Squamous cell carcinoma of lower lip: the results of wide V-shaped resection

  • Generally, if the size of a lip cancer defect exceeds 30% of the lower lip, a local flap or free flap is recommended. However, defects up to 50% of the lower lip in size have been reconstructed successfully by primary closure without a local flap or free flap. In one case, an 80-year-old male farmer who had smoked for more than 50 years presented with squamous cell carcinoma of the lower lip and underwent mass resection and supraomohyoid neck dissection. The defect accounted for almost 2/3 of the lower lip and was repaired by primary closure with V-shaped resection. Biopsy results confirmed pT2N0cM0 stage II disease with clear margins. In another case, a 68-year-old male also presented with squamous cell carcinoma of the lower lip and underwent mass resection. The defect accounted for about half the size of the lower lip but was repaired by primary closure with V-shaped resection. Both patients experienced no discomfort while eating or speaking and were satisfied with the cosmetic and functional outcomes with no evidence of recurrence. Thus, direct closure can be considered even in large lower lip cancers.

Primary cutaneous mucinous carcinoma in a periorbital lesion: two case reports and literature review

  • Primary cutaneous mucinous carcinoma (PCMC) is a rare malignancy of the sweat glands that most commonly affects the periorbital area. It is characterized by slow growth over a prolonged period, and its morphology can be easily confused with a benign tumor, such as an epidermal cyst. Consequently, many patients experience recurrence after undergoing multiple resections. However, there are few reports concerning the surgical management of PCMC. We present two cases of PCMC originating in the periorbital area. The first case involved a 76-year-old man with a mass measuring 3.0×1.5 cm that had been increasing in size. The second case was a 61-year-old man with two masses, each measuring 1.0×1.0 cm, that were also growing. Both patients underwent wide excision with a 5-mm safety margin, which was determined based on the widest view of the cross-section of the mass on the magnetic resonance imaging. Subsequently, based on the intraoperative frozen biopsy results, both patients underwent additional excision with a 5-mm safety margin in only one direction. This report shows that, when determining the surgical margin of PCMC in periorbital area, employing imaging modalities and intraoperative frozen biopsies can be helpful for narrowing the surgical margin.

Ultrasonographic Features of Triple-Negative Breast Cancer: a Comparison with Other Breast Cancer Subtypes

  • Background: Triple-negative breast cancer (TNBC) is known to be associated with aggressive biologic features and a poor clinical outcome. Therefore, early detection of TNBC without missed diagnosis is a requirement to improve prognosis. Preoperative ultrasound features of TNBC may potentially assist in early diagnosis as characteristics of disease. Purpose: To retrospectively evaluate the sonographic features of TNBC compared to ER (+) cancers which include HER(-) and HER2 (+), and HER2 (+) cancers which are ER (-). Materials and Methods: From June 2012 through June 2014, sonographic features of 321 surgically confirmed ER (+) cancers (n=214), HER2 (+) cancers (n=66), and TNBC (n=41) were retrospectively reviewed by two ultrasound specialists in consensus. The preoperative ultrasound and clinicopathological features were compared between the three subtypes. In addition, all cases were analyzed using morphologic criteria of the ACR BI-RADS lexicon. Results: Ultrasonographically, TNBC presented as microlobulated nodules without microcalcification (p=0.034). A lower incidence of ductal carcinoma in situ (p<0.001), invasive tumor size that is>2 cm (p=0.011) and BI-RADS category 4 (p<0.001) were significantly associated with TNBC. With regard to morphologic features of 41 TNBC cases, ultrasonographically were most likely to be masses with irregular (70.7%) microlobulated shape (48.8%), be circumscribed (17.1%) or have indistinct margins (17.1%) and parallel orientation (68.9%). Especially TNBC microlobulated mass margins were more more frequent than with ER (+) (2.0%) and HER2 (+) (4.8%) cancers. Conclusions: TNBC have specific characteristic in sonograms. Ultrasonography may be useful to avoid missed diagnosis and false-negative cases of TNBC.

정지궤도위성 광학탑재체 복사 열제어 시스템 개념 설계 (A CONCEPTUAL DESIGN OF RADIATIVE THERMAL CONTROL SYSTEM IN A GEOSTATIONARY SATELLITE OPTICAL PAYLOAD)

  • A conceptual thermal design is performed for the optical payload system of a geostationary satellite. The optical payload considered in this paper is GOCI(Geostationary Ocean Color Imager) of COMS of Korea. The radiative thermal control system is employed in order to expect a small thermal gradient in the telescope structure of GOCl. Two design margins are applied to the dedicated radiator dimensioning, and three kinds of configuration to the heater power sizing. A Monte-Carlo ray tracing method and a network analysis method are utilized to calculate radiative couplings and thermal responses respectively. At the level of conceptual design, sizing thresholds are presented for the radiator and heater on the purpose of determining the mass and power budget of the spacecraft.

기관에 발생한 Adenoid Cystic Carcinoma 치험 1예 (A case of cystic carcinoma of the trachea)

  • Adenoid cystic carcinoma is a rare primary tracheal tumor and this tumor behave slow growth, low-grade ma!ignancy, locally invasion and long-term host survival. Operation with the primary goal of complete excision is the treatment of choice but this tumor require excessive margins at surgical removal because of locally invasive cancer. A 45-years-old male patient had complained paroxysmal coughing from 1 year ago prior to admission and was diagnosed pre-operatively as endotracheal adenoma. He had been treated by operation, and combined with radiotherapy by 4 MeV. Lineal Accelerator. The tracheal mass was removed by tracheo-bronchotomy transpleurally and right total pneumonectomy was performed. There was post-operative course uneventfully and no post-operative complication. The patient Is free from cancer until post-operative 1 year clinically and alive with good healthy.

