Purpose: The aim of this study was to evaluate the marginal and internal gap of zirconia copings fabricated by CAD/CAM system and to research the effect of cement space on the fit of zirconia prosthesis. Methods: Zirconia copings were designed using 3shape software and then milled from presintered zirconia blocks by Kavo dental milling machine. The cement space was set at $0{\mu}m$ around the margins for all groups, and additional cement space appling 1 mm above the finish lines of tooth. The samples were classified into 4 groups according to the cement space setting(Cs-0, Cs-40, Cs-80, Cs-120). Replica technique used to investigate marginal and internal gap. The silicone film thickness was recorded using a optical microscope with image software analysis system at 75${\times}$magnification. The assessment performed on the six points in bucco-lingual and mesio-distal sections(4 groups, 10 copings per group, 12 measuring points per coping). The data were statistically analyzed with one-way ANOVA followed by pair-wise Tukey's post hoc tests(${\alpha}$<0.05). Results: Better marginal and internal fit values were exhibited by $80{\mu}m$ cement space. The results showed that different cement space values had statistically significant effect on the marginal and internal gap of zirconia copings(p<0.001). The gap of margin and axial wall area was smaller than that the gap of occlusal area in all groups(p<0.001). Conclusion : Effect of different cement spaces on the marginal and internal gap of zirconia copings.
This study was to evaluate the fabrication method and marginal adaptation of the conical inner crown fabricated with CAD/CAM. The informations on abutment teeth were transferred to a computer with a micro contact digitizer, which had a $50{\mu}m$ accuracy on the master die. A conical inner crown was designed on a computer and a real crown was machined based on this design using CAM. The marginal fit of a computer-machined conical inner crown was assessed using electron microscopy Measurement of the marginal gap between the conical inner crown and the abutment was performed on four different locations (mesial, distal, buccal, and lingual surfaces) of the finish line. The evaluation was based on 10 test specimens. The results were as follow. 1 The mean marginal gap between the conical inner crown and abutment tooth was $83.2{\pm}43{\mu}m$, 28.9% of the specimen showed marginal gap over $100{\mu}m$. 2. The fabrication method using CATRS and CAM provided clinically acceptable marginal fitness compared to conventional casting method (P<0.05).
본 연구는 수작업으로 제작된 metal coping과 CAD/CAM(Computer Aided Design/Computer Aided Manufacturing)으로 제작된 지르코니아 coping을 제작하여 변연과 내면 적합도를 비교 분석하고자 하였다. 모형은 우레탄 모형재를 사용하여 knife, chamfer변연 두 종류의 지대치를 제작하였다. 수작업과 CAD/CAM으로 제작된 보철물의 변연 적합도를 실리콘 리플리카 테크닉방법을 사용하여 측정하였다. 적합도 측정은 현미경의 CCD카메라로 캡쳐하였다. 이때 지대치와 보철물의 거리는 이미지분석 소프트웨어에서 거리 조정이 된 상태에서 실시하였다. 측정부위는 marginal opening을 MO, marginal gap을 MG, internal gap을 IG, Axial gap을 AG, occlusal gap을 OG로 하였다. 제작법과 변연 형태에 따른 비교분석을 위해 이원배치분산분석을 실시하였으며 서로 다른 평균값들의 비교 분석을 위해서 일원배치분산분석과 Scheffe's 사후 검정을 실시하였다. 그 결과 CAD/CAM의 OG와 knife 변연의 MO을 제외하고는 < $120{\mu}m$ 적합도를 나타냈다. CAD/CAM으로 제작된 coping은 chamfer 변연 MO에서 높은 적합도를 보였으나 MG에서는 knife변연이 chamfer변연 보다 더 좋은 적합도를 보였다. AG의 내면 적합도는 가장 좋은 값을 나타내었다(< $38{\mu}m$).

PURPOSE. To evaluate the fit of a three-unit metal framework of fixed dental prostheses made by subtractive and additive manufacturing. MATERIALS AND METHODS. One master model of metal was fabricated. Twenty silicone impressions were made on the master die, working die of 10 poured with Type 4 stone, and working die of 10 made of scannable stone. Ten three-unit wax frameworks were fabricated by wax-up from Type IV working die. Stereolithography files of 10 three-unit frameworks were obtained using a model scanner and three-dimensional design software on a scannable working die. The three-unit wax framework was fabricated using subtractive manufacturing (SM) by applying the prepared stereolithography file, and the resin framework was fabricated by additive manufacturing (AM); both used metal alloy castings for metal frameworks. Marginal and internal gap were measured using silicone replica technique and digital microscope. Measurement data were analyzed by Kruskal-Wallis H test and Mann-Whitney U-test (${\alpha}=.05$). RESULTS. The lowest and highest gaps between premolar and molar margins were in the SM group and the AM group, respectively. There was a statistically significant difference in the marginal gap among the 3 groups (P<.001). In the marginal area where pontic was present, the largest gap was $149.39{\pm}42.30{\mu}m$ in the AM group, and the lowest gap was $24.40{\pm}11.92{\mu}m$ in the SM group. CONCLUSION. Three-unit metal frameworks made by subtractive manufacturing are clinically applicable. However, additive manufacturing requires more research to be applied clinically.
