Glioma Mimicking a Hypertensive Intracerebral Hemorrhage

  • Here, we report a rare case of an anaplastic astrocytoma masquerading as a hypertensive basal ganglia hemorrhage. A 69-year-old woman who had been under medical management for hypertension during the past 3 years suddenly developed right hemiparesis with dysarthria. Brain computed tomography (CT) scans with contrast and CT angiograms revealed an intracerebral hemorrhage (ICH) in the left basal ganglia, without an underlying lesion. She was treated conservatively, but underwent a ventriculoperitoneal shunt operation 3 months after the initial attack due to deteriorated mental status and chronic hydrocephalus. Three months later, her mental status deteriorated further. Magnetic resonance imaging (MRI) with gadolinium demonstrated an irregular enhanced mass in which the previous hemorrhage occurred. The final histological diagnosis which made by stereotactic biopsy was an anaplastic astrocytoma. In the present case, the diagnosis of a high grade glioma was delayed due to tumor bleeding mimicking hypertensive ICH. Thus, a careful review of neuroradiological images including MRI with a suspicion of tumor bleeding is needed even in the patients with past medical history of hypertension.

  • In case of lateral cervical cystic lesions, the differential diagnoses include branchial cleft cyst(BCC), teratoma, dermoid, hemangioma and lymphangioma etc. But sometimes metastatic cystic lymph nodes may exist in lateral neck. In such circumstance, the primary lesions are known to stem from oropharynx, nasopharynx, salivary and thyroid gland etc. A-66-year-old-male came to our clinic, due to the lateral cervical mass for 5 years. We performed the neck CT, sonography and sono-guided FNAC. He was initially diagnosed with the benign cyst such as BCC. We performed the excisional biopsy on left level II, but the pathologic report was revealed as metastatic papillary thyroid carcinoma(PTC). And then he received the total thyroidectomy with neck dissection. The final diagnosis was cystic metastasis from PTC. We learn a valuable lesson form this case in the following. Even if the simple cervical cyst is presumed with radiology and clinical pattern, more careful considerations on the basis of history and radiologic findings are mandatory.

  • Dermoid cysts are developmental anomalies that represent the simplest form of teratoma. They are the result of the sequestration of the skin along the lines of embryonic closure. These cysts of the head and neck are uncommon and account for 7% of all dermoid cysts. They are predominantly found in the orbit, floor of mouth, and nose. As a dermoid cyst of the parotid gland is extremely rare, it is often misdiagnosed preoperatively. By way of imaging modalities such as computed tomography, MRI and ultrasongraphy along with FNAB, it can be differentiated from many other cystic lesions of the parotid gland. We report a case of dermoid cyst of the parotid gland which masqueraded as lipoma before complete surgical excision.

  • Safety-critical systems related to the railway communications are currently undergoing changes. Mechanical and electro-mechanical devices are being replaced by programmable electronics that are often controlled remotely via communication networks. Therefore designers and operators now not only have to contend with component failures and user errors, but also with the possibility that malicious entities are seeking to disrupt the services provided by theirs systems. Recognizing the safety-critical nature of the types of communications required in rail control operations, the communications infrastructure will be required to meet a number of safety requirements such as system faults, user errors and the robustness in the presence of malicious attackers who are willing to take determined action to interfere in the correct operation of a system. This paper discusses the safety strategies employed in the railway communications and proposes a security mechanism for Korean railway communication system. We present the developed communication safety evaluation tool based on the proposed security mechanism and also evaluate its protecting capability against the threats of masquerading, eavesdropping, and unauthorized message manipulation.

A Case of Lumbar Metastasis of Choriocarcinoma Masquerading as an Extraosseous Extension of Vertebral Hemangioma

  • We report here on an uncommon case of metastatic choriocarcinoma to the lung, brain and lumbar spine. A 33-year-old woman was admitted to the pulmonary department with headache, dyspnea and hemoptysis. There was a history of cesarean section due to intrauterine fetal death at 37-weeks gestation and this occurred 2 weeks before admission to the pulmonary department. The radiological studies revealed a nodular lung mass with hypervascularity in the left upper lobe and also a brain parenchymal lesion in the parietal lobe with marginal bleeding and surrounding edema. She underwent embolization for the lung lesion, which was suspected to be an arteriovenous malformation according to the pulmonary arteriogram. Approximately 10 days after discharge from the pulmonary department, she was readmitted due to back pain and progressive paraparesis. The neuroradiological studies revealed a hypervascular tumor occupying the entire L3 vertebral body and pedicle, and the tumor extended to the epidural area. She underwent embolization of the hypervascular lesion of the lumbar spine, and after which injection of polymethylmethacrylate in the L3 vertebral body, total laminectomy of L3, subtotal removal of the epidural mass and screw fixation of L2 and L4 were performed. The result of biopsy was a choriocarcinoma.