  • Theoretical-numerical analysis of wave instability is conducted with parametric response function model. Fluctuating instantaneous mass evaporation rate functionally coupled with pressure perturbations with phase lag is assumed to examine the validity of the method. With sufficiently large amplitude and less phase lag to perturbation, combustion response is resonant to pressure waves, unstable waves are amplified, and the system is driven to instability. Magnitude of response is a crucial instability parameter in the determination of a stability margins and makes a critical change of balancing conditions between the amplifying and damping acoustic energies. In the phase regime the unstable waves are amplified, whereas, the acoustic waves are attenuated in the out-of-phase regime. In the intermediate regime, no distinct tendency of unstable waves was determined.

Differences Between Breast Cancer Patients Younger and Older than 40 Years: Mammographic Findings

  • Objective: To compare the mammogarphic appearance between breast cancer patients aged <40 and ${\geq}40$ years. Methods: Needle localization and biopsy of suspicious mammographic lesions identified 1,959 breast carcinomas in a single institution from Jun 2012 to Apr 2013. According to the age, we divided patients into two groups: <40 and ${\geq}40$ years old, and analyzed mammographic appearance separately. Results: Young patients had 44.2% foci with calcification, but old patients only had 39.4% (P<0.001). In younger group, the ratios of cases according to mass density were 41.8% or higher, 58.2% equivalent and lower. In older group, the ratios were 55.5 % and 44.5%, respectively. There were statistical differences between high density and others (P<0.05). The ratios of cases according to mass margin were 13.9% circumscribed and microlobulated, 86.1% indistinct and spiculated in the younger group, as compared to 6.5% and 93.5%, respectively, in the older group (P<0.05). Conclusions: Mammographic findings differ between young and old patients with breast cancer, for example regarding mass density, mass margin and microcalcification ratios.

$\mu$-합성법을 이용한 사장교의 지진응답 제어 (Seismic Response Control of a Cable-Stayed Bridge using a $\mu$-Synthesis Method)

  • This paper presents a hybrid system combining lead rubber bearings and hydraulic actuators controlled by a μ-synthesis method for seismic response control of a cable-stayed bridge. A hybrid system could alleviate some of restrictions and limitations that exist when each system is acting alone because multiple control devices are operating. Therefore, the overall control performance of a hybrid system may be improved compared to each system, however the overall system robustness may be negatively impacted by active device in the hybrid system or active controller may cause instability due to small margins. Therefore, a f-synthesis method that guarantees the robust performance is considered to enhance the possibility of real applications of the control system. The control performances of the proposed control system are compared with those of passive, active, semiactive control systems and hybrid system controlled by LQG algorithm and an extensive robust analysis with respect to stiffness and mass matrices perturbation and time delay of actuator is performed. Numerical simulation results show that the control performance of the proposed control system is superior to that of the passive system and slightly better than that of the active and semiactive systems and two hybrid systems show similar control performances. Furthermore, the hybrid system controlled by a μ-synthesis method shows the good robustness without loss of control performances. Therefore, the proposed control system could effectively be used to seismically excited cable-stayed bridge which contains many uncertainties.

다단 압축기의 단 매칭 기법에 관한 연구 (A Study on The Stage Matching of Multistage Compressor)

  • A method to search the design parameters for optimum stage matching has been used based on a 1-D mathematical model of a compressor, which uses the data obtained from the preliminary test to identify the design parameters. This methodology was applied with a two-stage axial compressor, which was originally designed for a helicopter gas turbine engine. After Identifying design parameters using preliminary test data, an optimization process has been employed to achieve the best matching between the stages (i.e., maximum efficiency of the compressor at its operation modes within a given range of the rotor speed under given restrictions for required stall margins and mass flow). 3-D flow calculations have been performed to confirm the usefulness of the corrections based on 1-D mathematical model. Calculational results agree well with the experimental data in view of the performance characteristics. Some promising results were produced through the methodology proposed in this paper in conjunction with flow calculations.

Evaluation of correlations for prediction of onset of heat transfer deterioration for vertically upward flow of supercritical water in pipe

  • Supercritical water has great potential as a coolant for nuclear reactor. Its use will lead to higher thermal efficiency of Rankine cycle. However, in certain conditions heat transfer may get deteriorated which may lead to undesirable high clad surface temperature. It is necessary to estimate the operating conditions in which heat transfer deterioration (HTD) will take place, so as to establish thermal margins for safe reactor operation. In the present work, the heat flux corresponding to onset of HTD for vertically upward flow of supercritical water in a pipe is obtained over a wide range of system parameters, namely pressure, mass flux, and pipe diameter. This is done by performing large number of simulations using an in-house CFD code, which is especially developed and validated for this purpose. The identification of HTD is based on observance of one or more peak/s in the computed wall temperature profile. The existing correlations for predicting the onset of HTD are compared against the results obtained by present simulations as well as available sets of experimental data. It is found that the prediction accuracy of the correlation proposed by Dongliang et al. is best among the existing correlations.