To investigate the effect of resin cement, which had been known to increase the adhesive capacity of the cast gold inlay, on the gingival marginal fit and whether the tin-planting of the beveled area affects the marginal fit, Class II cast gold inlays were made on the 25 sound molars. Control group(ZPC goup) was cemented with the ZPC by conventional method. Experimental groups were cemented with the resin cement(Super-hond & $Panavia_{EX}$) and subdivided further by the existence or nonexistence of the tin-plating of the beveled area(ST & PT groups: with plating, SNT & PNT groups: without plating). So, each group was consisted of 5 teeth and the gingival margin of each specimen was mesiodistally sectioned by 3 times and the marginal and internal gap were evaluated by the Stereo Microscope (${\times}180$) and the Scanning Electron Micrascope(${\times}5,000$) was used for examining the adhesive relationship of the resin cement to the cavity wall and to the cast gold surface. The results were as follows : 1. Marginal gap was less than internal gap in all groups. 2. ZPC and SNT(bevel without tin-plating) groups showed the least gap and gap in PNT(bevel without tin-plating) group, ST(bevel with tin-plating) group, PT(bevel with tin-plating) group showed the greater value in order in evaluation of the both internal gap and marginal gap. 3. With the exception of the relationships between ZPC and SNT groups, ST and PNT groups, relationships between any other groups showed the statistical significance in the internal gap(p<0.05). 4. In the marginal gap, all relationships between groups showed the statistical significance (p<0.05) except the relationships between ZPC and SNT groups, ST and PNT groups, ZPC and PNT groups. 5. ZPC group showed more soluble phenamena than the resin groups(ST, SNT, PT, PNT). 6. Resin cement showed the void spaces in spite of good penetration into the micro-irregularities on both the tooth surface and the cast surface. The void was shown more in PT and PNT groups than in the ST and SNT groups. 7. After the treatment of heat and desiccation for SEM specimen, resin cements were detached more easily from the tooth surface than from the cast surface.
PURPOSE. This study evaluated marginal fit of glass infiltrated alumina cores fabricated using two techniques and their marginal stability after firing cycles of veneering porcelain. MATERIALS AND METHODS. Fifteen standardized all-ceramic crowns were fabricated on a metal die using each technique: slip cast technique of VITA In-Ceram sprint Alumina (Group A as control) and plastic foil matrix technique of Turkom-Cera fused alumina core system (Group B). Copings were compared between groups and within groups at coping stage and after firing each layer of veneering porcelain. A device was used to standardize seating of copings on the metal die and positioning of the specimens under the microscope after each stage of fabrication. The specimens were not cemented and marginal gap was measured using an image analyzing software (Imagepro Express) on the photographs captured under an optical microscope. Two tailed unpaired 't test' was used to compare marginal gaps in two groups and one way ANOVA was used to analyze marginal distortion within each group at 95% confidence interval. RESULTS. The marginal gap was smaller at the coping stage in group B ($60+30{\mu}M$) than group A ($81+21{\mu}M$) with statistical significance. After firing of veneering porcelain the difference was insignificant. At the final stage, both groups exhibited lower mean marginal gaps than at the initial coping stage with the difference of $11.75{\mu}M$ for group A and $11.94{\mu}M$ for group B, but it was statistically insignificant due to high value of standard deviation. CONCLUSION. Within the limitations of this study, it was concluded that both techniques produced copings with comparable and acceptable marginal fit and marginal stability on firing veneering porcelain.
PURPOSE. This study compared digital (reference point matching) and replica methods for measuring marginal and internal fit of full coverage restorations. MATERIALS AND METHODS. A maxillary left first molar typodent was fixed on to an aluminum base and prepared to receive all-ceramic full coverage restoration. The model was scanned with an intraoral scanner (CEREC Omnicam, Sirona, York, PA, USA). Twelve crowns were fabricated from lithium disilicate blocks (IPS emax CAD, Ivoclar Vivadent) and then crystalized. Marginal and internal fit of each restoration was measured by two examiners using replica and a new digital three-dimensional technique. Reliability between the two methods and two examiners was assessed by correlation and Cronbach's Alpha coefficient (P<.05). A Bland-Altman assessment for agreement was used to compare the two methods. RESULTS. Bland-Altman assessment showed that the mean of difference for marginal, absolute marginal, and axial gap was respectively -1.04 ㎛, -41.9 ㎛, and -29.53 ㎛ with limit of agreement (LOA) between -37.26 to 35.18 ㎛ for marginal, -105.85 to 22.05 ㎛ for absolute marginal and -80.52 to 22.02 ㎛ for axial gap. Positive correlation for repeatability (P<.05) in determining marginal and internal gaps by the two examiners in both techniques was revealed. Reliability of both techniques in all sites of measurements was at least good (0.8 ≤ α < 0.9). CONCLUSION. Both measuring techniques appeared highly reliable for evaluating fit of fixed dental restorations, while reference point matching provided higher values in axial and absolute marginal gap assessment.

