, 16, 20, 30sieve), and the electrical activity from selected muscles(Temporalis and Masseter muscle) was recorded simultaneously with electromyography (Bio-Pak system) as the subject masticated test foods (rice, peanut and gum) with four different occlusal schemes. Mandibular movement was, also, measured with Sirognathography(Bio-Pak system). These recordings were performed in immediately, after 1 week and after 2 weeks of insertion of complete denture. The results were as fellows; 1. The average masticatory performance of 0-degree artificial teeth was higher than any other artificial teeth. 2. Masticatory performance in denture wearer was affected preferentially by food and artificial occlusal schemes. 3. During chewing, there was a statistical difference of EMG activity between masseter and temporal muscle(p<0.01). Especially, EMG activity of working masticatory muscle was highly affected by food rather than by artificial occlusal schemes. 4. In denture wearer, the velocity of opening was not affected by food, whereas, the velocity of closing was faster in soft food chewing than in hard food chewing, and the amount of vertical displacement was grater in chewing of soft and large bolus than in chewing of hard and small bolus. However, the amount of lateral displacement showed conversely(p<0.05). 5. It was considered that masticatory performance in denture wearer is not affected by the condition of residual ridge. the history of denture wear, the preference, the adaptation to artificial teeth and the total mesiodistal length of artificial posterior teeth.

Assessment of swallowing and masticatory performance in obturator wearers: a clinical study

  • PURPOSE. To assess function by identifying changes in swallowing and masticatory performance in maxillary obturator prosthesis wearers. MATERIALS AND METHODS. Sixty subjects were recruited for the study, of which 20 were obturator wearers, 20 were completely dentulous and 20 had removable partial/complete dentures with similar Eichner's Index. Swallowing ability was evaluated with and without obturator using the "Water Drinking Test"; Masticatory performance was evaluated with the Sieve test; and maximum occlusal force was recorded with the help of a digital bite sensor. The data was analyzed using the Statistical Package for Social Science version 15.0 with a confidence level at 95%. RESULTS. Profile, behavior of drinking and time taken to drink were significantly improved (P<.001) in subjects after wearing obturator. Masticatory performance was not significantly different (P=.252) in obturator wearer when compared with dentulous or removable partial/complete denture wearer, but significantly (P<.001) high inter group difference in maximum occlusal force existed. Correlation between masticatory performance and maximum occlusal force was not significant (P=.124). CONCLUSION. Swallowing ability was significantly improved after wearing obturator but masticatory performance was not significantly different from those having similar occlusal support zone in their dentition.

A COMPARISON OF THE MASTICATORY FUNCTION BETWEEN TWO DIFFERENT TYPES OF IMPLANT SUPPORTED PROSTHESES AND COMPLETE DENTURE FOR FULLY EDENTULOUS PATIENTS

  • STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.

자연치열자와 가철성 국소의치 장착자의 저작효율과 근활성도에 관한 비교 연구 (A STUDY ON MASTICATORY PERFORMANCE AND MUSCLE ACTIVITY IN REMOVABLE PARTIAL DENTURE WEARERS)

  • The purpose of this study was to compare and evaluate the differences in masticatory performance, muscle activity, and patterns of occlusal contact between persons with natural dentition and removable partial denture wearers. Twenty healthy adult subjects with more than 28 teeth and thirteen removable partial denture wearers that classified Kennedy classification I was selected. The degree of pulverized rice and peanut was measured and analyzed by means of sieving method to compare the masticatory performance. For the muscle activity, EMG was recorded in selected muscles(Temporalis and masseter muscle) during mastication and resting state. The occlusal record in maximal intercuspation was taken with a silicone occlusal bite registration material for analysis of the patterns of occlusal contact. The obtained results were as follows: 1. When chewed peanuts, masticatory performance ratio at 10-sieve size was 81.31% in natural dentition group. In removable partial denture wearer, 27.01% without RPD and 69.09% with RPD. When chewed rice, 42.04%, 11.87%, and 21.58%, respectively. The differences of masticatory performance ratio between groups were statistically significant at the 0.05 level. 2. The mean EMG value in resting state was $1.06{\mu}V$ on temporal muscle, $0.98{\mu}V$ on masseter muscle in natural dentition group. In removable partial denture wearers, $1.13{\mu}V$ on temporal muscle, $1.05{\mu}V$ on masseter muscle without RPD and $1.11{\mu}V$ on temporal muscle, $1.04{\mu}V$ on masseter muscle with RPD. 3. The mean EMG value during mastication was $45.64{\mu}V$ in natural dentition group, and in removable partial denture wearers, $22.06{\mu}V$ without RPD and $31.01{\mu}V$ with RPD when chewed peanuts. When chewed rice, $45.24{\mu}V,\;25.53{\mu}V\;and\;32.14{\mu}V$, respectively. The differences of mean masticatory EMG value between groups were statistically significant at the 0.05 level. 4. The number of posterior occlusal contact point was 20.15 in natural dentition group and 11.92 in removable partial denture wearers. The area of perforated surface was $16.50mm^2$ in natural dentition group and $6.06mm^2$ in removable partial denture wearers. The area of contact surface was $78.93mm^2,\;51.52mm^2$, respectively. 5. The area of contact surface was effective to masticatory performance ratio in natural dentition group and removable partial denture wearers (p<0.05). From these results, it is concluded that in partially edentulous patient, masticatory efficiency can be improved by removable partial denture wearing, and for efficient mastication, tight occlusal contact surface shoud be maintained by maximum support that is provided from mucosa.