Giant cell tumor of temporomandibular joint masquerading as temporomandibular joint pain dysfunction syndrome: a rare case report

  • Giant cell tumor (GCT) of the craniofacial bones has been reported but they are not common. This tumor occurs more often in women than in men and predominantly affects patients around the third to fifth decade of life. GCTs are generally benign but can be locally aggressive as well. We report a case of GCT involving the temporomandibular joint (TMJ), which was initially thought to be temporomandibular disorder (TMD). A 22-year-old female presented with swelling and pain over the right temporal region for 18 months associated with jaw locking and clicking sounds. On examination, her jaw deviated to the right during opening and there was a $2{\times}2$ cm swelling over the right temporal region. Despite routine treatment for TMD, the swelling increased in size. Computed tomography and magnetic resonance imaging of the brain and TMJ revealed an erosive tumor of the temporal bone involving the TMJ which was displacing the temporal lobe. Surgical excision was done and the tumor removed completely. Histopathological examination was consistent with a GCT. No clinical or radiological recurrence was detected 10 months post-surgery.

  • NAT (Network Address Translation)은 임의의 사설 IP 주소를 인터넷의 정식 IP 주소로 변환하는 메커니즘으로써 네트워크 보안 및 인터넷 주소의 절약이 주요 기능이다. 일반적으로 NAT는 패킷 헤더의 IP정보를 이용하여 이러한 기능을 수행한다. 하지만 특정 응용 프로토콜들은 패킷의 헤더뿐만 아니라, 데이터 부분에도 종단간 통신에 필요한 정보가 존재하기 때문에, NAT는 이러한 정보를 적절하게 변화해 주어야 할 필요가 있다. 본 논문에서는 포트프락시(port proxy)서버를 이용하여, 패킷의 데이터에 있는 가상 IP정보를 실제 IP정보고 변환하여, 외부와 통신이 원할하게 이루어 질 수 있도록 하는 방안을 제시하고 실제로 구현하였다.

Immunoglobulin G4-Related Disease Masquerading Anaplastic Thyroid Carcinoma

  • Immunoglobulin G4-related disease (IgG4RD) is a chronic inflammatory condition characterized by tissue infiltration with lymphocytes and IgG4-secreting plasma cells, as well as varying degrees of fibrosis. We report a case of a 70-year-old man with a rapid-growing cervical mass for several months. Computed tomography and positron emission tomography showed a huge, ill-defined mass involving left thyroid lobe and encasing the common carotid artery, which was clinically and radiologically suspicious for anaplastic thyroid carcinoma. Ultrasonography-guided core needle biopsy was performed and histopathology examination revealed to be consistent with IgG4RD, and the IgG4/IgG ratio was 0.6. After oral corticosteroid was administered, the mass was dramatically resolved. Because IgG4RD often presents as a single localized and infiltrated mass lesion, it can be confused and misdiagnosed as a malignancy. Thus, clinicians should consider IgG4RD as a differential diagnosis in a rapid-growing neck mass to prevent unnecessary and excessive treatments.

  • Many neurologic disorders manifest as psychiatric symptoms. Anti-N-Methyl-D-Aspartate (NMDA) receptor encephalitis is an autoimmune disease of the brain characterized by numerous neurological and psychiatric features. Despite being rare, its prevalence is rapidly increasing and early management is critical in ensuring successful and sustainable recovery. Therefore, the illness should be considered as a differential diagnosis when clinically assessing patients. This report presents a case of a female child who was hospitalized for acute psychiatric manifestations, which was later confirmed as anti-NMDA receptor encephalitis. She recovered relatively successfully after combined neurological and psychiatric treatment. This report provides information on the clinical course of early onset anti-NMDA receptor encephalitis, including treatment strategy and prognosis.

Craving Jobs? Revisiting Labor and Educational Migration from Uzbekistan to Japan and South Korea

  • This paper focuses on the emerging patterns of educational mobility and unskilled labor migration from Uzbekistan to Japan and South Korea. Labor migration and educational mobility are becoming the next "horizon" in the expanded relationship between East and Central Asia, powered by several factors, including the efforts by Japan and South Korea to build "original" people-oriented policy engagements with the region and the demand from Central Asian states, such as Uzbekistan, to provide more labor opportunities to their young and growing populations. This paper presents the initial findings of a pilot survey that explores and occasionally compares the experiences of Uzbek migrants to Japan and South Korea, using datasets of face-to-face interviews related to various aspects of life in Japan and South Korea. The interviews were conducted face to face and online (Telegram, Skype, etc.) with 66 migrants and Japanese language school students (whom this paper treats as labor migrants masquerading as students) in Japan from November 2019 to January 2020 as well as online with 30 laborers and students in South Korea from August to September 2020.
