측두하악장애를 가진 성인여성에 있어 중절치부 최대교합력 유지양상 (A Feature of Maintaining the Maximum Bite Force on Central Incisors in Adult Females with Signs and Symptoms of Temporomandibular Disorders)

  • In order to evaluate the effect of signs and symptoms of temporomandibular disorders on the bite force of anterior teeth, the author estimated a feature of maintaining the maximum bite force on central incisors in Korean 34 adult females with signs and symptoms of temporomandibular disorders (TMD group) and in Korean 31 adult females within normal masticatory function far from any sign or symptom of TMD (control group), and analyzed the data statistically. The obtained results were as follows: 1. There was not a significant difference of the maximum bite force on central incisors between TMD group and control group (P>0.05). 2. The duration of maintaining the maximum bite force on central incisors in TMD group was shorter than it in control group (P<0.01).

Oral rehabilitation for a patient with oligodontia and maxillary hypoplasia

  • An 18 year old female with oligodontia and maxillary hypoplasia was treated using an interdisciplinary team approach involving orthodontists, maxillofacial surgeons and prosthodontists. Full mouth one-piece fixed partial dentures were the final restoration. The fixed partial dentures fabricated for the maxilla and mandible using the concept of a shortened dental arch resulted in improved esthetics and the masticatory function. This paper describes the treatment procedures for an oligodontia patient with alveolar bone hypoplasia.

  • Crown-type implant-assisted removable partial dentures (CIRPDs) can be a feasible treatment option for partially edentulous patients. Here we report a case with remaining unilateral mandibular teeth. Two implants were placed in the posterior portion of the mandible using a surgical guide, and a distal-extension removable partial denture with implant-supported surveyed crowns was fabricated. After 12 months, both the abutment teeth and implants were in good condition. The treatment outcomes were satisfactory in terms of masticatory function and esthetics. The advantages of CIRPDs and considerations for obtaining successful clinical outcomes with these dentures are also discussed.

Benign osteoblastoma of the mandible: a case report

  • Osteoblastoma is a rare benign neoplasm formed by osteoid tissue and well-vascularized bone that occurs mainly in children and adolescents. It appears primarily in the long bones, vertebral column, and small bones of the hands and feet, and not typically in the skull and maxillary bones. The purpose of this study is to present the case of an 8-year-old girl with a diagnosis of right mandibular osteoblastoma and a review of the relevant literature. The goals of treatment were to preserve dental occlusion, masticatory function and facial symmetry while minimizing the effects on patient body image and quality of life. Osteoblastoma, although it is benign, can be aggressive, and its treatment will depend on the timing of diagnosis, size and location. Early diagnosis is essential to avoid not only radical surgery as in the case presented, but also to help minimize the risk of possible relapse and potential malignancy of a benign osteoblastoma.

Eustachian tube calcification as an unusual finding on a panoramic radiograph

  • The author herein presents an unusual case of eustachian tube calcification masquerading as loose radiopacities in the temporomandibular joints on a panoramic image, creating a diagnostic challenge. The patient, a 72-year-old woman, presented to the dental service for implant treatment to improve her masticatory function. A cone-beam computed tomography scan was performed and reviewed by a board-certified oral and maxillofacial radiologist. The scan showed no evidence of calcifications in the temporomandibular joints; however, it revealed nodular calcifications within the cartilaginous portion of the eustachian tube bilaterally. Additionally, this report briefly reviews the differential diagnosis of calcified loose bodies in the temporomandibular joint and provides information that needs to be reinforced periodically.

  • 하악골 재건을 위한 여러 골이식 방법중 유리혈관화 견갑골은 다른 방법에 비해 여러 가지장점을 갖고 있다. 저자는 독일 Wuerzburg 대학에서 연수중 다수의 하악골 복합결손환자를혈관화 견갑골판을 이용 재건하여 우수한 결과를 얻는 것을 경험하였다. 견갑골 피판은 혈관분포를 보존하면서 여러 군데의 쐐기형 부분절골술에 의해 쉽게 하악골 외형에 따라 형성할 수 있었으며, 대부분의 증례에서 혈관경의 길이는 충분하였고, 수혜부의 조건에 따라 견갑골판 단독으로 또는 피부판을 포함하여 채취되었으며, 공여부의 유병율은 거의 없이 우수한 심미적 기능적 결과를 얻을 수 있었다. 결론적으로 이 유리골관은 다양한 피판의 유용성과 하악골의 3차원적인 재건 그리고 적은 유병율을 가지고 있어, 연조직을 포함한 하악골 복합결손부 재건에 매우 적합하리라 사료된다.

