The purpose of the this study was to provide objective evidence through a systematic review and meta-analysis regarding the effects of self-management in knee osteoarthritis. Articles published between 1999 and 2013 from periodicals indexed Ovid Medline, CINAHL, PubMed, RISS, KISS and other databases were selected, using the following key words: Osteoarthriti* OR OA, self-(management OR care OR help). A selection of 11 studies(7 randomized controlled trials and 4 quasi-experimental interventions that were used were exercise, education, massage and self-help group. As a result of mera-analysis, self-management significantly effected pain, physical function, activities of daily living, and self-efficacy. In particular, self-management showed great effect on ADL, and moderate effect on pain and physical function. Based upon these result, it is necessary to develop a standardized self-management program for knee osteoarthritis patients.
This study was aimed to analyze the variables measuring stress and immune responses, to identify the relationship between stress and immune responses, and to find out the effect of nursing interventions associated with stress and immune responses by reviewing thirty-four published articles since 1970 in Korea. The articles were selected in the field of nursing, stress management, and masters or doctoral dissertations and limited to human subject. Among these, the thirty-one articles were published since 1996 and mainly distributed in nursing (44.1%) and medicine(44.1%). The prevailing research design was nonequivalent control pre-post experimental design(41.1%). The research subjects were 55.9% for patients and 44.1% for healthy general persons including 20.6% of university students. To evaluate stress, both physiologic and psychosocial measures were adapted together in 35.3% of the articles. The most frequent two variables measuring stress and immune response were cortisol level(15.9%) and number or activity of natural killer cell(25.9%). The relation between stress and immune responses was positive in 4 articles, negative in 9 cases, and none in 12 cases. Decreased stress and enhanced immune function have been found when massage, abdominal breathing, exercise, relaxation, and touch were provided as nursing interventions. The articles to investigate the relationship between stress and immune function were limited and the tested variables were diverse. Also there was no consistent evidence to correlate the stress and immune function at present. Further studies are needed to construct a valid research design and to investigate the relationship between stress and immune responses. Nursing interventions to decrease stress should be developed to result in the increased immune function and the effect of these interventions would be verified.
Purpose: The aim of this study was to investigate the effects of physical therapy combined with a virtual reality (VR) game on pain, quality of life (QOL), engagement, and knee function in post-knee-surgery patients. Methods: Twenty-four patients who had undergone knee surgery four weeks or more before the study were recruited. Two withdrew from the study during the four-week experimental period, and a total of 22 patients were included in the final analysis. Routine physical therapy consisting of electrostimulation (10 min.) and therapeutic massage (10 min.) was the base intervention for all groups. The experimental group (n = 10) was additionally exposed to a VR game intervention, while the control group (n = 12) underwent an intervention involving similar motions as the experimental intervention but with no VR. The intervention for the experimental group used the game Rig Fit Adventure on Nintendo switch. Both groups underwent their respective interventions 3 times a week (35 min. per session) for 4 weeks. Pain was assessed using the numeric rating scale (NRS), and QOL was assessed using the EuroQol five-dimensional five-level questionnaire (EQ-5D-5L). Engagement was assessed using the Korea flow state scale (K-FSS). Finally, knee movement and function were assessed based on knee flexion and extension, range of motion (ROM), and Western Ontario and McMaster Universities Arthritis Index (WOMAC). Results: After the four-week physical therapy, both groups showed significant reductions in pain (on the NRS), increased knee ROM (flexion), better WOMAC scores, and increased EQ-5D-5L scores (p < 0.05), with the experimental group showing significantly better improvements in EQ-5D-5L and K-FSS scores (p < 0.05). Conclusion: The results of this study confirm that a VR-game-integrated intervention is effective for improving pain, QOL, engagement, and knee function in post-knee surgery patients and that VR-game-integrated interventions could be therapeutic alternatives for patients bedridden for prolonged periods with little motivation for rehabilitation.
As there has recently been growing interest in Alternative Medicine, Oriental Medicals have been trying to incorporate Alternative Medicine into Oriental Medicine evolutionarily. The purpose of this study is to make a comparative study of the historical background and present condition, the view of life, the principle of therapeutics, and the various therapeutics in Alterative Medicine and Oriental Medicine from the viewpoint of Oriental Medicine. The conclusion of this study is; 1. Although Alternative Medicine has become influential mainly in the effort of finding a solution the limit of western medicine, the clinical effect of Alternative Medicine should be verified continuously. 2. Alternative Medicine includes all of the therapeutics such as Traditional Medicine, Naturopathy, Complementary Medicine and Selective Medicine whose characters are different from Western Medicine and Oriental Medicine can be regarded as a sort of Alternative Medicine. 3. The holistic, systematic view of life of Alternative Medicine, from which viewpoint human considered as a harmonious vital existence, is a sort of recurrence to the view of Hipporcrates. 4. From the viewpoint of Alternative Medicine, a disease is not partial but holistic disorder and is treated by utilizing natural products as a medicine based on the principle that pursues harmonious and balanced psychosomatic function. This principle of therapeutic is the same as Oriental Medicine is based on. 5. Alternative Medicine may be classified largely five category, (1) Eastern Therapy, (2) Manipulative Therapy, (3) Natural Therapy, (4) Active Therapy, (5) Therapy Involving External Powers. 6. Besides Alternative Medicine which is used in clinical treatment, Massage, Homeopathy, Bach Flower remedies, Meditation, and other Music Therapy and Color Therapy is relatively accordant with therapeutics in Oriental Medicine and should be more actively accepted in clinics and improved.
Purpose: This study was performed to systematically review the recently published nursing intervention studies. Methods: The literature was identified through the Korean Education and Research Information Service (KERIS), the Korean Information Service System (KISS), and National Assembly Library websites. Key words such as breast cancer, nursing, and intervention were used. The factors analyzed are as follows: 1) the characteristics of studies and study populations, 2) the classification of interventions, 3) outcome indicators and their effects, and 4) effective interventions. Results: Thirty two studies were included. Seventeen studies used a single intervention such as aerobic dance, TaiChi, foot massage, aromatherapy, or a stress-reduction method. Fifteen studies used combined interventions, including education, exercise, counseling, support, yoga or meditation. The data on 47 outcome indicators and their effects were segregated into psycho/spiritual outcomes, stress coping, physical outcomes, cardiorespiratory function, symptom management, arm and shoulder functions, fatigue, and quality of life. Some interventions had positive effects on stress, fatigue, and functions of shoulder. Conclusion: Various interventions are available for breast cancer patients, and some have had positive effects. However, more studies are required to develop evidence-based practice guidelines for nursing interventions.