The purpose of this study was to compare the marginal fit of the anterior single restorations made by using the Digident CAD / CAM zirconia ceramic crowns with metal-ceramic restorations and to obtain more accurate information by using a large enough sample size and by making sufficient measurements per specimen. Material and Methods. The crowns were made from one extracted maxillary central incisor pre-pared with a 1mm shoulder margin and $6^{\circ}$ taper walls by milling machine. The in vitro marginal discrepancies of the digident CAD / CAM zirconia ceramic crowns and control groups(metal ceramic crowns) were evaluated and compared. Twenty crowns per each system were fabricated. Measurements of a crown were recorded at 50 points that were randomly selected for marginal gap evaluation. Parametric statistical analysis was performed for the results. Conclusion. Within the limitations of this in vitro study, the following conclusions were drawn: 1. Mean gap dimensions and standard deviations at the marginal opening for maxillary incisal crowns were $88{\pm}10{\mu}m$ for the control (metal-ceramic crowns), $92{\pm}4{\mu}m$ for Digident CAD / CAM zirconia ceramic crowns. 2. Marginal gap between Digident CAD / CAM zirconia ceramic crowns and metal ceramic crowns did not show significant difference (P>.05). 3. The Digident CAD/ CAM zirconia ceramic crowns and metal ceramic crowns showed clinically acceptable marginal discrepancy.

Material and methods. The in vitro marginal discrepancies of computer-aided milled Cercon crowns and control groups (metal ceramic crowns) were evaluated and compared. The crowns were made from one extracted maxillary central incisor prepared by milling machine. 30 crowns per each system were fabricated. Measurements of a crown were recorded at 50 points that were randomly selected for marginal gap evaluation. Parametric statistical analysis was performed for the results. Results. The means and standard deviations of the marginal fit were 85$\pm$22$\mu$m for the control group and 91$\pm$15$\mu$m for the Cercon crowns. The t-test of the marginal discrepancies between Cercon crowns and metal-ceramic crowns were performed. Significant differences were not found between groups (P=0.230>.05). Based on the criterion of 120$\mu$m as the limit of clinical acceptability, the mean marginal fits of Cercon crowns and metal-ceramic crowns were acceptable. Conclusion. Within the limitations of this in vitro study, the following conclusions were drawn: 1. Mean gap dimensions and standard deviations at the marginal opening for maxillary incisal crowns were 85$\pm$22$\mu$m for the control (metal-ceramic crowns), 91$\pm$15$\mu$m for Cercon crowns. 2. The Cercon crowns showed slightly larger marginal gap discrepancy than the control but marginal gap between Computer-aided milled Cercon crowns and metal ceramic crowns did not showed significant difference (P>.05). 3. The in vitro marginal discrepancies of three different all-ceramic crown systems (Celay In-Ceram. Conventional In-Ceram. IPS Empress 2 layering technique) and one control group (PFM) were evaluated and compared. The crowns were made from one extracted maxillary central incisor prepared with a 1mm shoulder margin and $6^{\circ}$ taper walls by milling machine. 10 crowns per each system were fabricated. Measurements or a crown were recorded at 50 points that were randomly selected for marginal gap evaluation. Non-parametric statistical analysis was performed for the results. Within the limits of this study, the following conclusions were drawn: 1 Mean gap dimensions and standard deviations at the marginal opening for the maxillary incisor crowns were $98.2{\pm}40.6{\mu}m$ for PFM, $83.5{\pm}18.7{\mu}m$ for Celay In-Ceram, $104.9{\pm}44.1{\mu}m$ for conventional In-Ceram, and $45.5{\pm}11.5{\mu}m$ for IPS Empress 2 layering technique. The IPS Empress 2 system showed the smallest marginal gap (P<0.05). The marginal openings of the other three groups were not significantly different (P<0.05). 2 The marginal discrepancies found in this study were all within clinically acceptable standards ($100\sim150{\mu}m$). 3. When the variable is so controlled that the system may be the only one, mean value is interpreted to be the marginal discrepancy of a restoration which is made by each system and standard deviation is to be technique-sensitivity of each one. 4. From the standard deviations. the copy-milling technique (Celay/In-Ceram) was not considered to be technique-sensitive in comparison with other methods. 5. Parametric analysis is more reliable than non-parametric one in interpretation of the mean and standard deviation. The sample size of each group has to be more than 30 to use parametric statistics. The level of clinically acceptable marginal fit has not been established. Further studies are needed.