대도시 노인의 주관적 구강증상과 저작능력이 삶의 질에 미치는 영향 (A Study on the Effects of Perceived Oral Symptoms and Masticatory Performance on the Quality of Life for the Elderly in an Urban Area)

  • The purpose of this study is to examine relations among the subjective oral symptoms, masticatory performance and life quality of aged people residing in some regions of a large city, ultimately providing basic information needed to develop and implement programs oral health project and oral health education programs for the oral health promotion of those people. For the above purpose, this researcher conducted a questionnaire survey of people aged 65 or over from July 14th to 30th, 2011. Among the copies of the questionnaire distributed to the subjects, 318 were recollected. Out the recollected forms, 18 which were deemed as in appropriately filled in were excluded, and the remaining 300 were finally analyzed. The results are as follows. Subjective oral symptoms and masticatory performance have correlation(p<0.001) with aged people's life quality. Factors of influencing life quality were indicated to be gender(p=0.046), age(p=0.008), appearance of living together(<0.001), masticatory performance(p<0.001), and temperature reaction(p=0.018). This study has limitation because of being information that was obtained just with questionnaire survey on subjective oral symptoms. It is considered that there is a need of surveying along with objective data on oral symptoms hereafter.

한국 소아청소년의 저작 능력 평가 및 관련 요인 (Masticatory Performance and the Related Factors in Korean Children and Adolescents )

  • 이 연구는 국내 소아청소년의 저작 능력을 평가하고 관련 요인들을 조사하고자 하였다. 유구치의 조기 상실, 제1대구치의 교합 정도, 제1대구치의 교합 관계, 전치부 반대교합의 유무를 고려하였다. 2020년 3월부터 2021년 7월까지 56명의 만 6세에서 12세 사이 소아청소년을 대상으로 하였다. 혼합능력지수(MAI)가 소아청소년의 저작 능력을 평가하기 위해 계산되었고 유구치의 조기 상실 개수에 따라 세 집단으로 분류한 연구대상자는 정상 치열, 유구치 1 - 2개 상실, 유구치 3개 이상 상실한 경우 각각 23명, 18명, 15명이었다. 정상치열군과 1 - 2개 유구치 상실군에서는 MAI 값에서 차이가 없었으나, 3개 이상 유구치를 상실한 경우 MAI 값이 감소했으며, 유의한 차이가 관찰되었다. 이 연구는 국내 소아청소년의 저작 능력을 평가한 최초의 연구로, 성인과 저작 능력을 비교하였으며, 임상에서 소아청소년의 저작 능력 증진을 위해 노력하는 소아치과 의사에게 도움이 될 수 있다.

Masticatory function following implants replacing a second molar

  • Purpose: The aim of this study was to obtain objective and standardized information on masticatory function and patient atisfaction following second molar single implant therap. Methods: Twenty adult patient, who had restored second molar single implants more than 1 month before the study, were enrolled in this stud. All patients received a chewing test using peanuts before and after insertion of the implant prosthesi, with a questionnaire and visual analogue scale (VAS) to evaluate the effect of second molar single implant therap. Results: his study obtained standardized information on the masticatory function objectively (e.g., P, R, $X_{50}$) before (Pre-insertion) and after insertion (Post-insertion) of the implant prosthesis. Masticatory performance (P) after insertion of the implant prosthesis significantly increased from $67.8{\pm}9.9$ to $84.3{\pm}8.5$% (P<0.0001). With the implant prosthesis, the P value increased by 24%. The masticatory efficiency index (R) of Post-insertion is higher than that of Pre-insertion (P<0.0001). With the implant prosthesis, the R value increased by 29%. The median particle size ($X_{50}$) of Post-insertion is lower than that of Preinsertion (P<0.0001). More than 90% of the patients were satisfied with the second molar single implant therapy from a functional point of view. Conclusions: These findings indicate that a second molar single implant can increase masticatory function.

저작운동으로 인한 진동 잡음 신호의 경감을 위한 측두골 이식형 마이크로폰의 설계 (The Design of Temporal Bone Type Implantable Microphone for Reduction of the Vibrational Noise due to Masticatory Movement)

  • A microphone for fully implantable hearing device was generally implanted under the skin of the temporal bone. So, the implanted microphone's characteristics can be affected by the accompanying noise due to masticatory movement. In this paper, the implantable microphone with 2-channels structure was designed for reduction of the generated noise signal by masticatory movement. And an experimental model for generation of the noise by masticatory movement was developed with considering the characteristics of human temporal bone and skin. Using the model, the speech signal by a speaker and the artificial noise by a vibrator were supplied simultaneously into the experimental model, the electrical signals were measured at the proposed microphone. The collected signals were processed using a general adaptive filter with least mean square(LMS) algorithm. To confirm performance of the proposed methods, the correlation coefficient and the signal to noise ratio(SNR) before and after the signal processing were calculated. Finally, the results were compared each other.