This study was designed to investigate the effect of a combined stimulus which was composed of chiropractic massage and thermotherapy on the relief of mental and physical stress. Fifteen healthy male subjects were treated with three type of stimuli; control (without any stimulus), a single stimulus (only chiropractic) and a combined stimulus. To evaluate the effects of stimuli, visual analogue scale (VAS), trunk extension, electromyogram (EMG) on erector spinae muscle and electrocardiogram (ECG) were measured and analyzed before and after stimuli. In the control group, there were no significant changes in EMG root mean square (RMS) value and low Frequency/high Frequency (LF/HF) ratio (p > 0.05, p > 0.05). However, both stimulus groups showed significant increases in trunk extension and EMG RMS values (p < 0.05, p < 0.05), but significant decreases in LF/HF ratios (p < 0.05). Particularly the decrement rate in LF/HF ratio was significantly higher in the combined stimulus group than that in the single stimulus group (p < 0.05). Also, a significantly lower VAS was recorded after combined stimulus. Our findings indicate that the combined stimulus might not only improve flexibility and strength of muscle, but also active parasympathetic nerve activity. The combined stimulus may thus contribute to relieve the stress and to enhance the muscle function.

Nowadays, NGN(Next Generation Network) is evolving into network with packet based QoS.\ulcorner This paper presents the recovery method as a selecting way of more efficient path in MPLS network that reduces the service interruption time. In order to increase the efficiency of usage of recovering resource which is one of major functions of disorder recovering, it opens the state information of each Node both to fault detector and bandwidth broker so that it reduces message transmission time for recovery when functional disorder occurs. in addition, by effective management and adjustment of required bandwidth of entire network depending on the changes of traffic load, it improves the efficiency of bandwidth usage. and in MPLS network supporting priority selection function, it allocates domains depending on the traffic relating to priority order to the entire network so that\ulcorner it efficiently allocates proper resources to the restoring process between the entry and the end for the traffic of high priority and for low priority traffic, it restores path oかy inside disordered domain. by doing this, it reduces resetting time and unnecessary massage transmission.

With increasing use of a personal mobile computer. the Mobile IPv6 is one of the main protocols that support mobility and complies with IPv6 specification. Similar to the mobile IPv6, the mobile IPv6 also has limitations on fast moving condition. The Hierarchical Mobile IPv6 is a solution that overcomes these limitations. The Hierarchical Mobile IPv6 is a micro mobility protocol that supports fast mobile IP handover and reduces signaling overhead with Mobility Anchor Point(MAP). But until now no paging method is applied to the Hierarchical Mobile IPv6 to reduce unnecessary signaling overhead and power consumption of mobile nodes. So, the paging mechanism for the Hierarchical Mobile IPv6 is proposed in this paper. the mechanism is implemented by making use of the destination option header and extension function and the last location algorithm. The results show that the Hierarchical Mobile IPv6 with the paging ability reduces the traffic of mobile networks by removing unnecessary binding update packet generated whenever handover takes place. Also, the larger the paging size is. the less the number of BU(Binding Update) massage generated.