국산 Avana implant의 임상결과에 관한 연구

  • Osseointegrated dental implants have now become and accepted form of treatment for patients with a fully or partially missing dentition. The purpose of this study was to evaluate the performance of AVANA implant-Korea. A total of 323 AVANA implants were placed at 9 centers in the edentulous and partially edentulous jaws of 88 consecutive patients ranging in age form 18 to 79 years. So we get the clinical results. 1. Both patients (94.4%) & doctors (94%) were satisfied with the results, in patient's aspect 'most satisfaction' case were more common(52%) 2. The length of fixture which was frequently used was 13mm (168/319,44.5%) followed by 15mm and 10mm, and 3.75mm width implant (239/319,74.9%) was the most popularly used. 3. Mandibutar posterior was the most common installation site (140/317.43%) 4. A mean implant survival rate was 96.2%. That showed little difference which reported other paper. Cause of failure consisted of inflammation $ infection-36%, fall of osseointegration-18%, paln during masticatory function-9%. improper prosthetic function $ procedure-9%. 5. The most popularly used types of abutment was the UCLA abutment(168/305,55.5%)

Occlusal rehabilitation of posterior fixed prostheses: A clinical report

  • Masticatory function is more important than esthetics in posterior fixed restoration. The usual technique-mounting the casts on a semi-adjustable articulator, etc. -cannot make all patients satisfied with their restorations. For example, functionally generated path technique can be an easier and more satisfactory method in the restoration of group function. These clinical reports describe various approaches for occlusal restoration of relatively simple posterior fixed protheses according to patients occlusal patterns. The 3-unit bridge restoration is one of the most popular treatment options in prosthodontics. Because dentists have much experiences of it, they restore a missing span of one tooth mechanically, that is, without special consideration. While esthetics is important in making an anterior 3-unit fixed prostheses, mastication is more focused on in posterior 3-unit bridge restoration. Many dentists are concerned about various aspects in esthetics, such as morphology of the tooth, value, chroma, hue, translucency, surface texture, etc. But they do not usually consider various methods to restore occlusion. They treat one-tooth-missing area in a similar way in spite of patients having variety of occlusal patterns. Three cases are presented here in 3 or 4-unit bridge restoration. They show some methods to restore patients' occlusal patterns.

의치의 안정성을 증대시키기 위한 고려 요소: 증례보고 (Considerations for increasing denture stability: a case report)

  • 총의치 장착시, 환자는 생리적으로 안정된 범위 내에서 자연스럽게 기능하기를 원하며 이를 위해서는, 안모의 회복(심미적 효과), 생물학적으로 안정된 인공치 배열과 외형, 의치 동요가 없는 교합접촉이 반드시 필요하다. 이에 본 증례보고에서는 총의치를 제작하기에 불리한 조건을 가진 환자를 대상으로 인상면, 연마면, 교합면에 있어서의 의치의 안정을 증대시키기 위해, 기능적 변연 형성을 통한 인상채득, 중립대 확인, 생리적 중심위 재확립, 안정된 교합을 형성시켜 주었으며, 임상적으로 기능과 심미적인 측면에 있어서 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

  • 전악 구강 수복에서 이상적인 교합평면을 형성하기 위한 기존의 진단 및 치료계획은 각각의 과정이 복잡하고 치과의사와 기공사간의 정보교환이 주관적이므로 어려움이 있다. OP finder® system은 동공간선이 평행하지 않은 환자의 정보를 보다 객관적이고 정확하게 전달하는데 도움이 된다. 본 증례에서는 오래된 불량한 고정성 보철물과 심하게 마모된 하악 치아를 가진 환자에게 OP finder® system을 이용하며 전악수복을 시행하였고, 적절한 교합평면 설정 및 저작기능 회복 결과를 얻었기에 보고하고자 한다.
